Abstract:OBJECTIVE:To analyze the clinical characteristics of unilateral conductive hearing loss with intact tympanic membrane, and summarize the key diagnostic points, differential diagnosis and observe the effects of surgical treatment.METHODS:We reviewed data from 82 patients with unilateral conductive hearing loss with intact tympanic membranes who accepted the exploratory tympanotomy from April 2011 to September 2013. There were 41 males and 41 females, aged from 7 to 66( averaged 26.5±13.7)years, with a history of one month to 50 years. The history, clinical symptoms, audiological evaluation, high resolution temporal bone CT, the results of surgical exploration and hearing reconstruction were analyzed.RESULTS:The exploratory tympanotomy revealed 43 cases of congenital middle ear malformations (52.4%), 22 cases of otosclerosis (26.8%), eight cases of congenital cholesteatoma (9.8%), six cases of trauma induced conductive hearing loss (7.3%), three cases of congenital ossicular malformations with congenital cholesteatoma (3.7%). Progressive hearing loss was common in patients with otosclerosis and congenital cholesteatoma, and patients with congenital middle ear malformations described their hearing loss since childhood. High resolution temporal bone CT of congenital middle ear malformation, trauma induced conductive hearing loss, congenital cholesteatoma diagnosis rate was 40.0%, 50.0%, and 83.3% respectively. The preoperative air-conductive threshold of patients with absence of the oval window were increased to (66.9±1.1)dBHL, the preoperative bone-conductive threshold achieved (28.3±10.4)dBHL at 2 000 Hz. While patients with stapes fixation and that with ossicular chain discontinuity were (27.2±9.7)dBHL and (17.8±8.8)dBHL(P=0.000)respectively. Through the tympanic exploration with endaural incision under the microscope, different hearing reconstruction were applied according to different lesions. After the operation, the hearing level of 52 patients with return visit were improved, the mean air-conductive threshold were decreased from (60.0±11.4)dBHL to (32.2±12.1)dBHL(P=0.000); and the mean ABG were decreased from (43.2±12.0)dB to (16.3±9.4)dB(P=0.000).CONCLUSIONS:Congenital middle ear malformations, otosclerosis, congenital cholesteatoma are the most common causes in unilateral conductive hearing loss with an intact tympanic membrane. The diagnosis rate can be improved by analyzing the clinical features. Through exploratory tympanotomy and hearing reconstruction, we can clarify the diagnosis and achieve a satisfying hearing recover.

Middle Ear Foreign Body Causing Cholesteatoma and External Auditory Canal Granuloma: a Case Report

Abstract:Patients with unilateral ear discharge and hearing loss often have external or middle ear diseases. We present a 55–year–old man who suffered from persistent ear discharge and hearing loss in the left ear. Local findings showed that his left ear canal was filled with a large amount of granulation tissue, with purulent, foul–smelling discharge. Computed tomography indicated left middle ear cholesteatoma and mass shadow in the left external auditory canal. Modified radical mastoidectomy was performed. A piece of white plastic stick was found in the middle ear during the operation. Foreign body–induced cholesteatoma and external auditory canal granuloma in adults are very rare. We present this rare case so that these conditions can be better recognized and understood.

Modified Translabyrinthine Approach for Massive Petrous Bone Cholesteatoma Removal and Cochlear Preservation Using Latent Spaces Anterior to the Cochlea

Abstract:OBJECTIVE:Microsurgery is the reference standard treatment of petrous bone cholesteatoma (PBC). In most cases, radical removal of an extensive PBC can only be achieved at the cost of sacrificing the cochlea. Such treatment will result in the impossibility of future cochlear implantation for hearing rehabilitation purposes. To address this issue, a modification of the traditional translabyrinthine (TL) approach with endoscopic assistance has been developed for radical removal of extensive PBC with preservation of the cochlea.METHODS:From June 2017 to December 2017, 3 patients with a massive PBC underwent surgical removal using the modified TL approach by the senior author in our department. We reviewed the patient characteristics and retrospectively studied the surgical outcomes and postoperative complications. In the present report, we have described our modified TL approach in detail.RESULTS:Complete resection of the PBC and successful cochlea preservation were achieved in all 3 patients. No recurrence had developed during the follow-up period. However, various degrees of cochlear ossification were observed in 2 patients postoperatively.CONCLUSIONS:This modified TL approach provides the possibility of fully exposing the whole petrous apex without removing the cochlea in selected cases. However, the development of long-term cochlear ossification requires further investigation to allow for successful cochlear implantation.

