Abstract:OBJECTIVETo evaluate the safety and efficacy of laparoscopic splenectomy (LS) and pericardial devascularization in treatment of portal hypertension due to liver cirrhosis.METHODSTwenty three cases with hepatitis B and schistosoma cirrhosis and portal hypertension underwent LS and paraesophagogastric devascularization performed by one treatment team. Follow-up was conducted for 9 months.RESULTSLS combined with pericardial devascularization was successfully performed on these 20 cases. Three cases were converted to open surgery due to intra-operative bleeding. The mean operative time was 235 min (180 - 350), and mean intra-operative blood loss was 520 ml (200 - 1600 ml). All patients were treated with plasma transfusion, antibiotics, and abdominal drainage post-operatively. Peristalsis of stomach and intestine recovered 24 - 72 hours after operation. The mean hospitalization time was 8.5 days (6 - 17 days). The peri-operative complication included plural effusion in 3 cases and subphrenic abscess in one case, .mild ascites in two cases, and wound dehiscence in one case. No mortality occurred. Rebleeding rare was 0%.CONCLUSIONLS combined with pericardial devascularization is relatively safe and effective in treatment of portal hypertension due to liver cirrhosis. The keys to success include experienced laparoscopic skills, use of harmonic scalpel and careful manipulation.
Laparoscopic Splenectomy Performed in 41 Cases

Abstract:Objective To evaluate the feasibility and efficacy of laparoscopic splenectomy(LS). Methods In this study 41 cases were enrolled including 10 cirrhotic splenomegaly cases and 17 idiopathic thrombocytopenic purpura cases. The clinical data of 41 cases undergoing LS was retrospectively reviewed. KG2Result Two cases were shifted to open surgery, LS was successful in 39 cases with average operating time of 238 min, the average spleen weight was 389 g, blood loss 318 ml, postoperative GI movement started from 12 to 24 hrs. The average postoperative hospital stay was 4 days. Postoperative complications occurred in 3 cases including intraabdominal bleeding, subcutaneous emphysema, and stabbing-caused abdominal wall bleeding in one each. Conclusion LS is safe and less traumatic in selected patients indicated for splenectomy, especially for those suffering from haemotologic diseases.

Abstract:Objective To compare laparoscopic splenectomy and pericardial devascularization with open procedures for the treatment of portal hypertension in cirrhotic patients. Methods In this retrospective study from December 2004 to December 2006, 20 cases underwent laparoscopic splenectomy and paraesophagogastric devascularization and twenty cases undergoing open surgery served as controls. Clinical data including Child grade, presentation, operative time, intraoperative blood loss, recovery time, hospital stay and postoperative complications were compared between the two groups. Results The operative time of laparoscopic group and the open group was respectively （235 ± 79） min and （230 ± 99 ） min（ P = 0. 068 ）. The mean intraoperaive blood loss was （ 520 ± 386 ） ml and （ 1856 ± 1799 ） ml respectively, （P = 0. 018 ）. The postoperative stay was （ 8.5 ± 2.6 ） days and （ 14. 5 ± 7.4） d （ P = 0. 0186）. There was no operative mortality in neither groups and there were no significant differences in postoperative complication rate between the two groups. All patients were followed up for 6 - 24 months, there was no recurrent bleeding. Laparoscopic Splenectomy without Using Endo-GIA Combined with Pericardial Devascularization

