Decortication of renal cyst by percutaneous approach: safety and effects

Abstract:Objective:To introduce an approach to decortication of renal cyst of kidney stone patients in one session operation of PCNL and evaluate its efficacy and morbidity.Method:Nineteen patients with both renal calculi and simple symptomatic renal cyst were enrolled to be decorticated by PCN approach.Stone burden was 15-32 mm in length,and the diameter of cyst was 42-95 mm.PCN procedure was guided by ultrasound,and the cystic wall was circumferentially excised from inside by monopolar electric hook or 2μm laser under epidural or combined spinal-epidural anesthesia.All patients were followed up at least 12 months including ultrasound and CT scan.Result:No mortality occurred and no open surgery was needed in all patients.Mean surgery time was (28.4±20.9)min,stone free rate was 94％,the area of cyst wall excised was (1 504.5±778.9) mm2.All pathological results confirmed the diagnosis of simple renal cyst.No severe complications with further intervention occurred except that one case had renal hematoma who accepted transfusion.No recurrence was observed by CT scan at 12th months follow-up.Conclusion:Decortication by PCN is a new reliable approach for renal cyst in one-session operation for patients burdened both kidney calculi and simple cyst.
