Abstract:Background Although public medical insurance covers over 95% of the population in China, disparities in health service use and out-of-pocket (OOP) health expenditure across income groups are still widely observed. This study aims to investigate the socio-economic disparities in perceived healthcare needs, informal care, formal care and payment for healthcare and explore their equity implication. Methods We assessed healthcare needs, service use and payment in 400 households in rural and urban areas in Jiangsu, China, and included only the adult sample ( N = 925). One baseline survey and 10 follow-up surveys were conducted during the 7-month monitoring period, and the Affordability Ladder Program (ALP) framework was adopted for data analysis. Negative binomial/zero-inflated negative binomial and logit regression models were used to explore factors associated with perceived needs of care and with the use of self-treatment, outpatient and inpatient care respectively. Two-part model and logit regression modeling were conducted to explore factors associated with OOP health expenditure and with the likelihood of incurring catastrophic health expenditure (CHE). Results After adjusting for covariates, rural residence was significantly associated with more perceived healthcare needs, more self-treatment, higher probability of using outpatient and inpatient service, more OOP health expenditure and higher likelihood of incurring catastrophic expenditure ( P < 0.05). Compared to the Urban Employee Basic Medical Insurance (UEBMI), enrollment in the New Rural Cooperative Medical Scheme (NRCMS) or in the Urban Resident Basic Medical Insurance (URBMI) was correlated with lower probability of ever using outpatient services, but with more outpatient visits when people were at risk of using outpatient service ( P < 0.05). NRCMS/URBMI enrollment was also associated with higher likelihood of incurring CHE compared to UEBMI enrollment (OR = 2.02, P < 0.05); in stratified analysis of the rural and urban sample this effect was only significant for the rural population. Conclusions The rural population in Jiangsu perceived more healthcare needs, had a higher probability of using both informal and formal healthcare services, and had more OOP health expenditure and a higher likelihood of incurring CHE. The inequity mainly exists in health care financing, and may be partially addressed through improving the benefit packages of NRCMS/URBMI.

Abstract:Introduction:Equity is the core of primary care. The issue of equity in health has become urgent and China has attached increasing attention to it. With rapid economic development and great change of the policy on medical insurance, the pattern of equity in health has changed a lot. Reform of healthcare in Zhejiang province is at the forefront of China; studies on Zhejiang are of great significance to the whole country. This paper aims to measure the equity in health from the perspectives of health needs and health seeking behavior, and provides suggestions for decision making.Methods:A household survey was conducted in August 2016. A sample of 1000 households, 2807 individuals in Zhejiang was obtained with the multi-stage stratified cluster sampling method. Descriptive analysis and Chi-square test were adopted in the analysis. The value of concentration index was used to measure the equity.Results:This study finds that the poor have more urgent health needs and poorer health situation compared with the rich. The utilization of outpatient services was almost equal, whilst the utilization of hospitalization was pro-rich (the rich use more).Individuals with employer-based medical insurance use more outpatient services than those with rural and urban medical insurance. Compared to the rich, there were more people in the poorer income groups who didn't use inpatient services due to financial difficulties.Conclusions:The issue of equity in health has attracted broad attention in the world, and China is no exception. We measured and analyzed the equity of health needs and absent rate of health services. We find that the poor have more urgent health needs and high absent rate of inpatient services compared with the rich. Income level and medical insurance may well explain the equity of outpatient and inequity of hospitalization. In view of the pro-rich inequity of hospitalization, more financial protection should be provided for the poor.

Abstract:Health equity is the basic theory of universal health insurance and the objective of reform of the medical welfare system. Based on health equity theory with the perspective of giving priority to disadvantaged groups, this article takes Zhejiang province as an example and adopts a mixed-method approach to explore the effects of the reform of the medical welfare system on health equity for older people. The structural equation model (SEM)-based nonrecursive path analysis demonstrates an improvement of health equity for insured elders of the Basic Medical Insurance for Employees (BMIE) and the Urban Basic Medical Insurance for Residents (UBMIR) as a result of an increased utilization of healthcare. However, there is little difference in terms of the utilization of healthcare and health outcomes between old people insured by the New Rural Cooperative Medical System (NRCMS) and those without medical insurance. This results from the low-level benefits of the insurance policy and defective policy design. A qualitative analysis further discovers the insufficient protection of health equity for several types of vulnerable aged groups in the process of healthcare delivery. Finally, this study advocates the perspective of health equity for vulnerable groups, a transition from emphasizing opportunity equity to stressing outcome equity, and the construction of universal health insurance and long-term care system for the disabled elderly.

