Abstract:Patients with unilateral ear discharge and hearing loss often have external or middle ear diseases. We present a 55–year–old man who suffered from persistent ear discharge and hearing loss in the left ear. Local findings showed that his left ear canal was filled with a large amount of granulation tissue, with purulent, foul–smelling discharge. Computed tomography indicated left middle ear cholesteatoma and mass shadow in the left external auditory canal. Modified radical mastoidectomy was performed. A piece of white plastic stick was found in the middle ear during the operation. Foreign body–induced cholesteatoma and external auditory canal granuloma in adults are very rare. We present this rare case so that these conditions can be better recognized and understood.

Abstract:Objective To investigate the clinical characteristics and surgical management of middle ear cholesteato-ma in children. Methods 27 cases of children with middle ear cholesteatoma were enrolled in this study;Clinical symp-toms, signs and surgical experience were reviewed. Of 27 patients, 2 were performed transmeatal atticotomy and tympano-plasty, 25 were performed canal-wall-down modified radical mastoidectomy, and 5 were performed first-stage ossicular chain reconstruction. Results Otorrhea, ear odour and hearing loss were the most common symptoms;Mastoid, sinus tympani and attic were involved in 25 cases;Mesotympanum, hypotympanum and eustachian tube were in-volved in 20 cases;88.9%of patients had ossicular chain destruction;Facial nerve canal was eroded in 7 cases;During the follow-up of one to five years, there was no one with significant complications as facial palsy and intracranial complica-tions, no recurrent cholesteatoma was observed. Conclusions The middle ear cholesteatoma in children show more aggres-sive in complicated condition, appropriate surgical approach should be selected according to the severity of the lesion, opened technique is benificial to complete removal of lesions and prevent recurrence in severe patient.

Abstract:The existence of acquired cholesteatoma has been recognized for more than three centuries; however, the nature of the disorder has yet to be determined. Without timely detection and intervention, cholesteatomas can become dangerously large and invade intratemporal structures, resulting in numerous intra- and extracranial complications. Due to its aggressive growth, invasive nature, and the potentially fatal consequences of intracranial complications, acquired cholesteatoma remains a cause of morbidity and death for those who lack access to advanced medical care. Currently, no viable nonsurgical therapies are available. Developing an effective management strategy for this disorder will require a comprehensive understanding of past progress and recent advances. This paper presents a brief review of background issues related to acquired middle ear cholesteatoma and deals with practical considerations regarding the history and etymology of the disorder. We also consider issues related to the classification, epidemiology, histopathology, clinical presentation, and complications of acquired cholesteatoma and examine current diagnosis and management strategies in detail.

Abstract:Middle ear cholesteatoma, as a common ear disease, has been further understood in terms of its molecular mechanism, classification and staging, and it is generally believed that surgical treatment is the first choice, but there is no final conclusion on its surgical choice. Microscopic surgery includes complete or open radical mastoidectomy with or without reconstruction of the middle ear cavity, and their pros and cons have been debated for decades. In the past decade, a variety of surgical methods have been developed, including various improvements and combined operations that combine the advantages of traditional open radical mastoidectomy and complete mastoidectomy. On the other hand, the emergence of ear endoscopic surgery has opened up new surgical methods, as well as the use of laser and other new technologies to improve the surgical effect and prognosis, but also can not avoid their limitations and shortcomings. The purpose of this paper is to summarize the advantages and disadvantages of various surgical treatments for middle ear cholesteatoma in order to provide guidance and help for otologists in the treatment of middle ear cholesteatoma.

