Abstract:But the future looks bleak for local scrutiny of health organisations The public inquiry into Mid Staffordshire NHS Foundation Trust has left few reputations undamaged. But amid the wreckage two emerge with credit: the group Cure the NHS (www.curethenhs.co.uk), set up by patients, relatives, and friends to draw attention to failings at Stafford Hospital, and a local journalist, Shaun Lintern, who gave them a public voice. The rest—boards, regulators, managers, and most doctors and nurses—failed in their professional duty, some more egregiously than others. In identifying failure it was the outsiders who proved to be the true professionals. This is a sobering conclusion, because the past decade has seen a systematic failure to involve local people effectively in the NHS. Since the abolition of the community health councils in 2002 under Tony Blair’s government, each effort to replace them has proved feebler than the last. The councils were replaced firstly by patient and public involvement forums and then, in 2005, by local involvement networks (LINks). With the unconscious irony for which NHS publications are rightly celebrated, the document setting up LINks was entitled A Stronger Local Voice . Now a new plan is afoot to replace LINks with Healthwatch, but with little evidence that any lessons have been learnt. Abstract:Abstract Tackling health inequalities (HI) has become a key policy objective in England in recent years. Yet, despite the wide-ranging policy response of the 1997–2010 Labour Government, socio-economic variations in health continued to widen. In this paper, we seek to explore why. We propose that a meta-narrative has emerged in which the health problems facing England's most deprived areas, and the solution to those problems, have increasingly come to be linked to levels of National Health Service (NHS) funding. This has been, in part, a response to key shortcomings in previous rounds of resource allocation. The very significant sums of money allocated with respect to ‘health inequalities’ reflects and reinforces the belief that the NHS can and should play a central role in promoting health equity. This medicalisation of HI focuses attention on the role of individual risk factors that lend themselves to medical management, but effectively sidelines the macroprocesses of social inequality, legitimising the kind of society that neo-liberal government has produced in the United Kingdom – one in which health (like other assets) has become a matter of individual and not collective responsibility.

Abstract:How can people who need specialist care get it more quickly? Say what you like about the NHS, but you have to admit it’s extraordinarily good at setting obstacles in the way of care. Sometimes it seems that the entire system is built on the principles of the medieval castle, with a series of redoubts, moats, and stout stone walls designed to defend against unwelcome intruders—that is to say, patients. Gatekeeping is the watchword. Today even the gatekeepers have gatekeepers, those much abused receptionists in general practices who ration access to the GPs so assiduously that patients often get better before they’ve managed to arrange an appointment. Who’d be a receptionist when the clocks strike 8 am and the phones begin to ring? Only a service dedicated to keeping customers and providers apart could have devised a system so utterly demeaning. It turns patients into supplicants and receptionists into bouncers, neither being roles to which they aspired. Two arguments are advanced for gatekeeping, one rather crude, the other more nuanced and intellectually pleasing. The Better Care Fund: a Disaster in Waiting?

Abstract:Although the fund is a worthwhile aspiration, confusion exists at almost every level Some projects in the NHS in England start well and then go sour—think of Connecting for Health. Others, despite being makeshift, at least put a poultice on a painful boil—the Cancer Drugs Fund, for example. And then there are those that sound good on paper, tick enough of the right boxes to face no intellectual challenge, but nevertheless look like a disaster in waiting from their very inception. Here is where we find the Better Care Fund. The idea is to take some money from acute care trusts and give it to local authorities to be spent in providing better health and social services in the community, thereby preventing so many admissions that hospitals won’t even notice the loss of income. It seeks a virtuous circle of fewer admissions and faster discharges, easing pressure on the acute sector while providing care in the places patients are supposed to want it—a worthwhile aspiration. But there is confusion at almost every level. The transfers involved, totalling £3.8bn (€4.7bn; $6.4bn), are large enough to destabilise many acute trusts but too small to replenish the losses local authorities have suffered in the years of retrenchment since 2010. …

Abstract:The associations between where you live and your health outcomes are increasingly understood. Healthy life expectancy is more than 18 years lower for people living in the most deprived areas compared with the least deprived.1 Some of this, but certainly not all, is related to the quality of your healthcare. In 2020, Quality Watch, a joint venture funded by the Nuffield Trust and the Health Foundation, found that long waits for emergency care were greater in the most deprived areas.2 They also found that rates of admissions for pressure sores, a largely preventable quality indicator, were three times higher in the most deprived areas. There is policy commitment to tackle this in the NHS Long Term Plan, which explicitly acknowledges that health inequalities create further costs. Universal coverage and cost control: the United Kingdom National Health Service

Abstract:The UK NHS has a number of important strengths. Its costs are relatively low compared to the health care systems of other developed countries due in part to cash limited central budgeting. It is extremely popular with the electorate and surveys show overall satisfaction with the NHS despite some dissatisfaction with waiting lists and a public perception of underfunding. The NHS model of general medical care provided by independent contractors has been acclaimed as "a British success" (General Medical Services Council, 1983). The role of the UK GP combines providing primary care and acting as a gatekeeper to secondary care. This increases equitable access to care for the population and assists in cost containment. As a model, it is currently being emulated in other countries including Sweden and US Health Maintenance Organizations but, as in these countries, the UK primary care model has been evaluated poorly. There are of course continuing weaknesses in the UK health care system. There is insufficient knowledge upon which to base health care services and increase efficiency. In the future, if a knowledge-based health care service is to be created, a considerable amount of research and evaluation is required to identify "what works" in health care (i.e., what is effective) and also the cost effective ways of altering provider behaviour to maximise the amount of health gain which can be achieved using a limited budget. The NHS reforms created a lot of enthusiasm and energy but its effects are difficult to disentangle from the simultaneous increases in funding. There is little evidence from the UK or elsewhere that competition in health care produces efficiency or improvements in resource allocation. Evaluation is required to identify which of the reforms are increasing efficiency. Competition needs to be used with caution and recognised as a mean and not an end in itself. It is remarkable how both clinical practice and health policy reform, in the UK and elsewhere, is poorly evaluated. Medical practice varies substantially locally, regionally, and internationally, e.g., patients with similar age and stage of cancer receive very different levels of radiotherapy across Europe. For most interventions, the appropriate level of treatment may be asserted but is not based on cost effectiveness knowledge. Health policy analysts, like clinicians, make assertions about competition and other health care reforms which are value- rather than knowledge-based. Both groups of decision-makers should be more cautious, informing their choices with research rather than relying on unsubstantiated optimism!
Lessons from New Zealand for England's NHS Foundation Trusts

Abstract:The legislation to devolve responsibility for the management and operation of England's top-performing NHS hospitals to community-owned NHS Foundation Trusts raises several issues relating to the challenges posed to governance structures by private non-profit ownership and control of assets used to provide government-financed services. Building upon the lessons learned from devolution of public hospital governance in New Zealand to boards at arm's-length from central control during the 1990s, this paper analyses the English NHS hospital changes. Whilst local political accountability and competition between hospitals indicate that the English reforms may be more successful in meeting patients' needs more efficiently than the New Zealand reforms, the English proposals may be compromised by the ability of staff to become members of Trusts, boards bearing risks of decisions outside their control whilst simultaneously being insulated from the consequences of their decisions by a 'soft budget constraint', and conflicts of interest as boards simultaneously act as agents of both central regulators and local beneficiaries.
