Abstract:Purpose: To introduce the physical background and technical characteristics of lithotripters made in China and to compare the efficacy of shockwave lithotripsy (SWL) for distal-ureteral stones between Chinese and Western lithotripters in consecutive use in our stone center. Patients and Methods: From December 1987 to December 2003, we used a series of four lithotripters to treat 1802 patients with distal-ureteral stones: the piezoelectric EDAP LT-01 with ultrasound B-mode scanning for localization (N = 179); the electrohydraulic JDPN-IV lithotripter using fluoroscopy, which is made in China (N = 294), the electromagnetic Dornier Compact S with fluoroscopy/ultrasound dual localization (N = 1105), and the Huikang MZ-SWL-V, which is an electromagnetic- and electrohydraulic-compatible machine with dual localization that is made in China (N = 224). The physical background and technical characteristics of Chinese lithotripters are different from those of Western machines, but the treatment strategies were identical in all series. Results: In the EDAP LT-01 series, the retreatment rate was 35.2%, and the auxiliary procedure rate was 6.7%, bring the efficiency quotient (EQ) to 69%. In the JDPN-IV series, the retreatment rate was 27.6%, the auxiliary procedure rate 1.0%, and the EQ 74%. In the Dornier Compact S series, the retreatment rate was 12.9%, and auxiliary procedures were performed in 10.9%, for an EQ of 81%. In the Huikang MZ-SWL-V series, the retreatment rate was 20.1%, the auxiliary procedure rate was 4.9%, and the EQ was 80%. Conclusion: The fragmentation results of Chinese electrohydraulic and electromagnetic lithotripters are superior to those of the EDAP LT-01. Chinese lithotripters are inferior to the Dornier Compact S in the efficacy of stone fragmentation, but Chinese machines cause less pain than this lithotripter.

Rock-Breaking Mechanism Study of Axial and Torsional Impact Hammer and Its Application in Deep Wells

Abstract:Abstract A brand new impact hammer–Axial & Torsional impact hammer (producing high-frequency axial and torsional impact forces simultaneously)–has been developed to improve the penetration rates and eliminate stick-slip vibrations in deep and ultra-deep wells. This paper presents its rock-breaking mechanism and evaluates its field application effects. Based on the finite element method, a numerical model was first proposed to investigate the rock-breaking processes of a single PDC tooth under axial, torsional and axial & torsional impact forces respectively. The simulation results show that the axial & torsional impact drilling method can break rock with least mechanical specific energy and increase the rate of penetration (ROP) significantly. The mechanism is that the axial component of the compound impact force can increase the penetration depth of PDC teeth into rock and enlarge the rock-breaking volume, and that the torsional component facilitates to reduce stick-slip vibrations and prohibit premature failures of bits. An Axial & Torsional impact hammer was then developed according to the principle of modulating hydraulic pulses. The hammer has been used in 20 deep and ultra-deep wells in western China, achieving 105.1%~163.4% higher ROP compared with the average ROP in the same block. Besides, the drilling process became more stable and the lives of PDC bits were greatly increased. This paper presents a comprehensive comparison of the penetration rate, torque fluctuation, bit wearing degrees and cuttings shape between this impact tool and other tools used in offset wells. Finally, the structure of the new hammer is simple and reliable, and it is also convenient to maintain.
Comparison of efficacy of German and Chinese electromagnetic shock wave lithotripters: Dornier Compact S vs Huikang MZ-SWL-V.

