Abstract:Objective To evaluate renal function changes and risk factors for acute kidney injury (AKI) after percutaneous nephrolithotomy (PCNL) in patients with renal calculi with a solitary kidney (SK) or normal bilateral kidneys (BKs). Patients and Methods Between 2012 and 2016, 859 patients undergoing PCNL were retrospectively reviewed at Changhai Hospital. In all, 53 patients with a SK were paired with 53 patients with normal BKs via a propensity score-matched analysis. Data for the following variables were collected: age, sex, body mass index, stone size, distribution, operation time, perioperative outcomes, and complications. The complications were graded according to the modified Clavien-Dindo system. Univariable and multivariable logistic regression models were constructed to evaluate risk factors for predicting AKI. Results The SK and BKs groups were comparable in terms of age, sex ratio, stone size, stone location distribution, comorbidities, and American Society of Anesthesiologists Physical Status classification. The initial and final stone-free rates were comparable between the SK and BKs groups (initial: 52.83% vs 58.49%, P = 0.696; final: 84.91% vs 92.45%, P = 0.359). There was no difference between the two groups for complications, according to the Clavien-Dindo grades. The estimated glomerular filtration rate (eGFR) increased dramatically after the stone burden was immediately relieved, and during the 6-month follow-up eGFR was lower in the SK group compared with the BKs group. We found a modest improvement in renal function immediately after PCNL in the BKs group, and renal function gain was delayed in the SK group. Through logistic regression analysis, we discovered that a SK, preoperative creatinine and diabetes were independent risk factors for predicting AKI after PCNL. Conclusion Considering the overall complication rates, PCNL is generally a safe procedure for treating renal calculi amongst patients with a SK or normal BKs. Follow-up renal function analysis showed a modest improvement in patients of both groups. Compared to patients with normal BKs, patients with a SK were more likely to develop AKI after PCNL.
The treatment of upper ureteral calculi with minimally invasive percutaneous nephrolithotomy in lateral decubitus position

Abstract:Objective To explore the clinical value of minimally invasive percutaneous nephrolithotomy(mPCNL) in lateral decubitus position in the management of upper ureteral calculi.Methods 25 patients were analyzed retrospectively,who suffered from upper ureteral calculi and were treated with minimally invasive percutaneous nephrolithotomy in lateral decubitus position between January 2007 and December 2007.These patients included 18 men and 7 women,with an average age of 38-years-old(23 to 65 years).All patients' ureteral calculi were unilateral and 6 cases were combined with kidney calculi. The diameter measured by KUB ranged from 1.0 cm to 2.7 cm.Results The mean operative duration was 45 min(ranging from 25 to 90 min) and the average hospital stay lasting for 8 days(ranging from 5 to 12 days).22 cases were treated with only one percutaneous access tract in a single session and 3 cases were only placed stoma tube and underwent a second-look procedure because of pyonephrosis.The average preoperative hemoglobin was(114±25)g/L.On the first to third postoperative day,hemoglobin was reexamined,with an average of(112.5±23.5)g/L.2 cases had high fever postoperatively.The stone-free rate was 100%.Excessive hemorrhage and other severe complications did not occur.Conclusions The lateral decubitus position,the most familiar to urologists,is also an ideal position for mPCNl in the management of upper ureteral calculi.It adds ease and comfort to the patient,anesthesiologist,and surgeon.
Percutaneous Nephrolithotomy Combined with Pneumatic and Ultrasonic Lithotripsy for Complex Renal Calculi:Report of 150 Cases

