Abstract:BACKGROUND:Use biomechanical technique to access the strain distribution and stability of Contour anti-protrusion cage with or without the whole circumference acetabular cup fixation by the transacetabular screws.METHODS:Pelvic specimens from 10 male cadavers were used for the biomechanical test. The models of type C bone defect were copied according to the AAOS classification. Group 1: the contour reconstruction ring was fixed only by flange screws; Group 2: the contour reconstruction ring was fixed both by flange screws and transacetabular screws in dome. Under load in Heel Strike, Midstance, and Toe off phase, the transverse and longitudinal strains were measured in both the superior measure site near the root of the iliac wing and the inferior measure site near the root of the ischium. The relative displacement between the anterior inferior spine and the root of ischium below acetabular component was also measured.FINDINGS:Compared with Group 1, the transverse and longitudinal pressure strain of Group 2 on the superior measure site decreased, and measure sites also showed reductions on the inferior under load under three different gait phases. The result of mean relative displacement showed transacetabular screws fixation couldn't provide significant improvement of displacement prevention.INTERPRETATION:Biomechanical test showed that under the premise of a satisfied bone allograft and well-fixed flange screws in iliac and ischium, the satisfactory initial stability of the reconstruction cup can be achieved, but could not be improved by transacetabular screw technique. However, the max strain in root part of the flange will be reduced using transacetabular screws in ilium, pubis and ischium together, and the strain distribution will also be improved.
Superior cortical screw in osteoporotic lumbar vertebrae: A biomechanics and microstructure-based study.

Combined Use of Extensively Porous Coated Femoral Components with Onlay Cortical Strut Allografts in Revision of Vancouver B2 and B3 Periprosthetic Femoral Fractures

Abstract:Background Periprosthetic femoral fractures following total hip arthroplasty are getting more prevalent. The aim of this study was to evaluate the clinical results of combined use of uncemented extensively porous coated femoral components with onlay cortical strut allografts in revision of Vancouver type B2 and B3 periprosthetic femoral fractures.Methods Thirteen hips after total hip arthroplasty in 13 patients who suffered a Vancouver B2 or B3 periprosthetic fracture were treated with an uncemented extensively porous coated femoral component combined with onlay cortical strut allografts. Each patient was assigned a Harris hip score, and a visual analog scale (VAS) score for pain and satisfaction. Radiographs were examined for component stability, fracture site and allograft-host union and allograft reconstruction.Results At an average of 5.3-year follow-up, no patient required repeat revision. The average Harris hip score was (71.8 +/- 6.3) points, the pain VAS score was (16.6 +/- 4.3) points, and the patient satisfaction VAS score was (81.5 +/- 5.7) points. Radiographic examination showed no detectable loosening of the prostheses, and 12 cases were presented fixation by osseointegration and the other one was stable fibrous ingrown fixation. All fractures united for (5.2 +/- 1.4) months in average. And all onlay strut allografts united to host bone for about (11.5 +/- 2.6) months.Conclusions Combined use of uncemented, long stemmed, extensively porous coated implant with cortical onlay strut allografts can achieve good clinic results and high rate of union for both fracture site and allograft-host bone junction. This technique could be used routinely to augment fixation and healing of Vancouver 132 and 133 periprosthetic fractures. Chin Med J 2009;122(21):2612-2615

Acetabular Revision Using an Anti-Protrusion Cage in Patients with Periprosthetic Acetabular Bone Loss

Abstract:Objective To investigate the risk factors related to the survival time of anti-protrusion cage in acetabular revision.Methods Data of 40 patients who had received acetabular revision using antiprotrusion cages between January 2002 and June 2010 were retrospectively analyzed.There were 16 males and 24 females,aged from 22 to 77 years (average,60.3 years).All patients were followed up for 12 to 82 months (average,39.2 months).According to the AAOS classification,there were 29 cases of type C and 11 cases of type D； while according to the Paprosky classification,there were 29 cases of type ⅢA and 11 cases of type Ⅲ B.The multiple regression analysis was utilized to investigate the relationships between different factors and anti-protrusion cage failure.The corresponding factors included gender,age,bone loss classification,superior or lateral migration of acetabular center of rotation,abduction angle,fixation manners (simple flange fixation or flange fixation plus transacetabular screw fixation),bone grafting techniques and cup type.Results At final follow-up,the average Harris score was 74.5±15.4.Two patients received rerevision due to prosthetic loosening.Obvious radiological loosening of prosthesis combined with pain was found in 2 cases.The statistical analysis indicated that superior migration of acetabular center of rotation,abduction angle and fixation manners were correlated with cup failure,especially when the superior migration was more than 8.5 mm or the abduction angle was larger than 53.5°.Flange fixation plus transacetabular screw fixation could reduce risk of cup failure.Conclusion There is a higher cup failure risk in acetabular revision using an anti-protrusion cage for patients with serious bone defect.However,implanting cup at the level of the true acetabulum as far as possible,decreasing abduction angle properly and using flange fixation plus transacetabular screw fixation can reduce cup failure risk.
Computer Aided Design and Evaluation of New Anatomic Fixation System on Entire Pelvic Model

