Comparative analysis of survival rate and quality of life in axial-flow pump left ventricular assist devices (LVADs)

Abstract:Background: The rising heart failure rates globally show the pressing demand for treatment progress, especially in Left Ventricular Assist Devices (LVADs). Axial-flow pump LVADs are gaining notice for their small size, few moving parts, and potential for miniaturization, providing a vital option for heart transplants during donor shortages. Objectives: Despite several studies on LVADs, there is a notable lack of research specifically comparing axial-flow pumps with similar technology. This gap hinders the identification of the most optimal technology to guide development efforts and meet patient needs. This study aims to comprehensively compare the most commonly used axial-flow pumps and provide a detailed analysis focusing on survival rates and quality of life parameters. Methods: As a developer of axial-flow pumps (LVADs), our group conducted a systematic review of the current axial-flow pump LVADs. We analyzed studies comparing these devices, focusing on key metrics such as survival rates and quality of life. Results: The HeartMate 2 and Jarvik 2000 show superior survival rates (up to 86.9 % at 6 months, 96.3 % at 3 years) and (6-month survival 67 %-91 %) respectively, compared to the other axial flow pumps LVAD. The results underscore the importance of choosing the optimal device and informing the direction of future developments. Conclusion: In this paper, we aim to inform future studies to enhance their effectiveness and advance the overall performance of these devices, ultimately benefiting patients and developers. This review furnishes evidence-based recommendations for the most appropriate axial-flow pumps based on survival rates and quality of life parameters.
Development of a Novel Axial Flow Ventricular Assist Device and its Preliminary Animal Experiment

Abstract:Objective: To design and develop blood pumps for left ventricular assist devices. Methods: Using computational fluid dynamics (CFD), five-axis machine, a prototype of the pump has been designed and fabricated. The pump has been used to conduct in vitro experiments to evaluate the performance of hydraulic performance. The pump has also been tested for hemolysis characteristic through animal experiment. Results: At 9000r/min, the pump generates more than 5L/min flow against 100mmHg. The plasma free hemoglobin was 0.05g/dL. Conclusions: The results obtained from the prototype hydraulic testing and hemolysis testing indicated an acceptable design for the blood pump, representing a significant step in the development phase of this device.
Evaluation of the Hemolysis and Fluid Dynamics of a Ventricular Assist Device under the Pulsatile Flow Condition

Abstract:When the rotary blood pumps are used as ventricular assist devices, the pump flow rate will have a pulsatile component even at a constant impeller rotational speed due to the remaining beating of the natural heart. However, previous studies on the in vitro hemolysis evaluation of a rotary blood pump have always been conducted under steady states and this pulsation was not taken into account. In this study, the hemolysis in a centrifugal blood pump is evaluated under the pulsatile flow condition in vitro. The required time-varying flow rate is obtained by conducting a system simulation of the pump-assisted cardiovascular system, and realized by controlling a pulsation unit in the experiments. The results of our tests indicate a significant increase in hemolysis under the pulsatile flow condition compared with the non-pulsatile condition. To reveal the flow characteristics responsible for the higher hemolysis, transient computational fluid dynamic simulations are then performed. This study suggests that traditional hemolysis evaluation under the steady states may not fully represent the hemolytic performance in the clinical use. For the ventricular assist pumps at the design stage, eliminating the concern about the extra hemolysis under the pulsatile condition will be helpful for the subsequent in vivo experiments.
CFD-Based Flow Channel Optimization and Performance Prediction for a Conical Axial Maglev Blood Pump

