Influence of Casein Phosphopeptide-Amorphous Calcium Phosphate Application, Smear Layer Removal, and Storage Time on Resin-Dentin Bonding

Abstract:The aim of this study is to evaluate the influence of Tooth Mousse (TM) application, smear layer removal, and storage time on resin-dentin microtensile bond strength (µTBS). Dentin specimens were divided into two groups: (1) smear layer covered; (2) smear layer removed using 15% EDTA for 90 s. In each group, half the specimens were treated once with TM for 60 min. After bonding procedures using a two-step self-etching adhesive (Clearfil SE Bond (CSE); Kuraray Medical, Tokyo, Japan), an all-in-one adhesive (G-Bond (GB); GC Corp, Tokyo, Japan), and a total-etch adhesive (Adper Single Bond 2 (SB); 3M ESPE, St. Paul, MN, USA), the specimens were stored for 3 d or 6 months in deionized water at 37 °C, and µTBS was tested and analyzed. With the exception of SB (no TM application) and GB, the μTBS was significantly increased for CSE and SB using EDTA pre-conditioning and 3 d of storage (P≤0.001). Bond strength of GB decreased significantly when using EDTA (3 d storage, P<0.05). TM application only increased the μTBS of GB (no EDTA) and SB (with EDTA) after 3 d (P≤0.02). Comparing the adhesives after 3 d of storage, CSE exhibited the greatest μTBS values followed by GB and SB (P≤0.02). The factors of adhesive, EDTA, and TM did not show any significant impact on μTBS when specimens were stored for 6 months (P>0.05). The additional application of TM and EDTA for cavity preparation seems only to have a short-term effect, and no influence on µTBS of dentin bonds after a period of 6 months.
Effects of Desensitizing Toothpastes on the Permeability of Dentin after Different Brushing Times: an in Vitro Study

Abstract:Purpose: To investigate the effects of three commercially available desensitizing toothpastes on dentin permeability, and compare the efficacy of each product for reducing dentin permeability in the short term according to the frequency and duration of usage. Methods: 100 dentin discs with no caries were prepared from freshly extracted human third molar teeth. The dentin discs were brushed with three desensitizing toothpastes or with a non-desensitizing toothpaste and distilled water, which served as control. The 100 dentin slices were randomly divided into two groups (n= 50): one group underwent continuous brushing (brushed for 3 minutes continuously), and the other group underwent discontinuous brushing (brushed three times, each time for 1 minute). Then, the two groups were divided into five subgroups (n = 10) for the five brushing applications. Dentin permeability was measured with a hydraulic permeability system before and after brushing. Results: All desensitizing toothpastes reduced dentin permeability significantly after treatment. Sensodyne Repair & Protect (calcium sodium phosphosilicate) and discontinuous brushing reduced dentin permeability significantly compared with continuous brushing. Dentin permeability values showed no significant difference between the three toothpastes after 3 minutes of continuous brushing. When comparing the three toothpastes under discontinuous brushing conditions after 3 minutes, Sensodyne Repair & Protect (calcium sodium phosphosilicate) reduced dentin peimeability significantly.

Abstract:Background/purpose: Few studies have comprehensively assessed the shear bonding strength of the luting cements between abutments and fixed partial dentures after dentin surface treatment with disinfectants. The purpose of the study was to evaluate the effect of three commonly used disinfectants (2.5% sodium hypochlorite, 0.2% chlorhexidine, and 0.2% benzalkonium chloride) on the shear bonding strength of four luting cements.Materials and methods: Teeth were mounted on Teflon cylinders and prepared for dentin exposure. Three different disinfectants were used to treat the dentin surface. Nickel-chromium posts were cemented with resin cement, glass ionomer cement, polycarboxylate cement, or zinc phosphate cement. The shear bonding strength of the cement was examined using an Instron testing machine. Two-way analysis of variance (ANOVA) was used to examine the differences in shear bonding strength between the cements. If a statistically significant difference was found through ANOVA, a post hoc test with Tukey's honest significant difference was conducted.Results: Disinfectants significantly decreased the shear bonding strength of resin cement, with 2.5% sodium hypochlorite causing the most substantial decrease. The zinc phosphate cement group displayed minimal shear bonding strength regardless of the disinfectant used.Conclusion: The presence of 2.5% sodium hypochlorite significantly reduced the shear bonding strength of resin cements. During permanent cementation of indirect restorations, the choice of luting cement paired with the proper disinfectant is of utmost importance to maintain the shear bonding strength.