Abstract:Objective To evaluate the clinical efficacy of percutaneous nephrolithotomy (PNL) in combination with pneumatic and ultrasonic lithotripsy in the treatment of complex renal calculi. Methods Under general anesthesia,an ureteral stent was placed into the patient by cystoscopy. With the prone position,percutaneous access was established by inserting an access needle into the intended calix under the guidance of ultrasonography or fluoroscopy. Afterwards,combined pneumatic and ultrasonic probes were used by nephroscopy for lithotripsy. Results A single tract was used in 137 patients,while two or three tracts were created in the other patients (11 and 2 respectively). One-step procedure to remove the stones was achieved in 119 patients,while a second intervention was performed in 27,and three procedures was carried out in 4. The mean time of the operations were (88±34) min. After the operation,one patient developed infectious shock,and then was cured by anti-shock therapy;two patients showed secondary hemorrhage in two weeks postoperation,and was cured by conservative therapy. In this series,the rate of stone clearance was 84.7% (127/150);23 cases had residual stones,10 of them received ESWL (the stone was removed completely in 7 of the 10 cases),and the other 13 were treated by oral medicines. Six months after the operation,follow-up examination showed a stone-free rate of 89.3% (134/150). Conclusion Combination of ultrasonic and pneumatic intracorporeal lithotripsy is effective and safe for complex renal calculi,which is intractable by open surgery.

Abstract:The availability of testosterone and estradiol to Sertoli and prostate cells is dependent upon 1) the permeability properties of the blood-tubular barrier (BTB) of the testis or prostate cell membrane, and 2) sex steroid binding to plasma proteins, such as albumin or testosterone-binding globulin (TeBG). Sex steroid influx into these tissues was studied after in vivo arterial bolus injections of [3H]testosterone or [3H]estradiol in anesthetized rats. Both testosterone and estradiol were readily cleared across the BTB or prostate cell membrane in the absence of plasma proteins and in the presence of human pregnancy serum, in which testosterone or estradiol are 80-95% distributed to TeBG. The extravascular extraction of [3H]TeBG across the BTB or prostate plasma membrane [73 +/- 2% (+/- SE) and 92 +/- 9%, respectively] was significantly greater than extraction of [3H]albumin or other plasma space markers and indicative of a rapid first pass clearance of TeBG by Sertoli or prostate cells. In summary, these studies indicate that 1) testosterone and estradiol are readily cleared by Sertoli and prostate cells; 2) albumin- and TeBG-bound sex steroids represent the major circulating pool of bioavailable hormone for testis or prostate; and 3) the TeBG-sex steroid complex may be nearly completely available for influx through the BTB or prostate plasma membrane.
Clinical Value of Lateral Decubitus Position for Minimally Invasive Percutaneous Nephrolithotomy

Abstract:Objective:To explore the clinical value of lateral decubitus position for minimally invasive percutaneous nephrolithotomy(mPCNL).Methods:64 renal units(56 patients) with renal calculi treated with mPCNL in lateral decubitus position from Jan 2007 to Dec 2007 were retrospectively reviewed.Among the 56 patients(64 renal units)reviewed,48 cases suffered from unilateral renal calculi,8 cases bilateral.38 cases'(45 renal units') calculi were single and 18 cases'(19 units') were staghorn.The stone measured by KUB ranged 2.4 to 6.7 cm.Results:Mean operative time was 85 min(ranged 45-180 min) and mean time of hospital stay was 12 days(8-21 days).Among 45 units with single calculus,38 units were treated with only one percutaneous access tract in a single session and 7 units underwent a second-look procedure because of pyonephrosis.All 19 units with staghorn calculi were treated with 2 tracts.Among them,8 units were treated in a single session and 11 required a secondary-look procedure.The mean preoperative hemoglobin was 112±28 g/L.On the 1-3 postoperative day,hemoglobin was reexamined,with a mean of 102.5±31.5 g/L.Postoperatively,ESWL was used to treat residual stones in 17 cases.The total calculi clearance rate was 92.3%.Conclusions:The lateral decubitus position,the most familiar to urologists,is also an ideal position for mPCNl.With the wide use of the puncture guided By B ultrasonics,the lateral decubitus positon would be acceped widely.
Percutaneous Nephrolithotomy in Semisupine Position: a Modified Approach for Renal Calculus

