Interventional effects of mecobalamin on spinal neuronal apoptosis and pain in neurogenic pain rats

Abstract:AIM: By ligating sciatic nerve of rats, to establish neurogenic pain models, explore the effect of mecobalamin on spinal neuronal apoptosis and ache after ligation in neurogenic pain rats. To analyze its pathway from the angle of drug intervention on the expression of pro-inflammatory cytokines in order to provide experimental evidence in the treatment of neurogenic pain. METHODS: The experiment was conducted at the center for animal experiment of Renmin Hospital, Wuhan University from October 2003 to January 2004. Sixty-six healthy adult male Wistar rats were selected and assigned randomly into 3 groups: sham-operated group (n=6), saline group (n=30) and mecobalamin group (n=30). ①Sciatic nerve ligature was performed in the saline group and the mecobalamin group with Bennett and Xie model. It was required that only retained constriction, but not blocked the blood supply of nerve. The sciatic nerve of the rats in the sham-operated group were exposured only, no ligation. ②After ligation the rats in the mecobalamin group were injected with 0.25 mL 600 μg/kg mecobalamin through intraperitoneal injection, for 3 days. The same volume saline was given at the same time point in the sham-operated group and the saline group. ③Praxiology determination was performed at 6 hours, 1, 3, 7 and 14 days before and after operation in every group. Liminal values of withdrawing foot and flipping tail in the hot water were recorded. Expressions of tumor necrosis factor (TNF) and interleukin 6 were detected with immunohistochemical method and imaging analysis system. The apoptosis of spinal neuron was observed by TUNEL and changes of spinal pathomorphism were measured. RESULTS: A total of 66 selected experimental rats were involved in the result analysis. ①Changes of liminal values of withdrawing foot and flipping tail in the hot water of rats at different time point after the legation on sciatic nerve: Compared with the sham-operated group, mechanical pain sensitivity and hot induced ache appeared in the rats of saline group and the mecobalamin group at 6 hours after operation (P < 0.05). The liminal values of withdrawing foot and flipping tail in the hot water of rats at 14 days after operation recovered to the base line, while it was low in the saline group (P < 0.01). ②Pathological changes of spinal in every group after ligation on sciatic nerve: There was insignificant change in the sham-operated group. Nissl's body in the neurons of spinal cord gray matter in the saline group after ligation on sciatic nerve showed central or peripheral lysis, and the number of cells decreased significantly, and the spinal cord white matter became edema. The pathological change in the mecobalamin group was similar to that of the saline group, but the swelling level of neurons and reduction degree were light as compared with the saline group (P < 0.05). ③Changes of the expression of TNF and interleukin 6 in spinal cord of rats in every group after ligation: There were a few expressions in the sham-operated group. The expressions of TNF and interleukin 6 were higher significantly than those in the sham-operated group at 6 hours, 1 and 3 days after ligation in the saline group (P < 0.01). The expressions became weak obviously at the 14th day after injury, and closed to normality. While the expression of TNF and interleukin 6 could be inhibited in the mecobalamin group, and had significant difference at every time point in the saline group (P < 0.01 or 0.05). ④Change of apoptosis index of spinal neurons in every group after ligation: Few tagged positive cells appeared at 6 hours and 1 day after operation in the saline group, and reached the peak at 3 days after operation. There were few expressions at 14 days after operation. The positive neural cells in spinal cord of rats of the mecobalamin group were less markedly than those in the saline group at every time point (P < 0.01), and reached the peak at 3 days after injury. The apoptosis index had positive correlation with ache, TNF and interleukin 6 expression (r=0.9,P < 0.01=. CONCLUSION: Sciatic nerve ligation can lead to spinal neuronal apoptosis and the overexpression of neurogenic cytokines, and is associated with the development and maintenance of neuropathic pain. The mecobalamin can protect the spinal of rats, which can be related to the inhibition of expression of neurogenic cytokines and spinal neuronal apoptosis and pain reduction.