Abstract:OBJECTIVES To compare the treatment efficacy of Dornier Compact S and a Chinese electromagnetic shock wave lithotripter (Huikang MZ-SWL-V) in a large series. METHODS We retrospectively reviewed the outcome of shock wave lithotripsy (SWL) in 13,954 consecutive patients with upper urinary tract stones from December 1996 to December 2008 with Dornier Compact S (9124 cases) and Huikang MZ-SWL-V (4830 cases). An intramuscular injection of 50 mg pethidine was given to the patients with Dornier Compact S but not to those with Huikang MZ-SWL-V. Treatment outcomes were assessed by the stone-free rate at three months, the retreatment rate, the auxiliary procedure rate, and the effectiveness quotient. The pain scores were evaluated by visual analog scales, where 0 = pain and 10 = unbearable pain. RESULTS The overall stone-free rate at 3 months, retreatment rate, and ancillary procedure rate for Dornier Compact S were 95.9%, 13.4%, and 2.2%, respectively, and they were 85%, 29.6%, and 10.2%, respectively, for Huikang MZ-SWL-V. The overall efficiency quotients of Dornier Compact S and Huikang MZ-SWL-V were 83% and 60.8%, respectively. However, the pain scores in the Dornier group (6-10) were higher than those in the Huikang group (1-5). Three clinically significant subcapsular hematomas were detected in the Dornier group but there were none in the Huikang group. CONCLUSIONS Dornier Compact S is more effective than Huikang MZ-SWL-V, but SWL with Huikang machine could be performed with milder pain and without analgesia, and a lower significant hematoma rate was observed in Huikang group. Abstract:Introduction: Since 1980, extracorporeal shock wave lithotripsy (SWL) has been employed in the treatment of urolithiasis, offering noninvasive alternatives to surgical techniques. In addition to being limited by the size and location of the stones, its efficacy is influenced by several factors. Despite the advancement of other surgical techniques, SWL could maintain its position with new improvements. Our objective is to review the existing literature on the latest advances in the extracorporeal treatment of lithiasis. Material and methods: A non-systematic literature review was carried out from 2017 to 2023 to obtain 26 articles on three different emerging technologies in extracorporeal lithotripsy: Burst Wave Lithotripsy (BWL), Histotripsy, and Microbubble Lithotripsy (ML). Results: The BWL uses sinusoidal bursts of US waves delivered at lower and higher frequencies than conventional SWL. Its mechanism of action generates a higher quality fragmentation (fine fragments) instead of generating tensile stresses for stone fracture resulting in larger fragments, as in traditional SWL. Studies in pigs and humans have shown effective fragmentation with a good safety profile. Based on High Intensity Focused Ultrasound (HIFU) technology, histotripsy fragments tissue through cavitation. Good in vitro results have been shown, but the formation of microbubbles between the stone and ultrasound waves hinders the progress of this technique. Microbubble Lithotripsy (ML) combines microbubbles and ultrasound for safe and effective stone fragmentation. In vitro and pig results are promising. This technique can help optimize treatments and reduce energy levels. Conclusions: Technological innovation is not only being applied to endourological techniques, but also to ESWL. New techniques such as BWL, histotripsy and ML are promising, with good results in the research phase.

Abstract:Bio-effects of non-thermal plasma include lethality of microorganisms and stimulation of mammalian cells,which brings up a series of techniques in disinfection and clinical medicine.In this paper,the applications of non-thermal plasma in medical device sterilization,water and air cleaning,as well as food and package treatment are reviewed.Commercialized plasma sterilizer for medical devices can be classified into vacuum and atmospheric pressure,the volume of the former is much larger while the latter is more simple and easy to operate.For disinfection of water,air and food,it is not yet commercialized due to its energy consumption,efficiency or chemical residues.In the last decade,the developed plasma sources and techniques for medicine are successfully applied to treatment of decayed tooth,skin,wound,and cancer,with some of them approved for clinical practice.In the future,the research focus will be identifying the interactions between plasmas and cells,and developing more effective and practical equipment for different applications.
Improving the lens design and performance of a contemporary electromagnetic shock wave lithotripter

Abstract:The efficiency of shock wave lithotripsy (SWL), a noninvasive first-line therapy for millions of nephrolithiasis patients, has not improved substantially in the past two decades, especially in regard to stone clearance. Here, we report a new acoustic lens design for a contemporary electromagnetic (EM) shock wave lithotripter, based on recently acquired knowledge of the key lithotripter field characteristics that correlate with efficient and safe SWL. The new lens design addresses concomitantly three fundamental drawbacks in EM lithotripters, namely, narrow focal width, nonidealized pulse profile, and significant misalignment in acoustic focus and cavitation activities with the target stone at high output settings. Key design features and performance of the new lens were evaluated using model calculations and experimental measurements against the original lens under comparable acoustic pulse energy (E+) of 40 mJ. The -6-dB focal width of the new lens was enhanced from 7.4 to 11 mm at this energy level, and peak pressure (41 MPa) and maximum cavitation activity were both realigned to be within 5 mm of the lithotripter focus. Stone comminution produced by the new lens was either statistically improved or similar to that of the original lens under various in vitro test conditions and was significantly improved in vivo in a swine model (89% vs. 54%, P = 0.01), and tissue injury was minimal using a clinical treatment protocol. The general principle and associated techniques described in this work can be applied to design improvement of all EM lithotripters.