Abstract:Objective To summarize and discuss the characteristics of endoscopic approach to manage the middle ear cholesteatoma,and to evaluate the operative safety and outcomes based on the data from the multicenter study.Methods The data of 242 cases diagnosed with the middle ear cholesteatoma and received operation through endoscopic approach between June 2016 and June 2017 in six tertiary hospitals in China were analyzed in this work.There were 130 males and 112 females,with the age ranging from 3 to 72 years old.We evaluated the strategy about how to manage the cholesteatoma,discussed the detailed techniques about how to remove the cholesteatoma and to improve the efficiency under endoscopic visualization.Meanwhile,the recurrence rate and residual rate of cholesteatoma as well as the complications in endoscopic approach were summarized.Results A total of 158 cases were operated in exclusively endoscopic transcanal approach,72 cases operated in combined approach,and 12 cases operated majorly under microscope and minorly under endoscope.219 cases were operated in one stage surgery,23 cases received second look.In the second look,3 cases were detected with residual cholesteatomas.Among them,2 cases were found by MRI-DWI examination after the first-stage operation.With endoscopic examination after operation,17 cases showed retracted pocket recurrence (7％,17/242).With introduction of endoscope in cholesteatoma,153 cases were achieved canal wall-up operation (63％,153/242).The complications in endoscopic approach included chord tympani never injury in 27 cases,skin injury of ear canal in 11 cases,tinnitus in 13 cases,vertigo in 7 cases,external ear canal stenosis in 1 case.Conclusions Using otoendoscope in cholesteatoma surgery would help keeping the normal structures of middle ear as much as possible,benefit to remove the hiding pathologies,help reducing residual cholesteatoma and lowering the rate of canal wall-down operation as well.This study showed good safety of otoendoscopic cholesteatoma surgery,however,strict evaluation of indication and quite good surgical techniques and skills are necessary for avoiding unexpected complication.

Scutum reconstruction technique and classification in endoscopic middle ear cholesteatoma surgery

Abstract:Background: Endoscopy has been increasingly used in middle ear cholesteatoma surgery. It can increase the proportion of canal wall up (CWU) surgery and effectively control the recurrence and residual rates. However, there is a lack of systematic description on scutum reconstruction technique in endoscopic middle ear cholesteatoma surgery. Objective: The technique and classification of scutum reconstruction in endoscopic cholesteatoma surgery were proposed, and the outcomes were preliminarily evaluated. Methods: According to the development of mastoid and the way of hearing reconstruction, the reconstruction of scutum was classified into three types. The rates of residual and recurrence and hearing outcomes were observed and analyzed. Results: A total of 57 patients underwent endoscopic cholesteatoma surgery, all of which were CWU surgery. The reconstruction of the scutum was performed using tragal cartilage according to three types. The residual of cholesteatoma was observed in three cases. The recurrence of cholesteatoma and retraction pocket was detected in two cases and five cases, respectively. Postoperative hearing results were generally satisfied in all three types of surgeries. Conclusion: The technique of scutum reconstruction in endoscopic cholesteatoma surgery in this work can effectively control the recurrent disease after CWU surgery.

Abstract:Surgery for cholesteatoma should be tailored to individual patients, considering demographic and disease factors, to obtain a dry, safe, and functional ear. The EAONO/JOS classification and staging system provide a valuable framework for data collection and outcome assessment. Canal wall-up and canal wall-down surgical approaches each have their advantages and disadvantages, though it is not definitive that one approach is clearly more advantageous than the other. Mastoid obliteration techniques show promise in reducing recidivistic disease rates but require further research and standardization. Endoscopic ear surgery further augments our surgical capabilities to visualize and eradicate cholesteatoma.

Abstract:Implementation of endoscopes in cholesteatoma surgery resulted in considerable changes in the management of cholesteatoma in the last two decades. Compared to the microscopic approach with an excellent but straight-line view and limited illumination, the introduction of endoscopes provides a wide-angled panoramic view. Moreover, angled lenses allow the surgeon to visualize the middle ear and its hidden recesses through a transcanal, minimally-invasive approach. The endoscope enables the surgeon to remove limited cholesteatoma of the middle ear and its recesses using an exclusive endoscopic technique by taking advantage of these benefits. This reduces the rate of residual disease and sparing external incisions and excessive temporal bone drilling as in a transmastoid approach. Since transcanal endoscopic access is mainly a one-handed technique, it implies the need for specific procedures and technical refinements. This article describes a step-by-step guide as a surgical manual for endoscopic removal of epitympanic cholesteatoma. Different techniques for cholesteatoma dissection and bone removal for epitympanectomy, including curettage and powered instruments such as drills and ultrasonic devices with their outcomes, are discussed. This may offer ear surgeons insight into technical refinements and the latest technological developments and open the horizon for different techniques.
Clinical Analysis of 16 Pediatric Patients with Cholesteatoma in Middle Ear

