Abstract:The number of GPs working in England has crept up in the past year, a rise almost entirely accounted for by an increase in GP trainees. In March 2019 England’s GPs totalled 44 847, a rise of 350 (0.8%) on the same month in 2018. The full time equivalent count, which allows for some GPs working only part time, showed a similar increase, rising from 34 424 to 34 736 (0.9%).1 But closer examination of the data released by NHS Digital shows that the headcount of fully qualified GPs fell by 494 year on year, while the number of trainees increased by 760. Abstract:Objective To describe trends in the organisational structure, workforce and recorded appointments by role in English general practice. Design Retrospective longitudinal study. Setting English general practice. Data sources and participants NHS England, Office for Health Improvement and Disparities and Care Quality Commission national administrative datasets covering between 5 to 10 years from 2013 to 2023. Results Between 2013 and 2023, the number of general practices fell by 20% from 8044 to 6419; the average practice list size increase by 40% from 6967 to 9724 patients. The total population covered by providers with over 100 000 registered patients reached 2.3 million in 2023 compared to 0.5 million in 2017. The proportion of practices under individual ownership decreased from 13% to 11% between 2018 and 2023; there was little change in the proportion owned by partnerships, incorporated companies or NHS bodies, which respectively averaged around 80.3%, 6.9% and 0.7%. Between 2015 and 2022, there was a 20% rise in the total full-time equivalent (FTE) general practice workforce, including Primary Care Network staff, from 1.97 to 2.37 per 1000 patients because of an increase in multidisciplinary other 'Direct Patient Care' (DPC) and administrative roles. The number of nurses remained stable, and the number of qualified general practitioners (GPs) decreased by 15%. In September 2022, there were 0.45 FTE qualified GPs per 1000 patients; GPs and other DPC roles, excluding nurses, each represented 19% of the FTE per 1000 patients workforce; administrative roles represented 51%. The general practice workforce is predominantly female. A quarter of GPs qualified overseas. Between 2018 and 2023, there was no clear upward or downward trend in total appointments per 1000 patients with, on average, half provided by GPs. Conclusions Since 2013, there has been a shift in general practice towards larger practices with more multidisciplinary teams, alongside a reduction in the number of FTE qualified GPs per 1000 patients. We recommend that the impacts of these changes on access, quality and costs are closely monitored.

Abstract:Background: There are not enough GPs in England. Access to general practice and continuity of care are declining. Aim: To investigate whether practice characteristics are associated with life expectancy of practice populations. Design and setting: A cross-sectional ecological study of patient life expectancy from 2015-2019. Method: Selection of independent variables was based on conceptual frameworks describing general practice's influence on outcomes. Sixteen non-correlated variables were entered into multivariable weighted regression models: population characteristics (Index of Multiple Deprivation, region, % White ethnicity, and % on diabetes register); practice organisation (total NHS payments to practices expressed as payment per registered patient, full-time equivalent fully qualified GPs, GP registrars, advanced nurse practitioners, other nurses, and receptionists per 1000 patients); access (% seen on the same day); clinical performance (% aged ≥45 years with blood pressure checked, % with chronic obstructive pulmonary disease vaccinated against flu, % with diabetes in glycaemic control, and % with coronary heart disease on antiplatelet therapy); and the therapeutic relationship (% continuity). Results: Deprivation was strongly negatively associated with life expectancy. Regions outside London and White ethnicity were associated with lower life expectancy. Higher payment per patient, full-time equivalent fully qualified GPs per 1000 patients, continuity, % with chronic obstructive pulmonary disease having the flu vaccination, and % with diabetes with glycaemic control were associated with higher life expectancy; the % being seen on the same day was associated with higher life expectancy in males only. The variable aged ≥45 years with blood pressure checked was a negative predictor in females. Conclusion: The number of GPs, continuity of care, and access in England are declining, and it is worrying that these features of general practice were positively associated with life expectancy.

Abstract:BackgroundEnglish primary care faces a reduction in GP supply and increased demand.AimTo explore trends in GP working time and supply, accounting for factors influencing demand for services.Design and settingRetrospective observational study in English primary care between 2015 and 2022.MethodTrends in median GP contracted time commitment were calculated using annual workforce datasets. Three measures of demand were calculated at practice-level: numbers of patients; numbers of older patients (≥65 years); and numbers of chronic conditions using 21 Quality and Outcomes Framework disease registers. Multi-level Poisson models were used to assess associations between GP supply and practice demand, adjusted for deprivation, region, and year.ResultsBetween 2015 and 2022, the median full-time equivalent (FTE) of a fully qualified GP decreased from 0.80 to 0.69. There was a 9% increase in registered population per GP FTE (incidence rate ratio [IRR] = 1.09; 95% confidence interval [CI] = 1.05 to 1.14). This increase was steeper using numbers of chronic conditions (32%, IRR = 1.32; 95% CI = 1.26 to 1.38). Practices in the most deprived decile had 17% more patients (IRR = 1.17; 95% CI = 1.08 to 1.27) and 19% more chronic conditions (IRR = 1.19; 95% CI = 1.06 to 1.33) per GP FTE, compared with the least deprived decile. These disparities persisted over time. All regions reported more chronic conditions per GP FTE than London.ConclusionPopulation demand per GP has increased, particularly in terms of chronic conditions. This increase is driven by several factors, including a reduction in GP contracted time commitments. Persistent deprivation gradients in GP supply highlight the need to recruit and retain GPs more equitably.