Abstract:Conventional percutaneous nephrolithotomy (PCNL) is usually performed in a prone position, which compresses the thorax and results in difficulty in rescue during operation. When PCNL is performed in a supine position, the flank renal puncture area is limited, so it is difficult to treat disseminated and complex renal calculi. Herein, we introduce a modified semisupine position for performing PCNL, which has numerous benefits as well as safe and effective. Between May 2002 and May 2009, a total of 452 patients with renal calculi were treated with semisupine PCNL. The patient was placed in 45° semisupine position during the procedure, with the affected flank arched as much as possible. In this series, no one converted to open surgery. The average operating time was (115.2 ± 44.5) min. Single tract PCNL was performed for 80.97% of the cases, two tracts 13.94%, three tracts 4.65%, and four tracts 0.44%. The upper, middle, and lower calix tracts accounted for 12.1, 63.0, and 24.9%, of procedures, respectively. Stone-free rate was 85.7% overall, 92.2% for single calculus (83/90), and 72.9% for staghorn calculi (78/107). Major postoperative complications occurred in 3.3% of the cases. This study demonstrated PCNL in a semisupine position is an effective alternative for treating renal calculi, which combines the advantages of PCNL in a prone position, and PCNL in a supine position. The semisupine position allows easier irrigation of stone fragments, is more comfortable for the patient, and facilitates monitoring of anesthesia.
Complication and safety of ultrasound guided percutaneous nephrolithotomy in 8,025 cases in China

Abstract:Background: Percutaneous nephrolithotomy (PCNL) was mostly performed with fluoroscopy and/or ultrasonography. The safety and feasibility of PCNL performed totally under ultrasound are not clearly defined. Therefore, we introduce the 9-year experience of 8 025 ultrasound guided PCNL procedures from multiple centers in China performed by the same surgeon, to evaluate the feasibility and security of this technique. Methods: From September 2004 to August 2013, 8 025 cases, 4 398 males (54.8%) and 3 627 females (45.2%), whose age ranged from 6 months to 85 years old, with upper urinary tract stones, underwent PCNL in our center and the supported hospitals. Puncture site selection and channel dilation were all guided using only Doppler ultrasound. Single stones were treated in 1 356 cases, there were 2 817 cases of multi stones, and 3 852 cases of staghorn calculi. The pre- and post-operative imaging data, the intraoperative findings, operation time, perioperative complications, and related parameters were recorded. Results: All procedures were successful. No patients died during the operation. Average operation time was 42 minutes (range 10 to 168 minutes), 4 cases converted to open surgery, and 2 patients lost the diseased kidney due to refractory bleeding in the early stage of the PCNL. Ninety-four (1.2%) patients received blood transfusions and 20 (0.25%) patients needed highly selective renal artery embolization. Fifteen (0.19%) patients had a pleural injury. 5 457 (68%) cases were completed by a single tract and 2 568 (32%) cases added more tracts. The mean stone size (longest diameter) was 2.8 cm (range 1.2 to 26.5 cm). The final stone-free rate was 85.5%. Residual stones occurred mainly in patients with renal dysfunction, medullary sponge kidney, and complete staghorn calculi with a slim calyceal neck. Conclusions: X-ray free Doppler ultrasound guided percutaneous nephrolithotomy is feasible and safe in a variety of cases of renal and/or upper ureteral stones. The probability of radiation hazard and adjacent organ injury is low. The morbidity from major complications was reduced remarkably after special training. It is worthy of wider use compared with fluoroscopy in patients with special kidneys (e.g. solitary kidney, spinal deformity, ectopic kidney) and in infants.

Abstract:Purpose:To evaluate the feasibility and availability of sonographically percutaneous nephrolithotomy (PCNL) in treatment of renal calculi.Methods:Here reported 102 cases of renal calculi in this group. The first 50 cases underwent two stage procedure for PCNL. That is a percutaneous micronephrostomy guided by ultrasound B established and one week later the PCNL was carried out in which a guiding wire was introduced through the nephrostomic tubing followed by a series of dilation solely under the guidance of B sonography.While a nephrostomy sheath was placed properly, a 26F nephroscope or a ureteroscope was used through to manage the calculi. Results:69 cases presented stone free on the first PCNL and 33 others still had residual stone and underwent the second management. So the total stone clearence amounted to 91.0 % by the first and second operation.The average operation time was 2.5 h;6 cases had transfusion during the PCNL and averaged 230 ml; The complications were discovered of extravasation of fluid in abdominal cavity on 2 cases, renal artery vein fistula on 1 case and UPJ stricture on 1 cases.All these complications were treated properly.Conclusions:Solely sonographic guidance is available to the PCNL and it is, indeed, applicable to all urologic centers.
Clinical value of minimally invasive percutaneous nephrolithotomy in the supine position under the guidance of real-time ultrasound: report of 92 cases

