Abstract:Health equity is the basic theory of universal health insurance and the objective of reform of the medical welfare system. Based on health equity theory with the perspective of giving priority to disadvantaged groups, this article takes Zhejiang province as an example and adopts a mixed-method approach to explore the effects of the reform of the medical welfare system on health equity for older people. The structural equation model (SEM)-based nonrecursive path analysis demonstrates an improvement of health equity for insured elders of the Basic Medical Insurance for Employees (BMIE) and the Urban Basic Medical Insurance for Residents (UBMIR) as a result of an increased utilization of healthcare. However, there is little difference in terms of the utilization of healthcare and health outcomes between old people insured by the New Rural Cooperative Medical System (NRCMS) and those without medical insurance. This results from the low-level benefits of the insurance policy and defective policy design. A qualitative analysis further discovers the insufficient protection of health equity for several types of vulnerable aged groups in the process of healthcare delivery. Finally, this study advocates the perspective of health equity for vulnerable groups, a transition from emphasizing opportunity equity to stressing outcome equity, and the construction of universal health insurance and long-term care system for the disabled elderly.

Abstract:Introduction:Equity is the core of primary care. The issue of equity in health has become urgent and China has attached increasing attention to it. With rapid economic development and great change of the policy on medical insurance, the pattern of equity in health has changed a lot. Reform of healthcare in Zhejiang province is at the forefront of China; studies on Zhejiang are of great significance to the whole country. This paper aims to measure the equity in health from the perspectives of health needs and health seeking behavior, and provides suggestions for decision making.Methods:A household survey was conducted in August 2016. A sample of 1000 households, 2807 individuals in Zhejiang was obtained with the multi-stage stratified cluster sampling method. Descriptive analysis and Chi-square test were adopted in the analysis. The value of concentration index was used to measure the equity.Results:This study finds that the poor have more urgent health needs and poorer health situation compared with the rich. The utilization of outpatient services was almost equal, whilst the utilization of hospitalization was pro-rich (the rich use more).Individuals with employer-based medical insurance use more outpatient services than those with rural and urban medical insurance. Compared to the rich, there were more people in the poorer income groups who didn't use inpatient services due to financial difficulties.Conclusions:The issue of equity in health has attracted broad attention in the world, and China is no exception. We measured and analyzed the equity of health needs and absent rate of health services. We find that the poor have more urgent health needs and high absent rate of inpatient services compared with the rich. Income level and medical insurance may well explain the equity of outpatient and inequity of hospitalization. In view of the pro-rich inequity of hospitalization, more financial protection should be provided for the poor.

Abstract:Objective: To evaluate the equity in hospitalized service utilization among China's urban residents to discuss the influence of urban resident basic medical insurance on equity of hospitalized service utilization.Methods: Using standardization and decomposition of concentration index.Results: Results show that the average number of need-but-not hospitalization of urban residents form 2007 to 2010 was declining in a downward trend.The corresponding standardization concentration indexes were 0.195,-0.218,0.160 and-0.212.Statistics differences were significant on 1% level.This fact indicates that the inequity still existed in hospitalized service utilization and the poor were more likely to miss necessary inpatient services.Personal income and urban resident basic medical insurance were two major factors that could improve the equity in hospitalized service utilization.Conclusion: The paper argued that urban resident basic medical insurance has essential effects on the equity of hospitalized service utilization.Yet,the effectiveness of the issue was limited.In order to guarantee people's basic right of improving health and access to medical services,it is suggested that a comprehensive reform,including financial and delivery system of health services,should be carried out.

Abstract:The equality of public health services is a current research focus in present socioeconomic life,which is also one of the five major tasks mentioned in the newly published government document "Opinions of Deeping the Health Care System Reform" within the next three years.In this article,the authors discuss the connotation of the equality in public service,analyze and summarize the status quo,issues and the influence of public finance system on equality.It recommends that government should take more responsibilities in public financing for realizing the equality of public health services.Meanwhile,correlative legislation must be reinforced.

Abstract:The will and capacity of the Chinese government in assuming healthcare functions have changed greatly since the reform. This paper explores the impact of such changes on equity in the financing and provision of service in healthcare in urban China. The author briefly traces how China's urban health insurance has evolved under acute fiscal stress, investigates how the shift of government policy in relation to public healthcare agencies has affected the behaviors of healthcare providers, and explains some phenomena such as unequal healthcare financing and rising costs from the perspectives of demand and supply. Based on three National Health Services Surveys (1993, 1998, 2003), the author empirically examines the degree of inequality in healthcare financing and delivery in urban China. The article concludes that the market-oriented health reform in urban China has exacerbated the cost problem that it intended to solve, reduced access to health services for the most vulnerable, and increased the instances of illness-induced poverty. To solve the problem, the government should reorient its policies and increase its extractive capacity, so as to play a more important role in restoring an equal and accessible basic healthcare system that covers all residents.