Abstract:This paper presented a special computer aided procedure to design a new sacroliliac anatomic bar-plate internal fixation system, and evaluated its biomechanical properties on an accurate patient-specific finite element model of entire pelvis, compared with two conventional internal fixation methods. Based on virtual anatomical measure of 30 digital pelvic models reconstructed from CT, an anatomic plate was designed according to the complicated structure of the outer table of the posterior ilium, and was integrated into the complete fixation system. Then, an ad hoc semi-automatic mesh generator was employed to construct a patient-specific finite element model of whole pelvis, including elaborate sacroiliac joints, important pelvic ligaments, and interpubic disc, as well as position-dependent cortical thickness and trabecular bone elastic modulus. Following, one side of sacroiliac joint related ligaments were deleted to simulate a complete unilateral sacroiliac joint disruption. Then the new anatomic fixation system was integrated to fix the fracture, and two comparing models including iliosacral screw fixation and front reconstruction plate fixation were also generated. Finally, all models were simulated under same loading conditions. The results demonstrated that the mechanical stability of the new anatomic fixation system was superior, with obviously improved stress distribution and little displacement, which implied an effective internal fixation method for potential clinical application.
A computer-aided system for longer transacetabular screw trajectory research

Abstract:Objective To overcome the inherent limitations of traditional technique, we promoted that long transacetabular screws could be safely implanted into ilium, pubis and ischium. Methods Twenty 3D hemi-pelvis models were constructed using CT data, and each was divided into four parts: posterior ilium, anterior ilium, pubis and ischium. Each part was undertaken parallel osteotomy and the contour coordinates of each section were recorded. The acetabular cup in each model was defined as a hemisphere with 52 mm diameter. An uniform lattice at equal intervals of 2° in longitude-latitude on the acetabular cup were regard-ed as entry points. A computer aided program was developed to simulate screw trajectories in all directions through every entry point, then select out all the useful entry points and corresponding trajectories. Then the optimum direction of screw trajectory of each entry point was also calculated. Topographic maps of entry zone were delineated in the acetabular quadrant system to present the results. Results According the bone anatomy, the posterior ilium was the most favorable area for the implantation of long screws, secondly the an-terior ilium, thirdly the pubis and minimally the ischium. The entry zone in posterior ilium has the longitude from -148.0° to 29.2°, latitude from 7.5° to 87.3°, and α angle for trajectory direction was from -85.3° to -53.8°,β angle from 46.3° to 60.2°. In anterior ilium has the longitude from -29.0° to 31.4°, latitude from 7.6° to 54.2°, and α angle was from -36.2° to -49.8° ,β angle from 19.7° to 33.8°. In pubis has the longi-tude from 56.4° to 100.6°, latitude from 6.8° to 23.2°, and α angle was from 91.3° to 117.2°, β angle from 10.5° to 17.9°. And the anatomy of ischium was not permitted transacetabular screws to reach the distant 1/3 part of the isehium ramus. Conclusion The long transacetabular screws were permitted to deeper insertion when the trajectories were not perpendicular to the cup wall. The computer-aided system developed in the study was useful for screw trajectory simulation and selection.
Application of Jumbo Cup in Acetabular Revisions for Patients with Massive Acetabular Bone Deficiency