Abstract:Objective To discuss the technique and clinical value of laparoscopic splenectomy (LS) without using Endo-GIA combined with pericardial devascularization for the treatment of cirrhotic portal hypertension. Methods From March 2005 to October 2006, 23 patients with cirrhotic portal hypertension-induced lower esophageal varices were treated with LS combined with pericardial devascularization. In 18 of the cases, the spleen was resected by disconnection of the secondary splenic pedicle without using endo-GIA. During the operation, the splenic vessels were separated and disconnected by using absorbable clip or ligation respectively. And then the lesser omentum was cut using ultrasonic knife, and the pericardial varices devascularization was performed using the absorbable clips or ultrasonic knife. Finally, the spleen was placed into a bag, broken into small pieces, and removed from an enlarged trocar hole. Results The operation was completed successfully in the 18 cases. The mean operation time was 255 min (range,180-320 min). The mean intraoperative blood loss was 450 ml (range, 200-1600 ml). After the operation, 2 patients developed plural effusion, 1 had subphrenic abscess, and 2 had mild ascites. The subphrenic abscess was cure by ultrasonography-guided puncture. The mean hospitalization was 7.5 days (range 6 to 17 days). No mortality occurred. All the patients were followed up for an averge of 16.4 months (range 5 to 24 months). No patient died after the operation. The mean hospital stay was 7.5 days (6-17 days). The cases were followed up for 5-24 months (mean, 16.4 months). One patient developed rebleeding 20 months after the operation, and was cured by injecting sclerosing agent under a gastroscope. The other 17 cases had no hemorrhage after the operation.Conclusions It is a low-cost and superior method to disconnect the secondary splenic pedicle without using endo-GIA in LS combined with pericardial devascularization for the treatment of cirrhotic portal hypertension.

Abstract:Objective To evaluate a modified totally laparoscopic splenectomy plus pericardial devascularization (LSPD) for the treatment of portal hypertension.Methods We applied the technique of ligating the splenic artery in advance followed by the establishment of a tunnel underneath the splenic hilum and the consecutive transection of secondary splenic pedicles and gastric coronary vein with Endo-GIA during the process of splenectomy.Results Totally laparoscopic procedures were performed successfully in 28 cases.Procedure was converted to open surgery in one case because of uncontrollable splenic bleeding.Of the 28 patients that received the total LSPD,the operation time was (207.2 ± 59.3) min,intraoperative blood loss was (287.7 ± 212) ml,intraoperative blood transfusion volume was (106.7 ± 98.3) ml,postoperative passing gas time was (4.6 ± 1.7) d,postoperative hospital stay was (11.8 ± 2.9) d.Abdominal infection,postoperative portal vein thrombosis and liver failure were found in 1,1,2 cases respectively.There were no pancreatic fistula,intraabdominal hemorrhage,postoperative upper gastrointestinal hemorrhage cases.One patient died of liver failure,the other was cured by artificial liver therapy.Conclusions The technique of modified LSPD could reduce the conversion rate of laparoscopic splenectomy and intraoperative blood loss.
Separate control of the splenic artery and vein in laparoscopic splenectomy and pericardial devascularization for the treatment of portal hypertension

Abstract:Objective To evaluate the technique of separate control of the splenic artery and vein in laparoscopic splenectomy (LS) and pericardial devascularization in the treatment of portal hypertension. Methods From March 2005 to October 2005 thirteen cases with cirrhotic portal hypertension underwent LS and paraesophagogastric devascularization. The splenic pedicles were all managed by separate control of the splenic artery and vein branches laparoscopically. Results Laparoscopic procedure was all successful in the 13 cases. The mean operative time was (232±47) mins (range 180 mins to 320 mins) , intraoperaive estimated blood loss was (480±232) ml (range 200 ml to 800 ml). All patients were treated with plasma transfusion, antibiotics, and abdominal drainage postoperatively. Alimentary tract function recovered 24 hours to 48 hours after operation. The mean postoperative hospital stay was 8. 7 days (range 6 to 14 days). Postoperative complications included plural effusion in 3 cases and subphrenic abscess in one case. No mortality occurred. Conclusions The technique of separate control of the splenic artery and vein in laparoscopic splenectomy (LS) and pericardial devascularization in the treatment of portal hypertension is feasible, safe and effective.

