Abstract:Objectives Elevated portal inflow is part of the pathogenesis of portal hypertension in patients with cirrhosis. Vasoactive substances appear to play a primary role in the regulation of portal flow. The aim of this study was to investigate the effects of somatostatin and octreotide on portal pressure and plasma levels of insulin-like growth factor (IGF-1), nitric oxide (NO), endothelin-1 (ET-1) and glucagon (GLU).Methods Portal pressures of 14 cirrhotic patients with portal hypertension who underwent transjugular intrahepatic portosystemic shunt (TIPS) were directly measured via a catheter placed in the portal vein. Portal pressure and IGF-1, NO, ET-1 and GLU plasma levels were determined at baseline, and at 8 h and 24 h after administration of somatostatin or octreotide via portal vein catheter in a randomized, double-blind, cross-over design.Results The average decrease in portal pressure after intravenous infusion of somatostatin and octreotide was 9.4 +/- 1.0 cmH(2)O and 5.0 +/- 1.0 cmH(2)O, respectively (P < 0.01). Plasma levels of GLU and IGF-1 decreased significantly 8 and 24 h after somatostatin and octreotide infusion (P < 0.05). However, there were no significant decreases in plasma NO or ET-1 levels. There was a significant difference between somatostatin and octreotide groups (P < 0.01).Conclusion Both somatostatin and octreotide can significantly reduce portal pressure, although somatostatin is more potent than octreotide. The underlying mechanisms may involve inhibition of the secretion of GLU, IGF-1 and other hormones as well as a decrease in hepatic metabolism and portal inflow leading to a reduction in portal pressure. The Effects of Serum Somatostatin Concentration on Portal Pressure in Patients with Portal Hypertension

Abstract:Objective To study the effects of serum somatostatin concentraton on portal pressure in patients with portal hypertensioa Methods The changes of portal pressure in 14 patients with portal hypertension had been evaluated for 24 hours by direct measurement of portal pressure through a catheter placed in portal vein when somatostatin(250 ng/h)or octreotide(50 ng/h)was infused intravenously by means of a micro-infusion pump. The serum concentration of somatostatin, Glu, NO and EF-1 were determined when portal pressure was measured. Results Serum concentration of somatostatin was increased and portal pressure was decreased significantly after intravenous infusion of somatostatin or octreotide. The serum concentration of somatostain in patients with the infusion of somatostain was higher than that in patients with octreotide. The decrease of portal pressure in patients with somatostain was more than that in patients with octreotide. There was a liner correlation between the concentration of somatostain and portal pressure in patients with the infusion of somatostain. The decrease of Glu concentration and no changes of NO,ET-1 occurred in patients with the infusion of somatostain and octreotide. Conclusion The high concentration of somatostain can be maintained by intravenous infusion of somatostatin or octreotide. Although the serum concentration of somatostain in patients with the infusion of somatostain was higher than that in patients with octreotide , the decrease of portal pressure in patients with somatostain was more than that in patients with octreotide.

The Effects of Octreotide on Portal Hemodynamics in Patients with Liver Cirrhosis

Abstract:OBJECTIVE:To compare the portal hemodynamic effects of different doses of somatostatin analogue octreotide (sandostatin) with beta-blocker (propranolol) on prevention of early variceal re-bleeding in patients with cirrhotic portal hypertension. METHODS:Thirty patients of cirrhosis with moderate to severe esophageal varices were randomly allocated into three groups. 10 patients in group A were given propranolol 10-20 mg orally three times a day for 7 days; 10 patients in group B were administrated octreotide 0.05 mg subcutaneously two times a day for 3 days, while 10 patients in group C received octreotide 0.1 mg subcutaneously two times a day for 3 days. The postprandial vessel diameter, maximal and average flow velocity (Vmax/V) and flow volume of portal vein (PV), splenic vein (SV) and superior mesenteric vein (SMV) were measured before and after therapy by using Echo-Doppler technology. RESULTS:There was a trend of decrease of the vessel diameter in all the three groups, but with no significant difference. However, the average velocity and flow volume of PV were significantly diminished in the propranolol group (P < 0.05), while there was no statistical change of them in SV and SMV groups. In octreotide 0.05 mg and 0.1 mg group, the average velocity and flow volume of PV, SV and SMV were significantly decreased (P > 0.05). Besides, octreotide 0.1 mg group but not 0.05 mg group was more active in decreasing flow volume of PV as compared with the propranolol group, while both octreotide groups were superior to the propranolol group in decreasing flow volume of SV and SMV. CONCLUSIONS:Octreotide may have potential prophylactic effect in prevention of early variceal re-bleeding in cirrhotic patients with portal hypertension.

