Abstract:BackgroundA percutaneous ventricular assist device (pVAD) is an effective method to treat heart failure, but its complications, mainly hemolysis and thrombus formation, cannot be ignored. Accurate evaluation of hemolysis and thrombus formation in pVAD is essential to guide the development of pVAD and reduce the incidence of complications.MethodsThis study optimized the numerical model to predict hemolysis and thrombus formation in pVAD. The hemolysis model is based on the power law function, and the multi-component thrombus prediction model is improved by introducing the von Willebrand factor.ResultsThe error between the numerical simulation and the hydraulic performance experiment is within 5%. The numerical results of hemolysis are in good agreement with those of in vitro experiments. Meanwhile, the thrombus location predicted by the numerical model is the same as that found in the in vivo experiment.ConclusionThe numerical model suggested in this study may therefore accurately assess the possible hemolytic and thrombotic dangers in pVAD, making it an effective tool to support the development of pVAD. A hemolysis model for the pVAD has been improved through previous studies and the in vitro hemolysis experiment. Meanwhile, the improved 11-component thrombus simulation model well predicts the location of thrombus generation in the pVAD. The numerical models help evaluate the hemodynamic performance of the pVAD during the design stage.image
Evaluation of the Hemolysis and Fluid Dynamics of a Ventricular Assist Device under the Pulsatile Flow Condition

Abstract:When the rotary blood pumps are used as ventricular assist devices, the pump flow rate will have a pulsatile component even at a constant impeller rotational speed due to the remaining beating of the natural heart. However, previous studies on the in vitro hemolysis evaluation of a rotary blood pump have always been conducted under steady states and this pulsation was not taken into account. In this study, the hemolysis in a centrifugal blood pump is evaluated under the pulsatile flow condition in vitro. The required time-varying flow rate is obtained by conducting a system simulation of the pump-assisted cardiovascular system, and realized by controlling a pulsation unit in the experiments. The results of our tests indicate a significant increase in hemolysis under the pulsatile flow condition compared with the non-pulsatile condition. To reveal the flow characteristics responsible for the higher hemolysis, transient computational fluid dynamic simulations are then performed. This study suggests that traditional hemolysis evaluation under the steady states may not fully represent the hemolytic performance in the clinical use. For the ventricular assist pumps at the design stage, eliminating the concern about the extra hemolysis under the pulsatile condition will be helpful for the subsequent in vivo experiments.

Abstract:BACKGROUND AND OBJECTIVE:Ventricular assist devices (VADs) are generally designed to perform continuous flow. However, it has been proven that continuous flow, which is not a physiological hemodynamic state, may cause severe complications such as gastrointestinal bleeding, pulmonary hypertension, and ventricular suction. For these reasons, many pulsating blood pump control strategies have been proposed and have the potential for application in percutaneous ventricular assist devices (pVADs) or microaxial blood pumps. A few cases report extra hemolysis when introducing pulsating speed, while none involve blood pumps. This research's primary purpose is to evaluate the potential hemolysis of pVAD under pulsating flow conditions.METHODS:First, the pulsating flow state is deduced using a heart failure model and varying speed. The heart model is established according to the pathology state collected from a clinical check. The rotation speed and boundary physical state are set to fit the heart failure model. The computational fluid dynamics (CFD) method with the hemolysis prediction model is performed. Furthermore, we used proper orthogonal decomposition (POD) analysis to reconstruct the flow field and obtain more details about shearing and transporting effects.RESULTS:(1) As a variable rotational speed was introduced, no significant gain in hemolysis accumulation appeared in pVAD. This is quite different from long-term implantable VADs. (2) Pulsation affects hemolysis mainly through pressure (or normal stress). Variable rotational speed affects hemolysis mainly through flow instability. (3) Variable rotational speed will increase the instability and influence hemolysis by transporting and shearing effects, while the transporting effect is more significant.CONCLUSIONS:The unsteady flow state will affect the spatial distribution of hemolysis, which should be taken into account during control strategy and impeller shape design.
Flow Evaluation and Hemolysis Analysis of BVAD Centrifugal Blood Pump by Computational Fluids Dynamics