Abstract:Background Although laparoscopic splenectomy (LS) has become the standard approach for most splenectomy cases, some areas still remain controversial. To date, the indications that preclude LS are not clearly defined. Portal hypertension from liver cirrhosis still is a contraindication to LS in the clinical practice guidelines of the European Association for Endoscopic Surgery published in 2008. This study aimed to evaluate the feasibility of LS for hypersplenism secondary to liver cirrhosis and portal hypertension. Methods The study retrospectively analyzed 206 laparoscopic splenectomies performed for a variety of indications over 13 years. According to diagnosis, the patients were divided into group A (hypersplenism secondary to liver cirrhosis and portal hypertension, n = 96) and group B (hematologic and other disorders, n = 110). A detailed review of medical records was conducted. The perioperative data for the two groups were compared including patient characteristics, diagnosis, operative details, complication rates, and postoperative hospital stay. Results Laparoscopic splenectomy was completed for 201 patients. Conversion from laparoscopic to open surgery was necessary for 5 patients (2.4%) because of hemorrhage, and 26 patients (12.6%) had complications. There were significant differences between groups A and B in terms of mean operation time (2.8 vs. 2.1 h), complication rates (17.7% vs. 8.2%), and postoperative stay (7.1 vs. 4.7 days). However, the two groups showed no significant differences with respect to intraoperative blood loss, blood transfusion, and conversion rate. Conclusion Laparoscopic splenectomy is a feasible, effective, and safe surgical procedure for patients who require splenectomy. Hypersplenism secondary to cirrhosis and portal hypertension should not be considered contraindications for LS.
The Selection of Surgical Treatment for Gastric Cancer Patients with Cirrhosis and Hypersplenism.

Abstract:BACKGROUND/AIMS:This study is to find the changes of hypersplenism, the leukocyte, platelets count, blood loss and operative time, hospitalization days in gastric cancer patients with cirrhosis and hypersplenism after splenectomy or splenic artery ligation. METHODOLOGY:32 gastric cancer patients with cirrhosis and hypersplenism were divided into two groups performed radical gastrectomy combined with splenectomy (Group A, n = 17) or splenic artery ligation (Group B, n = 15), 16 gastric cancer patients performed radical gastrectomy were selected as control group (Group C). Then, statistical analyzed. RESULTS:There were no significant differences of the leukocyteand platelets between Group A and B (p > 0.05), but significant compared with Group C (p < 0.01). After 6 chemotherapy, the differences of the leukocyteand platelets were obvious between Group A and C (p < 0.05), but not between Group B and C (p > 0.05). There were significant differences on the grading of hyperhepatia between Group A and B. Except the operative time of Group B and C (p > 0.05), there were significant differences on the blood loss, the operative time and hospitalization days among three groups (p < 0.05). CONCLUSIONS:The splenic artery ligation method was much more rational than splenectomy and it was preferred for the gastric cancer patients with cirrhosis and hypersplenism.
Laparoscopic Versus Open Splenectomy Plus Portaazygous Devascularization in the Treatment of Patients with Portal Hypertension with Recurrent Upper Gastrointestinal Bleeding

Abstract:Objective To compare the efficacies of laparoscopic versus open splenectomy plus portaazygous devascularization in the treatment of patients with portal hypertension with recurrent upper gastrointestinal bleeding.Methods The clinical data of patients who underwent laparoscopic (n =60) versus open (n =52) splenectomy plus open portaazygous devascularization in the Department of Hepatobiliary Surgery,the Center Hospital of Xianyang City,Xi'an Jiaotong University Health Science Center from March 2014 to February 2017 were retrospectively analyzed.Results There was no perioperative death in the 2 groups.The amounts of intraoperative bleeding,the durations of the operation,the time of first flatus passed after operation and the duration of postoperative hospitalization were compared,and the differences were significantly different (t =3.288,2.533,3.325,2.823,P ＜ 0.05).There was no significant difference in the total hospitalization expenses (t =0.651,P ＞0.05).The incidence of postoperative complications was significantly higher in the open surgery group (x2 =7.622,P ＜ 0.05).At 1-month after surgery,color doppler flow imaging (CDFI) showed no significant difference on the portal blood flow between the two groups (t =0.625,P ＞ 0.05).On gastroscopy,the esophageal and gastric varices were significantly improved,and there was no significant difference between the two groups (x2 =0.718,P ＞ 0.05).Liver function was better in the laparoscopic group than the open group (x2 =3.765,P ＜ 0.05).Comparison of the rebleeding rates and the incidences of hepatic encephalopathy for the two groups at 1 year after operation showed significantly better outcomes for the laparoscopic group (x2 =2.351,1.245,P ＜ 0.05).The 1-year and 3-year survival rates after operation (x2 =0.218,0.361,P ＞ 0.05) were not significantly different.Conclusion Compared with laparotomy,laparoscopic splenectomy plus portaazygous devascularization had the advantages of less trauma,better short-term and long-term efficacies.
Laparoscopic splenectomy and azygoportal disconnection with intraoperative splenic blood salvage