Despite being awash with paper, the NHS in England lacks real time information on primary care, and this report aims to fill the gap.Its sources included data collected over five years from 177 general practices by TTP, a supplier of GP information systems, together with a one week snapshot survey of 43 practices and interviews with 318 GP trainees. Abstract:Anyone keeping abreast of general practice workforce issues will have heard a common refrain over the past years: there are not enough GPs due to a recruitment and retention crisis. More GPs are leaving the NHS amidst an increasing demand for services, all of which is pushing general practice to the brink. Despite this, Pulse has recently published results of a survey of 612 GP partners across the UK, and found a 44% reduction in the number of GP vacancies advertised over the past year. If there's such a shortage of GPs, where did all the jobs go? The Pulse survey indicates that a lack of funding in general practice is impacting recruitment, alongside the increase in the additional roles reimbursement scheme (ARRS) in hiring other healthcare professionals to fill job roles in general practice.1 I've written here previously about the ARRS and some of the wider impacts of the scheme.2 The 2019 Conservative Party Manifesto promised an additional 6000 GPs. When it became clear that this goal would not be achieved, ARRS shifted the scope from meeting the manifesto pledge to instead increase the number of appointments offered in general practice by 50 million a year. Abstract:Purpose: Relationship continuity of care has declined across English primary health care, with cross-sectional and longitudinal variations between general practices predicted by population and service factors. We aimed to describe cross-sectional and longitudinal variations across the COVID-19 pandemic and determine whether practice factors predicted the variations. Methods: We conducted a longitudinal, ecological study of English general practices during 2018-2022 with continuity data, excluding practices with fewer than 750 patients or National Health Service (NHS) payments exceeding £500 per patient. Variables were derived from published data. The continuity measure was the product of weighted responses to 2 General Practice Patient Survey questions. In a multilevel mixed-effects model, the fixed effects were 11 variables' interactions with time: baseline continuity, NHS region, deprivation, location, percentage White ethnicity, list size, general practitioner and nurse numbers, contract type, NHS payments per patient, and percentage of patients seen on the same day as booking. The random effects were practices. Results: Main analyses were based on 6,010 practices (out of 7,190 active practices). During 2018-2022, mean continuity in these practices declined (from 29.3% to 19.0%) and the coefficient of variation across practices increased (from 48.1% to 63.6%). Both slopes were steepest between 2021 and 2022. Practices having more general practitioners and higher percentages of patients seen the same day had slower declines. Practices having higher baseline continuity, located in certain non-London regions, and having higher percentages of White patients had faster declines. The remaining variables were not predictors. Conclusions: Variables potentially associated with greater appointment availability predicted slower declines in continuity, with worsening declines and relative variability immediately after the COVID-19 lockdown, possibly reflecting surges in demand. To achieve better levels of continuity for those seeking it, practices can increase appointment availability within appointment systems that prioritize continuity.Annals Early Access article.

Abstract:Background: There is little evidence and no agreement on what constitutes full-time working for general practitioners (GPs). This is essential for workforce planning, resource allocation and accurately describing GP activity. Aim: To clarify the definition of full-time working for general practitioners, how this has changed over time and whether these changes are explained by GP demographics. Design and Setting: Repeated cross-sectional national surveys between 2010 and 2021. Method: Comparison of three measures of working time commitments (hours and sessions per week and hours per session) plus a measure of workload intensity across survey years. Multiple regression to adjust the changes over time for age, sex, ethnicity, contract type, area deprivation, and rurality. Unadjusted hours and sessions per week were compared to definitions of full-time working. Results: Average hours and sessions per week reduced from 40.5 (95% CI: 38.5, 42.5) to 38.0 (36.3, 39.6) and 7.3 (7.2, 7.3) to 6.2 (6.2, 6.3) respectively between 2010 and 2021. In 2021, 54.6% of GPs worked at least 37.5 hours per week and 9.5% worked at least 9 sessions. Hours per session increased from 5.7 (5.7, 5.7) to 6.2 (6.2, 6.3) between 2010 and 2021. Partners worked more hours, sessions and hours per session. Adjustments increased the increase in hours per session from 0.54 to 0.61. Conclusion: At the current average duration of sessions, six sessions per week aligns with the NHS definition of full-time hours. However, hours per week is a more consistent way to define full-time work for GPs.

Abstract:Do you feel that you are safely providing care to your patients? The recent Royal College of General Practitioners (RCGP) 'GP Voice' survey has highlighted the tension clinicians are feeling between workload, pressures on time, and concerns about patient safety. Overall, 76% of GPs responding to the survey think that patient safety is being compromised by their excessive workloads. This boils down to a matter of time, with 60% of responders reporting that they don't have enough time to adequately assess and treat their patients during appointments. Kamila Hawthorne has called these results 'alarming', and it's hard to disagree with this when patient safety should be one of our absolute priorities.1 How has increasing workload impacted on GPs and our sense of providing safe care to our patients? Workload and complexity of care is increasing in general practice, as is the volume of patients. RCGP tracking data show that in April 2024 there were 2294 patients per full-time equivalent GP in England, an increase of 7.2% since 2019.2 And perceived workload burden among British GPs has increased more than some of our international colleagues. A recent survey indicated that British GPs experienced higher rises in workload than their counterparts in 10 other Commonwealth countries.3 Reflecting the findings of the RCGP survey, British GPs were the least satisfied with their workload and time available to spend with patients, with only 7% of responders satisfied with the time spent with patients, and only 4% of GPs extremely or very satisfied with their level of workload.3 ...
