Abstract:Background Cardiac structure and function are well-studied in Western countries. However, epidemiological data is still scarce in China. Methods Our study was conducted in the framework of cardiovascular health examinations for the current and retired employees of a factory and their family members. According to the American Society of Echocardiography recommendations, we performed echocardiography to evaluate cardiac structure and function, including left atrial volume, left ventricular hypertrophy and diastolic dysfunction. Results The 843 participants (43.0 years) included 288 (34.2%) women, and 191 (22.7%) hypertensive patients, of whom 82 (42.9%) took antihypertensive drugs. The prevalence of left atrial enlargement, left ventricular hypertrophy and concentric remodeling was 2.4%, 5.0% and 12.7%, respectively. The prevalence of mild and moderate-to-severe left ventricular diastolic dysfunction was 14.2% and 3.3%, respectively. The prevalence of these cardiac abnormalities significantly ( P ≤ 0.002) increased with age, except for the moderate-to-severe left ventricular diastolic dysfunction. After adjustment for age, gender, body height and body weight, left atrial enlargement was associated with plasma glucose ( P = 0.009), and left ventricular hypertrophy and diastolic dysfunction were significantly associated with systolic and diastolic blood pressure ( P ≤ 0.03), respectively. Conclusions The prevalence of cardiac structural and functional abnormalities increased with age in this Chinese population. Current drinking and plasma glucose had an impact on left atrial enlargement, whereas systolic and diastolic blood pressures were major correlates for left ventricular hypertrophy and diastolic dysfunction, respectively.
Cardiac Dysfunction Investigation in Prehypertension

Abstract:To study the possible cardiac function alternation in hypertension, 13 cardiac function parameters such as blood pressure (BP), cardiac output (CO), ejection time (ET) and stroke volume (SV), etc. were measured in 898 subjects. The subjects were grouped into 3 groups by blood pressure, namely: normotension, prehypertension and hypertension group. Statistic analyses showed that in prehypertension subjects, peak velocity and cardiac output increased, while the systemic vascular resistance index had no significant increase compared with normotensive. Peak velocity, mean pressure gradient and velocity time integral in prehypertension were significantly correlated with systolic blood pressure, however, no correlation was found in either normotension or hypertension subjects. The results suggest that increasing cardiac output and peak velocity occurred at the phase of prehypertension, which may conduce to the elevation of blood pressure
Changes of Cardiac Structure and Function in the Hospitalized Patients With Essential Hypertension in Beijing

Abstract:Objective: To find out the echocardiographic characteristics of cardiac structure and function changes and its related factors in the hospitalized patients with essential hypertension in Beijing. Methods: This is an epidemiological retrospective survey performed in hypertensive patients in 20 hospitals in Beijing. Echocardiographic results and clinical parameters were analyzed. Results: The echocardiographic characteristics of all patients showed that the proportion of left atria enlargement and E/A abnormality were 50. 6% and 70. 0% respectively, which were the two most common cardiac abnormalities in hypertension. The elderly group (60-79 years old) complicated with more structural and functional damage than that in the middle-age group. The higher the blood pressure,the more patients complicated with the left atria enlargement and left ventricular hypertrophy. Patients with heart damages had been treated with more antihypertensive agents. The multiple-factor Logistic regression analysis showed that male, atrial fibrillation, obesity, aging, longer hypertensive history and elevated systolic blood pressure were the risk factors of left atria enlargement. The risk factors of left ventricular hypertrophy were male, longer hypertensive history, elevated systolic blood pressure and elder. The risk factors of left ventricular enlargement were female, atrial fibrillation and obesity. Conclusion :(1)Left atria enlargement and E/A abnormality were two most common changes of cardiac abnormalities in hypertension. (2)The elderly patients complicated with more cardiac structural and functional damages. (3)The higher the blood pressure, the more patients complicated with the left atria enlargement and left ventricular hypertrophy. (4) Male, longer hypertensive course, in-elevated systolic blood pressure and aging were common risk factors of left atria enlargement and left ventricular hypertrophy. More patients with atrial fibrillation complicated with enlargement of left atria and left ventricle.