Abstract:Computational fluid dynamics (CFD) and particle image velocimetry (PIV) are commonly used techniques to evaluate the flow characteristics in the development stage of blood pumps. CFD technique allows rapid change to pump parameters to optimize the pump performance without having to construct a costly prototype model. These techniques are used in the construction of a bi-ventricular assist device (BVAD) which combines the functions of LVAD and RVAD in a compact unit. The BVAD construction consists of two separate chambers with similar impellers, volutes, inlet and output sections. To achieve the required flow characteristics of an average flow rate of 5 l/min and different pressure heads (left – 100mmHg and right – 20mmHg), the impellers were set at different rotating speeds. From the CFD results, a six-blade impeller design was adopted for the development of the BVAD. It was also observed that the fluid can flow smoothly through the pump with minimum shear stress and area of stagnation which are related to haemolysis and thrombosis. Based on the compatible Reynolds number the flow through the model was calculated for the left and the right pumps. As it was not possible to have both the left and right chambers in the experimental model, the left and right pumps were tested separately.

Abstract:AbstractThrombosis is the main reason for the failure of ventricular assist devices (VADs). It has been acknowledged that the platelet activation induced by the nonphysiological blood flow leads to the increased thrombotic risk. However, due to the complicated influence of the VADs’ flow field and the difficulty in real‐time in situ observation, the mechanisms and process of thrombus formation in VADs remain unclear. In this work, the process of thrombus formation in VADs in vitro experiments is observed. The thrombus is found to form on the middle of the inlet guide vanes first and it is mainly caused by the immediate activation of platelets induced by the high shear rate of the flow field around the vanes and also affected by the rotation of the impeller. Then, subsequent thrombus is found in the tail of guide vanes and around the axle journal, where the blood flow is stagnated and the platelets are activated by the accumulated bioagonists. These findings clarify that the thrombus formed on the inlet guide vanes and the axle journal are dominated by two different mechanisms. This work provides unique insights into the initial formation of the thrombus on VADs and helps to reduce the thrombotic risk.

Abstract:Abstract Interventional blood pumps serve as a crucial component for temporary mechanical circulatory support in the treatment of heart failure, specifically designed to improve blood circulation recovery and survival rate in patients undergoing treatment for acute cardiovascular diseases. This study aims to design a novel interventional blood pump with a focus on achieving exceptional hydraulic performance and superior blood compatibility based on numerical simulation, which considers the interactions between the blood pump and the upstream (drainage cannula) and downstream (aorta) flow fields, establishing a full-scale flow field analysis. For the numerical method, the Reynolds-Averaged Navier-Stokes (RANS) equations coupling with the k-ɛ turbulence model are solved. The result indicates that high shear stress exists around the leading and trailing edges of impeller blades and there is a jet at the outlet of impeller, and the blade leading and trailing edge, and the outlet of the impeller are the dominant regions for higher hemolysis occurrence. It is also noted that the presence of an interventional blood pump generates significant vortex structures within the aorta. To effectively reduce the hemolysis index, back-sweep concept is applied to this study also optimize the impeller’s leading edge. The analysis result confirms that the back swept leading edge of impeller blade helps improve the blood compatibility for interventional blood pumps.
Numerical solution for blood flow in a centrifugal ventricular assist device

