Abstract:THE HIGH INCIDENCE of cerebral vascular accidents in the vicinity of the internal capsule is of major concern to all clinicians. The blood supply to this area is less well understood than some others, partly because of its complexity. Since the internal capsule fibers are intimately enclosed by two massive cellular regions, namely, the basal ganglia and the diencephalon, and since penetrating arteries distribute primarily to nuclear structures, the blood supply to these nuclei is conceivably of major importance to that of the internal capsule. The arterial blood supply to the cerebrum and diencephalon was first reliably detailed by the work of Duretl-5 in Charcot’s laboratory at the Salpktribre in Paris, although the results of Heubner’so (of Leipzig) investigations were published simultaneously in Berlin ( 1872). However much subsequent authors may disagree, the fine details of Duret’s concepts of the blood supply to the brain and the basic arterial patterns which he worked out are still remarkable. His work has persisted largely through the clinical application and emphasis given it by Charcot. In the intervening years studies limited to a few vessels or to those of a particular region have appeared,7-17 many of them with considerable merit. Only the general work of BeevorlsJ9 has influenced to any great extent the thinking about the blood supply to the various regions of the cerebral hemispheres, including the subcortical telencephalic nuclei (basal ganglia) and to the diencephalon. The venous drainage of the brain has been neglected by anatomists. Because of the need for such knowledge, data on the deep cerebral veins, mainly in con junction with applications of this information to roentgenologic case studies, have recently been reported by Johanson,2o Wolf and associates,21.22 Kaplan and F0rd,~3 and H a s ~ l e r . ~ ~ The 1939 paper of Schlesinger26 has been the one most cited, although others on this subject include papers by Schwartz and Fink,20 Schwartz,27 Stopford,28 Bailey,29 O’C~nne l l ,~~ Lin and co-workers,al Wolf and co-workers,32 Denny-Brown and associates,33 K a ~ l a n , ~ ~ and Bekov.36 The intracerebral veins are numerous and prominent and it is logical to suspect that they, as well as arteries, play a significant part in the normal cerebral circulation and in the abnormal processes which occur in the central nervous system.