Abstract:Objective To investigate the effects of the jumbo cup in acetabular revision for patients with massive acetabular bone deficiency.Methods We retrospectively studies 48 patients (49 hips) who underwent jumbo cup revisions between 2006 and 2015 (19 men and 29 women;average age:62± 12 years).The acetabular bone defects were classified according to Paprosky classification (Paprosky Ⅱ 27 hips,Paprosky ⅢA 18 hips,Paprosky ⅢB 4 hips).Radiological assessments were conduced to evaluate the acetabular prosthesis loosening,dislocation and infection.The reasons for revisions were mechanical loosening in 41 cases,and infections in another 8 cases with cement spacer putted by last surgery.Results The operation duration and blood loss was 110 min (85-160 min) and 315 ml (270-455 ml) respectively.After an average follow-up of 6.8 years (1-11 years),Harris hip score was increased from 47.6±7.1 preoperatively to 82.4± 6.2 postoperatively.Osseointegration occurred in the acetabular components at 6 months postoperatively.Radiograph analysis showed satisfied position of acetabular cup without complications such as damages of vessels or nerves.The successful rate of jumbo cup revisions was 89.8％±4.4％.However,two in Paprosky ⅢA failures for periprosthetic infection occurred and two in Paprosky ⅢA,one in Paprosky ⅢA and the other in Paprosky ⅢB for acetabular component loosening.One patient in Paprosky ⅢB had weight-bearing pain and relieve slightly after conservative treatment.The radiologic study showed that there was no relationship between failure rate and acetabular abduction angle (r=0.06,P=0.53),rotation center migration in vertical direction (r=0.11,P=0.14) and horizontal direction (r=0.04,P=0.89).Conclusion The mid-term results show that using jumbo cup in revising acetabular failure with massive bone deficiency are optimal,which can be achieved by simplified operation procedures,reduction in the need of bone graft and promotion in acetabulum osseointegration.However,higher failure rate may occur in Paprosky ⅢB patients.
Structural Bone-Grafting with Uncemented Cup for Acetabular Deficiency and Reconstruction

Abstract:Objective To introduce the structural bone-grafting in conjunction with an uncemented cup for reconstruction of the defected acetabula. Methods A retrospective study was done on 26 hips in 25 patients (female 14 and male 11; average age 56 years) with bone loss in acetabula treated by this procedure for acetabular reconstruction in revision surgery (16 cases) and primary arthroplasty (9 cases). According to the American Academy of Orthopaedic Surgeons (AAOS) grading system,bone deficiency of all cases are defined with segmental acetabular defects (AAOS Ⅰ). The acetabular cup was placed at the anatomical level of the primary acetabula,and bone grafts,harvested from autogenous femoral head or freeze-drying allografts,were required for bone loss in all patients. The coverage of the cementless acetabular cup by structural grafts was less than 50%in all cases. The clinical evaluations were conducted with modified Harris hip score,and radiological evaluations. Results All grafts were seen to be incorporated with the host -bone,and no collapse or fracture was found. Minor resorptions were confirmed in two cases. No clinical evidence of loosening of acetabular component was found in all patients,but the radiolucent line around the cup appeared in one case post-operatively,without widening progressively in the later follow-up. The average modified Harris score improved from 50 at preoperation to 88 at the final review. Conclusions The treatment for the acetabular deficiency with structural bone-grafting in conjunction with an cementless cup is effective. Satisfactory results could be obtained with less than 50%coverage of the cup by grafts,and the acetabular bone stock can be restored effectively.
Biomechanical Comparison of Fixation Systems in Posterior Wall Fracture of Acetabular by Finite Element Analysis

Abstract:Background: The use of reconstruction plates and lag screws has been recommended for fractures to the posterior wall of the acetabulum. However, little information about the rigidity of recommended forms of fracture fixation is available. This study aimed to evaluate the biomechanical difference among the fixation systems.Methods: A posterior wall fracture, which is represented by softer elements with lower elastic modulus, was created along an arc of 40-90 degrees about the acetabular rim. Three different fixation systems: screws alone, reconstruction plate with screws, reconstruction plate with T-shaped plates were used to fix the posterior wall fractures to the acetabulum.Results: All three fixation system can be used to obtain good functional outcomes. The reconstruction plate with T-shaped plates was beneficial to increasing the effective stiffness, decreasing the stress concentration and enhancing the rigidity of fracture fixation. So this fixation system served an ideal result in the analysis.Conclusion: Theoretically, the reconstruction plate with T-shaped plates system may reduce many of the risks and limitations compared to the other fixation systems. This fixation system may result in a clinical benefit.
Antegrade Posterior Column Acetabulum Fracture Screw Fixation via Posterior Approach: A Biomechanical Comparative Study