Abstract:A very small centrifugal pump, fully supported by magnetic bearings, is being developed for use as a ventricular assist device to be implanted in humans. In this paper, we apply computational fluid dynamics to model the blood flow to aid in the design of the ventricular assist device. The flow of blood through the pump has been modeled using computational fluid dynamics (CFD) software that is commercially available from AEA Technology, UK. The flow regions modeled in version 3 of the Continuous Flow Ventricular Assist Device (CF3) are the fully shrouded four bladed impeller and the two clearance regions around the impeller that are bounded by the pump hub and shroud. This paper describes the geometry and computational grids developed for the flow regions, and the equations of motion for the blood flow are developed. The overall numerically-evaluated flow rates and head rise have similar trends to the flow parameters experimentally measured, indicating that future pump designs can be effectively modeled numerically before being constructed and tested. Numerical solutions are presented and compared with experimentally-obtained overall pump performance results. These solutions are used to predict shear stress levels to be experienced by the blood flowing through the pump, and it is predicted that hemolysis will be insignificant. The solutions also indicate no regions of flow stagnation that can be a source of thrombosis in pumps. The calculations provide a viable design method to achieve improved efficiency in future versions of this pump.
Computational characterization of flow and blood damage potential of the new maglev CH-VAD pump versus the HVAD and HeartMate II pumps

Abstract:Left ventricular assist devices are routinely used to treat patients with advanced heart failure as a bridge to transplant or a destination therapy. However, non-physiological shear stress generated by left ventricular assist devices damages blood cells. The continued development of novel left ventricular assist devices is essential to improve the left ventricular assist device therapy for heart failure patients. The CH-VAD is a new maglev centrifugal left ventricular assist device. In this study, the CH-VAD pump was numerically analyzed and compared with the HVAD and HeartMate II pumps under two clinically relevant conditions (flow: 4.5 L/min, pressure head: normal ~80 and hypertension ~120 mmHg). The velocity and shear stress fields, washout, and hemolysis index of the three pumps were assessed with computational fluid dynamics analysis. Under the same condition, the CH-VAD hemolysis index was two times lower than the HVAD and HeartMate II pumps; the CH-VAD had the least percentage volume with shear stress larger than 100 Pa (i.e. normal condition: 0.4% vs HVAD 1.0%, and HeartMate II 2.9%). Under the normal condition, more than 98% was washed out of the three pumps within 0.4 s. The washout times were slightly shorter under the hypertension condition for the three pumps. No regions inside the CH-VAD or HVAD had extremely long residential time, while areas near the straightener of the HeartMate II pump had long residential time (>4 s) indicating elevated risks of thrombosis. The computational fluid dynamics results suggested that the CH-VAD pump has a better hemolytic biocompatibility than the HVAD and HeartMate II pumps under the normal and hypertension conditions.

CFD-Based Flow Channel Optimization and Performance Prediction for a Conical Axial Maglev Blood Pump

Abstract:Ventricular assist devices or total artificial hearts can be used to save patients with heart failure when there are no donors available for heart transplantation. Blood pumps are integral parts of such devices, but traditional axial flow blood pumps have several shortcomings. In particular, they cause hemolysis and thrombosis due to the mechanical contact and wear of the bearings, and they cause blood stagnation due to the separation of the front and rear guide wheel hubs and the impeller hub. By contrast, the implantable axial flow, maglev blood pump has the characteristics of no mechanical contact, no lubrication, low temperature rise, low hemolysis, and less thrombosis. Extensive studies of axial flow, maglev blood pumps have shown that these pumps can function in laminar flow, transitional flow, and turbulent flow, and the working state and performance of such pumps are determined by their support mechanisms and flow channel. Computational fluid dynamics (CFD) is an effective tool for understanding the physical and mechanical characteristics of the blood pump by accurately and effectively revealing the internal flow field, pressure-flow curve, and shear force distribution of the blood pump. In this study, magnetic levitation supports were used to reduce damages to the blood and increase the service life of the blood pump, and a conical impeller hub was used to reduce the speed, volume, and power consumption of the blood pump, thereby facilitating implantation. CFD numerical simulation was then carried out to optimize the structural parameters of the conical axial maglev blood pump, predict the hemolysis performance of the blood pump, and match the flow channel and impeller structure. An extracorporeal circulation simulation platform was designed to test whether the hydraulic characteristics of the blood pump met the physiological requirements. The results showed that the total pressure distribution in the blood pump was reasonable after optimization, with a uniform pressure gradient, and the hemolysis performance was improved.