Abstract:Objective: We explored the effects of Nuclear Factor-E2-related factor 2 (Nrf2) and Heme Oxygenase 1 (HO-1) on splanchnic hemodynamics in portal hypertensive rats.Methods: Experimental cirrhosis with portal hypertension was induced by intraperitoneal injection of carbon tetrachloride. The expression of proteins was examined by immunoblotting. Hemodynamic studies were performed by radioactive microspheres. The vascular perfusion system was used to measure the contractile response of mesentery arterioles in rats.Results: Nrf2 expression in the nucleus and HO-1 expression in cytoplasm was significantly enhanced in portal hypertensive rats. Portal pressure, as well as regional blood flow, increased significantly in portal hypertension and can be blocked by tin protoporphyrin IX. The expression of endogenous nitric oxide synthase and vascular endothelial growth factors increased significantly compared to normal rats, while HO-1 inhibition decreased the expression of these proteins significantly. The contractile response of mesenteric arteries decreased in portal hypertension, but can be partially recovered through tin protoporphyrin IX treatment.Conclusions: The expression of Nrf2/HO-1 increased in mesenteric arteries of portal hypertensive rats, which was related to oxidative stress. HO-1was involved in increased portal pressure and anomaly splanchnic hemodynamics in portal hypertensive rats. (C) 2016 Elsevier Inc. All rights reserved.
Immediate Effects of Different Schedules of Somatostatin on Portal Pressure in Patients with Liver Cirrhosis

Abstract:What is known and Objective: Somatostatin (SST) is used for the treatment of acute variceal bleeding based on its ability to decrease portal pressure and collateral blood flow. To date, no studies have focused on the immediate–early effects (between 1 and 30 min) of SST. The aim of this study was to compare the efficacy of different schedules of SST therapy with placebo on portal pressure in patients with portal hypertension treated with portal-azygous disconnection and to test whether an increase in bolus or infusion dose can improve the clinical efficacy of SST therapy. Methods: Patients were treated with four different schedules: (a) standard dose (n = 11): one 250 μg bolus + a continuous infusion of 250 μg/h; (b) medium dose (n = 10): 500 μg bolus + a continuous infusion of 250 μg/h; (c) high dose (n = 10): 250 μg bolus + a continuous infusion of 500 μg/h; (d) control (n = 10): an injection of placebo (saline) followed by a placebo infusion. Following SST or placebo administration, portal pressure, central venous pressure (CVP), systemic blood pressure and heart rate (HR) were measured at 1, 3, 5, 7, 10 and 30 min. Results and Discussion: The three schedules of SST induced a marked, rapid and highly significant decrease in portal pressure. The decline in portal pressure was moderate at 1 min (P < 0·040), achieved a peak effect at 5 min (P < 0·009) and remained decreased at 30 min. The effect of SST on portal pressure was significantly greater than placebo from 1 min after administration. There were no significant differences in portal pressure decrease between the three schedules of SST. The three schedules of SST and the placebo schedule did not induce significant changes in HR, systemic blood pressure and CVP. What is new and Conclusion: This study shows that SST is effective in decreasing portal pressure within 30 min of administration in patients with liver cirrhosis. The clinical schedule used in this study was reasonable and safe.
[A Clinical Study on Splanchnic Hemodynamic Changes after Orthotopic Liver Transplantation for Patients with Portal Hypertension].