Abstract:Corresponding to human anatomical structure,a bond graph model for cardiovascular circulation system including heart,head and neck circulation,pulmonary circulation,coronary circulation,abdomen circulation and extremity circulation was developed,which comprise of 54 mathematical equations.The blood pressure and blood flow wave on arbitrary site of circulation can be obtained by solving the equations,meanwhile,the stroke volume,cardiac output and eject fraction of heart can be computed.The model achieved the typical physiological parameters such as,systolic/diastolic blood pressure 115/78 mmHg,stroke volume 80 mL,cardiac output 5.76 L and eject fraction 64.5%.The simulation results were in agreement with the human physiological experimental data,so that the validity of the model was verified.By adjusting the initial condition parameters of the model,the simulations for pathological features of the cardiovascular system,such as atherosclerosis,hypertension and stenosis in left subclavicle artery were implemented,with the results being consistent with human pathological features.Furthermore,the components of the model are composed of identical but independent structures,so the model is flexible to be extended.

Abstract:Background Atrial fibrillation (AF) is the most common persistent cardiac arrhythmia. This study aimed to estimate its prevalence and explore associated factors in adults aged 18 years or older in China. Methods Study data were derived from a national sample from July 2020 to September 2021. Participants were recruited using a multistage stratified sampling method from twenty-two provinces, autonomous regions, and municipalities in China. AF was determined based on a history of diagnosed AF or electrocardiogram results. Findings A total of 114,039 respondents were included in the final analysis with a mean age of 55 years (standard deviation 17), 52.1% of whom were women. The crude prevalence of AF was 2.3% (95% confidence interval [CI] 1.7-2.8) and increased with age. The age-standardized AF prevalence was 1.6% (95% CI 1.6-1.7%) overall, and 1.7% (1.6-1.8%), 1.4% (1.3-1.5%), 1.6% (95% CI 1.5-1.7%), and 1.7% (1.6-1.9%) in men, women, urban areas, and rural areas, respectively. The prevalence was higher in the central regions (2.5%, 2.3-2.7%) than in the western regions (1.5%, 1.0-2.0%) and eastern regions (1.1%, 1.0-1.2%) in the overall population, either in the gender or residency subgroups. The associated factors for AF included age (per 10 years; odds ratio 1.41 [95% CI 1.38-1.46]; p < 0.001), men (1.34 [1.24-1.45]; p < 0.001), hypertension (1.22 [1.12-1.33]; p < 0.001), coronary heart disease (1.44 [1.28-1.62]; p < 0.001), chronic heart failure (3.70 [3.22-4.26]; p < 0.001), valvular heart disease (2.13 [1.72-2.63]; p < 0.001), and transient ischaemic attack/stroke (1.22 [1.04-1.43]; p = 0.013). Interpretation The prevalence of AF was 1.6% in the Chinese adult population and increased with age, with significant geographic variation. Older age, male sex, and cardiovascular disease were potent factors associated with AF. It is crucial to increase the awareness of AF and disseminate standardized treatment in clinical settings to reduce the disease burden. Copyright (c) 2022 Published by Elsevier Ltd. Abstract:Background: Many studies revealed that variations in cardiac ion channels would cause cardiac arrhythmias or act as genetic risk factors. We hypothesized that specific single nucleotide polymorphisms in cardiac ion channels were associated with cardiac rhythm disturbance in the Chinese population. Method: We analyzed 160 nonfamilial cardiac arrhythmia patients and 176 healthy individuals from which 81 individuals were selected for association study, and a total of 19 previously reported SNPs in four cardiac ion channel genes (KCNQ1, KCNH2, SCN5A, KCNE1) were genotyped. Results: The frequency of KCNQ1 1638G>A, as well as the haplotype harboring KCNQ1 1638A, KCNQ1 1685 + 23G and 1732 + 43T (haplotype AGT) was significantly higher in healthy controls than in arrhythmia patients. This finding implicated that this haplotype (AGT) might be a protective factor against arrhythmias. Conclusions: Our study provided important information to elucidate the effect of SNPs of cardiac ion channel genes on channel function and susceptibility to cardiac arrhythmias in Chinese population.