Abstract:Background: Intraoperative blood salvage can reduce or avoid perioperative allogeneic blood transfusion. Salvaging the blood in the portal hypertension-induced enlarged spleen becomes an issue of concern during devascularization surgery because an enlarged spleen accommodates a large red cell pool. We report 20 cases of laparoscopic splenectomy and azygoportal disconnection and present the advantages of the use of intraoperative splenic blood salvage during the procedure. Methods: A total of 20 cirrhotic patients with esophagogastric variceal bleeding refractory to treatment with β-blockers and endoscopic therapy were studied. Laparoscopic splenectomy with azygoportal disconnection was performed. During the procedure, an intraoperative autologous blood salvage device recovered the splenic blood. The perioperative data were recorded from various viewpoints. Results: The operative time was 3.1 ± 0.3 h and the blood loss was 70.5 ± 32.5 ml. The weight of the excised and morcellated spleen was 826.0 ± 155.1 g. The volume of autotransfused blood was 541.0 ± 150.4 ml. No patient received a perioperative allogeneic blood transfusion. There were no significant complications either intraoperatively or postoperatively. The hemoglobin value increased from 9.3 ± 0.8 to 11.5 ± 1.1 g/dl at postoperative day 1 (p < 0.01). During a postoperative follow-up period of 18.0 ± 9.0 months for 18 patients, neither esophageal variceal bleeding nor encephalopathy recurred. Conclusion: Laparoscopic splenectomy with azygoportal disconnection is a feasible, effective, and safe surgical method for the treatment of bleeding portal hypertension. Intraoperative splenic blood salvage can avoid the risk associated with allogeneic transfusion during the procedure, with an advantage of significantly increased postoperative hemoglobin levels.
The decreased risk of hepatocellular carcinoma in hepatitis B virus-related cirrhotic portal hypertension patients after laparoscopic splenectomy and azygoportal disconnection

Abstract:Background: Posthepatitic cirrhosis is one of the leading risk factors for hepatocellular carcinoma (HCC) worldwide, among which hepatitis B cirrhosis is the dominant one. This study explored whether laparoscopic splenectomy and azygoportal disconnection (LSD) can reduce the risk of HCC among patients with hepatitis B virus (HBV)-related cirrhotic portal hypertension (CPH). Methods: A total of 383 patients with HBV-related CPH diagnosed as gastroesophageal variceal bleeding and secondary hypersplenism were identified in our hepatobiliary pancreatic center between April 2012 and April 2022, and conducted an 11-year retrospective follow-up. We used inverse probability of treatment weighting (IPTW) to correct for potential confounders, weighted Kaplan-Meier curves, and logistic regression to estimate survival and risk differences. Results: Patients were divided into two groups based on treatment method: LSD (n = 230) and endoscopic therapy (ET; n = 153) groups. Whether it was processed through IPTW or not, LSD group showed a higher survival benefit than ET group according to Kaplan-Meier analysis (P < 0.001). The incidence density of HCC was higher in the ET group compared to LSD group at the end of follow-up [32.1/1000 vs 8.0/1000 person-years; Rate ratio: 3.998, 95% confidence intervals (CI) 1.928-8.293]. Additionally, in logistic regression analyses weighted by IPTW, LSD was an independent protective predictor of HCC incidence compared to ET (odds ratio 0.516, 95% CI 0.343-0.776; P = 0.002). Conclusion: Considering the ability of LSD to improve postoperative survival and prevent HCC in HBV-related CPH patients with gastroesophageal variceal bleeding and secondary hypersplenism, it is worth promoting in the context of the shortage of liver donors.