Abstract:To determine the clinical value of the real-time-ultrasound-guided minimally invasive percutaneous nephrolithotomy (m-PCNL) technique in the supine position, 92 patients suffering from renal or upper ureteral stones were treated by m-PCNL with a nephroscope/ureteroscope in the supine position. The ipsilateral flanks of the patients with different body sizes were elevated with a 1,000 or 3,000-ml water bag. Under cystoscopy, a ureteral catheter was inserted into the kidney. Normal saline was infused into the kidney via the ureteral catheter to dilate the entire urinary system. Under the guidance of real-time ultrasound, the needle was inserted into the urinary system to dilate the tract and establish the 16F mini-tract for percutaneous nephrolithotomy. All 92 (100%) m-PCNL procedures were successfully performed in the supine position. Primary stone clearance was achieved in 64 cases (69.6%). Residual stones occurred in 28 cases (30.4%). M-PCNL was performed for a second time in 16 cases to clear the residual stones. In 4 cases, stones remained after the second m-PCNL. Two of them were treated further by extracorporeal shockwave lithotripsy (ESWL). The total stone clearance rate of m-PCNL was 82.6%. Only one case required blood transfusion. No other serious complications occurred. The supine position is a favorable position for the patients, the surgeons and the anesthesiologists during the m-PCNL procedure. Real-time ultrasound is a valuable technique for guiding of the m-PCNL.
Safety And Efficacy Of Percutaneous Nephrolithotomy Through Upper Pole Access

Abstract:Objective To evaluate the efficacy and safety of percutaneous nephrolithotomy （PCNL）guided by ultrasonography through upper pole access. Methods From October 2007 to Oc tuber 2009, 42 patients with upper urinary tract calculi underwent PCNL through upper pole access. Among these cases, there were 10 cases of staghorn calculi, 22 cases of renal pelvis calculi, 7 cases of the upper calyx calculi, 3 cases of the lower calyx calculi, 4 cases combined with ureter calculi and 2 cases combined with ureteropelvic junction obstruction. The stone measured from 2. 0 to 6. 5 cm （average： 3.4 cm） in length. Working tunnels （F16--F26） were established through the 10th or llth intercostals. Pneumatic or holmium laser lithotripsy was used to disintegrate and remove stones by ne phroscopy or ureteroscopy. Clinical data including operation time, complications and stone free rate were analyzed retrospectively. Results All the operations were completed in one session, single tract was used in 36 cases（85.7%）, double tracts were used in the other 6 cases（14.3%）. The stonefree rate after one session was 88.1%（37/42）, 3 cases（7.1%） received a second-session PCNL, 2 cases （4.8%）underwent ESWL after operation. The mean operative time was 65 min（30--140 min）. Postoperative surgery-related infection rate was 9. 5% （4/42）. One patient （2. 4%） required blood transfusion. Perforation of the pelvis occurred in 1 patient（2.4 %）. No pleural or important organ injury occurred. Conclusion The upper pole access for PCNL can be convenient to remove stones, this method is a highly efficient and safe technique. Percutaneons nephrolithotomy for calculi in solitary kidney: challenge and risk

