Abstract:Instantaneous wave-free ratio (iFR) has been proposed as a hemodynamic parameter that can reliably reflect the blood flow in stenosed coronary arteries. Currently, there are few investigations on the quantitative analysis of iFR in the patients regarding the variation of microcirculatory resistance (MR). The data aim to provide geometric (cross-section area of branches) and hemodynamic (flow rate and iFR of branches) parameters of normal and stenosed coronary arteries derived from CFD simulation. The CFD simulation was performed on the three-dimensional artery models reconstructed from computed tomography (CT) images of four subjects. The hemodynamic parameters were obtained in six situations of MR to simulate coronary microvascular dysfunction (CMD). This dataset could be used as the reference to estimate the iFR and flow rate in patients with CMD and stenosis in coronary arteries. The geometric parameters could be used in the modelling of coronary arteries.
Blood Flow Pattern in the Middle Cerebral Artery in Relation to Indices of Arterial Stiffness in the Systemic Circulation

Abstract:BACKGROUNDThe brain is perfused at high-volume flow throughout systole and diastole. We explored the association of blood flow in the middle cerebral artery (MCA) with the pulsatile components of blood pressure in the systemic circulation and indices of arterial stiffness.METHODSWe enrolled 334 untreated subjects (mean age, 50.9 years; 45.4% women) who had been referred for ambulatory blood pressure monitoring to Ruijin Hospital, Shanghai, China. We measured the MCA pulsatility index (PI) by transcranial Doppler ultrasonography. The indices of arterial stiffness included pulse pressure (brachial (bPP) and central (cPP) measured at the office and 24-h ambulatory (24-h PP)) and carotid-femoral (cf-PWV) and brachial-ankle (ba-PWV) pulse wave velocity. Effect sizes, expressed per 1 s.d., were adjusted for sex, age, heart rate, and mean pressure.RESULTSWomen had faster MCA blood flow than men (68.0 vs. 58.3 cm/s), but lower PI (75.4 vs. 82.3%; P < 0.001). The five arterial stiffness indices were intercorrelated (r >= 0.37;P < 0.001). PI increased (P <= 0.045) with bPP (+6.78%), cPP (+5.56%), 24-h PP (+7.58%), cf-PWV (+1.59%), and ba-PWV (+3.46%). In explaining PI variance, bPP ranked first (partial r(2) = 0.25), 24-h PP second (0.20) and cPP third (0.14). In models including both cf-PWV and ba-PWV, only the latter was significant (-0.19%; P = 0.84 vs. +3.54%; P < 0.001). In models including both bPP and ba-PWV, only the former contributed to PI variance (+6.98%; P < 0.001 vs. -0.24%; P = 0.78).CONCLUSIONMCA blood flow is closely associated with the pulsatile pressure in the systemic circulation, which depends on arterial stiffness as measured by PWV.
A Novel Physics-Based Model for Fast Computation of Blood Flow in Coronary Arteries

Abstract:Blood flow and pressure calculated using the currently available methods have shown the potential to predict the progression of pathology, guide treatment strategies and help with postoperative recovery. However, the conspicuous disadvantage of these methods might be the time-consuming nature due to the simulation of virtual interventional treatment. The purpose of this study is to propose a fast novel physics-based model, called FAST, for the prediction of blood flow and pressure. More specifically, blood flow in a vessel is discretized into a number of micro-flow elements along the centerline of the artery, so that when using the equation of viscous fluid motion, the complex blood flow in the artery is simplified into a one-dimensional (1D) steady-state flow. We demonstrate that this method can compute the fractional flow reserve (FFR) derived from coronary computed tomography angiography (CCTA). 345 patients with 402 lesions are used to evaluate the feasibility of the FAST simulation through a comparison with three-dimensional (3D) computational fluid dynamics (CFD) simulation. Invasive FFR is also introduced to validate the diagnostic performance of the FAST method as a reference standard. The performance of the FAST method is comparable with the 3D CFD method. Compared with invasive FFR, the accuracy, sensitivity and specificity of FAST is 88.6%, 83.2% and 91.3%, respectively. The AUC of FFR FAST is 0.906. This demonstrates that the FAST algorithm and 3D CFD method show high consistency in predicting steady-state blood flow and pressure. Meanwhile, the FAST method also shows the potential in detecting lesion-specific ischemia.
The relationship between level of blood pressure and carotid blood velocity

