Reoperation for persistent or recurrent hemifacial spasm after microvascular decompression

Abstract:Objective To study the safety and efficacy of invalid or recurred hemifacial spasm with secondary microvascular decompression ( SMVD) .Methods 32 patients with invalid or recurred hemifacial spasms were inspected and assessed preoperatively , and treated with microvascular decompressions again .The reasons of invalid or recurrence were analyzed and curative effects were followed up postoperatively .Results 31 patients were cured after SMVD , among them 28 were immediate recovered , and 3 with delayed recovery ,only 1 was not cured and facial spasm still persist . 9 cases with facial paralysis and tinnitus in 5 in the early period were fully recovered in 2 months after operations ,however ,permanent hearing impairment occurred in 1 patient .Visible in the surgeries ,the huge and dense Teflon cotton pressed facial nerve root directly in 17 cases , 4 patients'responsibility vascular were missed , and 9 cases were inadequate decompressed .Conclusions It is feasible to choose the appropriate cases and treat with SMVD when the safety assessment is carried out on the recurrent case , SMVD has definite clinical effect , but the reoperation is difficult undoubtedly . Omissions of responsibility vascular and improper decompression are the main factors of relapse .