Abstract:Objective To evaluate the efficacy and safety of standard percutaneous nephrolithot- omy(PCNL)in the treatment of calculi in solitary kidney. Methods One hundred and fifty-six pa- tients of renal calculi with functional or anatomic solitary kidney had undergone PCNL accessed by two steps dilation to 24 F tract with ultrasound guidance from Aug 2003 to Oct 2008.Fifty-seven cases were with staghorn calculi，83 were with multiple calculi，16 were with single stone.There were 71 cases with abnormal renal function and 11with open nephrolithotomy history.All calculi were frag- mented and extracted by pneumatic and ultrasound lithotripter. Results The percutaneous renal ac- cess was successfully established in 1 56 patients under ultrasound guidance and immediate lithotripsy was performed.The calculi of 143(91.7％)patients were cleared by single access，9(5.8％)by double accesses，1 by triple accesses in one session and 3 by multiple accesses in multiple-session pro- cedures.The mean operating time was(46.7±11.2)min，the mean first accessing time was(4.5± 0.8)min.The stone-free rate after the first operation was 74.4％.There were 2 1 kidnevs accepted another PCNL to remove the residual calculi.The stone-free rate of this group was 87.8％.Five cases (3.2％)needed transfusion，4(2.6％)underwent hyperseleetive angioembolization for bleeding con- trol.No injury of adjacent organs or urosepsis happened.The plasma creatinine decreased after opera- tion in 81.7％of patients with pre-operative abnormal renal function.and no patient needed replace- ment therapy for deteriorated renal function. Abstract:Objective To evaluate the technique of performing mini-percutaneous nephrolithotomy ( mini-PCNL) in supine position. Methods A total number of 56 patients who underwent PCNL for renal or upper ureteral calculi were carried out by ureteroscope in supine position. The calculi were located in the left kidney of 23 cases, in the right kidney of 26 cases and in the upper ureter of 7 cases, respectively. Seventeen patients had single calculus while 26 others had multiple calculi. The mean calculus diameter was 2. 6 cm×1. 7 cm. Thirteen patients had staghorn calculi with the maximal dimension of 5. 7 cm×7. 2 cm×4. 3 cm. Severe hydronephrosis was found in 9 kidneys and mild to moderate hydronephrosis was found in 29 kidneys. The ipsilateral flank was raised with a 1000 ml saline bag as pad during the procedure. The skin was punctured and the tract was dilated under the guidance of ultrasound to set up the tract for mini-PCNL. Results The successful operation rate was 100% ,while the primary stone clearance rate was 71. 4% (40/56). Residual calculi were found in 16 cases. The additional PCNLs were performed on 9 cases,and the residual calculi were completely removed. Three cases were treated by ESWL in patients who were not willing to accept additional PCNL. The total stone clearance rate was 87. 5% (49/56). Only one case required blood transfusion during the procedure. There was no severe complication during and post-operation. Conclusions The supine position is the most comfortable position for patients accepting PCNL. The stone fragments are much easier to wash out than other positions as well. This position is convenient for anesthetist to observe the patient and to converse to general anesthesia if needed.