Abstract:Objective To probe into relationship between blood pressure level,type of hypertension and blood flow velocity of carotid artery.Methods 1084 participants aged 55 to 65 years and without history of stroke,heart disease and diabetes were selected from a stroke cohort.The participants were grouped according to systolic and diastolic blood pressure levels and types of hypertension,i.e.,the participants with various systolic pressures were divided into nine groups with 10 mm Hg difference.They were divided into eight groups according to 5 mmHg difference of diastolic pressure,and were divided into four groups according to types of hypertension.The blood flow velocity of carotid artery was compared between groups.Results The parameters,such as Vmax,Vmin,Vmean decreased gradually as systolic blood pressure ≥120 mm Hg or diastolic blood pressure ≥80 mm Hg.When systolic blood pressure was ≥160 mm Hg or diastolic blood pressure was ≥100 mm Hg,the extent of decrease in the parameters was obvious.The parameters showing differences among different types of hypertension were Vmin of left carotid and Vmax of right carotid.Conclusions In view of carotid blood flow,the ideal blood pressure is <120/80 mm Hg.The velocity of carotid blood flow decreases significantly when the blood pressure is ≥160/100 mm Hg.
Simulating Cardiovascular Haemodynamics By Bond Graph Technique

Abstract:A bond graph model for simulating cardiovascular haemodynamics is proposed. The model is given by an elastance variable description of heart, head and neck circulation, pulmonary circulation, coronary circulation, abdomen circulation and extremity circulation. Also, the carotid baroreflex control mechanism for circulation is embeded. The model responds to sympathetic and vagal activity by modifying systemic peripheral resistance, heart rate and ventricular end-systolic elastance. It is able to simulate the blood pressure and blood flow wave on arbitrary segment of circulation system, subsequently, the cardiovascular parameters such as stroke volume, cardiac output and eject fraction of heart can be computed. The simulation results under physiological condition are in agreement with human physiological experimental data. By modifying the arterial compliance and balance point of barorecpter, the hypertension is simulated and the results are consistent with the pathological features. In conclusion, the model could simulate the performance the cardiovascular system validly.

Abstract:Corresponding to human anatomical structure, a bond graph model for cardiovascular circulation system including heart, head and neck circulation, pulmonary circulation, coronary circulation, abdomen circulation and extremity circulation was developed, which comprise of 54 mathematical equations. The blood pressure and blood flow wave on arbitrary site of circulation can be obtained by solving the equations, meanwhile, the stroke volume, cardiac output and eject fraction of heart can be computed. The model achieved the typical physiological parameters such as, systolic/diastolic blood pressure 115/78 mm-Hg, stroke volume 80 mL, cardiac output 5.76 L and eject fraction 64.5%. The simulation results were in agreement with the human physiological experimental data, so that the validity of the model was verified. By adjusting the initial condition parameters of the model, the simulations for pathological features of the cardiovascular system, such as atherosclerosis, hypertension and stenosis in left subclavicle artery were implemented, with the results being consistent with human pathological features. Furthermore, the components of the model are composed of identical but independent structures, so the model is flexible to be extended.

Abstract:Continuous monitoring of blood pressure, an essential measure of health status, typically requires complex, costly, and invasive techniques that can expose patients to risks of complications. Continuous, cuffless, and noninvasive blood pressure monitoring methods that correlate measured pulse wave velocity (PWV) to the blood pressure via the Moens-Korteweg (MK) and Hughes Equations, offer promising alternatives. The MK Equation, however, involves two assumptions that do not hold for human arteries, and the Hughes Equation is empirical, without any theoretical basis. The results presented here establish a relation between the blood pressure P and PWV that does not rely on the Hughes Equation nor on the assumptions used in the MK Equation. This relation degenerates to the MK Equation under extremely low blood pressures, and it accurately captures the results of in vitro experiments using artificial blood vessels at comparatively high pressures. For human arteries, which are well characterized by the Fung hyperelastic model, a simple formula between P and PWV is established within the range of human blood pressures. This formula is validated by literature data as well as by experiments on human subjects, with applicability in the determination of blood pressure from PWV in continuous, cuffless, and noninvasive blood pressure monitoring systems.