Abstract:Background and Objective The implantation of ventricular assist devices (VADs) has become an important treatment option for patients with heart failure. Aortic valve insufficiency is a common complication caused by VADs implantation. Currently, there is very little quantitative research on the effects of transcatheter micro VADs or the intervention pumps on the aortic valves. Methods In this study, the multi-component arbitrary Lagrange-Eulerian method is used to perform fluid-structure interaction simulations of the aortic valve model with and without intervention pumps. The effects of intervention pumps implantation on the opening area of the aortic valves, the stress distribution, and the flow characteristics are quantitatively analyzed. Statistical results are consistent with clinical guidelines and experiments. Results The implantation of intervention pumps leads to the valve insufficiency and causes weak valve regurgitation. In the short-term treatment, the valve regurgitation is within a controllable range. The distribution and variation of stress on the leaflets are also affected by intervention pumps. The whirling flow in the flow direction affects the closing speed of the aortic valves and optimizes the stress distribution of the valves. In the models with whirling flow, the effects of intervention pumps implantation on valve motion and stress distribution differ from those without whirling flow. However, the valve insufficiency and valve regurgitation caused by intervention pumps still exist in the models with whirling flow. Conventional artificial bioprosthetic valves have limited effectiveness in treating the valve diseases caused by intervention pumps implantation. Conclusions This study quantitatively investigates the impact of intervention pumps on the aortic valves, and investigates the effect of blood rotation on the valve behavior, which is a gap in previous research. We suggest that in the short-term treatment, the implantation of intervention pumps has limited impact on aortic valves, caution should be exercised against valve regurgitation issues caused by intervention pumps.

Exploratory Simulation of Thrombosis in a Temporary LVAD Catheter Pump within a Virtual In-vivo Left Heart Environment

Abstract:Percutaneous catheter pumps are intraventricular temporary mechanical circulatory support (MCS) devices that are positioned across the aortic valve into the left ventricle (LV) and provide continuous antegrade blood flow from the LV into the ascending aorta (AA). MCS devices are most often computationally evaluated as isolated devices subject to idealized steady-state blood flow conditions. In clinical practice, MCS devices operate connected to or within diseased pulsatile native hearts and are often complicated by hemocompatibility related adverse events such as stroke, bleeding, and thrombosis. Whereas aspects of the human circulation are increasingly being simulated via computational methods, the precise interplay of pulsatile LV hemodynamics with MCS pump hemocompatibility remains mostly unknown and not well characterized. Technologies are rapidly converging such that next-generation MCS devices will soon be evaluated in virtual physiological environments that increasingly mimic clinical settings. The purpose of this brief communication is to report results and lessons learned from an exploratory CFD simulation of hemodynamics and thrombosis for a catheter pump situated within a virtual in-vivo left heart environment.

Abstract:For the optimization of ventricular assist devices (VADs), flow simulations are crucial. Typically, these simulations assume single-phase flow to represent blood flow. However, blood consists of plasma and blood cells, making it a multiphase flow. Cell migration in such flows leads to a heterogeneous cell distribution, significantly impacting flow dynamics, especially in narrow gaps of less than 300 μm found in VADs. In these areas, cells migrate away from the walls, forming a cell-free layer, a phenomenon not usually considered in current VAD simulations. This paper addresses this gap by introducing a viscosity model that accounts for cell migration in microchannels under VAD-relevant conditions. The model is based on local particle distributions measured in a microchannels with a blood analog fluid. We developed a local viscosity distribution for flows with particles/cells and a cell-free layer, applicable to both blood and analog fluids, with particle volume fractions of up to 5%, gap heights of 150 μm, and Reynolds numbers around 100. The model was validated by comparing simulation results with experimental data of blood and blood analog fluid flow on wall shear stresses and pressure losses, showing strong agreement. This model improves the accuracy of simulations by considering local viscosity changes rather than assuming a single-phase fluid. Future developments will extend the model to physiological volume fractions up to 40%.