Abstract:Based on the investigation of the current situation of demand and utilizion of health service in rural residents of the mountainous area,the study finds the main obstacle to transform health service demand into rational health service utilization is lack of money and unavailability of health resources.The major causes to dissatisfaction with health service is the insufficiency of the medical equipment and the high fee for service.To allocate community health service organization in rural areas,to improve its service quality,to perfect the fee compensation mechanism in the new rural CMS are good ways for rural residents utilizing health service.

Abstract:It is now about 15 years since the introduction of the market into health care in China. This produced fundamental changes in the way that health care is financed and resulted in the disappearance of universal free basic health care. Responsibility for provision of health services has been devolved to the provincial and county governments, and healthcare providers have been given considerable financial independence. A fee for service system has been introduced, and several different payment mechanisms are now in operation. The new financing and pricing structures are responsible for greater inequity of access to services and more inefficient use of resources. These problems are widely acknowledged, and a range of solutions is being developed and tested. Since the introduction of the reforms the measurable health status of the population has not declined, probably as a result of overall improved socioeconomic conditions and a continued emphasis on prevention.

Abstract:Background Getting medical treatment is still difficult and expensive in western China. Improving the equity of basic health services is one of the tasks of the new healthcare reform in China. This study aimed to analyze the parallel and vertical equity of health service utilization of urban residents and then find its influencing factors. Methods In August 2011, a household survey was conducted at 18 communities of Baoji City by multi-stage stratified random sampling. Based on the survey data, we calculated a concentration index of health service utilization for different income residents and a difference index of different ages. We then investigated the influencing factors of health service utilization by employing the Logistic regression model and log-linear regression model. Results The two-week morbidity rate of sampled residents was 19.43%, the morbidity rate of chronic diseases was 21.68%, and the required hospitalization rate after medical diagnosis was 11.36%. Among out-patient service utilization, the two-week out-patient rate, number of two-week out-patients, and out-patient expense had good parallel and vertical equity, while out-patient compensation expense had poor parallel and vertical equity. The inpatient service utilization, hospitalization rate, number of inpatients, days stayed in the hospital, and inpatient expense had good parallel equity, while inpatient compensation expense had poor parallel equity. While the hospitalization rate and number of inpatients had vertical equity, the days stayed in hospital, inpatient expense, and inpatient compensation expense had vertical inequity. Conclusions Urban residents' health was at a low level and there was not good health service utilization. There existed rather poor equity of out-patient compensation expense. The equity of inpatient service utilization was quite poor. Income difference and the type of medical insurance had great effects on the equity of health service utilization.

Abstract:This paper investigates equity with respect to one component of welfare in China--the provision and use of health services. Based upon a large-scale survey of almost 16,000 individuals in eight provinces in China, we examine a sub-sample of working-age adults who have identified themselves as injured or ill during the four weeks prior to being interviewed. We found that, beyond the level of severity of the reported condition, very few individual-level factors are related to the use of services when ill or injured. Only gender (female) and employment in state-run enterprises are associated with higher patterns of use. These results suggest that China has achieved a very wide distribution of clinics and other services at the local level, and that they are widely used by those who identify need for them. It is rare to be over half an hour away by bike from some form of care and the majority of care appears to be reasonably inexpensive. This broad availability of services contrasts with recent reports from China stressing declining accessibility, and paints a picture of relatively equal access to health care.

Abstract:Countries worldwide are making efforts to achieve health equity. China focuses on the implementation of the policy goal of “improving the primary level” to eliminate the health equity gap. The main purpose of this study is to examine the effects of the healthcare reforms at the primary level in China and to analyze the key factors that can help to improve their effectiveness. From the perspectives of the policy attention mechanism and public policy analysis, this study will explore primary care reforms from policy formulation to policy implementation on the basis of grounded theory and empirical research on primary care reforms in Shenzhen, China, that was conducted from 2018 to 2019. The present study found that the government pays close attention to the medical level and service level of primary care services at the policy formulation phase but less attention to talent level and information sharing. At the same time, this study combined with empirical data from primary care centers in Shenzhen for the period covering 2018 to 2019 evaluates policy implementation and its effect. Multiple regression analysis revealed that the medical level, talent level, service level, and information sharing helped to develop primary care services and improved health equity. Nevertheless, this study reflects a deviation between policy formulation and policy implementation for the development of primary care policies. Empirical experience shows that the development of talent level and information sharing can significantly promote primary care services and health management. Therefore, this study implies that in the process of promoting the health equity at the primary level, more attention should be paid to the consistency between policy formulation and policy implementation. Additionally, the policy promotion and influence mechanism can be improved, particularly in terms of talent development and information sharing, in order to effectively promote the development of health equity at the primary level.