Abstract:Congenital temporal bone cholesteatoma is a rare lesion in otolaryngology.The disease is locally invasive and may lead to significant complications,including hearing loss（conductive or sensorineural）, temporal bone destruction and intracranial invasion. This article reviews the characteristic symptoms of congenital temporal bone cholesteatoma, testing and imaging of the disease, stage and the current treatment options in order to promote awareness to this rare disease entity and perform early surgical treatment, effectively avoid the destruction of the temporal bone and its surrounding structures, thereby reducing the occurrence of complications. By improving the understanding of the disease and performing early surgical treatment, the destruction of the temporal bone and its surrounding structures can be effectively avoided, thereby reducing the occurrence of complications.
Transcanal Endoscopic Ear Surgery for Attic Cholesteatoma

Abstract:Objective To report surgical techniques, outcomes and application characteristics of transcanal endo-scopic ear surgery for attic cholesteatoma. Methods Data from 47 patients (28 males and 19 females;aged 15-68 years, mean=41 years) who underwent transcanal endoscopic ear surgery for attic cholesteatoma in our department from Janu-ary 2016 to January 2017 were retrospectively analyzed. During the surgery, the tragus cartilage-perichondrium compos-ite was routinely harvested for tympanoplasty after removal of cholesteatoma under an otoendoscope. Ossiculoplasty and attic reconstruction were individualized. Patients were followed up at postoperative 1, 2, 3 and 6month. Results Of the 47 patients, 32 (68%) had limited attic cholesteatoma, and 15 (32%) had protympanum/mesotympanum cholesteato-ma. Ossicles were intact in 21 cases (45%) and showed malleus/incus erosion in 26 cases (55%), with ossiculoplasty re-quired in 29 cases. There were no facial paralysis or otorrhea. The tympanic membrane and graft were intact and well healed, with no retraction or perforation after surgery. The mean preoperative air conduction threshold was 43.45± 9.56 dB HL, compared to 27.23 ± 6.89 dB HL postoperatively (P<0.01). The mean preoperative air-bone gap was 28.42 ± 6.78 dB, compared to 13.62 ± 8.67 dB postoperatively (P<0.01). Among the 29 patients who underwent ossiculoplasty, the postoperative air-bone gap was<10 dB in 11 cases, 10~20 dB in 13 cases, 20~30 dB HL in 3 cases and>30 dB in 2 cases. There were no recurrence, residual cholesteatoma or secondary surgery needed. Conclusions Stand alone transca-nal endoscopic ear surgery was a safe and effective for attic cholesteatoma.

Cholesteatoma With Cochlear Fistula and Carotid Dehiscence

Abstract:Cholesteatomas are non-neoplastic, invasive lesions created by the accumulation of keratinized squamous epithelium in the temporal bone. If left untreated, its expansion may cause local destruction of the surrounding structures, eventually leading to inner ear fistula, dehiscence of tegmen and possible intracranial pathology, and facial nerve paralysis. Surgical resection is the mainstay of curative treatment. We hereby present a case of a giant cholesteatoma in a 62-year-old patient who presented with a right-sided hemifacial spasm with later paralysis who was treated with botulinum toxin injection by a neurologist. CT scan imaging showed a very large cholesteatoma with involvement of the carotid canal, cochlea, and geniculate ganglion. The patient underwent transmastoid and subarcuate approach for resection which led to marked improvement of her symptoms.