Age- and Sex-Specific Reference Values for Atrial and Ventricular Structures in the Validated Normal Chinese Population: A Comprehensive Measurement by Cardiac MRI

Abstract:Background: Left and right cardiac structures have been shown to provide important prognostic information in patients with various cardiac disorders. They have always been important biomarkers in patients with heart failure and are crucial in the judgment of a variety of heart diseases. Purpose: To provide age- and sex-specific reference values of the normal cardiac structure of Chinese adults. Study type: Prospective study. Population: In all, 200 healthy adult volunteers with 20 men and 20 women in each of five age deciles from 20 to 70 years. Field strength: 3.0T, steady-state free precession (SSFP), turbo spin-echo (TSE) sequence. Assessment: The reference range of cardiac structure values was normalized to age, gender, and body surface area (BSA). The height, weight, blood pressure, and body mass index were measured as well. Statistic tests: Kolmogorov-Smirnov's, independent-sample t-tests, one-way analysis of variance (ANOVA), Pearson's coefficient, and linear regression. Results: The normalized left atrial (LA) transverse diameter (two-chamber view) at end-systolic, and left ventricular outflow tract diameter (LVOT) were significantly larger in females compared to males (all P < 0.001), while LA volume at end-systole was found higher in males (males: 36.25 ± 10.03 mL vs. females: 32.78 ± 10.27 mL). With increasing age, there was a weak but significant decrease in normalized LA transverse diameter (two-chamber view) at end- systolic (r = -0.25, P < 0.001). The normalized RA volume at end-systole and RVOT diameter showed significant decrease with advancing age (r = -0.22, P = 0.002; r = -0.34, P < 0.001). The indexed left and right ventricular volumes were mostly smaller with advancing age. The LA longitudinal parameters correlated significantly with LA area measured in the two-chamber view at end-systole (r = 0.78, P < 0.001). Data conclusion: Reference values for the morphological parameters of the intracardiac structure by sex and age distribution are provided in a Chinese population. Abstract:Compared to general population, China Cardiologists showed lower prevalences of hypertension, diabetes and hyperlipidemia and better management of cardiovascular risk. More attention and efforts are still needed to further improve the cardiovascular health of cardiologists in China, especially for males and intervention cardiologists.

Abstract:Heart-brain correlation is an important component of Chinese medicine about the theory of zang-fu organs, which is still valuable for acupuncture clinical practice. Nowadays, increasing evidence supports the close association between the heart-brain axis, central autonomic nerve network and cardiovascular diseases, as well as the extensive regulative effects of acupuncture intervention on the heart-brain axis, functional connectivity of the brain, automatic nerve activities and cardiac functions. Therefore, the authors of the present paper hold that from the viewpoint of the heart-brain relationship, and by combining non-invasive functional brain imaging techniques with the patients' subjective and objective clinical indexes, our researchers will possibly and systematically reveal the underlying central mechanisms of acupuncture therapy in the treatment of chronic stable angina pectoris. However, the concrete biochemical mechanism should be proved via other advanced biological techniques.

Abstract:Elevated central systolic blood pressure (BP) increases the risk of cardiovascular events and appears superior to peripheral BP for long term risk prediction. The objective of this study was to identify demographic and clinical factors associated with central pressures in patients with uncomplicated hypertension. We prospectively examined peripheral BP, central aortic BP, and arterial wall properties and wave reflection in 57 subjects with uncomplicated essential hypertension in the Pharmacogenomic Evaluation of Antihypertensive Responses (PEAR) Study. Significant predictors of central SBP included height, smoking status, heart rate (HR), and peripheral systolic BP (SBP), while central diastolic BP (DBP) was explained by peripheral DBP and HR. These variables accounted for nearly all of the variability in central SBP and central DBP (R2 = 0.94 and R2 = 0.98, respectively). Central pulse pressure variability was largely explained by gender, ex-smoking status, HR, peripheral SBP, and peripheral DBP (R2 = 0.94). Central augmented pressure had a direct relationship with smoking status, peripheral SBP, and duration of hypertension, whereas it was indirectly related to height, HR, and peripheral DBP. Easily obtainable demographic and clinical factors are associated with central pressures in essential hypertensive persons. These relationships should be considered in future studies to improve assessment of BP to reduce cardiovascular risk and mortality.

Abstract:Heart failure is a complex clinical syndrome,and impaired filling or ejection disorders result from any ventricular structure or dysfunction can cause heart failure.The prevalence of heart failure in adult populations in developed countries has reached 1% to 2%,while the prevalence in elderly people over 70 years of age has increased to ≥ 10%. With the population aging and the prevalence of coronary heart disease increased, the prevalence of heart failure has increased,becoming a disable and fatal disease.The changes of the central nervous system hormones such as renin-angiotensin system (RAS),inflammatory cytokines (proinflammatory cytokines,PIC),and reactive oxygen species (ROS)may be closely related to increased central activity in heart failure,which can significantly alter the activities of peripheral sympathetic nerves. Constant sympathetic nervous activity is an important cause of development of heart failure,so reducing the sympathetic excitability of heart failure is regarded as one of the focuses of treatment and research.This paper focuses on the influence of central neurohormone on heart failure and possible central mechanism.

