Abstract:Portal hypertension (PHT), as a disease with high incidence all over the world, badly affects the patients' health. The research on hemodynamics plays an important role in exploring the pathogenesis of PHT, assessing the risk of hemorrhage induced by the complications, selecting the scheme and time of surgical operations as well as evaluating the curative effects of medication. In this article, the main factors in the pathogeny of PHT are reviewed and the surgical treatments are discussed from the hemodynamic viewpoint. Then some recent results in the hemodynamic research of PHT are summarized. In addition, main ideas are put forward as to establishing a new global biomechanical model of PHT.

Abstract:Liver transplantation makes it possible to cure portal hypertension thoroughly.A new pattern of combined application of multiple treatments based on liver transplantation has emerged.Conventional surgical treatments such as splenectomy,devascularization and transjugular intrahepatic portosystemic shunting have important effects on the prognosis of liver transplant recipients.Portal vein stenosis,thrombosis as well as small-for-size liver graft lead to post-transplant portal hypertension and liver dysfunction.The author in this article provides an overview of the effects of various conventional treatments on the prognosis of recipients and the treatment of portal hypertension after liver transplantation,aiming to seek for optimal strategy.

Abstract:Portal hypertension(PH)is a clinical syndrome,character-ized by elevated pressure gradient between portal vein and inferior vena cava.These elevated pressures gradient due to increased vascular resistance and/or increased volume of blood flowing through the portal vein circulation,results in blood outflow difficulty from portal vein to hepatic veins and inferior vena cava,and eventually cause life-threatening complications including ascites,gastroesophageal varices,and hepatic encephalopathy.

Abstract:Background Cirrhosis patients often exhibit clinical symptoms such as right liver atrophy, portal hypertension, spleen enlargement and increased blood supply, which exhibit considerable variation between the left and right liver sections. These differences are hypothesized to stem from disparities in blood flow within the left and right portal vein (PV) branches. However, rigorous quantitative evidence remains scarce. Purpose We mainly aim at quantitatively revealing the relationship between the blood flow rates of two PV branches and liver volumes, and providing quantitative evidence and theoretical support for the diagnosis and treatment of cirrhosis from the perspective of hemodynamics. Methods Five cirrhotic patients and two healthy volunteers from Beijing Friendship Hospital are investigated. Their PV blood flow models are established based on computed tomography (CT) images and finite volume simulations. The volume of the left and right liver lobes are measured in 3‐matic. The distributions of blood source in the PV branches are tracked by streamline analysis. The blood flow rates are quantitatively counted by integrating the blood source velocities. Linear analysis is performed to build the relationship between liver volumes and PV blood flow distributions. Results Streamline analysis reveals significant differences in blood distribution between the left and right PV branches. The majority of blood from the superior mesenteric vein (SMV) flowed into the right portal vein (RPV), while most blood from the splenic vein (SV) entered the left portal vein (LPV). The main PV pressure drop linearly increases with the SV blood velocity for all PV structures of patients and healthy volunteers. The flow rate ratio QRPV/QLPV demonstrates an increase in tandem with the volume ratio VR/VL, exhibiting a linear correlation with the Pearson correlation coefficient being 0.93. Conclusion The differences in the blood distributions are consistent with the clinicians' knowledge and validate our simulations. We demonstrated a linear increase in PV pressure with elevated SV blood velocity. Additionally, the volumes of the left and right hepatic lobes exhibited a positive correlation with blood flow rates in the corresponding PV branches.