Abstract:Background and Objectives: Minimally invasive surgeries for acetabulum fracture fixation are gaining popularity due to their known advantages versus open reduction and internal fixation. Antegrade or retrograde screw fixation along the long axis of the posterior column of the acetabulum is increasingly applied in surgical practice. While there is sufficient justification in the literature for the application of the anterior approach, there is a deficit of reports related to the posterior approach. The aim of this study was to evaluate the biomechanical competence of posterior column acetabulum fracture fixation through antegrade screw placement using either a standard cannulated screw or a cannulated compression headless screw (CCHS) via posterior approach. Materials and Methods: Eight composite pelvises were used, and a posterior column acetabulum fracture according to the Letournel Classification was simulated on both their left and right sides via an osteotomy. The sixteen hemi-pelvic specimens were assigned to two groups (n = 8) for either posterior column standard screw (group PCSS) or posterior column CCHS (group PCCH) fixation. Biomechanical testing was performed by applying steadily increased cyclic load until failure. Interfragmentary movements were investigated by means of motion tracking. Results: Initial stiffness demonstrated significantly higher values in PCCH (163.1 ± 14.9 N/mm) versus PCSS (133.1 ± 27.5 N/mm), p = 0.024. Similarly, cycles and load at failure were significantly higher in PCCH (7176.7 ± 2057.0 and 917.7 ± 205.7 N) versus PCSS (3661.8 ± 1664.5 and 566.2 ± 166.5 N), p = 0.002. Conclusion: From a biomechanical perspective, CCHS fixation demonstrates superior stability and could be a valuable alternative option to the standard cannulated screw fixation of posterior column acetabulum fractures, thus increasing the confidence in postoperative full weight bearing for both the patient and treating surgeon. Whether uneventful immediate postoperative full weight bearing can be achieved with CCHS fixation should primarily be investigated in further human cadaveric studies with a larger sample size.
Preoperative Three-Dimensional Planning Using Computed Tomography Improves Screw Placement in Patients Undergoing Acetabular Revision Surgery

Abstract:Background: Stable fixation of joint replacement implants is essential to achieve osseointegration in uncemented implants. In acetabular revisions, screws often need to be utilized in quadrants other than the historically so-called "safe" zones to attain sufficient stability. The primary aim of this study was to determine whether preoperative three-dimensional (3D) planning for acetabular revision surgery influences screw length, specifically in the superior pubic ramus (SPR). Methods: Between March 2017 and December 2021, 20 patients underwent preoperative two-dimensional (2D) planning (2D group), and 30 patients underwent 3D planning following the implementation of a new 3D planning software into clinical practice in September 2019 (3D group). Two observers, blinded to the groups, measured the total screw length, screw penetration depth, and cup position on available postoperative computed tomography examinations. For statistical comparisons, the mean measurement from the 2 observers was used. Results: The median total screw lengths in the SPR were 16 mm in the 2D group and 25 mm in the 3D group (P = .004) and 40.5 mm compared with 50.5 mm in the ilium (P = .019). Median screw penetration depths in the SPR were 0 mm in the 2D group and 1.25 mm in the 3D group (P = .049). Conclusion: Longer screws were used in the SPR and ilium when preoperative 3D planning was conducted. Due to the study design, we were not able to evaluate whether longer screws lead to better fixation. Further studies are needed to elucidate this question.

Abstract:This study aimed to compare the fixation stability of a new device (SCCAB) for femoral neck fractures and the cancellous lag screws. Standardized femoral neck osteotomies were created in seven pairs of cadaver femurs and stabilized with either SCCAB or cancellous lag screws. The specimens were subjected to incremental axial loading to 1300 N and cyclic loading at 1300 N for 104 cycles. No significant differences in downward femoral head displacement, change in the superior osteotomy gapping, and load to failure were observed between the two groups. SCCAB may be potentially useful to treat femoral neck fractures.