Abstract:Objective To measure the equity of resources allocation in obstetric healthcare in Zhejiang Province, and provide evidence for decision making by healthcare departments of the government. Methods Lorenz curve and Gini coefficient were used to measure the equity of obstetric healthcare resources. Results The investigation found such allocation as equitable as measured by the layout of the number of live births, as the Gini coefficient of the distribution of obstetric doctors, nurses and beds was 0.1697,0.1971 and 0.1497 respectively by total birth, failing within the equitable range. However, when measured by geographic distribution, such coefficient were 0.2701, 0.3216 and 0.3341 respectively in cities, which were less equitable yet still within equitable range. Conclusions The government and health bureaus are recommended to scientifically predict the number of immigrants and rationally deploy healthcare resources according to the service radius of healthcare resources;moreover, more gynecologic doctors and nurses should be trained to improve the human resource structure in gynecologic departments. These effort can cater to the growing healthcare demands of the society resulting from a growing population of immigrants.
Study on the Equity of Health Service of New Rural Cooperative Medical Scheme in Shandong and Ningxia

Abstract:Objective:To explore the equity of health service among different income groups in Shandong and Ningxia. Methods:Using the data of the investigation from 2006 to 2008 and analyzing the date with rate,ratio,chi-square test,concentration index(CI). Results:The treatment-seeking rate among various income groups were 76.0%,79.7%,75.8%,80.0% and 79.7%,and the CI was 0.017 908. There was no significant difference among various income groups. The hospitalization rates were 8.9%,7.3%,6.2%,6.0% and 5.8% in five groups,and the CI was -0.017 44. Compared to 2006,the treatment-seeking rate and hospitalization rate had improved 32.9% and 1.4%. The prevalence rates in four weeks were 25.9%,25.2%,21.7%,19.6%,and 20.6%,and the rates of chronic disease were 24.7%,21.9%,19.9%,16.6% and 7.4% in five income groups. The CI were-0.037 68 and-0.046 28 respectively. The proportion of medicine to net income was 67.3% in the lowest income groups and 2.8% in the highest income groups. Conclusion:The health service utilization had increased,but the unfairness still existed in collection funds,health and health service utilization among different income groups.
The Impact of Expanded Health System Reform on Governmental Contributions and Individual Copayments in the New Chinese Rural Cooperative Medical System

Abstract:In 2002, the Chinese central government created a new rural cooperative medical system (NCMS), ensuring that both central and local governments partner with rural residents to reduce their copayments, thus making healthcare more affordable. Yet, significant gaps in health status and healthcare utilization persisted between urban and rural communities. Therefore, in 2009, healthcare reform was expanded, with (i) increased government financing and (ii) sharply reduced individual copayments for outpatient and inpatient care. Analyzing data from China's Ministry of Health, the Rural Cooperative Information Network, and Statistical Yearbooks, our findings suggest that healthcare reform has reached its preliminary objectives-government financing has grown significantly in most rural provinces, especially those in poorer western and central China, and copayments in most rural provinces have been reduced. Significant intraprovincial inequality of support remains. The central government contributes more money for poor provinces than for rich ones; however, NCMS schemes operate at the county level, which vary significantly in their level of economic development and per capital gross domestic products (GDP) within a province. Data reveal that the compensation ratios for both outpatient and inpatient care are not adjusted to compensate for a rural county's level of economic development or per capita GDP. Consequently, a greater financial burden for healthcare persists among persons in the poorest rural regions. A recommendation for next step in healthcare reform is to pool resources at prefectural/municipal level and also adjust central government contributions according to the GDP level at prefectural/municipal level.