Abstract:Objective: To study blood flow in cardiovascular system, laying a basic foundation for the development of non-invasive continuous blood pressure measuring system, a low-stage model of cardiovascular system has been developed in the article. Methods: Regard the whole cardiovascular system as one chamber based on Windkessel theory, and construct a one-stage Simulink model, while parameters of the model for people under different conditions are estimated by practical measurement, such as Doppler and Korotkoff's Sound Measure. Results: Different blood pressures are taken when varied parameters, which are in accordance with physiological facts. Conclusion: The structure of the model is useful, and it can mimic blood flow in cardiovascular system, and simulate this system under different physiological conditions, further researching can be taken according to it.
Analysis of periodic pulsating blood flow of fractional Maxwell power-law fluid in carotid artery with elastic vessel wall

Abstract:Hemodynamic analysis reveals a highly significant effect on the prevention, diagnosis, and treatment of human vascular diseases. This article goes deeply into the periodic pulsatile blood flow in the carotid artery with an elastic vessel wall. In view of blood rheological experimental data, the constitutive equation of fractional Maxwell power-law fluid with yield stress, which can describe the four characteristics of yield stress, viscoelasticity, shear thinning, and thixotropy is established. Meanwhile, drawing support from the data of pulsatile flow, the finite Fourier series of pressure gradient with a period of 1 s has been proposed. Leading into Hooke's law can build the fluid-structure coupling boundary condition of blood flow and elastic vessel wall. The numerical solutions are got hold of finite difference method integrated with the newly developed L1-algorithm, and their convergence and stability of which are verified. The axial velocities of blood under different constitutive relationships are compared. The results throw light that other constitutive relationships underestimate the velocity of blood. Furthermore, the flow rate and wall shear stress on different fluid are calculated. It can be concluded that compared with Bingham fluid, the maximum and minimum flow rate/wall shear stress of fractional Maxwell power-law fluid with yield stress increases by 19% and 32%, respectively. The flow rate lags behind the pressure gradient and has time delay effect, on the contrary, the velocity of blood vessel wall is keeping pace with the pressure gradient. The effects of relevant physical parameters on velocity are discussed. In addition, the spatiotemporal distribution of blood flow in cerebral artery and femoral artery are analyzed.

Abstract:Objective. The cerebral vasculature is formed of an intricate network of blood vessels over many different length scales. Changes in their structure and connection are implicated in multiple cerebrovascular and neurological disorders. In this study, we present a novel approach to the quantitative analysis of the cerebral macrovasculature using computational and mathematical tools in a large dataset. Approach. We analysed a publicly available vessel dataset from a cohort of 56 (32/24 F/M) healthy subjects. This dataset includes digital reconstructions of human brain macrovasculatures. We then propose a new mathematical model to compute blood flow dynamics and pressure distributions within these 56-representative cerebral macrovasculatures and quantify the results across this cohort. Main results. Statistical analysis showed that the steady state level of cerebrovascular resistance (CVR) gradually increases with age in both men and women. These age-related changes in CVR are in good agreement with previously reported values. All subjects were found to have only small phase angles (< 6°) between blood pressure and blood flow at the cardiac frequency. Significance. These results showed that the dynamic component of blood flow adds very little phase shift at the cardiac frequency, which implies that the cerebral macrocirculation can be regarded as close to steady state in its behaviour, at least in healthy populations, irrespective of age or sex. This implies that the phase shift observed in measurements of blood flow in cerebral vessels is caused by behaviour further down the vascular bed. This behaviour is important for future statistical models of the dynamic maintenance of oxygen and nutrient supply to the brain.

Abstract:The study of blood flow in curved arteries aims to understand how hemodynamic behavior is affected by geometric factors, which is crucial for revealing the formation and progression of vascular diseases. Currently, most studies on blood flow in curved vessels have utilized computational fluid dynamics software. Numerical calculations have typically concentrated on the steady flow of Newtonian fluid in curved vessels with circular cross sections, while non-Newtonian fluid flow has predominantly been calculated in curved pipes with rectangular cross sections. In this paper, the governing equation of Carreau fluid flow in a curved cerebral artery with constant curvature is established using a curvilinear coordinate system, with the flow driven by a physiological pulsatile pressure gradient. Deriving support from finite difference method, numerical solutions are acquired, whose convergence and validity are verified. It is concluded that time-averaged wall shear stress peaks at the inner side of the artery, resulting in the risk of atherosclerosis, while relative residence time peaks at the outer side, causing the likelihood of cerebral infarction there. While an increase in δ amplifies the amplitude of both, it has barely any effect on their values at θ = π/2 and θ = 3π/2. Time-averaged Dean number is first defined to evaluate the development of secondary flow in curved arteries over a cardiac cycle. Near the wall, it peaks at the inner side of the vessel and escalates markedly with larger δ. This study can provide an effective reference for the early prevention and diagnosis of cerebral artery infarction.