Abstract:Objective To study the diagnosis and treatment of cholesteatoma in the middle ear in children. Methods A retrospective analysis was conducted on 16 pediatric patients with cholestestoma in the middle ear who were hospitalized within September 2001 and November 2005.All of them were clinically diagnosed and pathologically confirmed.The clinical manifestations,imaging data and surgical precedures were reviewed.Of the 16 patients,13 were performed canal-wall-down tympanoplasty with mastoidectomy and 3 radical mastoidectomy. Results During the follow-up of 6 months to 4 years,no recurrent cholestestoma was observed and no one experienced facial palsy.Fifteen cases(95.8%)were cured in first-stage surgery,while the other one was removed granulation in the third month after surgery.The hearing threshold of 3 patients were improved,4 had no change,and 9 were decreased. Conclusion The cholesteatomas in children are characterized by a variety of presentations which are often invisible.CT and MRI examination may help to make early diagnosis and prevent the complications.Complete removal of diseased tissues(cholesteatoma and granulation),restoration of ossicular chain and prevention of compilications are the key points in the surgical procedures.

Abstract:Objective: investigate the clinical characteristics of external auditory canal cholesteatoma for improve the effect of treatment. Method: Sixteen patients with external auditory canal cholesteatoma were treated from 1988 to 2001 in our department. There were 10 males and 6 females, ranging in age from 15 to 78 years old. CT scaning was using for fefinition the ranging of lesions and the timum management surgery was completed. Results:The lesion ranging in size from a few millimeters to extensive mastoid destruction. The bone distraction was light in 5,heavy bone distraction but no invasive mastoid cavity or tympanic cavity in 6 ,invasion up tympanic cavity in 2,invasion mastoid cavity and formed fistulas between canal and mastoid cavity in 3. All patients were cure for the period of follow-up 5 years. Conclusion: The cholesteatoma of ear canal occurs was infrequently.It is most commonalty see in the elderly. The keratosis obturans and external auditory canal cholesteatoma could be two different clinical and pathologic processes. Congenital cholesteatoma clinical and surgical management

Abstract:Objectives: Congenital cholesteatoma (CC) is accompanied by hearing loss and an intact tympanic membrane. However, the hearing loss is usually associated with otitis media, and the diagnosis of CC is frequently delayed in patients with an intact tympanic membrane. This study aimed to describe the clinical characteristics, management and outcomes of patients with CC. Methods: We reviewed patients with cholesteatoma from January 2011 to May 2020 and selected those meeting the congenital cholesteatoma criteria. The primary outcome measures included presenting symptoms, surgical findings, stage of disease, recurrence rate and hearing outcomes. Results: We reviewed 1646 medical files of cholesteatoma patients and identified 18 patients with congenital cholesteatoma, the mean age at operation was 8.13 ± 1.36 years (range 3-18). The unilateral hearing loss included moderate 13 patients (72.2%), severe 4 patients (22.2%), and slight 1 (5.6%). There were 14 cases of conductive hearing loss (77.8%) and 4 cases of mixed hearing loss (22.2%). The mean course of disease was 1.41 ± 0.05 years (range 0.4-3). The surgical management was oto-endoscope exploratory tympanotomy in 1（5.6%）, canal wall up mastoidectomy in 12 (66.7%) and canal wall down in 5 (27.8%), with 17 (94.4%) ossicular replacements. Seventeen (94.4%) patients presented with Potsic stage III-IV. Recurrence occurred in 5.6% of patients in stage III and 11.1% of patients in stage IV. After surgery, patients achieved normal voice tone hearing. Conclusions: To diagnose it early, congenital cholesteatoma should be considered as a possible aetiology for hearing loss patients with an intact tympanic membrane. In our study, most patients were diagnosed at III and IV stage. This highlights the need to promote awareness of the disease among primary physicians in the community healthcare system. Surgical management with removal of the cholesteatoma and reconstruction of the tympanum and ossicular chain is an effective treatment.

