Abstract:Patients with unilateral ear discharge and hearing loss often have external or middle ear diseases. We present a 55–year–old man who suffered from persistent ear discharge and hearing loss in the left ear. Local findings showed that his left ear canal was filled with a large amount of granulation tissue, with purulent, foul–smelling discharge. Computed tomography indicated left middle ear cholesteatoma and mass shadow in the left external auditory canal. Modified radical mastoidectomy was performed. A piece of white plastic stick was found in the middle ear during the operation. Foreign body–induced cholesteatoma and external auditory canal granuloma in adults are very rare. We present this rare case so that these conditions can be better recognized and understood.

Abstract:OBJECTIVE:To analyze the clinical characteristics of unilateral conductive hearing loss with intact tympanic membrane, and summarize the key diagnostic points, differential diagnosis and observe the effects of surgical treatment.METHODS:We reviewed data from 82 patients with unilateral conductive hearing loss with intact tympanic membranes who accepted the exploratory tympanotomy from April 2011 to September 2013. There were 41 males and 41 females, aged from 7 to 66( averaged 26.5±13.7)years, with a history of one month to 50 years. The history, clinical symptoms, audiological evaluation, high resolution temporal bone CT, the results of surgical exploration and hearing reconstruction were analyzed.RESULTS:The exploratory tympanotomy revealed 43 cases of congenital middle ear malformations (52.4%), 22 cases of otosclerosis (26.8%), eight cases of congenital cholesteatoma (9.8%), six cases of trauma induced conductive hearing loss (7.3%), three cases of congenital ossicular malformations with congenital cholesteatoma (3.7%). Progressive hearing loss was common in patients with otosclerosis and congenital cholesteatoma, and patients with congenital middle ear malformations described their hearing loss since childhood. High resolution temporal bone CT of congenital middle ear malformation, trauma induced conductive hearing loss, congenital cholesteatoma diagnosis rate was 40.0%, 50.0%, and 83.3% respectively. The preoperative air-conductive threshold of patients with absence of the oval window were increased to (66.9±1.1)dBHL, the preoperative bone-conductive threshold achieved (28.3±10.4)dBHL at 2 000 Hz. While patients with stapes fixation and that with ossicular chain discontinuity were (27.2±9.7)dBHL and (17.8±8.8)dBHL(P=0.000)respectively. Through the tympanic exploration with endaural incision under the microscope, different hearing reconstruction were applied according to different lesions. After the operation, the hearing level of 52 patients with return visit were improved, the mean air-conductive threshold were decreased from (60.0±11.4)dBHL to (32.2±12.1)dBHL(P=0.000); and the mean ABG were decreased from (43.2±12.0)dB to (16.3±9.4)dB(P=0.000).CONCLUSIONS:Congenital middle ear malformations, otosclerosis, congenital cholesteatoma are the most common causes in unilateral conductive hearing loss with an intact tympanic membrane. The diagnosis rate can be improved by analyzing the clinical features. Through exploratory tympanotomy and hearing reconstruction, we can clarify the diagnosis and achieve a satisfying hearing recover.

Abstract:Objective To investigate the clinical characteristics and surgical management of middle ear cholesteato-ma in children. Methods 27 cases of children with middle ear cholesteatoma were enrolled in this study;Clinical symp-toms, signs and surgical experience were reviewed. Of 27 patients, 2 were performed transmeatal atticotomy and tympano-plasty, 25 were performed canal-wall-down modified radical mastoidectomy, and 5 were performed first-stage ossicular chain reconstruction. Results Otorrhea, ear odour and hearing loss were the most common symptoms;Mastoid, sinus tympani and attic were involved in 25 cases;Mesotympanum, hypotympanum and eustachian tube were in-volved in 20 cases;88.9%of patients had ossicular chain destruction;Facial nerve canal was eroded in 7 cases;During the follow-up of one to five years, there was no one with significant complications as facial palsy and intracranial complica-tions, no recurrent cholesteatoma was observed. Conclusions The middle ear cholesteatoma in children show more aggres-sive in complicated condition, appropriate surgical approach should be selected according to the severity of the lesion, opened technique is benificial to complete removal of lesions and prevent recurrence in severe patient.