Auditory Outcomes and Prognostic Factors in Canal Wall-Down Tympanoplasty with Single-Stage Ossiculoplasty

Abstract:Objective To explore the auditory outcomes and prognostic factors in canal wall-down tympanoplasty with ossiculoplasty. Methods A retrospective review was performed in 142 patients with cholesteatoma, who underwent canal wall-down tympanoplasty (CWDT) with ossiculoplasty in a single stage. The postoperative complication and hearing thresholds were analyzed in the 24 months follow-up. Results We had not found any postoperative retraction pocket or recurrence of cholestatoma during the follow-up. The rate of dry ear was 96.5%. But the extrusion of prosthesis occurred in 4 patients (2.8%). Auditory outcomes showed air threshold improved by 11.6 dB while the air-bone gaps were 7.4 dB. The successful auditory reconstruction rate was 49.3% on the whole. And we deemed the control of infection, function of eustachian tube, extent of cholesteatoma, material of prosthesis and surgical technique as the prognostic factors in CWDT with single stage of ossiculoplasty. Conclusion Influenced by several factors, CWDT with single stage of ossiculoplasty are safe and effective for cholesteatoma with low complication, low recurrence, and satisfactory outcome of auditory reconstruction.
Outcomes of transcanal endoscopic middle ear surgery for congenital cholesteatoma

Abstract:Background: Transcanal endoscopic surgery offers advantages for congenital cholesteatoma (CC) which is expanding cystic mass located on an intact tympanic membrane. Objectives: To determine the efficacy of transcanal endoscopic ear surgery for the treatment of CC. Material and methods: Eleven patients with CC confined to the middle ear underwent transcanal endoscopic ear surgery by surgeons who were skilled in otoscopic surgery. According to the Potsic classification, four, three, and four patients had lesions in stages 1, 2, and 3, respectively. The mean follow-up duration was 24.5 months. We retrospectively analyzed the recurrence rate, complications, and average hearing thresholds of the patients with CC. Results: All CCs were successfully resected via the transcanal endoscopic approach, without any immediate or serious complications. The only complication was external auditory canal stenosis due to postoperative scarring, which occurred in a patient with a stage-3 cholesteatoma that recurred and required reoperation and second-stage ossicular chain reconstruction. Two more patients underwent one-stage ossicular chain reconstruction. All three patients received a total artificial ossicular replacement prosthesis, and two of them also received cartilage grafts. Conclusions and significance: Transcanal endoscopic ear surgery is an effective method to treat CC belonging to Potsic stages 3 or below.
[Cochlear Implantation with Suprameatal Approach in Chinese Children].

Abstract:OBJECTIVE:To investigate the technique of the suprameatal approach for cochlear implantation in Chinese profound sensory hearing loss children.METHODS:Suprameatal approach for cochlear implantation were used in 50 cases (total 53 ears) with profound sensory hearing loss from May 2005 to January 2007. The electrode was passed through the suprameatal tunnel and went between the incus and chorda tympani into the scala tympani.RESULTS:Electrodes were completely inserted in 51 ears. There were no postoperative complications in all cases. Although the long effect need to be observed, all cases received better hearing and speech development benefit from cochlear implantation in the follow-up period. Among the 50 cases, 26 had speech perception in the open condition; 18 patients could speak short sentences although not clearly; and 6 patients learned to speak individual words only.CONCLUSIONS:The suprameatal approach was found to be a simple and safe technique that does not need mastoidectomy and avoid endangering the facial nerve and the chorda tympani. It enables wide exposure of middle ear and is especially suitable for cases with narrow facial recess or anteriorly located facial nerve.
[Clinical Analysis of Petrous Bone Cholesteatoma: Characteristics, Diagnosis and Treatment].

