Abstract:The NHS remains under immense pressure.1 Each part of the system is experiencing demand beyond its capacity, which is continually increasing the problem. This is most vividly illustrated in urgent and emergency care settings. Media images in the next few weeks will portray long queues of ambulances outside emergency departments and patients experiencing prolonged periods of degrading corridor care. Publications will feature stories of patients who have experienced significant harm and trauma due to delayed ambulance responses.Each winter in the past decade has become slightly worse,2 and that trend, which has not been reversed, has resulted in a dire situation that may not yet have reached its nadir. A continuum of often predictable perfect storms has caused a struggling system to reach collapse. Last winter this was exacerbated by a further wave of covid-19, high levels of influenza, and an outbreak of group A streptococcus infection with challenging cold weather....

Abstract:Concerns are mounting over whether the NHS can meet key performance targets this winter, NHS Providers has said, after a survey of trust leaders highlighted pressure on hospitals, ambulance services, and community and mental health teams.1Over nine in 10 of the leaders who responded (96%) said that they were extremely or moderately concerned about the effect of winter pressures on their trust and local area. Work on the NHS 10 year plan for health is underway and publication of the government's strategy has been promised for next Spring. Abstract:Our world is a world of crises, in a disconcerting variety and manifestations: announced, reported, advertised, analysed, silent, but also, forgotten and ignored. The paper’s argument is that there are definitely many more crises than solutions as the climate of uncertainty in Europe and in the entire world is not beneficial for the solutioning of crises. The water crisis is announced, but recurrent and tends to be forgotten. The educational crisis is silent, as is the ecological one, for the most part. Crisis is a current reality, aggravated by the ever-greater expectations from the people. The deficit of attention in terms of finding solutions, agreeing on solutions and implementing solutions goes hand in hand with people’s impatience with politics and paradoxical support for populism. Other crises, such as familial violence, marginality and even the loneliness crisis identified since 2019 seem pale in front of war, famine, disease and hurricanes. However, they are all seriously affecting our peers and diminish everyone as a true human being – the ones in suffering and the others, turning a blind eye.

Abstract:This article assesses how and why the National Health Service (NHS) has remained at the forefront of British politics and society for 75 years amid significant economic pressures, organizational strain, shifting ideological trends, ongoing reforms, and an unprecedented public health emergency. The postwar "years of consensus" evolved into alleged decline and ostensible neglect during the 1980s, while New Labour sought to rejuvenate this core public service after 1997, featuring investment and often controversial reforms that challenged the party's social democratic values. Amid the New Labour era, NHS powers filtered down to devolved administrations, while from 2008 retrenchment and austerity ensued, fueled by global recession. Austerity eventually subsided, yet from early 2020 the NHS swiftly faced the extreme conditions of the globalized Covid-19 pandemic. The service continues to face challenges regarding its longer-term viability, and this article analyzes this scenario, within the context of the NHS's 75-year historical legacy, its contemporary status, comparative international trends, and likely future evolution.