Abstract:As pump thrombosis is reduced in current-generation ventricular assist devices (VAD), adverse events such as bleeding or stroke remain at unacceptable rates. Thrombosis around the VAD inlet cannula (IC) has been highlighted as a possible source of stroke events. Recent computational fluid dynamics (CFD) studies have attempted to characterize the thrombosis risk of different IC-ventricle configurations. However, purely CFD simulations relate thrombosis risk to ad-hoc criteria based on flow characteristics, with little consideration of biochemical factors. This study investigates the genesis of IC thrombosis including two elements of the Virchow's triad: Endothelial injury and Hypercoagulability. To this end a multi-scale thrombosis simulation that includes platelet activity and coagulation reactions was performed. Our results show significant thrombin formation in stagnation regions (|u|< 0.002 m/s) close to the IC wall. In addition, high shear-mediated platelet activation was observed over the leading-edge tip of the cannula which mirrors the thrombus deposition pattern observed clinically. The current study reveals the importance of biochemical factors to the genesis of thrombosis at the ventricular-cannula junction which can inform clinical decisions in terms of anticoagulation/antiplatelet therapy and guide engineers to develop more robust designs.
Comparison and experimental validation of fluid dynamic numerical models for a clinical ventricular assist device

Abstract:With the recent advances in computer technology, computational fluid dynamics (CFDs) has become an important tool to design and improve blood-contacting artificial organs, and to study the device-induced blood damage. Commercial CFD software packages are readily available, and multiple CFD models are provided by CFD software developers. However, the best approach of using CFD effectively to characterize fluid flow and to predict blood damage in these medical devices remains debatable. This study aimed to compare these CFD models and provide useful information on the accuracy of each model in modeling blood flow in circulatory assist devices. The laminar and five turbulence models (Spalart-Allmaras, k-ε (k-epsilon), k-ω (k-omega), SST [Menter's Shear Stress Transport], and Reynolds Stress) were implemented to predict blood flow in a clinically used circulatory assist device, the CentriMag centrifugal blood pump. In parallel, a transparent replica of the CentriMag pump was constructed and selected views of the flow fields were measured with digital particle image velocimetry (DPIV). CFD results were compared with the DPIV experimental results. Compared with the experiment, all the selected CFD models predicted the flow pattern fairly well except the area of the outlet. However, quantitatively, the laminar model results were the most deviated from the experimental data. On the other hand, k-ε renormalization group theory models and Reynolds Stress model are the most accurate. In conclusion, for the circulatory assist devices, turbulence models provide more accurate results than the laminar model. Among the selected turbulence models, k-ε and Reynolds Stress Method models are recommended.
Numerical Analyses of Idealized Total Cavopulmonary Connection Physiologies with Single and Bilateral Superior Vena Cava Assisted by an Axial Blood Pump

Abstract:Our study evaluated the hemodynamic performance of an axial flow blood pump surgically implanted in idealized total cavopulmonary connection (TCPC) models. This blood pump was designed to augment pressure from the inferior vena cava (IVC) to the pulmonary circulation. Two Fontan procedures with single and bilateral superior vena cava (SVC) were compared to fit the mechanical supported TCPC physiologies. Computational fluid dynamics (CFD) analyses of two Pump-TCPC models were performed in the analyses. Pressure-flow characteristics, energy efficiency, fluid streamlines, hemolysis and thrombosis analyses were implemented. Numerical simulations indicate that the pump produces pressure generations of 1 mm to 24 mm Hg for rotational speeds ranging from 2000 RPM to 5000 RPM and flow rates of 2 LPM to 4 LPM. Two surgical models incorporated with the pump were found to be insignificant in pressure augmentation and energy boost. The risk assessment of blood trauma and thrombosis generation was evaluated representatively through blood damage index (BDI), particle resident time (PRT) and relative resistant time (RRT). The hemolysis and thrombosis analyses declare the advantage of the pump supported bilateral SVC surgical scheme in balancing flow distribution and reducing the risk of endothelial cell destruction and trauma generation.
Simulation of the effect of hemolysis on thrombosis in blood-contacting medical devices