Abstract:PurposeIntracorporeal lithotripsy is becoming the most commonly used surgical method of stone treatment in Urology. The five major types of intracorporeal lithotripters are ultrasonic, ballistic, and combination lithotripters as well as laser and electrohydraulic lithotripters. The advantages and disadvantages of choosing each of these treatment modalities are reviewed.MethodsExtensive review of literature was performed to identify the types of intracorporeal lithotripters. An investigation was undertaken of the early development of each modality of intracorporeal lithotripsy and/or the mechanism of action. Challenges of each technique were identified and presented. Finally, a determination was made of how these lithotripters compare on the basis of effectiveness of action and cost based on information provided in primary literature as well as previous reviews of these modalities.ResultsContemporary lithotripters have found widespread use in the management of urinary lithiasis. Holmium laser lithotripsy has become one of the most commonly used tools for intracorporeal lithotripsy.ConclusionThere is a wide variety of intracorporeal lithotripters which can be chosen based on the characteristics of each modality and the requirements of the urologist.

Abstract:Objective To evaluate the effectiveness of shock wave lithortripsy (SWL) for treating patients with calculus after failure of endoscopic lithortripsy. Methods From Feb. 2006 to Mar. 2007, 60 patients presented to our department with the upper urinary tract stones. Thirty-seven patients were renal stones and twenty-three were ureteral stones. All patients with average stone burden of 1.5 cm were treated using a Dornier Compact S lithotripter. The total number of shock waves varied from 1 600 to 3 000 and the energy levels ranged from 1 to 6 units. Results For 37 renal stones, the fragmentation was 75.67% (28 cases) after one SWL session, 10.81% (4 cases) after 2 SWL sessions, 5.40% (2 cases) after 3 SWL sessions, and 2.70% (1 case) after 4 SWL sessions. Two patients (5.40%) failed and changed to open surgery. For 23 ureter calculus, the fragmentation was 82.60% (19 cases) after one SWL session and 17.40% (4 cases) after 2 SWL sessions. Conclusion SWL is an effective and reliable treatment for patients with calculus after failure of endoscopic lithortripsy. However, it is very important for doctor to choose appropriate therapy. SWL is a remedy.
Accurate low-energy pulse system for intravascular lithotripsy

Abstract:In response to biomedical imperatives, a novel approach is presented for the endovascular management of atherosclerosis. This approach is grounded in the electrohydraulic effect of a precisely controlled low-energy pulse discharge system, coupled with sequential control within the incomplete discharge electrode, to generate shockwaves aimed at softening blood vessels and rupturing calcified plaques. Theoretical analysis encompassed circuit design, control timing, and operational processes, while experimental investigations demonstrated precise control of non-oscillation and incomplete discharge by adjusting key parameters. Utilizing a 3 kV DC high-voltage power supply, a 220 nF capacitor, and two sets of 3.3 Ω resistors in parallel, with pin–plate electrodes as the load for discharge in normal saline, resulted in a 1 kV reduction in voltage across the capacitor and a peak current of 170 A. Energy release occurred through non-oscillatory and incomplete discharge, effectively generating shockwaves with peak pressures of 5–10 MPa at distances of 1–2 mm from the discharge center to treat calcified lesions. Compared with the existing intravascular lithotripsy devices, the shockwave system realizes accurate control of discharge energy by adjusting the turn-off time of incomplete discharge so as to realize accurate control of shockwave energy. Doctors can choose different intensities of shockwaves according to different degrees of calcification in the treatment process so as to minimize the discomfort caused by shockwaves and improve safety. Valuable insights are provided for intravascular lithotripsy devices in engineering.