Abstract:Objectives: To evaluating the ability of New-type Rural Cooperative Medical System(NCMS) of Zhejiang province to improve income equity of residents in rural areas. Methods: Making use of the principle of Gini coefficient and Lorenze curve, we analyzed the influence of financing, medical expense and reimbursement of NCMS on income equity of residents in rural areas. Results: Financing of NCMS increased coefficient of Gini by 1.7 percent; Coefficient of Gini decreased by 42.6% after reimbursement of NCMS. Conclusions: Economy burden of diseases increase income inequity of residents in rural areas; Reimbursement of NCMS improved income equity of residents in rural areas at quite a bit extent.

Abstract:Objective:To measure the fairness of health financing in rural areas of China after implementing new rural cooperative medical system(NCMS).Methods:Through field investigation, the date of health service utilization and health financing of 4 295 families in three sample areas of Zhejiang, Gansu and Chongqing in 2012 were collected. Health Financing Contribution(HFC) and Fairness of Financial Contribution(FFC) were used to estimate the equity of family health financing.Result:The HFC of rural families in China was 0.195, and FFC was 0.774, which was improved compared with 2000. However, the differences between eastern areas and western areas varied obviously.Conclusions:There was improvement in the equity of health financing after NCMS, however, the total level was relatively low and differences among areas were large.

Abstract:BackgroundMany countries are developing health mechanisms to pursue the goal of universal coverage. In China, a rural health insurance system entitled New Cooperative Medical System (NCMS) has being developed since 2003. This paper aims to explore the changes in the health service needs and utilization among rural residents in Ningbo, China after the implementation of the new rural cooperative medical system (NCMS), and provide evidence to further improve the strategies of NCMS in China.MethodsStratified multistage cluster sampling was used to randomly select 10 villages from 5 townships in Yuyao and Fenghua counties of Ningbo Municipality. Eighty families were selected from each village, and face-to-face interviews were conducted by trained investigators to collect data using questionnaires.ResultsThe two-week visiting rate and prevalence of chronic diseases among the farmers included in the study was 25.40 and 22.50 %, respectively, which were higher than the levels in 2003 and 2008. The rate of not visiting the healthcare facility amongst those with illness, and the rate of non- hospitalization amongst those who required it were 32.36 and 0.60 %, respectively, which was lower than the levels in 2003 and 2008. Most of the outpatient visits were to the village clinics, while the hospitalizations were mainly to county hospitals.ConclusionNCMS greatly affected the utilization of healthcare services from outpatient clinics and improved the hospitalization rate in county hospitals. Financial difficulties are not the major causes of non-hospitalization and non-visiting any longer. These findings suggest that the NCMS policies alleviated the medical burdens of farmers in a certain degree.

Choice of and Equity in First-Contact Care in China’s Zhejiang and Qinghai Province: A Comparative Study

Abstract:Abstract Background The imbalance between health needs and healthcare resources has become more serious in China. The hierarchical diagnosis and treatment system has played an important role in solving the problem, one goal of which was to increase the use of basic-level medical facilities. This study aimed to compare the choice of and equity in first-contact care among residents in Zhejiang and Qinghai provinces, China. Methods First-hand data were obtained through a cross-sectional survey of residents of Zhejiang and Qinghai. We first compared the proportion of residents who select primary healthcare institutions as the setting for first-contact care. Then we compared horizontal equity using the concentration index decomposition method. An urban-rural subgroup analysis was carried out for both provinces. Results A total of 4899 residents were enrolled, 2872 from Zhejiang and 2027 from Qinghai. The proportion of residents selecting a PHI as their first-contact care provider was 82.5% in Qinghai, and 54.4% in Zhejiang. The horizontal inequity index of selecting a PHI as first-contact care was − 0.180 and 0.007 in Zhejiang and Qinghai, respectively. The HI was − 0.088, -0.160, 0.069, and − 0.058 in rural Zhejiang, urban Zhejiang, rural Qinghai, and urban Qinghai, respectively. Conclusions The proportion of residents selecting a PHI as their first-contact care provider in Zhejiang was lower and the extents of horizontal inequity were more serious compared with Qinghai’s. We suggest that medical reform should be implemented based on socioeconomic development, because of China’s geographic vastness and the considerable amount of difference in development among geographic areas. Larger gaps in the medical insurance reimbursement ratio between different levels of medical institutions might aid in increasing the equitable utilization of PHIs.
How Does the New Cooperative Medical Scheme Influence Health Service Utilization? A Study in Two Provinces in Rural China