A Blueprint of China's Health Reform and Development

Abstract:Solving the present issue of the hardship and expensive to seek medical care is related to the access, equity and cost of health service. But it is not the final outcomes of health reform and development. It reflects the cause of institutional issue in China. The paper puts forward a blueprint of China's health reform and development. It contains one goal: universal coverage of basic health service, strengthening three systems: basic medical insurance system, basic healthcare system and basic essential drug system as well as six strategic measurements. In the future 15 years, China will under the ideology of people-oriented and scientific development develop a harmonious society and socialist "Xiao Kang" society. Universal coverage of basic health service is the vision of Chinese population. To strengthening the responsibility of government and promoting the equity of health service, the author suggests making the universal primary education and universal coverage of basic health service as two basic social policies. However, the goal of universal coverage of basic health service is a long-term process, it requires mobilizing community involvement and coping with the social and economic development.

Abstract:Shanghai is facing three underlying problems in the health reform. First is that the relationship between government and market in the health sector is unclear, second is that the environment for the health settings , owners of all kinds to deliver medical care needs to be optimized, and third is that the system and mechanism of public hospitals needs to be improved. Targeting the above three problems, the paper proposes ideas and policy suggestions to deepen Shanghai health sector reform.

Abstract:In India, despite improvements in access to health care, inequalities are related to socioeconomic status, geography, and gender, and are compounded by high out-of-pocket expenditures, with more than three-quarters of the increasing financial burden of health care being met by households. Health-care expenditures exacerbate poverty, with about 39 million additional people falling into poverty every year as a result of such expenditures. We identify key challenges for the achievement of equity in service provision, and equity in financing and financial risk protection in India. These challenges include an imbalance in resource allocation, inadequate physical access to high-quality health services and human resources for health, high out-of-pocket health expenditures, inflation in health spending, and behavioural factors that affect the demand for appropriate health care. Use of equity metrics in monitoring, assessment, and strategic planning; investment in development of a rigorous knowledge base of health-systems research; development of a refined equity-focused process of deliberative decision making in health reform; and redefinition of the specific responsibilities and accountabilities of key actors are needed to try to achieve equity in health care in India. The implementation of these principles with strengthened public health and primary-care services will help to ensure a more equitable health care for India's population.

New evidence on financing equity in China's health care reform - A case study on Gansu province, China

Abstract:Background In the transition from a planned economy to a market-oriented economy, China’s state funding for health care declined and traditional coverage plans collapsed, leaving China’s poor exposed to potentially ruinous health care costs. In reforming health care for the 21st century, equity in health care financing has become a major policy goal. To assess progress towards this goal, this paper examines the equity characteristics of health care financing in a province of northwestern China, comparing the equity performance between urban and rural areas at two different points in time. Methods Analysis of whether health care financing contributions were progressive according to income were made using the Kakwani index for each of the four health care financing channels of general taxes, public and private health insurance, and out-of-pocket payments. Two rounds of surveys were conducted, the first in 2003 (13,619 individuals in 3946 households) and the second in 2008 (12,973 individuals in 3958 households). Household socio-economic, health care payment, and utilization information were recorded in household interviews. Results Low-income households have undertaken a larger share of the health care financing burden in recent years, reflected by negative Kakwani indices, which indicate a regressive system. We found that the indices for general taxation were −0.0024 (urban) and −0.0281 (rural) in 2002, and −0.0177 (urban) and −0.0097 (rural) in 2007. Public health insurance presented different financing distributions in urban and rural areas (urban: 0.0742 in 2002, 0.0661 in 2007; rural: –0.0615 in 2002,–0.1436 in 2007.). Out-of-pocket payments were progressive but not equitable. Public health insurance coverage has expanded but financing equity has decreased. Conclusions Health care financing policies in China need ongoing reform. Given the inequity of general consumption taxes, elimination of these would improve financing equity considerably. Optimizing benefit packages in public health insurance is as important as expanding coverage, both for health care financing and for utilization management as well. Although they are progressive, out-of-pocket payments are not equitable in China and have the effect of excluding the poor from health care as they cannot afford to pay for medical care and so withdraw from treatment.