Abstract:Objective: To explore the efficacy of ossiculoplasty surgery under oto-endoscope in patients of conductive hearing loss with intact tympanic membrane. Methods: A retrospective study was conducted. The clinical data of 45 patients with conductive hearing loss who had undergone simple ossiculoplasty surgery between October 2015 and December 2017 from five hospitals in China (West China Hospital of Sichuan University, General Hospital of the People's Liberation Army, Shanghai Ninth People's Hospital, Shanghai JiaoTong University School of Medicine, Xijing Hospital, Fourth Military Medical University and Shenzhen Sixth People's Hospital) were collected. There were 28 males and 17 females, with the age ranging from 12 to 69 years old. The tympanic membranes of those patients were intact before surgery. The lesion only occurred in the ossicular chain from CT and intraoperative exploration. The cases of otosclerosis, tympanosclerosis, cholesteatoma of middle ear, chronic suppurative otitis media, and tumor of middle ear were excluded. The postoperative complications, hearing improvements and operation time were observed. SPSS 23.0 software was used for statistic analysis. Results: There was neither sensorineural hearing loss nor facial paralysis after surgery in this group. Seven patients had transient mild tinnitus after surgery and gradually relieved during the follow-up period. Nine patients developed dysgeusia after surgery. Two patients developed dizziness after surgery, and the symptoms were mild and relieved during 1 week without special intervention. The air-bone gap (ABG) range of pre-operation was 22.5-45.7 dB, and the average ABG was (25.3±8.6) dB. The ABG range in the third month after surgery was 5.7-26.8 dB, and the average ABG was (9.3±8.6) dB. The mean value of ABG was significantly lower in the third month after surgery compared with that of pre-operation (t=2.31, P<0.05). The operation time of the surgeons in each research center was similar. The overall average of operation time was (43.56±18.25) min. There was no significant difference in the duration of operation between the centers (F=3.26, P>0.05). Conclusion: The ossiculoplasty surgery under oto-endoscope has good efficacy.
Auditory Outcomes and Prognostic Factors in Canal Wall-Down Tympanoplasty with Single-Stage Ossiculoplasty

Abstract:Objective To explore the auditory outcomes and prognostic factors in canal wall-down tympanoplasty with ossiculoplasty. Methods A retrospective review was performed in 142 patients with cholesteatoma, who underwent canal wall-down tympanoplasty (CWDT) with ossiculoplasty in a single stage. The postoperative complication and hearing thresholds were analyzed in the 24 months follow-up. Results We had not found any postoperative retraction pocket or recurrence of cholestatoma during the follow-up. The rate of dry ear was 96.5%. But the extrusion of prosthesis occurred in 4 patients (2.8%). Auditory outcomes showed air threshold improved by 11.6 dB while the air-bone gaps were 7.4 dB. The successful auditory reconstruction rate was 49.3% on the whole. And we deemed the control of infection, function of eustachian tube, extent of cholesteatoma, material of prosthesis and surgical technique as the prognostic factors in CWDT with single stage of ossiculoplasty. Conclusion Influenced by several factors, CWDT with single stage of ossiculoplasty are safe and effective for cholesteatoma with low complication, low recurrence, and satisfactory outcome of auditory reconstruction.
Comparison of the Outcome between Hydroxyapatite and Titanium Prosthesis Used in Ossiculoplasty and Mastoidectomy to treat Cholesteatomatous Otitis Media

Abstract:Objective To study the effects of different prosthesis used in ossiculoplasty on mastoidectomy of treating cholesteatomatous otitis media.Methods 142 patients with cholesteatoma were treated by mastoidectomy with ossiculoplasty in a single stage.They were divided into 2 groups according to material of prosthesis (hydroxypatite or titanium).Hearing thresholds were reviewed in the 12 months follow-up.Results The both groups showed air threshold improved by 11.88 and 11.41 while the air-bone gaps were 7.80 and 6.98 dB.The success rates were 54.69% and 44.87% with the titanium group being better.The titanium prosthesis performed well at the high frequency (4 kHz).Conclusion The outcomes of ossiculoplasty with mastoidectomy in treating cholesteatoma is influenced by prosthesis made of different material.Either hydroxypatite or titanium has showed great effects in the treatment with the latter being better.A long-term follow up to evaluate the influence of different prosthesis is needed.

