Abstract:Objective:To evaluate the clinical efficacy on vesical calculus treated with open surgery, holmium laser, and ultrasonic and pneumatic lithotripsy associated by negative pressure. Methods: A total of 104 patients with bladder stones in our hospital between May 2006 to August 2009 were retrospectively analyzed. The patients , aged from 28 to 64 (44.5±7.4), were divided into 3 groups. Group A: 23 cases was treated with open surgery; Group B:42 cases was treated with holmium laser; Group C:39 cases was treated with ultrasonic and pneumatic lithotripsy associated by negative pressure. The differences in the three methods were compared and analyzed in the treatment of bladder stones. Results:The operating time on vesical calculus of group A, B, C was 24. 04±3. 70, 41. 88±17. 30, and 34. 05±13. 03 minutes. The operating time of group A was significantly shorter than that of group B and C(P<0. 01), and that of group C was significantly shorter than group B(P<0. 05). As long diameter of calculi increased, the operating time of minimally invasive surgical treatment prolonged obviously. The hospitalization time of group A, B, C was 13. 13±4. 79, 11. 38±3. 36, 9. 51±1. 72 days. The hospitalization time of group A was significantly longer than that of group B(P<0. 05) and group C(P<0. 01), and that of group C was significantly shorter than group B(P<0. 05). The time of indwelling catheter after operation of group A, B, and C was 7. 00±2. 54, 5. 05±1. 51, 4. 03±0. 99 days. That time of group A was significantly longer than that of group B and group C(P<0. 01), and that of group C was significantly shorter than group B(P<0. 01). Conclusions:The treatment of vesical calculi with ultrasonic and pneumatic lithotripsy associated by negative pressure was mini-invasive, safe, highly effective, and was beneficial for recovery. Minimally invasive therapy can be applied to patients with benign prostatic hyperplasia complicated with bladder stones. Open surgery could be taken into account as long diameter of calculi is more than 4 cm.