Abstract:We investigate the relationship between the blood flow velocities (BFV) in the middle cerebral arteries and beat-to-beat blood pressure (BP) recorded from a finger in healthy and post-stroke subjects during the quasisteady state after perturbation for four different physiologic conditions: supine rest, head-up tilt, hyperventilation, and CO2 rebreathing in upright position. To evaluate whether instantaneous BP changes in the steady state are coupled with instantaneous changes in the BFV, we compare dynamical patterns in the instantaneous phases of these signals, obtained from the Hilbert transform, as a function of time. We find that in post-stroke subjects the instantaneous phase increments of BP and BFV exhibit well-pronounced patterns that remain stable in time for all four physiologic conditions, while in healthy subjects these patterns are different, less pronounced, and more variable. We propose an approach based on the cross-correlation of the instantaneous phase increments to quantify the coupling between BP and BFV signals. We find that the maximum correlation strength is different for the two groups and for the different conditions. For healthy subjects the amplitude of the cross-correlation between the instantaneous phase increments of BP and BFV is small and attenuates within 3-5 heartbeats. In contrast, for post-stroke subjects, this amplitude is significantly larger and cross-correlations persist up to 20 heartbeats. Further, we show that the instantaneous phase increments of BP and BFV are cross-correlated even within a single heartbeat cycle. We compare the results of our approach with three complementary methods: direct BP-BFV cross-correlation, transfer function analysis, and phase synchronization analysis. Our findings provide insight into the mechanism of cerebral vascular control in healthy subjects, suggesting that this control mechanism may involve rapid adjustments (within a heartbeat) of the cerebral vessels, so that BFV remains steady in response to changes in peripheral BP.

A Hemodynamic Model for Simulation of Cerebral Blood Flow and Ventricular Power Changes in Stroke

Abstract:The aim of this work is to analyze the effect of stroke on the ventricular work and the correlation with hypertension and to provide the hemodynamic evidences for the precaution of stroke and cardiovascular diseases. A dynamic model for heart and cerebrovascular coupling was proposed based on asymmetric T-tube model, and its arterial hemodynamic behaviors were computed and analyzed. Changes of the pressure, cerebral blood flow and ventricular power were simulated by altering the level of cerebral arterial resistance and compliance. Results showed that cerebral peripheral resistance in patients with stroke was significantly higher than that in the normal. The patients whose cerebral peripheral resistance was more than twice of normal value was 83% of the total. Blood pressure in stroke, especially the systolic pressure and the pulse pressure, was significantly higher than that of the normal. The systolic pressure was higher than 140 mmHg according to the simulated results. Compensatory flow and autoregulation existed in cerebral circulation. Ventricular power as well as the proportion of the pulsatile to total power increased with the increment higher than 10%. The cerebral pathological change in stroke was closely associated with hypertension and affected the ventricular work. Thereofore in the treatment of stroke, heart protection should also be strengthened.
[Flow Simulation of Normal Pulmonary Artery Branches Based on CT Image].

Abstract:OBJECTIVE:To study the hemodynamics of the pulmonary artery (PA) in pulmonary artery hypertension (PAH).METHODS:With combined clinical hemodynamic measurements and CT data, the 3D model of PA and its branches was reconstructed to obtain the real 3D geometric structure of PA and its branches. Computational fluid dynamics (CFD) simulation was carried out for the reconstructed 3D geometric model of the PA and its branches with Womersley velocity as main pulmonary entrance conditions; then hemodynamic parameters of the PA and its branches in cardiac cycle were calculated and the changes of flow field, pressure, wall shear stress (WSS) at different times, and its impact on physiological processes were analyzed.RESULT:Simulation showed that the pressure of the main PA and the branches of the left and right PA was higher in the systolic period than that in the diastolic period; especially, the that of right PA and its interlobular artery was significantly higher than that of the left. The blood flow velocity of the main PA and its branches of right and left PA was significant different, the velocity was much higher at the distal of the right PA than that at the left. High WSS field formed at the proximal of the right PA before the branches during the systolic.CONCLUSION:The right PA at the proximal and lower lobe artery are affected by much larger pressure and change firstly, so the change of morphological and functional of theses segments will be have more important value to prompt existence of the early PAH.