Abstract:BACKGROUND:Many countries are developing health financing mechanisms to pursue the goal of universal coverage. In China, a rural health insurance system entitled New Cooperative Medical Scheme (NCMS) is being developed since 2003. Although there is concern about whether the NCMS will influence the serious situation of inequity in health service utilization in rural China, there is only limited evidence available. This paper aims to assess the utilisation of outpatient and inpatient services among different income groups and provinces under NCMS in rural China.METHODS:Using multistage sampling processes, a cross-sectional household survey including 6,147 rural households and 22,636 individuals, was conducted in six counties in Shandong and Ningxia Provinces, China. Chi-square test, Poisson regression and log-linear regression were applied to analyze the association between NCMS and the utilization of outpatient and inpatient services and the length of stay for inpatients. Qualitative methods including individual interview and focus group discussion were applied to explain and complement the findings from the household survey.RESULTS:NCMS coverage was 95.9% in Shandong and 88.0% in Ningxia in 2006. NCMS membership had no significant association with outpatient service utilization regardless of income level and location.Inpatient service utilization has increased for the high income group under NCMS, but for the middle and low income, the change was not significant. Compared with non-members, NCMS members from Ningxia used inpatient services more frequently, while members from Shandong had a longer stay in hospital.High medical expenditure, low reimbursement rate and difference in NCMS policy design between regions were identified as the main reasons for the differences in health service utilization.CONCLUSIONS:Outpatient service utilization has not significantly changed under NCMS. Although utilization of inpatient service in general has increased under NCMS, people with high income tend to benefit more than the low income group. While providing financial protection against catastrophic medical expenditure is the principal focus of NCMS, this study recommends that outpatient services should be incorporated in future NCMS policy development. NCMS policy should also be more equity oriented to achieve its policy goal.

Abstract:Sapplying calculating method of Gini coefficient, this paper analyses the effects of cooperativemedical scheme (NCMS) financing, medical expenses and NCMS reimbursement on income equity of ruralpopulation. Result shows that Gini coefficient can be used to define the impact size and direction of the new-typerural cooperative medical scheme on income equity, and to measure the alleviation of NCMS on change of incomeequity caused by medical expenses.

Abstract:This paper analyzed the development of new rural cooperative medical scheme in Shandong and Ningxia Hui autonomous Region.The results shows that low financing,unreasonable payment systems,poor of management and rapidly growth of medical expenditure were found in this study.Some comments such as increasing financial level,improving payment systems reasonable and timely,training management staffs and alleviating the rapid growth of medical expenditure were introduced to policy makers.

Abstract:Background: Many countries and regions worldwide are improving their healthcare systems through the integration and unification of health insurance programs covering different groups of people. In China, the past 10 years has been the time when Chinese government promote the Urban and Rural Residents Basic Medical Insurance (URRBMI) by integrating the Urban Residents' Basic Medical Insurance (URBMI) and New Rural Cooperative Medical Scheme (NRCMS). Objectives: To evaluate the impact of the URRBMI on equity in relation to health services. Methods: The quantitative data used in this study were obtained from the CFPS 2014–2020 database, and all respondents with health insurance type UEBMI, URBMI, and NRCMS were included. UEBMI respondents were set as the control group and URBMI or NRCMS as the intervention group, and a DID method model was used to analyze the impact of integrating health insurance on health service utilization, costs and health status. Heterogeneity analysis was also conducted after stratifying the sample according to income level and chronic disease status. This was done to investigate whether there were differences in the effects of the integrated health insurance program across different social groups. Results: The implementation of URRBMI is found to be associated with a significant increase in inpatient service utilization (OR = 1.51, P < 0.01) among rural Chinese residents. Regression results by income stratum show that the utilization of inpatient services increased in rural areas for high-, middle- and low-income groups, with the fastest increase (OR = 1.78, P < 0.05) emerging for low-income groups. Analysis by chronic disease status shows that rural residents with chronic disease are associated with a higher increase in hospitalization rates (OR = 1.64, P < 0.01). Conclusion: The implementation of URRBMI is found to have improved health insurance's ability to withstand risks and effectively improve access to health services for rural residents. In this regard, it can be considered as playing a positive role in bridging the gap in health service utilization between rural and urban areas and in improving regional equity.