Abstract:OBJECTIVE:To study the regularity of splanchnic hemodynamic changes after orthotopic liver transplantation (OLT) for patients with portal hypertension. At the same time, effect of such changes on splenomegaly, hypersplenism, collateral circulation and the postoperative liver function was discussed.METHODS:Between June 2002 and October 2005, 173 liver transplantations were performed. In 38 patients with portal hypertension undergoing OLT, the following parameters were measured before surgery and subsequently at 1, 3, 5, 7 days, 1, 6 months and 1, 2, 3 years after operation by using Color Doppler sonography: portal blood flow mean velocity (PBV), portal blood flow volume (PBF), hepatic artery resistance indexes (HA-RI) and spleen size. The same parameters were measured in 8 patients with acute liver failure and 20 healthy controls. Meanwhile to observe liver function and varicose vein of esophagus.RESULTS:In cirrhotics, PBV and PBF increased immediately after transplantation [from (13.7 +/- 4.2) cm/s to (58.4 +/- 25.2) cm/s and from (958 +/- 445) ml/min to (3024 +/- 1207) ml/min respectively, P < 0.05]. HA-RI also augmented [from (0.65 +/- 0.11) to (0.74 +/- 0.12), P < 0.05]. PBV returned to normal values after 6 months, PBF returned to normal value after 2 years. Spleen size decreased significantly, but splenomegaly persisted after 3 years. In addition the esophagogastric varix ameliorated significantly.CONCLUSIONS:Abnormal splanchnic hemodynamic changes for patients with portal hypertension still will long-term exist after OLT, but does not effect recovery of hypersplenism, esophagogastric varix and liver function.
Changes of Blood Humoral Substances in Experimental Cirrhosis and Their Effects on Portal Hemodynamics.

Abstract:The changes of humoral substances in the blood of cirrhotic rats were studied together with their effects on portal hemodynamics at different stages during the development of cirrhosis. The profiles of humoral substances and hemodynamics in two different cirrhotic rat models were also investigated. During the development of cirrhosis, glucagon increased markedly in all stages, histamine and vasoactive intestinal polypeptide (VIP) increased in the early stage, serotonin (5-HT) and somatostatin (SS) increased in the middle and late stages. There were different patterns of humoral substances in different cirrhotic models. Glucagon was the main humoral substance elevated in CCL4 induced cirrhosis, but histamine and 5-HT were mainly elevated in the blood in thioacetamide (TAA) induced cirrhosis. The hemodynamics altered differently in different stages during the development of cirrhosis and differently in the two cirrhotic rat models. Exchange transfusions between normal and cirrhotic rats resulted in an elevation of portal flow in normal rats, but no such changes were found after exchange pressure and an increase of portal blood transfusions between normal rats. The relationship between the humoral substances and portal hemodynamics is discussed. The results of this study strongly support the hypothesis of "humoral mechanism" in the pathogenesis of portal hypertension due to cirrhosis.
The effect of partial splenic embolization on the hemodynamics of splenic and portal venous pressure

Abstract:Objective To investigate the changes of splenic and portal venous pressure after partial splenic embolization (PSE),and to discuss its influence factors.Methods PSE was carried out in 14 patients with portal hypertension complicated by hypersplenism.Before and after PSE,the splenic and portal vein pressure was determined by direct manometry method.The post-operative venous pressure was compared with pre-operative one.Contrast-enhanced CT scan was performed 1-2 months after the treatment.Results After PSE,the splenic venous pressure was decreased obviously,a reduction of more than 2 cmH2O in portal venous pressure was observed in 7 patients.Post-operative reexamination with contrast-enhanced CT scan showed that varying degrees of improvement of esophageal and gastric varices could be observed in most patients.Conclusion PSE can effectively decrease the splenic venous pressure,and there are a lot of factors that may influence the short-term reduction in portal vein pressure.

