Getting to the Root of Dental Pain—a Hard Palate Ulcer

Abstract:A 69-year-old female with type 2 diabetes (A1C 12.4%), hypertension, and hyperlipidemia presented to her dentist with a 2-day history of left-lower jaw pain (worse when chewing) and swelling. Her dentist identified pulpitis and periodontitis under the crown of her first molar (tooth number 19) and prescribed a 3-day course of methylprednisolone with clindamycin to reduce jaw swelling prior to an elective dental procedure. Three days later, she underwent an uneventful root canal treatment and crown replacement of tooth number 19. After the procedure, she noted worsening swelling and new development of facial numbness over the left masseter with resolution of the oral pain when chewing foods. By post-operative day 4, the swelling and pain progressed and radiated to the left temple and periauricular region, eventually culminating in left sided ptosis and trismus 7 days after her dental procedure. She presented to urgent care and was prescribed high-dose corticosteroids as her syndrome was suspected to be from giant cell arteritis and intraoral local anesthetics usage. A 2-day trial of steroids was ineffective—and she presented to the emergency department 10 days after her dental procedure with persistent left facial numbness, progressive left ptosis and visual loss, and hyperalgesia to her left maxilla and temple. There was no headache, nasal obstruction, purulent nasal discharge, or history of fever. On examination, she was afebrile with normal vital signs. Neurological examination was significant for left cranial nerve (CN) III, IV, and VI palsies. She had preserved visual acuity in her right eye and hand motion in the left eye. She was tender to palpation of the left jaw, temporal area, and sinuses. Oral examination revealed a painless and non-purulent hard palate ulcer (Figure 1).
