Application value of APACHE Ⅱ scoring system and serum bilirubin level for patients after cardiac surgery

Abstract:Objective To explore the relationship between acute physiologic and chronic health evaluation Ⅱ (APACHE Ⅱ)score and level of serum bilirubin and prognosis of patients after cardiac surgery.Methods We retrospectively analyzed the clinical data of 196 adult patients who underwent cardiac surgery in Xinhua Hospital in 2016.All the patients were score by the worst index level in the APACHE Ⅱ score system within 24 hours after operation.The patients were divided into 4 groups:< 15 score,15-24 score,25-34 score,>35 score.The values of serum total bilirubin were collected from preoperative to the fifth day after operation.Hyperbilirubinemia is defined as the value of serum total bilirubin more than ≥34.2 μmol/L.The patients were divided into 2 groups based on their prognosis:survival group and death group.According to the level of serum total bilirubin,all the patients were divided into normal bilirubin group and hyperbilirubinemia group.The relationships between APACHE Ⅱ score and level of serum total bilirubin and prognosis were analyzed.Results The level of serum total bilirubin increased as the APACHE Ⅱ scores increased.The level of serum total bilirubin in death group (105.1 ± 48.8)μmol/L was significantly higher than the survival group (30.9±9.1)μmol/L (P<0.05).The APACHE Ⅱ score in death group was (27.8±10.0)scores,which was higher than (9.8±4.6)scores in the survival group (P< 0.05).The level of serum total bilirubin was positively related to APACHE Ⅱ score (r=0.712,P<0.05).Conclusion The level of serum total bilirubin was positively related to APACHE Ⅱ score,and can be used to evaluate the severity and prognosis of patients after cardiac surgery.