Abstract:Introduction: Ultrasound guidance is a unique alternative to fluoroscopy for percutaneous renal access. Besides being free of ionizing radiation exposure to the patient and intraoperative personnel, it offers several advantages including easier identification of the posterior renal calix and surrounding visceral structures. In this video, we demonstrate how ultrasound can be used to guide percutaneous nephrolithotomy (PCNL) in a step-by-step manner. Materials and Methods: From March to June 2016, 16 consecutive patients of age 18 and more with kidney or proximal ureteral stones underwent completely X-ray-free ultrasound-guided PCNL. No patients were excluded during the study period. Under general anesthesia, we place an externalized ureteral catheter through a flexible cystoscope with the patient in a frog-leg position. Retrograde saline injection is used to distend the collecting system only when needed. Then the patient is placed in a prone position. An ultrasound machine with a 3.5-MHz convex abdominal probe (Hitachi Aloka Medical America) is used to guide all steps of PCNL. For renal access, a longitudinal approach for needle insertion is usually chosen.1 An 18-gauge Echotip needle (Cook Medical) is slowly advanced through the skin either in front of or behind the probe. On the ultrasound screen, the entire needle should be fully seen from skin to kidney and into the targeted calix. Entry into the collecting system is confirmed with either aspiration of urine or efflux of urine through the puncture needle. Then, the needle stylet is removed and a J-tip coaxial guidewire (Bard Medical) is inserted into the renal pelvis or down the proximal ureter under ultrasound monitoring. Gently moving the wire back and forth will help identify the location of the wire tip relative to the collecting system. Subsequently, the needle is withdrawn, a 1-cm skin incision is made surrounding the wire, and a 10F fascial dilator and a safety wire introducer are then passed over the wire. Although the wire appears with a bright echogenic signal, the dilator and the safety wire introducer are not echogenic. Their advancement over the wire can be observed ultrasonographically as they obscure the echogenic appearance of the wire. A second wire is subsequently advanced into the collecting system through the safety wire introducer. A high-pressure balloon dilator (BARD X-Force, Bard Medical) is then advanced into the collecting system over one of the wires. Because the deflated balloon tip can be difficult to identify on the ultrasound screen, the wire should be moved back and forth while passing the balloon and the operative surgeon should look for a change in the wire contour to judge where the balloon tip is relative to the wire. The placement of the tip of this balloon dilator is crucial, as ideally, it should be just within the collecting system of the target calix.2 A working tract is then dilated and the access sheath is carefully advanced until the back end of the balloon is seen. Then the balloon is withdrawn, an offset rigid nephroscope is inserted, and the stone is treated. Results: The mean age of patients was 48.8 ± 19.9 years. Forty-four percent of patients were male with a mean body mass index of 29.9 ± 7.9 kg/m2 and a mean stone size of 33.7 ± 15.0 mm. All procedures were effectively performed with ultrasound guidance with a mean operative time of 101.3 ± 32.2 minutes. Patients and intraoperative personnel were not exposed to any ionizing radiation during the surgery. No patients experienced any significant immediate postoperative complication. All patients were stone free (no visible stone fragments) based on intraoperative visual inspection using a flexible nephroscope and a renal ultrasound and KUB at 30 days after surgery, and no secondary procedures were required. Conclusions: Ultrasound guidance for renal access and tract dilation in prone PCNL is feasible and efficient. Although some situations such as obese patients or nondilated collecting system may present a challenge, the benefits of adopting this technique-namely eliminating ionizing radiation exposure and live imaging of anatomy surrounding the collecting system-provide value during PCNL. Safety and curative effects of ultrasound-guided percutaneous nephrolithotomy for renal calculi in solitary kidneys

Abstract:Objective: To evaluate the safety and curative effects of ultrasound-guided percutaneous nephrolithotomy(PCNL) for renal calculi in solitary kidneys. Methods: The clinical data of 138 renal calculi with functional or anatomic solitary kidney undergone PCNL in our department from July 2005 to December 2012 were retrospectively analyzed. They comprised of 16 anatomical solitary kidneys and 122 functional solitary kidneys(the GFR of contralateral kidney <10 m L/min). The total renal function was abnormal in 35 cases. There were staghorn calculi in 41 cases, multiple calculi in 85 cases, and single stone in 12 cases. 11 cases had a history of open nephrolithotomy. Percutaneous nephrostomy was performed to establish F18 minimally invasive channels or F24 standard channel with the guide of color Doppler ultrasound, and all calculi were broken and extracted by EMS lithotripsy and clearance system or holmium laser. Results: The percutaneous renal accesses were successfully established in 138 cases, including single access in 125 cases(90.6%) and double accesses in 13 cases(9.4%). The mean access setting time was(4.7±0.9) min, and the mean operation time was(49.5±12.6) min. Postoperative hemoglobin averagely declined for 5.8 g/L. Due to intra- or postoperative hemorrhoea, 6 patients received blood transfusion, and superselective renal angiographic embolization was performed to control bleeding in 3 of them. Of 35 cases of renal dysfunction, serum creatinine increased for(19.8±7.4) μmol/L in 32 cases(91.4%) on postoperative day 1, and declined to normal limits in 26 cases(74.3%) on postoperative day 7. Stones were cleared in 117 cases(84.8%) only by one time of PCNL. Total stone-free rate was 91.3% on postoperative month 1, and the remaining 12 cases with residual stones underwent ESWL and watchful follow-up. 15 cases(10.9%) had postoperative fever(temperature >38.5 ℃), but there were no cases with organ injury and septic shock in this group. Conclusion: Ultrasound-guided percutaneous nephrolithotomy is safe and effective to treat renal calculi in functional or anatomical solitary kidneys.
Minimally invasive percutaneous nephrolithotomy vs standard PCNL for management of renal stones in the flank-free modified supine position: single-center experience