Abstract:This study was evaluating how three desensitizing toothpastes used at home influence the effect associated with desensitizing agents after application in the clinic. Fifty dentine disks measure it permeability and 32 dentine disks with similar permeability levels were selected. Following Dental desensitizer treatment, dentine disks were randomly divided into three subgroups (n=10) that received applications of three toothpastes, respectively. The permeability (Lp) of each specimen was measured after each treatment. One specimen was selected from each group for scanning electron microscopy (SEM) observation. After each treatment, the Lp values decreased significantly for each group (p<0.05) and either completely or partially blocked the dentine tubules upon SEM observation. However, no significant differences in Lp values were observed amongst subgroups (p>0.05). After using the Dental desensitizer, Sensodyne, Crest and Colgate desensitizing toothpastes both can continued to reduce the permeability of the dentine disk, and no significant differences were found amongst them.
Effect of carbamide peroxide on the permeability of dentinal tubules and dentinal tubule occlusion by casein phosphopeptide-amorphouscalcium phosphate

Abstract:Objective To investigate the effect of carbamide peroxide(CP) on the permeability of dentinal tubules and dentinal tubule occlusion by casein phosphopeptide-amorphouscalcium phosphate(CPP-ACP).Methods Unerupted,surgical extracted human third molars were sectioned mesio-distally to provide two 2-mm dentine discs after removal of the crown.In dentinal tubule patency experiment,the test halves were treated with 10% CP,35% CP and 37% phosphoric acid,respectively,while the controls were treated with distilled water.The difference of the patency of dentinal tubules between the test halves and the controls were compared using SEM.In tubule occlusion test,after exposed to 10% CP for 8 hours,the dentinal discs were treated with CPP-ACP,Ms coat and UltraEZ desensitizing gel.The effects of dentinal tubule occlusion were also observed by SEM.Results After treated with 10% or 35% CP,the test halves showed increased number of open tubules and were wider in diameters than those of the controls.The diameters of test halves treated with 37% phosphoric acid were significantly wider than those treated with 10% or 35% CP.Dentinal tubule occlusion was observed when CPP-ACP was applied to 10% CP treated dentinal discs.Conclusions CP increased the number of open dentinal tubules,which was associated with bleaching-induced sensitivity.The effect of dentinal tubule occlusion by CPP-ACP will help to decrease bleaching-induced sensitivity.

Abstract:OBJECTIVES:This in vitro study evaluated the sealing ability of dentin adhesives and a desensitizer.METHODS:Standardized Class V cavities were prepared on the buccal and lingual surfaces of 55 freshly extracted human molars. The teeth were cut into two sections in a mesio-distal direction. All specimens were randomly divided into 11 groups (n = 10). In each group, the cavities were treated with one of eight different dentin adhesives, a dentin desensitizer or a special combination of these, except for the control group. After all the cavities had been stained for 24 hours with 0.5% methlyene blue solution, the teeth were longitudinally cut into two sections through the center of the cavities. Dye penetration was recorded according to the stained areas. Epoxy resin replicas of two specimens per group were analyzed by SEM.RESULTS:All dentin adhesives/desensitizer significantly reduced dentinal permeability (p < 0.05). The sealing ability of the different dentin adhesives/desensitizer was significantly different (p < 0.001). The degree of dye penetration corresponded well with the surface morphology of the dentin surfaces after the various treatments.CONCLUSIONS:None of the dentin adhesives/desensitizer could completely block fluid percolation through the dentinal tubules, but current dentin adhesives/desensitizers can significantly reduce dentin permeability.