Abstract:Heart failure, broadly characterized by the gradual decline of the ability of the heart to maintain adequate blood flow throughout the body's vascular network of veins and arteries, is one of the leading causes of death worldwide. Mechanical Circulatory Support is one of the few available alternative interventions for late-stage heart failure with reduced ejection fraction. A ventricular assist device is surgically implanted and connected to the left and or right heart ventricles to provide additional bloodflow, off-loading the work required by the heart to maintain circulation. Modern mechanical circulatory support devices generate non-physiological flow conditions that can lead to the damage and rupture of blood cells (hemolysis), and the formation of blood clots (thrombosis), which pose severe health risks to the patient. It is essential to improve prediction tools for blood damage to reduce the risk of hemolysis and thrombosis. A simulation-based approach examines the interaction between hemolysis and thrombosis. Incompressible finite-volume computational fluid dynamics simulations are executed on an open-hub axial flow ventricular assist device. A continuum model of thrombosis and the intrinsic coagulation process is extended to include the effect of hemolysis. The model accounts for the effect of activation of platelets by shear stress, paracrine signaling, adhesion, and hemoglobin and ADP released during hemolysis. The effect of hemolysis with thrombosis is modelled by accounting for the hyper-adhesivity of von-Willebrand Factor on extracellular hemoglobin, and the increased rate of platelet activation induced by ADP release. Thrombosis is assessed at varying inflow rates and rotor speeds, and cases are executed where thrombosis is affected by ADP release and Hb-induced hyper-adhesivity. It is found that there is a non-negligible effect from hemolysis on thrombosis across a range of rotor speeds, and that hyperadhesivity plays a dominant role in thrombus formation in the presence of hemolysis.
Experimental Investigation of the Flow Inside the Rotor Passage of an Axial Ventricular Assist Device

Abstract:Abstract Ventricular assist devices (VAD) are designed to provide circulatory support to patients suffering from advanced-stage heart failure. While not pulsatile, the advantages of continuous axial flow VADs include a compact size and low mechanical failure rate. However, being compact, they operate at high speed, resulting in adverse effects, such as hemolysis caused by high flow shear and thrombosis formation in stagnant regions are common and threaten the successful use of the device. While state-of-the-art computational fluid dynamics (CFD) is widely used in designing these devices, detailed high-resolution experimental measurements of the flow within them are not readily available in the literature. Such experimental data is crucial for understanding the flow inside the VAD and its interaction with blood cells as well as for validating the CFD predictions. The present study investigates the flow inside a 1:1 exact replica of a VAD – ReliantHeart HeartAssist5®. This 12mm diameter device consists of an inlet guide vane (IGV), a rotor and a stator. However, unlike in the real machine, the rotor is driven by a thin shaft that penetrates through the center of the IGV. Refractive index-matching is used to facilitate optical measurements. Hence, all the blades and housing of the pump are made of transparent acrylic. The working fluid is a mixture of water, sodium iodide and glycerin, which matches the refractive index of acrylic, and the kinematic viscosity of blood. Performance tests have been carried out at speeds ranging from 7000 to 9000 RPM. Trends of the results are consistent with those of the actual machine. While scaled data for the pressure rise across the rotor at different speeds collapse, the total head rise across the entire machine does not. High-resolution 2D PIV measurements with vector spacing of 30μm have been conducted in meridional planes of the rotor passage at several blade orientations. They have been performed at 8000RPM and flowrate of 4.5L/min, consistent with physiological requirements. They show that near the rotor front end, the flow in the tip region is dominated by the tip leakage vortex (TLV), associated blockage effects, and very high turbulence level. The upstream end of this domain remains aligned with the leading edge, implying that blades persistently dissect the remnants of a previous TLVs. Interaction of the hub boundary layer with the blades also generates secondary flows. Downstream of the helical section of the rotor, the axial flow is high near the hub and low in the outer part of the passage due to leakage flow-related blockage. Hub boundary layer separation occurs upstream of the stator, generating a large circumferential vortex that occupies nearly half of the rotor span. The complexity of the flow structure and turbulence in this machine would be a challenge to model.