Abstract:Aim:The transjugular intrahepatic portosystemic shunt(TIPS)procedure has been performed in China for 30 years,but our understanding of the status and difficulties associated with the use of TIPS for portal hypertension is incomplete.This study aims to investigate the current situation of the use of TIPS for portal hypertension,which should aid the development of TIPS in China.Methods:This study involved a questionnaire survey,initiated by the China Portal Hypertension Alliance(CHESS)to investigate the use of TIPS for portal hypertension in China.Questionnaires were released on the Internet between August 16,2022 and September 16,2022.Detailed information was collected from 545 hospitals contracted by the CHESS in China,which performed TIPS surgery in 2021.Results:The 545 hospitals covers the majority of hospitals in China that conduct TIPS intervention.In the survey,a total of 13,527 TIPS interventions were carried out with 94.1％(498/545)of them had a system of routine follow-up after TIPS.The majority(42.6％[232/545])believed that the main indication of TIPS was the control of acute bleeding.Overall,48.1％(262/545)carried out early or pre-emptive TIPS,53.0％(289/545)carried out TIPS for cavernous transformation of the portal vein,and 81.0％(441/545)chose routine embolization of collateral circulation during operation.Most used coils(97.0％[475/490])and biological glue(72.0％[353/490])as embolic materials,and 78.5％(428/545)routinely performed intraoperative portal pressure gradient measurements.When selecting TIPS stents,57.1％(311/545)chose viator-specific stents,and 57.2％(312/545)chose conventional anticoagulation after TIPS.Conventional anticoagulation after TIPS is over a time interval of 3-6 months(55.4％[173/312]).The limitation of TIPS surgery is mainly its cost(72.3％[394/545])and insufficient understanding of the procedure by doctors in other related departments(77.4％[422/545]).Most of the hospital teams accept the use of domestic instruments in TIPS surgery(92.7％[505/545]).Conclusions:This survey shows that TIPS is an essential part of the treatment of portal hypertension in China.TIPS is widely used in Chinese patients with portal hypertension,and this survey presents a more comprehensive picture of the options and difficulties encountered in the implementation of TIPS.In the future,it is necessary to promote TIPS technology and further standardize surgical indications,routine operations,and instrument application.
Application of Hepatic Transit Time and Shear Wave Velocity in Assessing Portal Pressure in Patients with Cirrhotic Portal Hypertension

Abstract:The aim of the study described here was to explore the value of hepatic transit time (HTT) and shear wave velocity (SWV) in diagnosing cirrhotic portal hypertension. Fifty-seven patients had undergone esophagogastric varicose vein embolization (group OBS) and 50 healthy controls (group CON) were retrospectively compared with respect to HTT (arterial–hepatic vein [HA-HVTT], portal vein–hepatic vein [PV-HVTT], liver parenchyma–hepatic vein [PA-HVTT]) and SWV to analyze their efficacy in diagnosing cirrhotic portal hypertension. The correlations between SWV/HTT and free portal pressure (FPP) in group OBS were also analyzed. Compared with group CON, group OBS had a shorter HTT and faster SWV. The area under the curve (AUC) of PV-HVTT (0.93) was higher than those of HA-HVTT (0.75) and PA-HVTT (0.64), the AUCs of PV-HVTT (0.93, threshold 7.9 s) and SWV (0.91, threshold 2.0 cm/s) did not statistically differ (z = 0.35, p = 0.73). HTT and FPP in group OBS had a negative correlation. In conclusion, HTT and SWV can be used to diagnose cirrhotic portal hypertension without difference in diagnostic efficacy, and HTT is more meaningful for assessing the changes in portal pressure.

The relationship of portal hemodynamics to nitric oxide and endothelin levels in patients with liver cirrhosis

Abstract:To study the relationships of portal hemodynamics to vasoactive substances - nitric oxide (NO) and endothelin (ET) in cirrhosis patients, 48 patients with liver cirrhosis (LC) and 32 normal controls (NC) were studied, including 18 patients with compensated LC and 30 patients with decompensated LC. Portal venous and splenic venous blood flow (PVBF and SVBF) were measured by color duplex doppler ultrasonography, and the blood coucentrations of ET and NO were detected. After analyzing the correlation between PVBF and SVBF to NO(ET), the results suggest that protal hy- perdynamic changes, which exist in cirrhosis patients from beginning to the end, were possibly due to the high NO level and the low sensitiving to ET, and it is useful for the clinical treatment.