Abstract:Background/purpose Tubular occlusion is an effective method to treat dentin hypersensitivity. This study aimed to determine the effect of a modified methyl methacrylate-p-styrene sulfonic acid copolymer-based gel desensitizer on dentin permeability and tubule occlusion in extracted human premolars. Materials and methods Hydraulic conductance (HC) measurement (n = 50) and scanning electron microscopy (SEM; n = 64) were performed. Tooth specimens were divided into 6 groups and treated with: G1, distilled water for 30 s; G2, distilled water for 5 min; G3, gel desensitizer for 30 s; G4, gel desensitizer for 5 min; G5, 3% potassium oxalate for 30 s; G6, 3% potassium oxalate for 5 min. HC of dentin were measured before and after 30 s-etching with 35% phosphoric acid, at 0, 30, 60 min after group treatment and after 5-min acid challenge with 6% citric acid. The degree of tubule occlusion and the penetration depth of each agent were also determined. ANOVA and multiple comparison tests were used for data analysis. Results G3, G4, G5 and G6 significantly decreased in HC after group treatment every observation period, compared to after acid-etching ( P < 0.001), and had 100% degree of tubule occlusion with penetration depth about 7.62, 7.94, 8.59 and 8.66 μm, respectively. However, G6 showed the greatest reduction in HC ( P < 0.05). Conclusion Gel desensitizer treatment, for only 30 s, could reduce dentin permeability and completely occlude the dentinal tubules, even though after acid challenge. However, 5-min treatment with potassium oxalate showed the greatest decrease in dentin permeability.

Abstract:Aim This in vitro study compared the effect of two commercially available silver diamine fluoride (SDF) products on dentinal tubule (DT) occlusion and dentin permeability. Methods Sixty‐six dentin discs (1.0 mm thick) were divided equally into six groups: acid‐etched with no treatment (negative control—NC), etched and sealed with nail varnish (positive control—PC), non‐etched and treated with Advantage Arrest (AA), non‐etched and treated with Topamine (TP), etched and treated with AA, and etched and treated with TP. Eight specimens from each group were tested for permeability using the dye percolation method. The remaining three samples from each group were assessed for dentinal tubule occlusion, depth of SDF penetration, and elemental analysis using a scanning electron microscope coupled with an energy‐dispersive X‐ray (SEM/EDX) system. One‐way ANOVA and Tukey's post hoc test were used to analyze the results. Results TP exhibited lower (p 0.05) penetration depth in the DT, none of them had the ability to completely occlude all the DTs. AA exhibited more silver and fluoride ion deposition on etched dentin than TP, and both materials showed a similar increase in the deposition of Ca and P compared to NC. Conclusions Whereas the effectiveness of dentin tubule (DT) occlusion varies based on the material used, both tested SDF products exhibit the capability to seal dentinal tubules, decrease dentin permeability, and promote dentin remineralization. These findings suggest their potential advantage in managing dentin hypersensitivity.

Abstract:Objectives: The present study examined a technique for reducing dentin permeability through the application of a calcium phosphate (CaP)-based desensitiser with a laser-assisted process and evaluated adhesive-dentin bond strength. Methods: Thirty dentin discs were divided into two groups according to whether the selected desensitiser (TeethMate; Kuraray Noritake) was used prior to dentin bonding. Each group was subdivided into three subgroups (n = 5): A- Adhesive (Single Bond Universal, 3M ESPE), AL- Adhesive + Laser (Nd:YAG 60 mJ) and LAL- Laser + Adhesive + Laser. Dentin permeability values (%) were recorded before and after desensitiser application. Resin composites were placed over the bonded specimens; the latter were aged prior to microtensile bond strength evaluation. Gelatinolytic activity within the hybrid layers was examined with in-situ zymography using confocal laser scanning microscopy. Data were analysed with ANOVA and Tukey test (α = 0.05). Results: Significant differences in dentin permeability were identified for all groups (p = 0.00). Both laser treatment (p = 0.182) and desensitiser application (p = 0.687) did not significantly improve dentin bond strength. Ultrastructure of the resin-dentin interface identified presence of calcium phosphate within dentinal tubules. Laser treatment did not affect hybrid layer ultrastructure. Both treatment modalities (intratubular CaP occlusion and laser) had no influence on gelatinolytic activity within hybrid layers. Conclusion: Although intratubular CaP occlusion and laser treatment were effective in reducing dentin permeability, they did not affect bond strength, interfacial ultrastructure and gelatinolytic activity within hybrid layers. Clinical relevance: Treatment of etched dentin with Nd:YAG Laser at 60 mJ does not adversely affect collagen ultrastructure and gelatinolytic activity within the hybrid layer. The application of a calcium phosphate-based desensitiser to etch dentin does not affect dentin bond strength.