Abstract:To assess the safety and efficacy of minimally invasive percutaneous nephrolithotomy (mPCNL) as compared to standard PCNL (sPCNL) for management of 2–3-cm renal stones in the flank-free modified supine position. Between September 2010 and December 2013, 150 patients (168 renal units) with 2–3-cm renal stones were prospectively randomized into two treatment groups; Group A (75 patients/87 renal units) treated by mPCNL and Group B (75 patients/81 renal units) treated by sPCNL. In both groups, the patients were placed in the flank-free modified supine position. In mPCNL group, the tract was dilated up to 16.5 F whereas in sPCNL group the tract was dilated up to 30 F. Both groups were compared regarding several perioperative parameters. No significant difference was recorded among both groups regarding fluoroscopy time (4.3 ± 1.3 vs 4.8 ± 2.1 min, p = 0.06), operative time (83.2 ± 17.3 vs 78.6 ± 24.4 min, p = 0.16), hospital stay (4.3 vs 4.5 days, p = 0.76), VAS score (3.2 ± 0.6 vs 3.3 ± 0.8, p = 0.36) and need for analgesia. The mean drop in hemoglobin level and the incidence of bleeding that necessitated blood transfusion were significantly lower in the mPCNL group (0.6 ± 0.1 vs 1.9 ± 1.1 g/dl, p < 0.0001 and 1.2 vs 9.8%, p = 0.03, respectively). Although the stone-free rate was higher in the sPCNL group, but this was statistically insignificant (97.1 vs 95.4%, p = 0.86). Mini-PCNL is effective for managing renal calculi with comparable operative time and stone-free rate to standard PCNL with the merit of higher safety due to lower incidence of bleeding that necessitates blood transfusion.

Abstract:ObjectivesTo evaluate the efficacy and safety of ultrasound (US)‐guided percutaneous nephrolithotomy (PCNL) in managing upper urinary tract calculi in patients with spinal deformity based on our experience over the last decade.Patients and MethodsA retrospective study was conducted on patients with spinal deformity who were admitted to Beijing Tsinghua Changgung Hospital and collaborative hospitals from August 2004 to August 2017. Patients diagnosed with urolithiasis and who underwent US‐guided PCNL, performed by the same surgeon, were included. Patients’ data were collected and the following variables were analysed: age; sex; plain abdominal radiograph of the kidneys, ureters and bladder (KUB); computed tomography (CT); stone burden; evaluation of spinal anatomy; respiratory function; operative time; and postoperative findings. The severity of surgical complications was determined according to the modified Clavien classification system (MCCS).ResultsA total of 72 patients (41 males and 31 females) were included, with a mean (range) age of 35 (21–69) years. The mean (range) stone diameter was 38 (19–68) mm. Seven patients had bilateral PCNL, completed in staged surgery with a 1‐week interval. Five kidneys received staged PCNL because of either large residual stones or the potential risk of a long operation time. In all, 19 patients underwent the surgery in a prone position, 29 lateral, nine oblique supine, and the other 27 in unconventional positions (lateral prone, lateral supine, and lateral jackknife position). All procedures were successfully completed with no major complications during the operation. The mean (SD) tract building time was 8.5 (1.7) min. The mean (SD) stone disintegration time was 39.3 (14.6) min. The mean (range) temperature and postoperative haemoglobin (Hb) drop on the first postoperative day were 37.6 (36.7–39.2) °C and 1.5 (0.2–4.3) g/dL, respectively. Four patients needed blood transfusions and none of the patients had septic shock or severe renal bleeding requiring angiographic renal embolisation or nephrectomy. A stone‐free status was achieved in 71 kidneys (89.87%). Eight patients with residual fragments ≥4 mm were either observed without treatment during follow‐up or re‐treated with shockwave lithotripsy (SWL) according to clinical significance.ConclusionPCNL in patients with spinal deformities is challenging but safe and effective. US‐guided puncture and access establishment has its unique advantages when such patients are placed in unconventional positions.
Percutaneous Nephroscopic Lithotripsy for Bilateral Upper Urinary Calculi

