Effect of Local Administration of Mecobalamin at the Nerve Suture Site:An Experimental Study

Abstract:Objective To investigate the effect of continuous local administration of mecobalamin at the nerve suture site on the nerve regeneration. Methods 60 New Zealand rabbits were involved in the study. The right sciatic nerves were severed and sutured directly. They were divided into 3 groups according to different medication, with 20 in each. In group A, an elastic infusion pump with mecobalamin solution (100 μg -1 ·d -1 × 5 d ) was connected with the nerve suture site. In group B muscle injection of mecobalamin (100 μg -1 ·d -1 × 5 d ) were done. Group C was treated at the same way as group A except using normal saline as control. The nerve regeneration was evaluated by morphology, electrophysiology and histology study at postoperative 6 and 12 weeks.Results Morphological study showed the quality of nerve regeneration in group A was superior ,and in group C inferior to group B. Electrophysiological results showed that the latency of motor nerve action potential(MNAP) in group A was shorter ,in group C longer than group B, while the amplitude of MNAP in group A was higher, in group C lower than group B. The lateral neucleus of spinal anterior horn at both sides and spinal ganglion (ACP staining) showed that the percentage of positive granule areas in group A was superior to those in group B and C. Conclusions Continuous local administration of Mecobalamin by elastic infusion pump at the nerve suture site could obviously enhance peripheral nerve regeneration ,whose treatment outcome was superior to that of injection of mecobalamin.