Abstract:Abstract Background To investigate the effect of transparent tray-based application of bioactive glasses (BGs) desensitizer on the permeability of enamel and dentin to hydrogen peroxide (H 2 O 2 ). Methods Freshly extracted human first premolars were divided into 6 groups ( n = 8). Group A and B: without pretreatments; Group C and E: treated with BGs desensitizer only; Group D and F: treated with BGs desensitizer dispensed with a transparent tray. After roots and pulp tissues of the treated tooth specimens were thoroughly removed, acetate buffer was added into pulp chambers and the treated specimens were immersed in distilled water (Groups A, E, and F) or 30% H 2 O 2 (Groups B, C, and D) for 30 min at 37 °C. The amount of H 2 O 2 in the pulp chamber of each group was measured using ultraviolet-visible spectrophotometry. Results In control groups (Group A, E, and F), H 2 O 2 was not detected. The amount of pulpal H 2 O 2 in Group B, C, and D were 21.149 ± 0.489 μg, 9.813 ± 0.426 μg, and 4.065 ± 0.268 μg respectively. One-way ANOVA analysis indicated that significant differences existed in these groups (F = 459.748, p < 0.05). Conclusions The effect of BGs desensitizer in reducing the permeability of enamel and dentin to H 2 O 2 could be enhanced when dispensed with a transparent tray.

Abstract:The purpose of this study was to evaluate the effect of different irrigating solutions on smear layer removal and dentinal tubule opening on root canal surfaces after post space preparation and to study whether additional ultrasonic irrigation has any effect on smear layer removal. Forty-eight anterior teeth were treated endodontically. After post space preparation, they were assigned to six groups: group 1, EDTA; group 2, EDTA with ultrasonic activation; group 3, sodium hypochlorite (NaOCl); group 4, NaOCl with ultrasonic activation; group 5, sodium chloride (NaCl); and group 6, NaCl with ultrasonic activation. Specimens were examined under a field-emission scanning electron micro;cope and scored for debris removal and dentinal tubule opening at the coronal, middle, and apical thirds of the root canal. The results showed that EDTA performed significantly better than NaCl and NaOCl in smear layer removal and dentinal tubule opening. Additional ultrasonic irrigation did not improve smear layer removal significantly. (J Endod 2009;35:583-586)

Abstract:OBJECTIVE: Cavity preparation is a surgical procedure that attempts to remove all infected dentin.1 Bacteria left beneath the filling material is greatest threat to the pulp. To reduce the potential for residual caries development and sensitivity, an antibacterial solution with the ability to disinfect the prepared tooth surface would be of help.2 So this study was conducted to evaluate and compare the effect of dentin disinfection with 2% chlorhexidine gluconate (Consepsis) and 0.3% iodine (Ora5) on shear bond strength (SBS) of self-etch adhesives to dentin.MATERIALS AND METHODS: Buccal surfaces of 36 caries-free permanent third molars were ground to expose dentin. All specimens were mounted on acrylic block, divided randomly into three groups, namely group I (control), group II (Con-sepsis), and group III (Ora5). After the application of cavity disinfectant and bonding procedures as per manufacturer's instructions, composite cylinders were built. Then SBS was measured using universal testing machine.RESULTS: Statistical analysis of the measurements were made using one-way analysis of variance (ANOVA), which showed that when cavity disinfectants (Consepsis and Ora5) were used there was significant reduction in SBS of composite to dentin when compared with that of control group.INTERPRETATION AND CONCLUSION: The results indicate that the use of commercially available cavity disinfectants, Consepsis containing 2% chlorhexidine gluconate and Ora5 containing 0.3% iodine and 0.15% potassium iodide with self-etch adhesive (Adper Prompt), would significantly lower SBS of composite to dentin.HOW TO CITE THIS ARTICLE: Suma NK, Shashibhushan KK, Reddy VVS. Effects of Hybrid Coat Protective Coat for Dentin in the Full Crown Restoration of Vital Teeth