The Changes of Biomechanical Properties of the Portal Veins in the Rats During the Pathogenesis of Intrahepatic Portal Hypertension

Abstract:Objective To observe the dynamic changes of opening angle in zero-stress state and the relationship between tension stress (T) and extension ratio (λ) in longitudinal extension of the portal vein during the pathogenesis of intrahepatic portal hypertension, and investigate the role of biomechanical properties of the portal veins in the process of the portal hypertension. Methods The models of intrahepatic portal hypertension were established in SD rats by means of carbon tetrachloride (CCl4) injection. Opening angle, the relationship between T and λ in longitudinal extension of the portal vein were determined in 2, 4, 6, 8, 10 weeks after CCl4 injection. Meanwhile the changes of several hemodynamic indexes such as portal venous pressure (PVP), portal venous flow (PVF), mean arterial pressure (MAP), portal vascular resistance (PVR) and splanchnic vascular resistance (SVR) were detected. Results After CCl4 injection, hemodynamic indexes in the CCl4 rats had changed markedly. Accordingly, opening angles, Parameter b in longitudinal extension were increased. At the tenth week, they were much greater than those of corresponding controls (P0.05). Conclusion The portal hypertensive rats were in hyperdynamic circulatory state (HCS). Changes of biomechanical properties of portal vein can be induced by HCS.

Noninvasive Assessment of Portal Hypertension by Ultrasonography

Abstract:To explore the feasibility of noninvasive assessment of portal hypertension (PHT) by ultrasonography. A total of 18 PHT patients who were planning to undergo splenectomy combiend with pericardial devascularization received ultrasonography one week before surgery (PHT group). In addition, 20 healthy volunteers were enrolled as the controls (control group). The conventional two-dimentional (2D) ultrasound, Doppler ultrasound and contrast-enhanced ultrasound (CEUS) parameters were compared between these two groups. For patients in the PHT group, the portal vein pressure was measured using portal vein puncture, and its relationship with the above parameters were further analyzed. The 2D ultrasound, Doppler ultrasound and CEUS parameters showed significant differences between the PHT group and control group (p < 0.01). Three ultrasonographic features including area under the time-intensity curve (TIC) of portal venous phase/hepatic arterial phase (Qp/Qa), intensity of portal venous phase/hepatic arterial phase (Ip/Ia), and base-to-peak ascending slope (βp/βa) were negatively correlated with the portal vein pressure (correlation coefficient: -0.701, -0.625, and -0.494, respectively). In this study, Qp/Qa and Ip/Ia have reduced significantly in PHT group compared with that in the control group (p <0.01), suggesting that in PHT patients, intensity and proportion of portal vein perfusion declined; while intensity and proportion of hepatic artery perfusion gradually rise due to buffering effect, thus gradually reversed the proportion of blood supplying through portal vein and hepatic arteries. Meanwhile, the decline of βp/βa, TTP, and RT confirmed that the increased flow velocity of the hepatic artery and decreased flow velocity of portal vein led to increased slope of TIC curve of the ascending branch of hepatic artery segment and decreased slope in portal vein segment, moreover; meanwhile, the increased overall slope of curve caused an advanced time to peak. In PHT group, Qp/Qa, Ip / Ia, and βp/βa were negatively correlated with the portal vein pressure, with correlation coefficients of -0.701, -0.625 and -0.494, respectively, indicating that these three ultrasound contrast parameters not only objectively reflected the pathophysiological process and the hemodynamic characteristics of chronic liver diseases but also showed a good correlation with portal pressure, therefore it was expected for clinical use as a new method for noninvasive assessment of portal vein pressure in PTH patients.
Clinical analysis of surgical treatment of portal hypertension.

