Abstract:But the future looks bleak for local scrutiny of health organisations The public inquiry into Mid Staffordshire NHS Foundation Trust has left few reputations undamaged. But amid the wreckage two emerge with credit: the group Cure the NHS (www.curethenhs.co.uk), set up by patients, relatives, and friends to draw attention to failings at Stafford Hospital, and a local journalist, Shaun Lintern, who gave them a public voice. The rest—boards, regulators, managers, and most doctors and nurses—failed in their professional duty, some more egregiously than others. In identifying failure it was the outsiders who proved to be the true professionals. This is a sobering conclusion, because the past decade has seen a systematic failure to involve local people effectively in the NHS. Since the abolition of the community health councils in 2002 under Tony Blair’s government, each effort to replace them has proved feebler than the last. The councils were replaced firstly by patient and public involvement forums and then, in 2005, by local involvement networks (LINks). With the unconscious irony for which NHS publications are rightly celebrated, the document setting up LINks was entitled A Stronger Local Voice . Now a new plan is afoot to replace LINks with Healthwatch, but with little evidence that any lessons have been learnt. The regulations tabled by the government to govern the running of Healthwatch at local level will prevent these bodies from doing anything that promotes or opposes …

Abstract:This article assesses how and why the National Health Service (NHS) has remained at the forefront of British politics and society for 75 years amid significant economic pressures, organizational strain, shifting ideological trends, ongoing reforms, and an unprecedented public health emergency. The postwar "years of consensus" evolved into alleged decline and ostensible neglect during the 1980s, while New Labour sought to rejuvenate this core public service after 1997, featuring investment and often controversial reforms that challenged the party's social democratic values. Amid the New Labour era, NHS powers filtered down to devolved administrations, while from 2008 retrenchment and austerity ensued, fueled by global recession. Austerity eventually subsided, yet from early 2020 the NHS swiftly faced the extreme conditions of the globalized Covid-19 pandemic. The service continues to face challenges regarding its longer-term viability, and this article analyzes this scenario, within the context of the NHS's 75-year historical legacy, its contemporary status, comparative international trends, and likely future evolution.

Abstract:Purpose: This paper compares how the two interacting themes of "Whistleblowing" or "Speaking Up" and the duty of candour (DoC), which are both concerned with safety and quality improvement in health care, got onto the agenda of the British National Health Service (NHS). Design/methodology/approach: It uses the approach of multiple streams and the methodology of interpretive content analysis in a deductive approach that focusses on both manifest and latent content. It examines official documents that discuss the DoC or whistleblowing or cognate terms in connection with the British NHS from 1999 to 2019. Findings: The main conceptual finding, which mirrors many previous studies, is that it seems difficult to operationalise many of the sub-components of the multiple streams approach. The main empirical finding points to the "focusing event" of the Francis Report into the Mid Staffordshire Trust of 2013 and the importance of its Chair, Sir Robert Francis, as a policy entrepreneur. Originality/value: This is one of the first studies to focus on both issues of whistleblowing and the DoC and the first to compare them through the lens of the multiple streams approach. It has two main conceptual advantages over most previous studies in the field: it compares whistleblowing and the duty of candour rather than the dominant approach of a single case study and explores the different outcomes of failed as well as successful couplings of the streams.

Abstract:So much for decentralisation: England’s health secretary can’t keep his hands off the NHS It hasn’t taken long for former health secretary Andrew Lansley’s plan for a “hands-off” NHS to hit the rocks. His successor, Jeremy Hunt, is behaving as if the Health and Social Care Act 2012, with its careful separation of powers, had never passed into law. If there’s a problem—and there are plenty—it isn’t long before Hunt comes up with a new wheeze for dealing with it, as health secretaries always have. From pressure on emergency care to hospital quality or the supposed iniquities of the general practitioner contract, he’s there with an opinion and an initiative. His junior minister, Dan Poulter, has echoed his master’s voice with a plan of his own, suggesting with no great originality that the NHS could save money by better procurement.1 The merits of these interventions matter less than their frequency and specificity, which tell us that the Lansley plan is in tatters. Abstract:How realistic are the predictions of the Five Year Forward View ? Since he published his Five Year Forward View in October, Simon Stevens has been walking on water. The chief executive of NHS England is (temporarily) able to do no wrong. Just as Melanesian islanders attribute magical powers to charismatic outsiders, Stevens has acquired a cargo cult of his own, deliciously lampooned in a series of tweets by the journalist Andy Cowper under the hashtag #SimonStevensfacts. Here’s a sample. “Hot? Bothered? Chant NHS England three times and Simon Stevens will appear with a chilled bottle of mineral water.” Impressive, but not quite as awe inspiring as another fact unearthed by Cowper. “Simon Stevens mined the Large Hadron Collider’s 27-km particle accelerator tunnels in a lunch break, using a plastic spoon.” Joking aside, it does seem that Stevens can currently get away with things that would bring derision on the heads of ordinary folk. For example, he has redefined the intervention process designed to rescue failing NHS services as a “success regime” rather than a “failure regime” to avoid demotivation and stigma.