Jumbo Bionic Trabecular Metal Acetabular Cups Improve Cup Stability During Acetabular Bone Defect Reconstruction: A Finite Element Analysis Study

Abstract:The biomechanical effects of acetabular revision with jumbo cups are unclear. This study aimed to compare the biomechanical effects of bionic trabecular metal vs . titanium jumbo cups for the revision of acetabular bone defects. We designed and reconstructed American Academy of Orthopaedic Surgeons (AAOS) type I–III acetabular bone defect models using computed tomography scans of a man without acetabular bone defects. The implantation of titanium and trabecular metal jumbo cups was simulated. Stress distribution and relative micromotion between the cup and host bone were assessed using finite element analysis. Contact stress on the screws fixing the cups was also analyzed. The contact stress analysis showed that the peak contact stress between the titanium jumbo cup and the host bone was 21.7, 20.1, and 23.8 MPa in the AAOS I–III models, respectively; the corresponding values for bionic tantalum jumbo cups decreased to 4.7, 6.7, and 11.1 MPa. Analysis of the relative micromotion showed that the peak relative micromotion between the host bone and the titanium metal cup was 10.2, 9.1, and 11.5 μm in the AAOS I–III models, respectively; the corresponding values for bionic trabecular metal cups were 17.2, 18.2, and 31.3 μm. The peak contact stress on the screws was similar for the 2 cup types, and was concentrated on the screw rods. Hence, acetabular reconstruction with jumbo cups is biomechanically feasible. We recommend trabecular metal cups due to their superior stress distribution and higher relative micromotion, which is within the threshold for adequate bone ingrowth.

Biomechanical Evaluation of an Adaptive-Motion Pedicle Screw Fixation System: Experimental and Numerical Analysis

Abstract:Rigid fixation is mostly used in thoracolumbar spine surgery, which restricts the thoracolumbar spine segments moving and is not conducive to postoperative rehabilitation. We developed an adaptive-motion pedicle screw and established a finite element model of the T12-L3 segments of the thoracolumbar spine in osteoporosis patients based on the CT image data. A variety of internal fixation finite element models were established for mechanical simulation analysis and comparison. The simulation results showed that compared with the conventional internal fixation system, the mobility of the new adaptive-motion internal fixation system was improved by about 13.8% and 7.7% under the classic conditions such as lateral bending and flexion. in vitro experiments were conducted simultaneously with fresh porcine thoracolumbar spine vertebrae, and the axial rotation condition was taken as an example to analyze the mobility. The in vitro results showed that the mobility of the adaptive-motion internal fixation system had better mobility characteristics under axial rotation conditions, which was consistent with the finite element analysis. The adaptive-motion pedicle screws can preserve a certain degree of vertebral mobility, and avoid excessive vertebral restriction. It also increases the stress value of the intervertebral disk, which is closer to the normal mechanical transmission of the human body, avoiding stress masking and slowing down the degeneration of the intervertebral disk. The adaptive-motion pedicle screws can reduce the peak stress of the implant and avoid surgical failure due to implant fracture.

Abstract:STUDY DESIGN: Cadaveric biomechanical investigation.OBJECTIVES: To test the feasibility of transfacet screws as a minimally invasive posterior fixation device for the cervical spine by comparing the biomechanical stability of transfacet screws to lateral mass screws and rods in a multilevel cervical corpectomy model.SUMMARY OF BACKGROUND DATA: Minimally invasive surgery (MIS) of the spine has gained increasing acceptance and popularity. However, a minimally invasive means of instrumenting the posterior cervical spine has yet to be discovered. Posterior transfacet screws have been described as a means of posterior fixation. In addition, they have the potential of being placed percutaneously through stab incisions. However, validation of transfacet screws in an unstable cervical model in which posterior instrumentation may be necessary has not been carried out till date.METHODS: Sixteen cadaveric cervical spines were randomized to transfacet or lateral mass instrumentation groups. The spines were tested in the following conditions: (a) intact, (b) after multilevel corpectomies with strut graft placement with stand-alone posterior fixation, and (c) with an additional anterior plate over the strut graft. Corpectomy site loading was measured with a custom-designed strut graft. Data were collected for spinal stiffness, range of motion, and strut graft loading, and was analyzed using 2-way analysis of variance (P<0.05).RESULTS: Stand-alone transfacet screw fixation was found to provide inferior spinal stability and resulted in increased spinal motion and graft loading compared with the other constructs (P<0.05 for all).CONCLUSIONS: It is unclear what kind of mechanical stiffness is necessary to stabilize the cervical spine and obtain solid fusion. However, decreased stability and increased graft loading suggest that transfacet screws may not be the ideal method of posterior fixation to supplement multilevel anterior cervical corpectomies and fusions despite their potential as a minimally invasive method for posterior cervical instrumentation.
