Analysis of the safety and efficiency of ultrasound guided percutaneous nephrolithotomy for treatment of caliceal diverticular calculi

Abstract:Objective:To evaluate the efficiency and safety of percutaneous nephrolithotomy (PCNL) for caliceal diverticular calculi.Method:In this study,the clinical data of 35 patients with caliceal diverticular calculi who received PCNL were retrospectively reviewed from May 2009 to September 2016.There were 14 males and 21 females whose mean age was (39.9± 13.0) years old.Fourteen cases were found caliceal diverticular in the left kidney,and 21 cases in the right kidney.The mean maximum diameter of caliceal diverticular was (23.4±7.4) mm,and the mean maximum diameter of stone was (19.6±7.4) mm.Besides,eight patients had received extracorporeal shock wave lithotripsy (ESWL).Result:The procedure of PCNL was successfully performed in all 35 patinets.The mean operative time was (64.6±29.9) min,and the postoperative hospital stay was (7.2±2.4) d.The concentration of hemoglobin decreased in 30 patients and the mean hemoglobin drop was (11.5 ± 8.0) g/L.The major postoperative complications were occurred in two (5.7 %) patients including fever in one patient and blood transfusion in another patient.None of patients occurred hemopneumothorax,sepsis or adjacent organ damage.The postoperative symptom-free rate was 100% and the stone free rate of hospital discharge was 94.3% (32/35).The caliceal diverticular disappeared in 29 (82.9%) patients and decreased in six (17.1%) patients after three months.Conclusion:PCNL has an satisfactory safety and efficiency for treatment of posterior caliceal diverticular calculi.
