Analysis of Causes and solutions of Delayed Facial Palsy Following Microvascular Decompression for Hemi-Facial Spasm

Abstract:Objective To evaluate the causes and treatments of delayed facial palsy following microvascular decompression operation(MVD) for hemi-facial spasm(HFS).To explore the effective solutions and prevention for the complication.Methods We followed up 64 patients with HFS who received microvascular decompression and developed delayed facial palsy in our department from January 2011 to 2016.Different vascular compression ways of the root entry zone and facial nerve was found by magnetic resonance imaging scan.Clinical effects and delayed facial palsy were followed up in the study.Neurophysiological monitoring was used in the operation to prevent any injury of the nerve.We also analyzed the probable causes of delayed facial palsy in both preoperative and postoperative periods.Results A total of 64 cases got rid of the spasm immediately and 3 cases showed no relief of the spasm in the follow-up period (1 ～ 5years,3.1 years on average).All cases showed delayed facial palsy(43 cases showed House-Brackmann Grade Ⅰ～Ⅲ and 24 cases showed Grade Ⅳ～Ⅴ).However,56 cases(83.6％) showed significant decline of facial motor evoked potential (FMEP) in intraoperative monitoring.All patients recovered rapidly and only 2 cases showed slightly recovery(Grade Ⅰ～Ⅱ).Conclusion Microvascular decompression is a kind of safe and effective way to cure HFS in long-term follow-ups.Delayed facial palsy may be one of the most common side effects of MVD.Effective ways to avoid this complication may turn out to be proper selection of decompression of responsible vascular,realtime intraoperative monitoring and rational drug using.