Abstract:Objective:To study the effects of Hybrid coat protective coat for dentin on desensitization of vital teeth in the full crown restoration.Methods:A total of 84 vital teeth from 50 patients were randomly divided into three groups,28 vital teeth in each group.One group for applying Hybrid coat after tooth preparation,one group for applying Colgate effective anti-acid toothpaste after tooth preparation,the other group was the control.Take the model after tooth preparation,cement the temporary crown.Week and one month to referral after the restoration,evaluate the extent of dentine hypersensitivity of experimental and control groups.Results: After a week and one month,Hybrid coat group had significant desensitization compared with Colgate group and control group(P<0.05).Colgate group and control group had no significant difference(P>0.05).Conclusions: Hybrid coat protective coat for dentin can effectively prevent dentine hypersensitivity after tooth preparation.

Abstract:Background: This in-vitro study aimed to investigate the effect of two different antibacterial disinfectants on the microleakage performance of newly developed bulk-fill composite, bonded to different tooth structures. Methods: Class V cavities were prepared in 30 sound premolar teeth, with enamel occlusal margins (OM) and dentin cervical margins (CM). Two disinfectants, 2% chlorhexidine gluconate (CHX) and Listerine Miswak (ListM), were used. Teeth were divided into three groups (n = 10): G1, Control; G2, CHX; and G3, ListM. Disinfectants were applied to the cavity preparation walls after they were etched with 35% phosphoric acid. The Single Bond Universal adhesive system was then used, and teeth were restored with Filtek One Bulk Fill composite. Samples were examined, after thermocycling aging, by stereomicroscopy for the evaluation of marginal dye penetration. Results: The highest mean microleakage score was reported in the CM of G1 (2.60 ± 1.174), which was significant compared with that of G2 only (p = 0.02). OM in G1 showed no microleakage, with no significant differences found among groups (χ2 = 1.39, p = 0.50). No significant differences were reported between G2 and G3 (p = 0.45 OM; p = 0.17 CM). Conclusions: Cavity pretreatment with CHX is not significantly different to pretreatment with CHX. In contrast, CHX improved the cervical marginal seal as compare to the control group (G1).

Abstract:Human dentin is known for its hierarchical structure through long-term evolution. Dental caries, embodied by dentin demineralization, is ascribed to a different reaction between peritubular dentin (PTD) and intertubular dentin (ITD) to acid dissolution. This study sheds light on the adverse effect of acid on dentin in terms of degradation of its fracture toughness (FT) due to the acid dissolution-induced corrosion of PTD. A scanning electronic microscope (SEM) is utilized to visualize the difference between normal and acid-treated dentin subjected to the same loading method in terms of crack propagation performance. 3D simulative representative volume elements (RVEs) are developed to analyze the effect of PTD missing on the performance of dentin fracture resistance (FR). The results indicate PTD plays a significant role in enhancing dentin FR capability and thus reveals the importance of structural integrity for dentin.

Effects of Carbodiimide Dentin Surface Treatment on Resin-Dentin Bonding

Abstract:Purpose: To assess the effects of ethanol-based 1-ethyl-3-(3-dimethyl-aminopropyl) carbodiimide (EDC) dentin surface treatment on resin-dentin bonding and dentin collagen fibril biodegradation. Methods: Acid-etched dentin surfaces were pretreated with different concentrations of ethanol-based EDC solutions (0.01-2M) for 60 seconds, followed by two-step etch-and-rinse dentin adhesive application and resin composite bonding. Dentin surfaces pretreated with either ethanol alone or no pretreatment were used as controls. The specimens were subjected to microtensile bond strength testing after storage in 0.9% NaCl solution at 37 degrees C for either 24 hours or 90 days. Furthermore, demineralized dentin slabs with and without ethanol-based EDC pretreatment were exposed to a collagenase solution for 24 hours, and subsequent hydroxyproline release was measured using ELISA. Data were analyzed with ANOVA and multiple comparison tests at alpha=0.05. Results: The bond strength values were significantly lower for dentin surfaces pretreated with 1 and 2 M ethanol based EDC than for the control surfaces (P<0.05). The 0.01, 0.1, and 0.3 M ethanol-based EDC pretreated groups obtained significantly higher bond strength values at 90 days compared to controls. Hydroxyproline release measurements revealed that there were significantly lower levels released in the 0.3 and 1 M ethanol-based EDC pretreated specimens than for controls (P<0.05).
Effects of disinfectants on physical properties of denture base resins: A systematic review and meta-analysis