Abstract:Chronic Otitis Media-Squamosal type is an erosive process, which when confined to ossicular chain causes varying degrees of hearing impairment. As the disease progresses to involve surrounding vital structures, it causes various complications like facial palsy, vertigo, mastoid abscess, which are more common than the other intracranial complications, and require a definitive surgical intervention i.e., mastoidectomy at the earliest. A retrospective study on 60 patients who had been operated for squamosal type were analysed for the demographics, symptomatology, intraoperative extent of cholesteatoma, type of mastoidectomy done, various graft materials used for reconstruction, post operatively for graft uptake, hearing improvement and the results were analysed using ChOLE classification of cholesteatoma. Although Intact Canal Wall mastoidectomy had improved post op PTA values, there was no significant difference in the Air-Bone gap closure when Intact Canal Wall mastoidectomy was compared to Canal Wall Down Mastoidectomy.
Concurrent Occurrence of Congenital Ossicular Anomaly and Localized Cholesteatoma: Series of 10 Cases

Abstract:Objective: The objective of this study is to describe the clinical features, managements and outcomes of a rare coexistence of congenital ossicular anomaly and localized cholesteatoma. A literature review on these cases and each congenital disorder is also presented. Methods: A retrospective chart review was performed on patients diagnosed with congenital ossicular anomaly with concurrent localized cholesteatoma from 2008 to 2017. Clinical data of these patients were collected. Results: A total of 10 patients were identified. All patients presented with unilateral hearing loss. Pure-tone audiometry showed conductive hearing loss in all affected ears with an average air conduction (AC) threshold of 59 dB. High-resolution computed tomography scans of the temporal bone diagnosed ossicular anomaly for 90% (9/10); however, only 50% (5/10) had a diagnosis of localized cholesteatoma. A transcanal exploratory tympanotomy under the microscope was performed to discover whether the localized tiny-sized cholesteatoma around the ossicular chain did not have direct contact with the ossicular chain, which could be diagnosed as congenital cholesteatoma. We removed the localized cholesteatoma and reconstructed the ossicular chain in each patient. All localized cholesteatomas were found in the posterior-superior quadrant of the middle ear. Ossicular chain anomalies were associated with the incus and/or the stapes in all cases. Hearing improvement was achieved in each of the 6 patients who were followed up postoperatively, with an average AC threshold of 35 dB. The clinical features of congenital ossicular anomaly with concurrent congenital cholesteatoma were compared with those of each congenital disorder. The pathogenesis of each condition was also discussed. Conclusions: Congenital ossicular anomaly with concurrent congenital cholesteatoma is rare. It shares similar clinical features with congenital ossicular anomaly occurring alone, therefore awareness should be raised for a possible concurrent congenital cholesteatoma which was easy to miss in the diagnosis (50%) by the radiologist. A patient’s hearing level can be improved by removal of the cholesteatoma and reconstruction of the ossicular chain. Localized cholesteatoma does not usually show residuals or recurrence.
Transcanal Endoscopic Ear Surgery for Attic Cholesteatoma

Abstract:Objective To report surgical techniques, outcomes and application characteristics of transcanal endo-scopic ear surgery for attic cholesteatoma. Methods Data from 47 patients (28 males and 19 females;aged 15-68 years, mean=41 years) who underwent transcanal endoscopic ear surgery for attic cholesteatoma in our department from Janu-ary 2016 to January 2017 were retrospectively analyzed. During the surgery, the tragus cartilage-perichondrium compos-ite was routinely harvested for tympanoplasty after removal of cholesteatoma under an otoendoscope. Ossiculoplasty and attic reconstruction were individualized. Patients were followed up at postoperative 1, 2, 3 and 6month. Results Of the 47 patients, 32 (68%) had limited attic cholesteatoma, and 15 (32%) had protympanum/mesotympanum cholesteato-ma. Ossicles were intact in 21 cases (45%) and showed malleus/incus erosion in 26 cases (55%), with ossiculoplasty re-quired in 29 cases. There were no facial paralysis or otorrhea. The tympanic membrane and graft were intact and well healed, with no retraction or perforation after surgery. The mean preoperative air conduction threshold was 43.45± 9.56 dB HL, compared to 27.23 ± 6.89 dB HL postoperatively (P<0.01). The mean preoperative air-bone gap was 28.42 ± 6.78 dB, compared to 13.62 ± 8.67 dB postoperatively (P<0.01). Among the 29 patients who underwent ossiculoplasty, the postoperative air-bone gap was<10 dB in 11 cases, 10~20 dB in 13 cases, 20~30 dB HL in 3 cases and>30 dB in 2 cases. There were no recurrence, residual cholesteatoma or secondary surgery needed. Conclusions Stand alone transca-nal endoscopic ear surgery was a safe and effective for attic cholesteatoma.