Abstract:We assess the impact of the New Rural Cooperative Medical Scheme(NRCMS) from a new perspective by estimating the contribution of health insurance to the inequality of health care utilization in rural China.The paper decomposes income-related inequality in the utilization of health care services into four sources:direct income effect;need related indicators;non-need related indicators and a residual term.NRCMS increases the utilization of health care,but not for using higher level (township,county and city) facilities.Accordingly,NRCMS has a positive contribution in increasing the pro-poor inequality in health care utilization both in men and women.But it causes no improvement in inequality in using township and county level hospitals,which are employed more qualified health care staffs and provide health services of relatively higher quality.NRCMS contributes more in the pro-poor outcome of health care in women.

Abstract:Background: China in 2009 committed to reach universal health coverage by promoting three forms of health insurance; NCMS for the rural population, UEBMI for formally employed urban residents and URBMI for other urban residents. NCMS has expanded to near universal coverage in rural China since launching in 2003. The objective of this study aimed to assess the effect of NCMS on inpatient care utilization from 2003 to 2012 at Longyou county hospital, Zhejiang province.Methods: The research was conducted at Longyou county, Zhejiang province. All registered inpatient admissions from January 1, 2003, to June 30, 2012, were included in the study. The PLSQL Developer software was used to select the interesting variables in the hospital information database and saved in an Excel 2003 file. The interesting variables included the patients' general information (name, gender, age, payment method), discharge diagnosis, length of hospital stay, and expenditure (total expenditure and out-of-pocket payment). Two common diseases (coronary arteriosclerotic disease and pneumonia) were selected as tracer conditions.Results: 292,400 rural residents were enrolled in the Longyou county NCMS by 2011, 95.4% of the eligible population. A total of 145,744 inpatient admissions were registered from 1 January 2003 to 30 June 2012. The proportion of inpatients covered by NCMS increased from 30.3% in 2004 to 54.2% in 2012 while the proportion of inpatients covered by UEBMI increased from 7.7% in 2003 to 14.7% in 2012. The average expenditure for UEBMI insured inpatients was higher than the average for NCMS insured inpatients, although the gap was narrowing. The average length of hospital stay increased every year for all inpatients, but was higher for UEBMI inpatients than for NCMS insured inpatients. For both tracer conditions the results were similar to the above findings.Conclusions: NCMS has improved coverage height for its enrollees and resulted in increased cost of care per inpatient admission at the county hospital. However, wide differences persist between the two insurance systems in coverage height. Both systems are associated with increasing lengths of stay and rising cost per inpatient admission. We found that around 30% of inpatients were not covered by any of the two public health insurance systems, which calls for further studies.

Abstract:Background Getting medical treatment is still difficult and expensive in western China. Improving the equity of basic health services is one of the tasks of the new healthcare reform in China. This study aimed to analyze the parallel and vertical equity of health service utilization of urban residents and then find its influencing factors. Methods In August 2011, a household survey was conducted at 18 communities of Baoji City by multi-stage stratified random sampling. Based on the survey data, we calculated a concentration index of health service utilization for different income residents and a difference index of different ages. We then investigated the influencing factors of health service utilization by employing the Logistic regression model and log-linear regression model. Results The two-week morbidity rate of sampled residents was 19.43%, the morbidity rate of chronic diseases was 21.68%, and the required hospitalization rate after medical diagnosis was 11.36%. Among out-patient service utilization, the two-week out-patient rate, number of two-week out-patients, and out-patient expense had good parallel and vertical equity, while out-patient compensation expense had poor parallel and vertical equity. The inpatient service utilization, hospitalization rate, number of inpatients, days stayed in the hospital, and inpatient expense had good parallel equity, while inpatient compensation expense had poor parallel equity. While the hospitalization rate and number of inpatients had vertical equity, the days stayed in hospital, inpatient expense, and inpatient compensation expense had vertical inequity. Conclusions Urban residents' health was at a low level and there was not good health service utilization. There existed rather poor equity of out-patient compensation expense. The equity of inpatient service utilization was quite poor. Income difference and the type of medical insurance had great effects on the equity of health service utilization.