Abstract:Objective To investigate the role and mechanism of prostacyclin in the formation of splanchnic hyperdynamic circulation in portal hypertension(PHT).Methods We investigated the portal vein pressure(PP),norepinephrine(NE) response of isolated mesenteric micro-artery,expression of cyclooxygenase(COX),and concentration of serum 6-keto-prostaglandin F1α(6-keto-PGF1α) in CCl4 induced cirrhotic portal hypertension rats(PHT,n=8) and control rats(N,n=8),and the effect of indomethin(INDO) on PHT rats.Results(1) The PP was significantly higher in PHT group[(14.50±0.87) mm Hg] than in control group[(7.63±0.48) mm Hg],as well as concentration of 6-keto-PGF1α [(78.73±4.57) ng/L vs.(51.60±5.23) ng/L].INDO could not decrease the PP in PHT rats[INDO vs.saline group:(14.63±1.70) mm Hg vs.(15.13±1.36) mm Hg],but it reduced the concentration of 6-keto-PGF1α[INDO vs.saline group:(76.57±5.58) ng/L vs.(42.00±8.75) ng/L].(2) The NE response in mesenteric micro-artery was significantly impaired in PHT rats compared with control rats.INDO could partly improve such hyporesponse.(3)The expression of COX-1 in mesenteric artery significantly increased in PHT rats than in control rats.However,COX-2 were not detected in either PHT or control rats.Conclusions Overproduction of prostacyclin induced by COX-1 plays a role in the formation of hyperdynamic circulation by attenuating the NE response in cirrhotic portal hypertension.

Abstract:Objective To detect the variation of vascular endothelial growth factor (VEGF) in the splanchnic vessels under portal hypertension (PHT) and explore its mechanism and influence on the process of hyperdynamic circulation.Methods In humans,the level of VEGF pathway related proteins were detected in the splenic artery of PHT patients in clinical trials.In animal experiments,the following parameters were observed for rats in the control group (group N,n =7) and the CCl4 induced portal hypertension group (group PHT,n=7):portal vein diameter,portal vein blood flow velocity (PBV),portal vein blood flow (PBF),portal vein pressure (PP),norepinephrine (NE) reactivity in the isolated mesenteric artery microcirculation,contractile reactivity and degree of endothelial ni tric oxide synthase (eNOS) activation in the mesenteric artery by selectively inhibiting the VEGF signal pathway with SU5416.In cell experiments,primary culturing of arterial endothelial cells in vitro were used to verify the effects of VEGF on eNOS activation.Results The results showed that VEGF expression levels in the splenic artery of PHT patients significantly increased.In animal experiments,there was not a significant difference in portal vein diameter between group N and group PHT.How ever,the PBV and PBF of group PHT were lower than those of group N,and SU5416 had no clear effect on PBV and PBF in group PHT.PP of group PHT was much higher than that of group N,and SU5416 had little influence on reducing PP in group PHT.The contractile response of mesenteric artery microcirculation to NE in group PHT decreased significantly,EC50 increased a lot,and SU5416 improved this hypoergia partially.The protein levels of VEGF,VEGFR-2,eNOS,and p-eNOS in the mesentery artery of group PHT raised quite a lot compared to group N,and SU5416 decreased the protein level of VEGFR-2 and activation of eNOS significantly.Experiments in vitro confirmed that VEGF could promote the activation of eNOS.Conclusion The excessive VEGF produced in visceral arteries under PHT may participate in the process of hyperdynamic circulation partially through promoting the synthesis and activation of eNOS and then reducing visceral arteries\u0027 response to NE.\r\n\r\nKey words: \r\nHypertension, portal; Splanchnic circulation; Vascular endothelial growth factors
Doppler Ultrasound Evaluation of Effect of Spleen Spontaneous Shunt on Portal Vein Hemodynamics in Patients with Liver Cirrhosis

Abstract:Objective To investigate the effect of spleen spontaneous shunt on portal vein hemodynamics in patients with liver cirrhosis with Doppler ultrasound.Methods Eighty-seven patients were divided into chronic hepatitis group,liver cirrhosis group and control group.Liver function of cirrhosis patients was classified into Child A and Child B/C according to Child-Pugh categorization criteria.Hemodynamic parameters of hepatic artery,portal vein,superior mesenteric artery/vein and spleen artery/vein were examined on resting condition.Then the blood shunt ratio of spleen vein and superior mesenteric vein with portal vein,as well as hepatic circulation index(HCI)were calculated.The relationship between spleen blood shunt and HCI was analyzed.Results Portal vein blood flow was not significantly different among groups.Spleen vein blood flow increased in cirrhosis group,which was significantly different to that of control group and chronic hepatitis group.The spleen shunt ratio of cirrhosis group was greater than that of control group and chronic hepatitis group,as well as the Child B/C and Child A in cirrhosis group;the ratio of spleen vein flow to portal vein flow and spleen vein flow to superior mesenteric vein flow increased,but the ratio of superior mesenteric vein flow to portal vein flow decreased with the liver function decreased in cirrhosis group.There was non-linear negative correlation between HCI and the spleen shunt ratio.Conclusion Spleen spontaneous shunt and splenic hyperdynamic circulation play an important role in liver perfusion.Detecting V spv /V pv ratio with Doppler ultrasound in patients with liver cirrhosis is helpful in assessing liver function reserve.