Abstract:Ventricular assist devices or total artificial hearts can be used to save patients with heart failure when there are no donors available for heart transplantation. Blood pumps are integral parts of such devices, but traditional axial flow blood pumps have several shortcomings. In particular, they cause hemolysis and thrombosis due to the mechanical contact and wear of the bearings, and they cause blood stagnation due to the separation of the front and rear guide wheel hubs and the impeller hub. By contrast, the implantable axial flow, maglev blood pump has the characteristics of no mechanical contact, no lubrication, low temperature rise, low hemolysis, and less thrombosis. Extensive studies of axial flow, maglev blood pumps have shown that these pumps can function in laminar flow, transitional flow, and turbulent flow, and the working state and performance of such pumps are determined by their support mechanisms and flow channel. Computational fluid dynamics (CFD) is an effective tool for understanding the physical and mechanical characteristics of the blood pump by accurately and effectively revealing the internal flow field, pressure-flow curve, and shear force distribution of the blood pump. In this study, magnetic levitation supports were used to reduce damages to the blood and increase the service life of the blood pump, and a conical impeller hub was used to reduce the speed, volume, and power consumption of the blood pump, thereby facilitating implantation. CFD numerical simulation was then carried out to optimize the structural parameters of the conical axial maglev blood pump, predict the hemolysis performance of the blood pump, and match the flow channel and impeller structure. An extracorporeal circulation simulation platform was designed to test whether the hydraulic characteristics of the blood pump met the physiological requirements. The results showed that the total pressure distribution in the blood pump was reasonable after optimization, with a uniform pressure gradient, and the hemolysis performance was improved.

Using the Hemopump as an example, we developed a model of a canine circulatory system assisted by an axial flow pump, and used computer simulation to predict the effects of the assist device under various hemodynamic conditions. In general, the results from the simulation are in good agreement with that observed in clinical and animal experiments. The same model is further used to explore the techniques and strategy for optimum control of the assist device by introducing an objective function, and choosing suitable membership functions with associated weighting factors, the model offers great flexibility in choosing the targeted hemodynamic variables, in specifying the particular way that each of these variables is to be optimized, and in assigning the relative importance of each targeted variable. The methods used and the results obtained in this study can be incorporated into the design of an advanced physiological controller for a long-term operation of the axial flow pump-based assist device as well as other types of continuous-flow LVAD. Experimental Investigation of the Flow Inside the Rotor Passage of an Axial Ventricular Assist Device

Abstract:Abstract Ventricular assist devices (VAD) are designed to provide circulatory support to patients suffering from advanced-stage heart failure. While not pulsatile, the advantages of continuous axial flow VADs include a compact size and low mechanical failure rate. However, being compact, they operate at high speed, resulting in adverse effects, such as hemolysis caused by high flow shear and thrombosis formation in stagnant regions are common and threaten the successful use of the device. While state-of-the-art computational fluid dynamics (CFD) is widely used in designing these devices, detailed high-resolution experimental measurements of the flow within them are not readily available in the literature. Such experimental data is crucial for understanding the flow inside the VAD and its interaction with blood cells as well as for validating the CFD predictions. The present study investigates the flow inside a 1:1 exact replica of a VAD – ReliantHeart HeartAssist5®. This 12mm diameter device consists of an inlet guide vane (IGV), a rotor and a stator. However, unlike in the real machine, the rotor is driven by a thin shaft that penetrates through the center of the IGV. Refractive index-matching is used to facilitate optical measurements. Hence, all the blades and housing of the pump are made of transparent acrylic. The working fluid is a mixture of water, sodium iodide and glycerin, which matches the refractive index of acrylic, and the kinematic viscosity of blood. Performance tests have been carried out at speeds ranging from 7000 to 9000 RPM. Trends of the results are consistent with those of the actual machine. While scaled data for the pressure rise across the rotor at different speeds collapse, the total head rise across the entire machine does not. High-resolution 2D PIV measurements with vector spacing of 30μm have been conducted in meridional planes of the rotor passage at several blade orientations. They have been performed at 8000RPM and flowrate of 4.5L/min, consistent with physiological requirements. They show that near the rotor front end, the flow in the tip region is dominated by the tip leakage vortex (TLV), associated blockage effects, and very high turbulence level. The upstream end of this domain remains aligned with the leading edge, implying that blades persistently dissect the remnants of a previous TLVs. Interaction of the hub boundary layer with the blades also generates secondary flows. Downstream of the helical section of the rotor, the axial flow is high near the hub and low in the outer part of the passage due to leakage flow-related blockage. Hub boundary layer separation occurs upstream of the stator, generating a large circumferential vortex that occupies nearly half of the rotor span. The complexity of the flow structure and turbulence in this machine would be a challenge to model.
Impeller Optimization and Computational Fluid Dynamics Analysis of Micro-Axial Blood Pumps