Abstract:Ethnopharmacological relevance: Cardiac hypertrophy (CH) is an incurable heart disease, contributing to an increased risk of heart failure due to the lack of safe and effective strategies. Therefore, searching for new approaches to treat CH is urgent. Centella asiatica (L.) Urb. (CA), a traditional food and medicinal natural plant, has been turned out to be effective in the treatment of cardiovascular disease, but its efficacy and potential mechanisms in alleviating CH have not yet been investigated. Aim of study: In this study, we aimed to elucidate the multi-level mechanisms underlying the effect of CA against CH. Study design and methods: A systems pharmacology approach was employed to screen active ingredients, identify potential targets, construct visual networks and systematically investigate the pathways and mechanisms of CA for CH treatment. The cardiac therapeutic potential and mechanism of action of CA on CH were verified with in vivo and in vitro experiments. Results: Firstly, we demonstrated the therapeutic effect of CA on CH and then screened 13 active compounds of CA according to the pharmacokinetic properties. Then, asiatic acid (AA) was identified as the major active molecule of CA for CH treatment. Afterwards, network and functional enrichment analyses showed that CA exerted cardioprotective effects by modulating multiple pathways mainly involved in anti-apoptotic, antioxidant and anti-inflammatory processes. Abstract:Objective:To explore the relationship between the plasma amino-terminal pro-brain natriuretic peptide (NT-proBNP) and cardiac hemodynamics.Methods: A total of 37 patients(aged 36-84,male 32) with acute coronary syndrome(ACS) from the cardiac medical intensive care unit of Zhongshan Hospital in Shanghai were enrolled.Each patient was tested by non-invasive cardiac hemodynamic monitor,meanwhile,NT-proBNP and other blood indicators were also measured.Spearman rank correlation test was used for the correlation analysis,and the multiple stepwise regression analysis was used for the multi-factor correlation analysis of the Blood parameters and hemodynamic parameters.Results: NT-proBNP levels correlated well with echocardiographic finding such as SV,CO,SVI,CI,EF,AMP C,IC,HI,EDV,AC,SW,CW,SWI and CWI,while positively correlated with PEP/LVET and AMP O.Conclusions: There exists certain relevance between the index of cardiac hemodynamics and the level of plasma NT-proBNP in Patients with ACS.

Abstract:Coronary atherosclerotic heart disease (CHD) is defined as heart disease caused by narrowing or obstruction of the lumen of blood vessels due to coronary atherosclerosis and ischemia or necrosis of the myocardium due to functional changes in the coronary arteries. CHD patients do not present with overt clinical symptoms at an early stage, but with prolonged illness symptoms such as chest pain and shortness of breath can occur. And often worsens following activity, which may affect the patient's ability to carry out daily activities and may even lead to adverse outcomes such as myocardial infarction and heart failure. Population growth and aging are leading to a significant increase in epidemiological indicators of CHD among cardiovascular diseases worldwide, becoming one of the most important threats to public health. [ 1 ] China has an existing population of approximately 330 million people with cardiovascular disease, including approximately 11.39 million patients with CHD. The last 30 years have also seen significant changes in the spectrum of cardiovascular disease in China: the burden of rheumatic heart disease in China has been on a declining trend, although atherosclerotic cardiovascular disease has greatly increased and is likely to become the number 1 cause of death in the nation in the near future instead of stroke. [ 2 , 3 ]

Abstract:Objective To survey the influences of diabetes,hypertension, gender on cardiac structure and function in type 2 diabetic patients.Methods 160 recepts including 80 cases of type 2 diabetic patients and 80 cases of healthy control were divided into eight groups according to gender blood sugar and blood pressure. Echocardiography, multiple variance analysis and three factors analysis were performed. Results Left atrium (LA), interventricular septal thickness(IVST), left ventricular posterior wall thickness(LVPWT) were affected by diabetes, hypertension and gender, respectively and by the different combinations. No influence on ejection fnaotion(EF) and left ventricular internal diameter at end diastole(LVIDd) were shown by the three factors in this study. Conclusion Not only diabetes, hypertension and gender have effects on cardiac structure respectively, but also the diabetes and hypertension; diabetes and gender have interact between them. This can be explained by the metabolic and hormonal changes in these patients acting directly or indirectly on the remodeling of the heart.
