Abstract:Andrew Lansley, architect of the latest NHS reorganisation in England, has been removed from his job as health secretary in the mid-term reshuffle, with the changes he began still incomplete. He will be replaced by Jeremy Hunt, who moves to health from culture, media, and sport. Lansley becomes leader of the House of Commons. Though his complex and controversial Health and Social Care Act made Lansley deeply unpopular with many in the NHS, moving him now represents a major gamble by the prime minister, David Cameron, who defended the bill on its passage through parliament and committed huge amounts of political capital to it. Abstract:So much for decentralisation: England’s health secretary can’t keep his hands off the NHS It hasn’t taken long for former health secretary Andrew Lansley’s plan for a “hands-off” NHS to hit the rocks. His successor, Jeremy Hunt, is behaving as if the Health and Social Care Act 2012, with its careful separation of powers, had never passed into law. If there’s a problem—and there are plenty—it isn’t long before Hunt comes up with a new wheeze for dealing with it, as health secretaries always have. From pressure on emergency care to hospital quality or the supposed iniquities of the general practitioner contract, he’s there with an opinion and an initiative. His junior minister, Dan Poulter, has echoed his master’s voice with a plan of his own, suggesting with no great originality that the NHS could save money by better procurement.1 The merits of these interventions matter less than their frequency and specificity, which tell us that the Lansley plan is in tatters. Abstract:But the future looks bleak for local scrutiny of health organisations The public inquiry into Mid Staffordshire NHS Foundation Trust has left few reputations undamaged. But amid the wreckage two emerge with credit: the group Cure the NHS (www.curethenhs.co.uk), set up by patients, relatives, and friends to draw attention to failings at Stafford Hospital, and a local journalist, Shaun Lintern, who gave them a public voice. The rest—boards, regulators, managers, and most doctors and nurses—failed in their professional duty, some more egregiously than others. In identifying failure it was the outsiders who proved to be the true professionals. This is a sobering conclusion, because the past decade has seen a systematic failure to involve local people effectively in the NHS. Since the abolition of the community health councils in 2002 under Tony Blair’s government, each effort to replace them has proved feebler than the last. The councils were replaced firstly by patient and public involvement forums and then, in 2005, by local involvement networks (LINks). With the unconscious irony for which NHS publications are rightly celebrated, the document setting up LINks was entitled A Stronger Local Voice . Now a new plan is afoot to replace LINks with Healthwatch, but with little evidence that any lessons have been learnt. David Nicholson told the annual general meeting of NHS England on Thursday 12 September that in the three years since he sat in a room with then health secretary, Andrew Lansley, and heard about his vision for the NHS, important steps had been taken to build an organisation that was consistent with its values. Abstract:To those, like me, who lived through the miners' strike in South Yorkshire in the 1980s, it's clear that the then prime minister Margaret Thatcher was driven by hard ideology and harder economics rather than, as retrospectively claimed, that she championed climate action. The end of the slave trade was driven by hard economics more than the principle of ending slavery. Indeed, slavery continued for decades afterwards.