Abstract:OBJECTIVE:To explore the clinical characteristics, diagnosis and surgical management of petrous bone cholesteatoma.METHOD:The data of 12 patients who underwent surgery for petrous bone cholesteatoma(PBC) were retrospectively analyzed with respects to the clinical characteristics, diagnosis and management.RESULT:Seven cases were characterized by a long otitis media history, severe hearing loss, and facial paralysis. In contrast, 5 cases were characterized with the symptoms of facial paralysis, hearing loss and vertigo attack and the absence of early otorrhea history. Trans labyrinth approach was chosen for 2 cases,while trans labyrinth-cochlear approach for 10. Cochlea was involved in 10 cases, while internal auditory canal in 9 and semicircular canal in 11. Otoscope was used in 1 case. Four patients were treated by partial resection of petrous apex and ear canal closure with good results. After years of follow-up, three recurrence cases were operated for a second time. Simultaneous facial nerve anastomosis or decompression was performed. The function of facial nerve recovered from V-VI to Ill-IV (House and Brackmann grading) in 6 anastomosis cases and from IV-V to II- IlI in 3 cases of decompression.CONCLUSION:Petrous bone cholesteatoma was characterized by severe hearing loss, and facial paralysis. Surgical approaches are decided according to the extent of lesion and hearing status. Our study indicated that otoscope would help to ensure a radical removal of the pathology in cases with deep involvement and restricted vision. Partial resection of petrous bone and ear canal closure could be effective solution for challenging cerebrospinal fluid otorrhea with large dural defects and protecting vital neurovascular structures.
The Misdiagnosis of Congenital Cholesteatoma of the Temporal Bone (5 Case Report)

Abstract:OBJECTIVE:The congenital cholesteatoma of the temporal bone is a rarer disease which might easily be misdiagnosed and wrongly treated. The clinical material and management of five cases are hereby introduced in order to call common attention.METHOD:There were 4 males and 1 female with age rage 10-48 years old. All cases revealed intact drum membrane. The complaints of these 5 cases are: 2 cases suffered from facial palsy; 2 cases from purulent bloody otorrhea; 1 case from otalgia. Among these five congenital cholesteatomas, four cases were originated from petromastoid region and 1 case from undeveloped. external ear canal. Surgical managements were performed after preoperative radiological examination in these five cases.RESULT:In each case, the diagnosis was confirmed by pathological examination and the disease was cured by operation.CONCLUSION:If a patient is suffered from facial palsy or otorrhea or otalgia, but the eardrum is intact, the correct diagnosis and suitable treatment of the congenital cholesteatoma are easily to be delayed and failed. Radiological evaluation, carefully topical examination and paying attention to this disease play important role in the early diagnosis and surgical management, thus the serious complications might be avoided.
Management of the Temporal Bone Fibrous Dysplasia with External Auditory Canal Stenosis and Secondary Cholesteatoma in an Asian Population: A 11-Case Series.

Abstract:OBJECTIVE:This article summarizes the experience of diagnosis and treatment of temporal bone fibrous dysplasia (FD) with external auditory canal (EAC) stenosis and secondary cholesteatoma in the Chinese population, in order to improve the quality of life of patients in the future.METHODS:Eleven patients with FD of the temporal bone who underwent surgery were retrospectively reviewed.RESULTS:All lesions originated from the temporal bone, and all involved of the EAC. There were 11 cases of cholesteatoma in the EAC, 4 cases of cholesteatoma in the middle ear. The most common symptoms were hearing loss (100%), tinnitus (36.4%), and otorrhea (36.4%). Two patients were severe-profound sensorineural hearing loss, and one patient was complicated with subperiosteal abscesses. All 11 patients underwent surgery. There were no perioperative complications in this series and median follow-up time was 4.2 years.CONCLUSION:Temporal bone FD remains a rare diagnosis, especially in the Asian population. The lesions mainly lead to stenosis of the EAC, especially at the osteochondral junction. Cholesteatoma is the main complication of this disease, which is secondary to occlusion of the EAC with the growth of the lesion. Canaloplasty of EAC combined with wide meatoplasty can provide excellent prognosis in most cases.
Initial Exploration on the Diagnosis and Treatment of the Petrous Bone Cholesteatoma

Abstract:Objective To summarize the experience on the diagnosis and treatment of the petrous bone cholesteatoma. Methods Based on the relative literatures, the findings during the operation and postoperation of two cases with petrous bone cholesteatoma were analyzed. Results One case was treated with a supralabyrinthine approach accompaning with facial nerve decompression,and left the petrous cavity exposed,the function of facial nerve was recovered from Ⅵ to Ⅴ of House-Brackmann grading and the hearing result had not been improved; Another case was treated with a translabyrinthine approach accompaining with facial nerve anastomosis,closed the petrous cavity,the function of the facial nerve had not been improved and the hearing had been lost.There had been no evidence of recurrence in two cases up to now. Conclusion The surgical approach must be chosen according to the place of the lesions,exposing and closing of the petrous cavity can be used in proper patients because of their own advantages.