Abstract:Statement of problem: The disinfection of removable dental prostheses and orthodontic appliances is essential to preventing transmission of pathogens. However, whether different disinfection solutions and durations affect the physical properties of denture base resins is unclear. Purpose: The purpose of this systematic review and meta-analysis was to statistically analyze the influence of disinfectants on the physical properties of denture base resins. Material and methods: A systematic search in Medline, Embase, PubMed, and Cochrane Library databases was conducted to evaluate the effects of chemical disinfection on the physical properties of denture base resins such as surface morphology, roughness, hardness, and flexural strength. Of 1909 studies, 44 studies were included in the systematic review and 41 in the meta-analysis. Heterogeneity was analyzed by using I2 statistics. The influence of different disinfection solutions and durations on the physical properties was further analyzed, and the risk of bias evaluated. Statistical analyses were performed by using the RevMan 5.4 software program with the standardized mean differences (SMDs) and 95% confidence intervals (CIs). Results: Of the 44 included studies, 40 studies were assessed as having a low risk of bias, and 4 had an unclear risk of bias. Meta-analysis results showed that compared with the control, disinfection could not significantly affect surface roughness and hardness within 60 minutes of immersion in disinfectant solutions or flexural strength within 30 minutes (roughness: P=.79, I2=0%; flexural strength: P=.08, I2=0%; hardness: P=.05, I2=19%). In addition, the physical properties were not significantly affected when glutaraldehyde, chlorhexidine, and peracetic acid were repeatedly used for more than 30 minutes. Conclusions: Most of the disinfectants did not reduce the physical properties of denture base resin within 30 minutes of immersion. Glutaraldehyde, chlorhexidine, and peracetic acid are recommended if longer immersion or repeated disinfection is required.

Abstract:Objectives: To compare the permeability of human dentin and a porous ceramic with two different compositions of perfusion fluid, in vitro. Methods: Dentin discs were cut from extracted third molar at a level close to the pulp chamber and ceramic discs were cut from a porous ceramic block. The convective permeability of the discs was measured in a fluid transport model using de-ionised water or 1:3 diluted bovine serum and the hydraulic conductance was determined. Results: In the negative controls, no flow was detected. The hydraulic conductance values for the dentin discs and the ceramic discs when water was used as the perfusion fluid were 0.069±0.023and 0.047±0.019ml min-1 cm-2 cm H2O-1, respectively. When the discs were perfused with the diluted bovine serum the mean (standard error of the mean) hydraulic conductance values for the dentin and the ceramic discs were 0.036±0.016 and 0.012±0.0055 ml min-1 cm-2 cm H2O-1, respectively. Conclusion: Basing on the modern technique it is possible to fabricate discs of hydroxylapatite ceramic to act as substitutes for dentin barriers in cytotoxicity test.

Abstract:The use of cavity disinfectants with composite resins has raised important questions regarding their potential adverse effects on bond strength. The purpose of this study was to evaluate the effect of cavity disinfection on the bond strength of composite resin to dentin. Buccal surfaces of 64 caries-free extracted human third molars were ground flat by diamond flat-end cylinder bur, polished with a series of silicon carbide abrasive papers and mounted in autopolymerizing acrylic resin. They were randomly distributed into four groups (of 16). In group 1 (control group), dentin surfaces were treated with 35% phosphoric acid, Permagen primer and Permagen bonding resin and no disinfectant was used. In group 2, cavity disinfectant was applied and acid etching, priming and bonding procedures were performed as in group 1. In group 3, cavity disinfectant was applied after acid etching. In group 4, cavity disinfectant was applied as in group 3, but it was rinsed off before priming and bonding procedures. Then a Teflon mould 3 mm thick and 4 mm in diameter was attached to the dentin surfaces, filled with composite resin and light polymerized. After the specimens had been thermocycled, shear bond strengths were determined. Application of the disinfectant before or after acid etching significantly reduced the shear bond strength of composite to dentin (P<0.05). Rinsing off the cavity disinfectant before the bonding procedure did not affect the bond strength (P>0.05). The use of cavity disinfectant with composite resin restorations can influence the dentin bond strength.