Abstract:Commonly,micro-axial blood pump is widely utilized for the cardiac assist of failing natural heart.The pressure-flow,rotation speed and flow fluid can be studied through CFD analyses.Because its performance differs from industrial axial blood pumps,traditional industrial designing method doesn't work.Two types of micro-axial pumps-DAVa and DAVb,are put forward,with impeller number 4,outer diameter 18mm,inner diameter 6.3mm and length 21mm.NACA10-NACA6 are applied to construct the impeller's 5 radial sections,each radial section's discharge angle is as 4/3 or 3/2 as the inner one.CFD analyses result shows:DAVb's pressure head is larger than that of DAVa,and with the inner discharge angle increases from 50° to 80°,DAVb's pressure head increases continuously while DAVa suddenly increases at discharge angle 80°.Besides,DAVa's has torrential and radial flow less than DAVb.Therefore,pressure head increases with discharge angle increases,and proper impeller torsion helps to decrease turbulence,wake and radial flow.
Numerical solution for blood flow in a centrifugal ventricular assist device

Abstract:A very small centrifugal pump, fully supported by magnetic bearings, is being developed for use as a ventricular assist device to be implanted in humans. In this paper, we apply computational fluid dynamics to model the blood flow to aid in the design of the ventricular assist device. The flow of blood through the pump has been modeled using computational fluid dynamics (CFD) software that is commercially available from AEA Technology, UK. The flow regions modeled in version 3 of the Continuous Flow Ventricular Assist Device (CF3) are the fully shrouded four bladed impeller and the two clearance regions around the impeller that are bounded by the pump hub and shroud. This paper describes the geometry and computational grids developed for the flow regions, and the equations of motion for the blood flow are developed. The overall numerically-evaluated flow rates and head rise have similar trends to the flow parameters experimentally measured, indicating that future pump designs can be effectively modeled numerically before being constructed and tested. Numerical solutions are presented and compared with experimentally-obtained overall pump performance results. These solutions are used to predict shear stress levels to be experienced by the blood flowing through the pump, and it is predicted that hemolysis will be insignificant. The solutions also indicate no regions of flow stagnation that can be a source of thrombosis in pumps. The calculations provide a viable design method to achieve improved efficiency in future versions of this pump.

Abstract:BACKGROUND:Hemocompatibility is highly affected by the flow field in Left Ventricular Assistant Devices (LVAD).OBJECTIVE:An asymmetric inflow and outflow channel arrangement with a 45° intersection angle with respect to the blood chamber is proposed to approximate the vascular structure of the aorta and left atrium on the left ventricle. The structure is expected to develop uninterruptible vortex flow state which is similar to the flow state in human left ventricle.METHODS:The Computational Fluid Dynamics (CFD) asymmetric model is simulated using ANSYS workbench. To validate the velocity field calculated by CFD, a Particle Image Velocimetry (PIV) experiment is conducted.RESULTS:The CFD results show that the proposed blood chamber could generate a shifting vortex flow that would be redirected to the aorta during ejection to form a persistent recirculating flow state, which is similar to the echocardiographic flow state in left ventricle. Both the PIV and the CFD results show the development of a persistent vortex during the pulsatile period. Comparison of the qualitative flow pattern and quantitative probed velocity histories in a pulsatile period shows a good agreement between the CFD and PIV data.CONCLUSION:The goal of developing persistent quasi intra-ventricle vortex flow state in LVAD is realized.