Abstract:Background: This study presents a comprehensive analysis of cholesteatoma of the middle ear, focusing on its clinical presentation, diagnostic imaging, and treatment outcomes. Cholesteatomas are defined by the keratinized squamous epithelium within the middle ear, leading to significant bone erosion, often affecting the ossicular chain and surrounding structures. Methods: The study explores various mechanisms involved in cholesteatoma progression, including enzymatic lysis, inflammatory responses, and neurotrophic disturbances. The study conducted a retrospective clinical and statistical review of 580 patients over a 20-year period (2003–2023), highlighting the role of advanced imaging, including computed tomography (CT) and diffusion-weighted magnetic resonance imaging (DWI), in preoperative planning and postoperative follow-up. Results: Findings revealed that early detection and intervention are crucial in preventing severe complications such as intracranial infection and hearing loss. Surgical treatment primarily involved tympanoplasty and mastoidectomy, with a recurrence rate of 1.55% within two years. The study underscores the importance of integrating imaging advancements into clinical decision-making to enhance patient outcomes and suggests further investigation into molecular mechanisms underlying cholesteatoma progression and recurrence. Histopathological and microbiological analysis was performed to identify pathological patterns and microbial agents. Conclusions: The study highlights the importance of early diagnosis and intervention to prevent complications such as intracranial infections and permanent hearing loss, while also emphasizing the role of advanced imaging techniques in the management and long-term monitoring of cholesteatoma patients.

Abstract:Hypothesis: Autophagy and its enhancement may have a role in the pathogenesis of acquired cholesteatoma. Background: The etiopathogenesis of acquired cholesteatoma remains unclear. Some clinical features of cholesteatoma are similar to those of cancer. The study of autophagy in cancer has indicated that enhanced autophagy enables tumor cell survival and growth. Methods: Cholesteatoma epithelium and normal external auditory canal (EAC) epithelium were obtained from patients with acquired cholesteatoma, and marginal epithelium of the tympanic membrane perforation was obtained from patients with chronic otitis media (COM). Immunohistochemistry (IHC) was performed to detect the expression of light chain 3 (LC3) in cholesteatoma and EAC epithelium. Western blotting (WB) was performed to detect the expression of LC3, Beclin-1, or the PI3K/AKT pathway in cholesteatoma, EAC, and COM epithelium. Results: LC3 staining of IHC was stronger in cholesteatoma epithelium compared with normal EAC epithelium. The ratios of LC3-II/I and Beclin-1 expression on WB were significantly higher in cholesteatoma epithelium compared with EAC epithelium or COM epithelium, and there was a significantly higher ratio of p-PI3K/PI3K and p-AKT/AKT in cholesteatoma epithelium compared with EAC epithelium. Conclusions: Enhanced autophagy might play a role in the pathogenesis of acquired cholesteatoma. PI3Ks might have different regulatory functions on autophagy in the cholesteatoma epithelium.

Abstract:Objective To investigate the clinical characteristics and treatment of external auditory canal chol‐esteatoma (EACC) .Methods The clinical data of 38 cases(39 ears)with external auditory canal cholesteatoma from August 2006 to December 2014 were retrospectively analyzed .Results All the cases of EACC in this study had the external ear canal full of impacted squamous material or granulation tissue .The Holt and CT imaging examinations disclosed that 10 ears were phase I ,lesions were confined to the external auditory with no bone destruction .There were 23 ears were phase II ,the lesions were located in external auditory meatus with destruction of bone ,but with no involvement of the middle ear .There were 6 ears were phase III ,showing the lesions with disrupt external audi‐tory meatus and involvement of the middle ear ( mastoid and/or tympanic sinus) .Hearing impairment and aural fullness were the most common symptoms .The phase I cases were treated by removing cholesteatomas from the ex‐ternal auditory canal .Canaloplasty and/or tympanoplasty were performed in phase II cases .The phase III cases were successfully managed by modified radical mastoidectomy and/or tympanoplasty .There were 30 ears of tympan‐ic membrane were perfect and invaginate .There were 4 ears of ossicular chain were disrupted with one ear of facial nerve exposed .All surgeries were performed at once .No recurrence except in one patient was found during the fol‐low -up period .Conclusion The clinical stages can help identify the primary lesions and determine the choice of the best surgical approach .