Abstract:Objective: To summarize the clinical features and postoperative efficacy of patients with oval window atresia accompanied by facial nerve aberration. Methods: The clinical data of patients with congenital middle ear malformation with facial nerve aberration admitted to our hospital from January 2015 to March 2023 were retrospectively analyzed. There were 97 cases (133 ears) in total. Among them, 39 patients (44 ears) had complete follow-up data, including 27 male patients and 12 females, aged 7-48 years old, with an average age of 17.8 years old. Of these, 14 cases (16 ears) were patients combined with facial nerve aberration, and 25 cases (28 ears) were without facial nerve aberration. The results of imaging examination, pure-tone audiometry, selection of surgical strategy, intraoperative findings and postoperative hearing improvement were summarized and analyzed. The malformations of malleus, incus, stapes, oval window and facial nerve were recorded. Prism 9 software was used to statistically analyze the mean bone conductance and air-bone gap of patients before and after surgery. Results: All the 14 patients (16 ears) with middle ear malformation accompanied by facial nerve aberration and oval window atresia showed poor hearing and no facial palsy since childhood. High resolution CT (HRCT) examination of temporal bone, pure tone audiometry and Gelle test were performed before surgery. The malformations of malleus, incus, stapes, oval window and facial nerve were recorded. Preoperative high-resolution CT (HRCT) examination of temporal bone found 12 ears with 4 or more deformities, accounting for 75.00%, in the group of patients with facial nerve malformation. The preoperative average bone conductive threshold was (15.3±10.4) dB and the average air-bone gap was (46.3±10.6) dB in pure-tone audiometry (0.5, 1, 2, 4kHz). According to the different degrees of facial nerve and ossicle malformation, we performed three different hearing reconstruction strategies for the 14 patients (16 ears) with facial nerve aberration and oval window atresia, including 7 ears of incus bypass artificial stape implantation, 7 ears of Malleostapedotomy (MS) and 2 ears of Malleus-cochlear-prothesis (MCP). After 3 months to 18 months of follow-up, all patients showed no facial paralysis. The postoperative mean bone conductive threshold was (15.7±7.9) dB and air-bone gap was (19.8±8.5) dB. There were significant differences in mean air-bone gap before and after operation (t=7.766, P<0.05), and there was no significant difference between the mean bone conductive threshold before and after surgery (t=0.225, P=0.824). There was no significant difference of mean reduction of air-bone gap between patients with and without facial nerve aberration (t=1.412, P=0.165). There was no significant difference between the three hearing reconstruction strategies. There was no significant displacement of the Piston examined by U-HRCT. Conclusion: For patients of middle ear malformation whose facial nerve cover the oval window partially, incus bypass artificial stape implantation or Malleostapedotomy (MS) can be selected according to the specific condition of auditory ossis malformation, and for patients whose facial nerve completely covers the oval window area, Malleus-cochlear-prothesis (MCP) can be selected. Three types of stapes surgery are safe and reliable for patients with oval window atresia accompanied by facial nerve aberration. There was no significant difference in efficacy between them. Preoperative HRCT assessment of middle ear malformation is effective. There is no significant difference of surgical effect with or without facial nerve aberration. The U-HRCT can be used to evaluate the middle ear malformation before surgery and the Piston implantation status after surgery. Due to the risks of surgery, those who do not want to undergo surgery can choose artificial hearing AIDS, such as hearing aid, vibrating soundbridge, bone bridge or bone-anchored hearing aid.