Abstract:Many varieties of pulsatile blood pumps exist in the fields of artificial hearts and ventricular assist devices. Effective sorts can be achieved with the differences in power source and transmission mechanism. Horizontal comparison across different pulsatile blood pumps, together with evolution of similar species is studied to find the commonness and evolution laws for pulsatile blood pumps. After a review of typical pulsatile blood pumps from the angle of power source and transmission mechanism, much analysis is focus on a pulsatile drive structure with flexible electro-hydraulic transmission, and importance of hydraulic transmission to improve the implantation property of pulsatile blood pumps is discussed. Finally new application of electro-hydraulic pulsatile blood pumps in the future, such as the application in Direct Mechanical Ventricular Assistant Device (DMVAD) is given.
Computational characterization of flow and blood damage potential of the new maglev CH-VAD pump versus the HVAD and HeartMate II pumps

Abstract:Left ventricular assist devices are routinely used to treat patients with advanced heart failure as a bridge to transplant or a destination therapy. However, non-physiological shear stress generated by left ventricular assist devices damages blood cells. The continued development of novel left ventricular assist devices is essential to improve the left ventricular assist device therapy for heart failure patients. The CH-VAD is a new maglev centrifugal left ventricular assist device. In this study, the CH-VAD pump was numerically analyzed and compared with the HVAD and HeartMate II pumps under two clinically relevant conditions (flow: 4.5 L/min, pressure head: normal ~80 and hypertension ~120 mmHg). The velocity and shear stress fields, washout, and hemolysis index of the three pumps were assessed with computational fluid dynamics analysis. Under the same condition, the CH-VAD hemolysis index was two times lower than the HVAD and HeartMate II pumps; the CH-VAD had the least percentage volume with shear stress larger than 100 Pa (i.e. normal condition: 0.4% vs HVAD 1.0%, and HeartMate II 2.9%). Under the normal condition, more than 98% was washed out of the three pumps within 0.4 s. The washout times were slightly shorter under the hypertension condition for the three pumps. No regions inside the CH-VAD or HVAD had extremely long residential time, while areas near the straightener of the HeartMate II pump had long residential time (>4 s) indicating elevated risks of thrombosis. The computational fluid dynamics results suggested that the CH-VAD pump has a better hemolytic biocompatibility than the HVAD and HeartMate II pumps under the normal and hypertension conditions.

Abstract:Computational fluid dynamics (CFD) and particle image velocimetry (PIV) are commonly used techniques to evaluate the flow characteristics in the development stage of blood pumps. CFD technique allows rapid change to pump parameters to optimize the pump performance without having to construct a costly prototype model. These techniques are used in the construction of a bi-ventricular assist device (BVAD) which combines the functions of LVAD and RVAD in a compact unit. The BVAD construction consists of two separate chambers with similar impellers, volutes, inlet and output sections. To achieve the required flow characteristics of an average flow rate of 5 l/min and different pressure heads (left – 100mmHg and right – 20mmHg), the impellers were set at different rotating speeds. From the CFD results, a six-blade impeller design was adopted for the development of the BVAD. It was also observed that the fluid can flow smoothly through the pump with minimum shear stress and area of stagnation which are related to haemolysis and thrombosis. Based on the compatible Reynolds number the flow through the model was calculated for the left and the right pumps. As it was not possible to have both the left and right chambers in the experimental model, the left and right pumps were tested separately.
A hemodynamic and hemolysis study of the axial pump with hydrodynamic-magnetically levitated impeller in swine

Abstract:Objective To explore the in-vivo hemodynamic and hemolysis effect of a newly designed axial continuousflow ventricular assist device(VAD) in swine.Methods Under general anesthesia,each of 5 swine [weight (40.0 ± 5.2)kg] was implanted with the axial continuous-flow VAD into the apex of left heart ventricle,and the outflow graft was anastomosised to descending aorta.Results All of the axial continuous-flow VAD were implanted successfully with post-operative survival rate 100％.All 5 animals survived over one week.There was a positive correlation between pump speed and assistance effect.The mean left ventricular systolic pressure was (131.6 ± 28.0) mmHg(1 mmHg =0.133 kPa).While the axial continuous-flow VAD was working,left ventricular end diastolic pressure decreased,along with mean intraventricular pressure declined.Peripheral hemodynamics was stable and peripheral blood pressure was not remarkably different from the pressure preoperation.Daily urine volume was in normal range within 1 week post operation.Free hemoglobin in plasma was slightly elevated on the surgery day,and gradually dropped to normal level within 1 week.International Normalized Ratio(INR) was maintained between 2.0-2.5 with oral adminiatration of warfarin of 3 mg/day.There was no thrombosis existing in VAD at autopsy.Conclusion The application of the axial pump with hydrodynamic-magnetically levitated impeller in animal experiment can provide stable hemodynamics,advanced heart unloaded effect,favorable peripheral perfusion,and blood compatibility is satisfactory.