Abstract:Extracorporeal Shock Wave Lithotriptors are very popular for the treatment of urinary stones all over the world. They depend basically upon either X-ray fluoroscopy or ultrasound scans to detect the stones before therapy begins. To increase the effectiveness of treatment this study took advantage of both X-ray and ultrasound to develop a dual stone locating system with image processing modules. Its functions include the initial stone locating mode with stone detection by fluorescent images and the follow-up automatic stone tracking mode made by constant ultrasound scanning. The authors have integrated both apparatus and present the operating principles for both modes. The system used two in vitro experiments to justify its abilities of stone location in all procedures.
EMS Lithoclast Trilogy™: an effective single-probe dual-energy lithotripter for mini and standard PCNL

Abstract:Purpose: Several energy sources are available to clear stones during PCNL. Required improvements are faster stone clearance, optimized suction and ease of use while maintaining high patient safety standard. EMS LithoClast® Trilogy, is the first device combining electromagnetic impactor with ultrasonic energy and suction, all-in-one probe. Animal studies and in vitro phantom stone studies have proven safety and efficacy of this device. We aim to study safety and clinical efficacy of Trilogy in our patients. Methods: 31 patients with renal stones were included. Amplatz sheath sizes/Trilogy probe size was 22-28 Fr/10.2 Fr for standard PNL (n = 20) and 15 Fr./5.7 Fr for mini PNL access (n = 11). Analysis was done with respect to demography, stone characteristics, operation duration (total time and lithotripter activation time), post op Hb drop, clearance rate and adverse events. Stone area/volume was calculated based on CT using 3D doctor. Efficacy was determined by stone volume clearance rate (mm3/min). Results: Male:female ratio was 6:5 and 16:4 for mini (MPNL)/standard PNL (SPNL). Stone densities were 1229 ± 206 vs. 1168 ± 344 HU (MPNL vs. SPNL). Mean stone volumes were 3776.1 ± 2132 mm3 for MPNL and 7096 ± 6441 for SPNL. Mean stone volume clearance ratios were 370.5 ± 171 mm3/min and 590.7 ± 250mm3/min for MPNL and SPNL, respectively. Hb drop was 1.24 ± 0.64 g/dL (MPNL) and 1.23 ± 0.89gm/dL (SPNL). Total procedure time/lithotripter activation time was 53.4 ± 23.8/14.7 ± 12.4 min for MPNL and 65.2 ± 23.5/12.0 ± 8.9 for SPNL. Immediate post-operative/1 month stone clearance rates were 93%/96% with one clinically insignificant residual fragment (< 3 mm) and no necessity for auxiliary procedures. No device failure occured and three Clavien grade I and one grade II complications were observed. Conclusion: Swiss LithoClast® Trilogy provides fast stone clearance in standard/mini-PCNL procedures. Ease of use, high tissue safety and optimized suction that avoids fragment blockings are other key features.

Abstract:It is well recognized that the popularity of extracorporeal shock wave lithotripsy (SWL), despite its non-invasive character, has decreased during recent years. This is partly explained by the technological achievements in endoscopy and urologists’ enthusiasm for such procedures. Another explanation is that many urologists have been insufficiently successful with SWL. The latter effect might to some extent be a result of the performance of the lithotripter used, but in too many cases, it is evident that the principles of how shock wave lithotripsy should be carried out are poorly applied. The purpose of this article is to emphasize some important aspects on how SWL best should be used. Based on decades of experience, it stands to reason that success with SWL does not come automatically and attention has to be paid to all details of this technique.

In Vitro Comparison of Two Types of Introcorporeal Ultrasound Lithotripsy Device (CQS-01 and EMS)

Abstract:Objective To assess the efficacy of stone fragmentation and clearance of this new intracorporeal ultrasound lithotripter (CQS-01) compared with currently available ultrasound units (EMS-Ⅲ/Ⅳ).Methods Twenty phantom stones composed of dental gypsum were randomly divided into four groups,and CQS-01 ultrasound lithotripter (70％ power and 70％ duty factor),EMS-Ⅲ ultrasound lithotripter (70％ power and 70％ duty factor),EMS-Ⅳ ultrasound lithotripter (type A,70％ power and 70％ duty factor) and EMS-Ⅳ ultrasound lithotripter (type B,70％ power and 100％ duty factor) were used to fragment and removepbantom stones.The mean stone breakdown time and fragment removal time and stone fragmental sizes for the standard ultrasound devices were compared to determine the completeness and efficiency of stone fragmentation and removal.Results The average time for stone breakdown was 7.4 ± 1.9 s,9.4 ± 1.6 s,82.2 ± 12.6 s and 51.4 ± 18.7 s,respectively.There was no significant difference between CQS-01 and EMS-Ⅲ (P ＞ 0.05),but there was significant difference between CQS-01 and EMS-Ⅳ (A or B) (P ＜ 0.001).The average time for stone clearance using the ultrasound devices was 387.8 ± 68.0 s,41 1.6 ± 57.6 s,568.0 ± 119.1 s and 383.6 ± 75.6 s,respectively.In addition,the average size of the largest fragments removed was the same among the groups (＜ 3 mm).Conclusion The ultrasound capabilities in a newly developed lithotriter (CQS-01) exhibited the same ability to fragment and clear phantom stones compared with standard ultrasound devices.

