Compare effects of inverse ratio ventilation with conventional ventilatory modes on cardiopulmonary function in acute lung injury

Abstract:Objective: To compare the effects of inverse ratio ventilation with conventional ventilatory modes on cardiopulmomaryh function in dog′s acute lung injury. Methods: Used oleic-acid to reproduce the model of acute lung injury in dogs. Each had been provided ventilation for 30 minutes, and then was measured all the parameters. Result: Without using extrinsic PEEP, the peak airway pressure of IRV was lower than the IPPV′s, if there was a certain level of IRV was PEEP, both inverse ratio ventilation′s mean airway pressure would be higher than IPPV′s. Without extrinsic PEEP, high proportion inverse ratio evntilation could rise the value of PaO2. If used a certain level of PEEP, the value of PaO2 would descend. Comparing to conventional ventilatory modes, all parameters of circulatory system didn't have apparent difference in IRV. Whereas the value of PEEP arrived to 15 cm H2O, the cardiac output indexes of inverse ratio ventilation and conventional ventilation were lower; at the same time, there were apparent changing in LVSWI and PAWP. Conclusions: Inverse ratio ventilation can be result in increasing the mean airway pressure and reducing the ratio of lung injury.