Abstract:AIM: To review the experience in surgery for 508 patients with portal hypertension and to explore the selection of reasonable operation under different conditions. METHODS: The data of 508 patients with portal hypertension treated surgically in 1991-2001 in our centers were analyzed. Of the 508 patients, 256 were treated with portaazygous devascularization (PAD), 167 with portasystemic shunt (PSS), 62 with selective shunt (SS), 11 with combined portasystemic shunt and portaazygous devascularization (PSS+PAD), 9 with liver transplantation (LT), 3 with union operation for hepatic carcinoma and portal hypertension (HCC+PH). RESULTS: In the 167 patients treated with PSS, free portal pressure (FPP) was significantly higher in the patients with a longer diameter of the anastomotic stoma than in those with a shorter diameter before the operation (P<0.01). After the operation, FPP in the former patients markedly decreased compared to the latter ones (P<0.01). The incidence rate of hemorrhage in patients treated with PAD, PSS, SS, PSS+PAD, and HCC+PH was 21.09% (54/256), 13.77 (23/167), 11.29 (7/62), 36.36% (4/11), and 100% (3/3), respectively. The incidence rate of hepatic encephalopathy was 3.91% (10/256), 9.58% (16/167), 4.84% (3/62), 9.09% (1/11), and 100% (3/3), respectively while the operative mortality was 5.49% (15/256), 4.22% (7/167), 4.84% (3/62), 9.09% (1/11), and 66.67% (2/3) respectively. The operative mortality of liver transplantation was 22.22% (2/9). CONCLUSION: Five kinds of operation in surgical treatment of portal hypertension have their advantages and disadvantages. Effects of Somatostatin-14 and -8 on Portal Hemodynamics in Patients with Portal Hypertension after Intrahepatic Portosystemic Shunt

Abstract:Objective:To study the effects of Somatostatin-14 and-8 on portal hemodynamics and plasma levels of the insulin-like growth factor(IGF-1),nitric oxide(NO),endothelin-1(ET-1) and glucagon(GLU) in patients with portal hypertension after transjugular intrahepatic portosystemic shunt(TIPS). Methods:Fourteen portal hypertension patients underwent TIPS,their portal pressures directly measured by the intravenous catheter placed in the portal vein,the hemodynamic changes observed and the plasma levels of IGF-1,NO,ET-1 and GLU determined before and 8 and 24 hours after the infusion of Somatostatin-14 or-8 according to a cross-over design.Results:The average decrease in portal pressure after the intravenous infusion of Somatostatin-14 and-8 was(9.4±1.0) cmH2O and(5.0±1.0) cmH2O,respectively(P 0.01),with significant differences before and after the infusion as well as between the Somatostatin-14 and-8 groups(P 0.01).Significant reduction was observed in GLU and IGF-1(P0.05),but not in NO and ET-1(P0.05) 8 and 24 hours after the infusion.Conclusion:Both Somatostatin-14 and-8 can significantly reduce portal pressure,although the former is more potent than the latter.The underlying mechanism may involve their inhibition of the secretion of GLU,IGF-1 and other hormones as well as their reduction of hepatic metabolism and portal inflow.
Long-term Study of Hemodynamic Changes in Portal Venous Flow after Liver Transplantation with Ultrasound

Abstract:Objective To evaluate the long-term changes of portal venous flow after liver transplantation with ultrasound.Methods The effect of liver transplantation on portal venous hemodynamics was recorded by color Doppler sonography in 121 patients who were followed for up to 4 years.Sonographic parameters such as portal blood flow mean velocity and spleen size were analyzed among 40 controls.Results (1) Portal blood velocity increased dramatically from 1 to 3 days after transplantation and then declined gradually.There was no significant difference between control and transplantation groups after 12 months(P>0.01);(2)Spleen size decreased gradually but still remained significantly bigger than that in the control group after 4 years(P<0.01).Conclusions Portal blood velocity was not different from normal subjects 12 months after transplantation,but splenomegaly lasted a long period,which probably indicated that normalization of splanchncic hemodynamics parameters required more time.