Abstract:The principle of the vibration drilling technology is introduced. The domestic and foreign status of the research and development on vibration drilling micro-hole is summarized, and the applying prospect and the existing problems are also analyzed. To solve the problems such as tool wear, machining accuracy, and surface quality of the micro-hole, a new scheme of electric hot vibration compound drilling is presented. Thus it can improve the machining efficiency and quality of the micro-hole.

Abstract:Objective To perspectively compare the clinical outcomes of China-and Germany-made electromagnetic lithotripters in the treatment of upper urinary tract calculi.Methods Between January 2005 and August 2005,60 cases of upper urinary tract calculi were randomly divided into two groups:China-made and Germany-made groups(30 in each).Extracorporeal shock wave lithotripsy(ESWL)was performed by a same group of surgeons.Results The stone-free rate was 73%(22/30)in China-made group,and was 90%(27/30)in Germany-made group 3 weeks after ESWL(χ2=2.283,P=0.095);and was 100% in both the groups 3 months postoperation.The retreatment rate was 27%(8/30)in China-made group,and 10%(3/30)in Germany-made group(χ2=2.283,P=0.095).The rate of auxiliary procedure in both the groups was 0.The efficacy quotient(EQ)was 0.79 in China-made group,and was 0.91 in Germany-made group.Totally,39 ESWLs were performed on the patients in China-made group,while 33 ESWLs were carried out in Germany-made group.The rates of macroscopic hematuria,skin injury,and renal colic were 87%(34/39),64%(25/39),and 8%(3/39)respectively in China-made group,and were 91%(30/33),6%(2/33),and 6%(2/33)in Germany-made group(χ2=0.016,P=0.900;χ2=25.693,P=0.000;and χ2=0.000,P=1.000;respectively).One patient in China-made group developed fever after ESWL,while no one in Germany-made group had the symptom.In both the groups,no patients had hematuria,renal edema,or subcutaneous perinephric hemorrhage.Conclusion Both China-and Germany-made electromagnetic lithotripters are effective,safety,and non-invasive.

Abstract:Over the last decade, urinary lithiasis' prevalence has dramatically increased due to diet and lifestyle changes, growing 10.6% and 7.1% in men and women respectively. Extracorporeal shock wave lithotripsy has lost relevance in current practice due to endoscopic device development and unpredictability of results. Instrument miniaturization is leading to an increase of the percutaneous approach of increasingly smaller stones, while most flexible ureteroscopes durability and digitalization has allowed urologists to address larger stones. So that, decision algorithm is now impossible to define, but what is clear is that ESWL has declined worldwide. Can it disappear as a urinary lithiasis treatment modality? If we don't improve appropriate candidate selection and optimize disintegration efficiency, guidelines are going to replace the more "boring" ESWL by popular and more attractive endoscopes. Shock wave technology has evolved in the last two decades, however lithotripsy fundamental principle has not changed. ESWL has passed the test of time and centers dedicated to stone treatment should have a lithotripter in order to offer an appropriate balance in different options for different clinical situations. New developments will be focused on improvements in location (in-line navigation systems; Vision track system) and automatic ultrasound location on a robotic arm; monitoring and stone fixation, implementation of different focal sizes with new acoustic lenses, multitask working stations that allow endourological approach, coupling control (avoiding microbubbles) and low cost devices for different applications. On the other hand, optimizing outcomes by: slower pulse rates, ramping strategies and patient selection with soft stones, short stone-skin distance, low BMI and favorable collecting system anatomy, allow us to achieve better outcomes in shock wave treatments. SWL still represents a unique non invasive method of stone disease treatment with no anesthesia and low complication rates; and a high proportion of stones could still be treated with shock waves and remains among patient's first options. This update objective has been to review the evolution, identify shock wave new developments and clarify their impact on our daily practice in urinary stones treatment.