Abstract:BACKGROUND AND OBJECTIVE:Ventricular assist devices (VADs) are generally designed to perform continuous flow. However, it has been proven that continuous flow, which is not a physiological hemodynamic state, may cause severe complications such as gastrointestinal bleeding, pulmonary hypertension, and ventricular suction. For these reasons, many pulsating blood pump control strategies have been proposed and have the potential for application in percutaneous ventricular assist devices (pVADs) or microaxial blood pumps. A few cases report extra hemolysis when introducing pulsating speed, while none involve blood pumps. This research's primary purpose is to evaluate the potential hemolysis of pVAD under pulsating flow conditions.METHODS:First, the pulsating flow state is deduced using a heart failure model and varying speed. The heart model is established according to the pathology state collected from a clinical check. The rotation speed and boundary physical state are set to fit the heart failure model. The computational fluid dynamics (CFD) method with the hemolysis prediction model is performed. Furthermore, we used proper orthogonal decomposition (POD) analysis to reconstruct the flow field and obtain more details about shearing and transporting effects.RESULTS:(1) As a variable rotational speed was introduced, no significant gain in hemolysis accumulation appeared in pVAD. This is quite different from long-term implantable VADs. (2) Pulsation affects hemolysis mainly through pressure (or normal stress). Variable rotational speed affects hemolysis mainly through flow instability. (3) Variable rotational speed will increase the instability and influence hemolysis by transporting and shearing effects, while the transporting effect is more significant.CONCLUSIONS:The unsteady flow state will affect the spatial distribution of hemolysis, which should be taken into account during control strategy and impeller shape design.

Abstract:Thrombosis and its related events have become a major concern during the development and optimization of ventricular assist devices (VADs, also called blood pumps), and limit their clinical use and economic benefits. Attempts have been made to model the thrombosis formation, considering hemodynamic and biochemical processes. However, the complexities and computational expenses are prohibitive. Blood stasis is one of the key factors which may lead to the formation of thrombosis and excessive thromboembolic risks for patients. This study proposed a novel approach for modeling blood stasis, based on a two-phase flow principle. The locations of blood residual can be tracked over time, so that regions of blood stasis can be identified. The blood stasis in an axial blood pump is simulated under various working conditions, the results agree well with the experimental results. In contrast, conventional hemodynamic metrics such as velocity, time-averaged wall shear stress (TAWSS), and relative residence time (RRT), were contradictory in judging risk of blood stasis and thrombosis, and inconsistent with experimental results. We also found that the pump operating at the designed rotational speed is less prone to blood stasis. The model provides an efficient and fast alternative for evaluating blood stasis and thrombosis potential in blood pumps, and will be a valuable addition to the tools to support the design and improvement of VADs.

Abstract:In this study,we investigated the effect of two different driving ways,integral drive and individual one,on the axial flow ventricular assist device.Under the same experiment condition,we compared the pump stability,the vitro hemolysis(the NIH values) and the blood temperature changing curves of these two kinds of drive way.Experimental results indicated that the integrally driven pump showed better blood compatibility,less temperature rise,but weaker stability.We inferred that it's the heat generated by the motor which results in hemolysis.

Abstract:A new streamlined axial blood pump was developed and manufactured, commercial CFD software CFX was used to numerically obtain the pressure head and flow flux relations validated by in-vitro test. A comparison of CFD data with experimental measurements shows the agreement, and the hemolytic properties of the pump was analyzed. The flow field in the pump was also displayed and examined in the view of its impact on efficiency and hemolytic characteristics, to offer the basic support for the further optimization of the blood pump inducing better flow field.