Abstract:OBJECTIVE:To explore the relationship between the portal hemodynamics or the indices of liver fibrosis and the liver function score in patients with different traditional Chinese medical syndromes of liver cirrhosis.METHODS:One hundred and forty-seven cases of liver cirrhosis with different syndromes were included in the study. Diameters and blood flow velocities of the portal vein and splenic vein were tested by color Doppler sonarography. The indices of liver function (TBIL, ALT, AST, Alb, Glb, PTA) and the indices of hepatic fibrosis (HA, PC-III, LN, VI-C) were tested.RESULTS:The diameters of portal vein in liver cirrhosis patients with different syndromes (internal accumulation of of damp-heat, stagnation of liver-qi, superabundance of dampness due to spleen-asthenia, asthenia of liver and kidney yin, asthenia of spleen and kidney yang, blood stasis) were not significantly different. The blood flow velocities and flow volumes of portal vein in patients with the blood stasis syndrome and the internal accumulation of damp-heat syndrome were significantly different (P < 0.05). The diameter of splenic vein in patients with the internal accumulation of damp-heat syndrome was significantly different to that of the asthenia of liver and kidney yin syndrome, or the blood stasis syndrome, or the asthenia of spleen and kidney yang syndrome, respectively (P < 0.05 or P < 0.01). The blood flow velocity of splenic vein in patients with the blood stasis syndrome or the asthenia of spleen and kidney yang syndrome was significantly different to that of the internal accumulation of damp-heat syndrome, or the stagnation of liver-qi syndrome, or the asthenia of liver and kidney yin syndrome, respectively (P < 0.01). The blood flow volume of splenic vein in patients with the internal accumulation of damp-heat syndrome was significantly different to that of the asthenia of liver and kidney yin syndrome, or the asthenia of spleen and kidney yang syndrome, or the blood stasis syndrome, respectively (P < 0.01). The liver cirrhosis indices HA and PC-III in patients with the asthenia of liver and kidney yin syndrome were significantly different to those of the internal accumulation of damp-heat syndrome (P < 0.05). In Child-Pugh classification, the incidence of Child-Pugh C was the highest in the asthenia of spleen and kidney yang syndrome, while the incidence of Child-Pugh A was the highest in the stagnation of liver qi syndrome.CONCLUSION:The changes of portal vein dynamics in liver cirrhosis patients with the asthenia of liver and kidney yin syndrome, or the asthenia of spleen and kidney yang syndrome, or the blood stasis syndrome are significantly different to those of the internal accumulation of damp-heat syndrome and the stagnation of liver-qi syndrome.
The Dynamic Changes of Plasma Nitric Oxide and Endothelin-1 in Prehepatic Portal Hypertension Rats

Abstract:AIM: To observe the dynamic changes of plasma levels of nitric oxide(NO) and endothelin (ET-1) in portal veins of the rats during prehepatic portal hypertension, and investigate the role of them in hyperdynamic circulation. METHODS: The models of prehepatic portal hypertension were established in Sprague-Dawley rats by means of partial portal vein ligation (PVL). The plasma levels of nitrite/nitrate (NO - 2/NO - 3) and ET-1 in the portal veins were detected by the method of nitric reductase and radioimmunoassay, respectively. In this study, rats were divided into normal, sham operation (SO) and PVL group. SO and PVL rats were divided into several subgroups according to different time after operations. Meanwhile, the changes of several hemodynamic indexes in these rats were also measured. RESULTS: The levels of NO - 2/NO - 3 were significantly increased and ET-1 were significantly decreased in rats at different time after PVL compared with normal control, whereas the hemodynamic indexes changed accordingly. CONCLUSION: The portal hypertensive rats are in hyperdynamic circulatory state (HCS). NO and ET-1 may play an important role in the induction and maintenance of HCS.

Abstract:Paroxysmal nocturnal hemoglobinuria is a rare intravascular hemolytic anemia, and thrombosis is the leading cause of mortality rate. The hepatic veins is the common site where Budd-Chiari syndrome usually occurs. We confronted a patient who simultaneously happened to have portal vein and superior mesenteric vein thrombosis leading to prehepatic portal hypertension and upper gastrointestinal bleeding. Percutaneous thrombolysis is an efficacious treatment.