Abstract:Background and purpose: The objective of this study is to determine if and how cerebral blood flow velocity and stroke risk factors are related in a stroke free population.Methods: This cross-sectional study recruited 1323 stroke-free subjects, aged 42-73 years, from a county region in China. Systolic blood flow velocities in cerebral arteries were detected with trans-cranial Doppler. Key factors, such as anthropometry, life-style, medication use, blood pressure and blood biochemical analysis were investigated at the same time.Results: In women, even controlled for major cardiovascular risk factors, cerebral blood flow velocities of common carotid artery, internal carotid artery, middle cerebral artery, anterior cerebral artery and posterior cerebral arteries decreased significantly with age (from 43.8 to 39.3, 48.0 to 42.6, 97.7 to 82.8, 79.2 to 69.4 and 44.1 to 39.7 cm/s in the range of 42-73 years old, p < 0.01 for each). In men, however, this inverse trend only existed in common carotid artery (from 45.6 to 41.7 cm/s, p < 0.01). Multivariate regression showed that most established cardiovascular risk factors (e.g. hypertension, hypercholesterolemia, diabetes mellitus and overweight/obesity) were associated with decreased blood flow velocity in the extra-cranial arteries. However, this association was not present (e.g. diabetes and overweight/obesity) or was inversed (e.g. hypertension and hypercholesterolemia) in the intra-cranial arteries.Conclusion: Extra-cranial blood flow velocity is significantly and inversely related to age and cardiovascular risk factors. The relationship between intra-cranial blood flow and risk factors changes and may depend on gender. These finding may have importance in the determination of mechanisms of stroke. Abstract:The aim of this paper is to study the relationship between change of cerebral arterial mechanics parameters and blood pressure for stroke warning and diagnosis. Heart and cerebral circulation is regarded as a global system. Equivalent circuit model of upper (head) and lower (trunk) circulation are in parallel together. Considering least cardiac work of heart, a dynamic model for heart and cerebrovascular coupling is proposed. Aortic impedance, blood pressure and velocity waveform are simulated by this model. Further, a common cerebrovascular disease is simulated. The result proves that heart and cerebral circulation are interrelated. When cerebral vascular resistance and elasticity change, the blood pressure also changes. People with hypertension have a high risk of stroke.

Abstract:The purpose of this paper is to provide a novel reference for the early diagnosis and treatment of atherosclerosis. Two-dimensional governing equations of fractional-order Maxwell fluid flow in a local stenotic artery are established, taking real physiological pulsating blood flow at inlet into consideration. Drawing support from the finite difference method as well as the L1 formula, vorticity and stream functions are introduced to acquire numerical solutions for velocity, stream function, and pressure. The distribution of blood flow in narrowed arteries within a real physiological pulse cycle is discussed. Furthermore, the influences of the degree of stenosis δ, the stenosis length parameter L0, fractional order parameter α, and relaxation time λ on crucial medical indicators, including the time average of the wall shear stress, oscillatory shear index, relative residence time, and pressure distribution are revealed. The results show that the deceleration and reversal phases of real physiological pulsatile flow critically affect the progression of arterial stenosis, and increasing the fractional order parameter α weakens the development of stenosis, while increasing λ has the opposite effect. This study is expected to serve as a reference for formulating standards of key medical indicators in the early diagnosis of vascular stenosis.

Decreased CSF Dynamics in Treatment-Naive Patients with Essential Hypertension: A Study with Phase-Contrast Cine MR Imaging

Abstract:BACKGROUND AND PURPOSE: Arterial sclerosis resulting from hypertension slows CSF transportation in the perivascular spaces, showing the intrinsic relationship between the CSF and the blood vasculature. However, the exact effect of hypertension on human CSF flow dynamics remains unclear. The present study aimed to evaluate CSF flow dynamics in treatment-naive patients with essential hypertension using phase-contrast cine MR imaging. MATERIALS AND METHODS: The study included 60 never-treated patients with essential hypertension and 60 subjects without symptomatic atherosclerosis. CSF flow parameters, such as forward flow volume, forward peak velocity, reverse flow volume, reverse peak velocity, average flow, and net flow volume, were measured with phase-contrast cine MR imaging. Differences between the 2 groups were assessed to determine the independent determinants of these CSF flow parameters. RESULTS: Forward flow volume, forward peak velocity, reverse flow volume, reverse peak velocity, and average flow in the patients with hypertension significantly decreased (all, P?<?.05). Increasing systolic blood pressure was significantly associated with lower forward flow volume (? = ?0.44?mL/mL/mm Hg; 95% CI, ?0.83 to ?0.06?mL/mL/mm Hg), forward peak velocity (? = ?0.50?cm/s/mm Hg; 95% CI, ?0.88 to ?0.12?cm/s/mm Hg), reverse flow volume (? = ?0.61?mL/mL/mm Hg; 95% CI, ?0.97 to ?0.26?mL/mL/mm Hg), reverse peak velocity (? = ?0.55?cm/s/mm Hg; 95% CI, ?0.91?0.18?cm/s/mm Hg), and average flow (? = ?0.50?mL/min/mm Hg; 95% CI, ?0.93 to ?0.08?mL/min/mm Hg). CONCLUSIONS: The CSF flow dynamics in patients with hypertension are decreased, and increasing systolic blood pressure is strongly associated with lower CSF flow dynamics.