Abstract:Otitis media (OM), defined as infection or inflammation of the middle ear (ME), remains a major public health problem worldwide. Cholesteatoma is a non-cancerous, cyst-like lesion in the ME that may be acquired due to chronic OM and cause disabling complications. Surgery is required for treatment, with high rates of recurrence. Current antibiotic treatments have been largely targeted to previous culturable bacteria, which may lead to antibiotic resistance or treatment failures. For this study, our goal was to determine the microbiota of cholesteatoma tissue in comparison with other ME tissues in patients with long-standing chronic OM. ME samples including cholesteatoma, granulation tissue, ME mucosa and discharge were collected from patients undergoing tympanomastoidectomy surgery for chronic OM. Bacteria were profiled by 16S rRNA gene sequencing in 103 ME samples from 53 patients. Respiratory viruses were also screened in 115 specimens from 45 patients. Differences in bacterial profiles (beta-diversity) and the relative abundances of individual taxa were observed between cholesteatoma and ME sample-types. Additionally, patient age was associated with differences in overall microbiota composition while numerous individual taxa were differentially abundant across age quartiles. No viruses were identified in screened ME samples. Biodiversity was moderately lower in cholesteatoma and ME discharge compared to ME mucosal tissues. We also present overall bacterial profiles of ME tissues by sample-type, age, cholesteatoma diagnosis and quinolone use, including prevalent bacterial taxa. Our findings will be useful for fine-tuning treatment protocols for cholesteatoma and chronic OM in settings with limited health care resources.

Abstract:objective To report treatment outcomes of external auditory canal cholesteatoma combined with mastoiditis and tympanic membrane perforation. Methods This is a retrospective study of 40 cases with external auditory canal choleste?atoma. All cases were followed six months to 1 year. Treatment outcomes by different surgical approaches were compared us?ing Fisher's exact probability test. Results (1) There was no recurrence among the cases during the following up to 1 year;(2) There was no statistically significant difference regarding eradication of middle ear infection among different surgical ap?proaches (p=0.375);(3) There was no statistically significant difference among different surgical approaches regarding tym?panic membrane healing (p=0.426). Conclusion The key in treatment of external auditory canal cholesteatoma with mastoid?itis and tympanic membrane perforation is the removal of cholesteatoma. Tympanoplasty and ear drum repair should be based on findings during mastoidotympanectomy and may need to be completed in phaseⅡ.
Clinical Analysis of 16 Pediatric Patients with Cholesteatoma in Middle Ear

Abstract:Objective To study the diagnosis and treatment of cholesteatoma in the middle ear in children. Methods A retrospective analysis was conducted on 16 pediatric patients with cholestestoma in the middle ear who were hospitalized within September 2001 and November 2005.All of them were clinically diagnosed and pathologically confirmed.The clinical manifestations,imaging data and surgical precedures were reviewed.Of the 16 patients,13 were performed canal-wall-down tympanoplasty with mastoidectomy and 3 radical mastoidectomy. Results During the follow-up of 6 months to 4 years,no recurrent cholestestoma was observed and no one experienced facial palsy.Fifteen cases(95.8%)were cured in first-stage surgery,while the other one was removed granulation in the third month after surgery.The hearing threshold of 3 patients were improved,4 had no change,and 9 were decreased. Conclusion The cholesteatomas in children are characterized by a variety of presentations which are often invisible.CT and MRI examination may help to make early diagnosis and prevent the complications.Complete removal of diseased tissues(cholesteatoma and granulation),restoration of ossicular chain and prevention of compilications are the key points in the surgical procedures.