Abstract:Portal hypertension is the major cause of complications in decompensated liver cirrhosis.Research results showed that non-selective β-blockers,angiotensin receptor antagonists,and statins can improve portal hypertension by reducing portal vein blood flow and intrahepatic resistance,and have certain prevention and treatment effect on hemodynamic disorders and portal hypertensive complications in chronic liver diseases.Herein,we review the mechanism of action,clinical effects and limitations of these three types of drugs on portal hypertension of cirrhosis.

Abstract:Objective To study the impact of splenectomy and esophagogastric devascularization on the nutritional status of patients with cirrhosis and portal hypertension.Methods Sixty consecutive patients with cirrhosis and portal hypertension who underwent splenectomy and esophagogastric devascularization at the Beijing YouAn Hospital from April 5,2015 to January 23,2017 were included in this study.The body mass index (BMI),albumin (Alb),prealbumin (PA) and lymphocyte counts were prospectively collected at the end of 1-week,1-month,3-month,6-month and 1-year after surgery.The postoperative results were compared with the preoperative results in these patients.Results The BMI results obtained at 1-week and 1-month after surgery were significantly lower than the preoperative level [(22.14 ± 3.08)kg/m2 vs.(22.85 ± 3.14) kg/m2,(21.72 ± 3.05) kg/m2 vs.(22.86 ± 3.16) kg/m2,P ＜ 0.05].The BMI result at the end of 1-year after surgery was significantly elevated when compared with the preoperative level [(23.24 ± 3.64) kg/m2 vs.(22.68 ± 3.47) kg/m2,P ＜ 0.05].The ALB levels at 1-month and 3-month after surgery were significantly higher than the preoperative level [(39.87 ± 4.22)g/L vs.(35.35 ±5.15) g/L,(39.35 ± 4.75) g/L vs.(34.82 ± 5.50) g/L,P ＜ 0.05].The PA obtained at 1-week after surgery was significantly lower than the preoperative levels [(79.59 26.52)mg/L vs.(121.77 ±39.96)mg/L,P ＜ 0.05].The lymphocyte counts at all the points after surgery were significantly higher than the preoperative level (P ＜ 0.05).Conclusion Short term and long term nutritional status improved in patients with cirrhosis and portal hypertension after splenectomy and esophagogastric devascularization.

Abstract:Emerging evidences showed the promoting role of spleen in the development of cirrhosis and hepatocellular carcinoma, in the context of portal hypertension and hypersplenism due to hepatitis B virus-related cirrhosis. In this study, we tried to explore the precise mechanism of how spleen regulates this process from the serum cytokines profile level. Compared with the 16 healthy subjects, the RayBio Human Cytokine Antibody Array identified 136 cytokines differentially expressed in 36 cirrhotic patients. Splenectomy resulted in significant changes in 28 cytokines. Differentially expressed cytokines were mainly involved in cellular processes, responses to stimuli, immune processes, binding, extracellular regions, and extracellular matrix. These cytokines were mainly enriched in cytokine-cytokine receptor interactions, Jak-STAT and MAPK signaling pathways, and pathways in cancer. These results may provide new clues to the function of the spleen, and modulation of cytokine expression maybe a potential alternative therapeutic strategy for these patients.
Study on Mrna Expression of Subtypes of Somatostatin Receptors in the Liver Tissue of Liver Cirrhotic Rats