Abstract:Objective To evaluate the efficacy and safety of percutaneous nephroscopic lithotripsy for the treatment of bilateral upper urinary calculi. Methods From July 2004 to December 2008,we performed percutaneous nephroscopic lithotripsy under X-ray or ultrasonic guidance for 35 patients with bilateral upper urinary calculi. Of the 35 patients,bilateral renal calculi were found in 18 patients,unilateral renal and unilateral ureteral calculus in 10,bilateral ureteral calculi in 7. Results In 35 patients (70 sides),the calculi was removed through a single tract in 49 sides,through double tracts in 18 sides,and through triple tracts in 3 sides. The mean operation time for unilateral PCNL was 66 min (ranged from 20 to 185 min). The stone-free rate was 80% (56/70) on one session. A second-look stone removal was needed in 8 laterals,and 3 laterals underwent three times of lithotomy. In three sides,a few residual renal stones were found after the first operation,but no second operation was performed. The final stone-free rate was 91.4% (64/70) in all of the cases,among which the stone-free rate was 100% (24/24) for ureteral stones and was 87% (40/46) for renal stones. In this series,no chest or abdominal injuries,intestinal perforation,or injuries to surrounding organs were detected. The patients were discharged from hospital in 5 to 30 days,and then were followed up for 3 to 6 months. During the follow-up,B-ultrasonography and KUB+IVP found no recurrence of calculi. Conclusions Percutaneous nephrolithotomy is effective and safe for bilateral upper urinary calculi. By skilled urologists in conditioned hospital,simultaneous bilateral percutaneous nephrolithotomy performed on bilateral upper urinary calculi can shorten the cycle time of treatment and hospitalization.

Abstract:ObjectiveTo report our experience and assess the safety and efficacy of ultrasound-guided percutaneous nephrolithotomy (PCNL) with ultrasonic and pneumatic lithotripters for complete renal staghorn stones.MethodsFrom Jan 2006 to Jan 2015, 128 patients (142 kidneys) with complete staghorn stones underwent PCNL, included 14 patients with bilateral renal complete stones,13 patients with solitary or founctional solitary kidney, 22 patients with renal insufficiency, 25 patients with renal empyema, 4 patients with spine malformation, ultrasound-guided PCNL were performed. Stones were fragmented and cleared using a combination of ultrasonic and pneumatic lithotripters.ResultsAll PCNL procedures were successful. Successful access to the collecting system was 100%. Mean operation time was 64.5±39. 1min (range 30–150). The primary stone-free rate of PCNL was 76.8%(109/142). Most of the renal stones(63.4%, 90/142)were managed satisfactorily by a single tract,a second tract was used in 25 renal stones(17.6%, 25/142). There were 36 auxiliary procedures (24 second PCNL and 3 third PCNL and 9 extracorporeal shock wave lithotripsy). One month after treatment, the overall stone-free rate was 85.2%(121/142). Six patients (4.2%) received blood transfusion,three patients(2.1%) received angiography and embolization . Three patients (2.1%) with infective shock were cured by intravenous antibiotics and antishock treatments. Three patients with temporary renal function receded were observed in solitary kidney for two tracts PCNL. No other severe complications occurred. The mean postoperative stay was 5.2 days (range 2–12). Conclusion: Ultrasound-guided Percutaneous Nephrolithotomy with ultrasonic and pneumatic lithotripters for management of renal complete staghorn stones is safe, effective, and worthy of wider use in clinical practice.