Abstract:A computational fluid dynamics study of blood flow in the continuous flow ventricular assist device, Prototype No. 3 (CFVAD3), which consists of a 4 blade shrouded impeller fully supported in magnetic bearings, was performed. This study focused on the regions within the pump where return flow occurs to the pump inlet, and where potentially damaging shear stresses and flow stagnation might occur: the impeller blade passages and the narrow gap clearance regions between the impeller-rotor and pump housing. Two separate geometry models define the spacing between the pump housing and the impeller's hub and shroud, and a third geometry model defines the pump's impeller and curved blades. The flow fields in these regions were calculated for various operating conditions of the pump. Pump performance curves were calculated, which compare well with experimentally obtained data. For all pump operating conditions, the flow rates within the gap regions were predicted to be toward the inlet of the pump, thus recirculating a portion of the impeller flow. Two smaller gap clearance regions were numerically examined to reduce the recirculation and to improve pump efficiency. The computational and geometry models will be used in future studies of a smaller pump to determine increased pump efficiency and the risk of hemolysis due to shear stress, and to insure the washing of blood through the clearance regions to prevent thrombosis.
A two-fluid blood stasis model for false lumen thrombosis after type B dissection repair

Abstract:The formation of thrombosis is a major concern in the false lumen (FL) for post-TEVAR (thoracic endovascular aortic repair) patients. Blood stasis is one of the key factors which lead to the formation of thrombosis in the arterial systems. This study proposed a computational model for blood stasis, using a two-fluid principle to track the locations of blood residual over time. The current study applied this novel model to evaluate blood stasis and thrombosis potential in four patient-specific post-TEVAR FLs of type B aortic dissection, with their follow-up in-vivo observations two years after TEVAR. The locations and topologies of residual blood in the FL predicted by the model agreed well with the in-vivo observations of thrombus. In addition, the results corresponded better with clinical observations in terms of interpatient comparison of degree of thrombosis, compared with conventional hemodynamic parameters. The blood stasis model serves as a valuable addition to conventional metrics to better predict thrombosis potential. Collectively, these metrics can provide an efficient non-invasive method for evaluating blood stasis and thrombosis potential in arterial system, and useful guidance for clinicians' operative planning and postoperative evaluation.
Computational fluid dynamics analysis of blade tip clearances on hemodynamic performance and blood damage in a centrifugal ventricular assist device

Abstract:An important challenge facing the design of turbodynamic ventricular assist devices (VADs) intended for long-term support is the optimization of the flow path geometry to maximize hydraulic performance while minimizing shear-stress-induced hemolysis and thrombosis. For unshrouded centrifugal, mixed-flow and axial-flow blood pumps, the complex flow patterns within the blade tip clearance between the lengthwise upper surface of the rotating impeller blades and the stationary pump housing have a dramatic effect on both the hydrodynamic performance and the blood damage production. Detailed computational fluid dynamics (CFD) analyses were performed in this study to investigate such flow behavior in blade tip clearance region for a centrifugal blood pump representing a scaled-up version of a prototype pediatric VAD. Nominal flow conditions were analyzed at a flow rate of 2.5 L/min and rotor speed of 3000 rpm with three blade tip clearances of 50, 100, and 200 microm. CFD simulations predicted a decrease in the averaged tip leakage flow rate and an increase in pump head and axial thrust with decreasing blade tip clearances from 200 to 50 microm. The predicted hemolysis, however, exhibited a unimodal relationship, having a minimum at 100 microm compared to 50 microm and 200 microm. Experimental data corroborate these predictions. Detailed flow patterns observed in this study revealed interesting fluid dynamic features associated with the blade tip clearances, such as the generation and dissipation of tip leakage vortex and its interaction with the primary flow in the blade-blade passages. Quantitative calculations suggested the existence of an optimal blade tip clearance by which hydraulic efficiency can be maximized and hemolysis minimized.