Abstract:Objective To investigate the changes in mRNA expression of subtypes of somatostatin receptors (rSSTR) during the development of portal hypertension in liver cirrhotic rats. Methods A total of 30 rats were randomly divided into 3 groups: control group, necrosis group and cirrhosis group. Total RNA was extracted from each sample and semi-quantitative reverse transcription and ploymerase chain reaction(RT-PCR) were used to detect the expression of mRNA of rSSTR. Results In the control group, all 5 subtypes of rSSTR were expressed in liver, and rSSTR4,rSSTR1 and rSSTR5 mRNA expression was significantly higher than rSSTR3 and rSSTR2 mRNA expression ( P 0.05). In the necrosis group, mRNA expression of rSSTR was significantly decreased as compared with that in the control group and cirrhosis group ( P 0.05). In the cirrhosis group, mRNA expression of rSSTR2 (161.97±40.42) was significantly increased as compared with that in the control group (105.57±37.91, P 0.05), while mRNA expression of rSSTR1(171.33±44.59) and rSSTR4(158.26±37.79) was significantly decreased as compared with that in the control group ( P 0.05). Conclusion The stimulated gene of specific rSSTR subtypes could increase the synthesis of mRNA of specific rSSTRs to regulate the rSSTRs-mediated effect of the analogues of somatostatin in the treatment of portal hypertension of liver cirrhosis.\;

Abstract:Objective: To detect the variation of calmodulin( CaM) in the splanchnic arteries under portal hypertension(PHT),and investigate its effect on the hyperdynamic circulation of PHT and its possible mechanism.Method: Portal vein blood flow,portal vein pressure,the reaction of mesenteric microcirculation artery to norepinephrine(NE) were detected and compared between the control group(n = 8) and PHT group(the rats with the liver cirrhosis caused by CCl4,n = 8). The influence of the specific inhibitor of CaM-W-7 on the contractile response of isolated mesenteric microcirculation artery and the protein expression of CaM and phosphorylation of endothelial nitric oxide synthase(p-eNOS) in mesentery artery were observed. Results:(1) Portal vein blood flow of PHT group was apparently lower than that of control group,W-7 had no significant effect on portal vein blood flow of PHT group.(2) Portal vein pressure of PHT group was much higher than that of control group,W-7 had little influence on reducing portal vein pressure of PHT group either.( 3) The contractile response of mesenteric microcirculation artery to NE in PHT group was decreased significantly,EC50increases significantly,W-7 could improve this kind of hypoergia partially.(4) Compared with control group,the protein expression of CaM and p-eNOS in mesentery artery of PHT group increased quite a lot,W-7 could decrease the protein expression of p-eNOS significantly. Conclusion: The overproduced CaM in the mesenteric artery of the PHT rats may activate the eNOS by participating in or cooperating with the producing of p-eNOS,then enhance the synthesis of nitric oxide(NO),reduce the contractile response to norepinephrine of mesenteric microcirculation artery,cause the splanchnic vasodilatation,involve in the formation of splanchnic hyperdynamic circulation.

Abstract:AIM:To investigate the effects of somatostatin on prolifera- tion and apoptosis of activated rat hepatic stellate cells (HSCs). METHODS:Being isolated from liver of SD rat by in situ perfusion with collagenase and pronase and density gradient, HSCs became activated after 10 days. Then the rates of apoptosis and proliferation of HSCs, which had been treated with different dosage of somatostatin, were measured by acridine orange/ethidium bromide fluorescent stain, termi- nal deoxynucleotidyl transferase-mediated dUTP nick end labeling, transmission electron microscopy, flow cytometry and MTT assay. Furthermore, the mechanisms of somatosta- tin were investigated with the aid of cytodynamic analysis. RESULTS:somatostatin was able to decrease the prolifera- tion rate as well as promote the programmed cell death of activated rat HSCs.Its dose-dependent action,in which G0/G1 arrest played an important role, was strongest when the con- centration of somatostatin reached 10 -6 mol/L or 10 -7 mol/L. CONCLUSION:Anti-proliferative and proapoptotic effects on activated rat HSCs can be obtained by a low dose of somatostatin. These findings reveal the potential antifibrotic value of somatostatin.