Abstract:Background Laparoscopic splenectomy has become the standard procedure for the normal to moderately enlarged spleens. We performed this study to investigate the safety, feasibility, and effectiveness of laparoscopic splenectomy for hypersplenism secondary to liver cirrhosis. Methods We performed a retrospective chart review of 24 cases of laparoscopic splenectomy (group 1), 24 cases of open splenectomy (group 2) for hypersplenism secondary to liver cirrhosis, and 68 cases of laparoscopic splenectomy for immune thrombocytopenic purpura (group 3). We performed comparisons between groups 1 and 2 and groups 1 and 3 in terms of demographic, intraoperative, postoperative variables, and changes in blood counts and liver function. Results Patients in groups 1 and 2 had comparable demographic characteristics, but those in group 1 had less estimated blood loss, fewer complications, and shorter duration of oral intake, and they required less analgesia and shorter post-hospital stays. In both groups, leukocyte and platelet counts increased significantly and transaminase and total bilirubin decreased postoperatively, but not significantly, and there was no significant difference between the two groups. Compared with group 3, patients in group 1 were older, had lower preoperative hemoglobin levels and leukocyte counts, poorer Child-Pugh class, required more operation time, and suffered more estimated blood loss; however, there were no statistically significant differences in terms of conversion rates, transfusion rates, complication rates, and postoperative course. Conclusions Laparoscopic splenectomy is a safe, feasible, and effective procedure for hypersplenism secondary to liver cirrhosis.

Abstract:Hepatic cirrhosis is relatively common and is related to hepatitis B and C, alcohol and other factors. It mainly manifests as portal hypertension, splenomegaly, hypoproteinemia, ascites, and esophagogastric fundal varices. [ 1 , 2 ] From initial compensatory cirrhosis to life-threatening decompensated cirrhosis, the pathological process of cirrhosis is irreversible. The most definitive treatment is liver transplantation [ 3 ] ; however, due to the lack of donor livers, many patients with end-stage cirrhosis die while waiting for a liver. [ 3 ] Rupture and hemorrhage of esophageal and gastric varices are the main causes of death. [ 4 ] In addition, patients with cirrhosis have poor coagulation function. Coagulation function is further worsened when combined with hypersplenism and thrombocytopenia increasing the risk of spontaneous bleeding. Pericardial vascular dissection combined with splenectomy can effectively control massive gastrointestinal tract bleeding and also correct thrombocytopenia. In addition, to preserve gastric vascular and vagus nerve function, selective esophagogastric fundal vascular dissection is commonly used. [ 5 , 6 ]

Surgery for gastric cancer with cirrhosis portal hypertension

Abstract:Objective To discuss the surgical procedures and prognostic factors for gastric carcinoma associated with liver cirrhosis and portal hypertension.Methods Thirty-one patients with gastric carcinoma associated with liver cirrhosis and portal hypertension were treated surgically at our hospital from October 1999 to December 2008.The clinical data of these patients were retrospectively analyzed.Results Of the patients,15 had the tumor of Child-Pugh score A,11 B and 5 C.Distal subtotal radical gastrectomy was performed in 9 patients,proximal subtotal radical gastrectomy in 5,distal subtotal gastrectomy+splenectomy in 2,distal subtotal gastrectomy+splenectomy+ devascularization in 5,proximal subtotal gastrectomy+spenectomy in 7,total gastrctomy+splenectomy in 1,palliative tumor excision+splenectomy+ devascularization in 1,and palliative excision in 1.Complications were seen in 19 patients,of whom 17 developed ascites.Six patients died at the hospital after surgery.12/18 patients who had been undergone D2 lymph node dissection developed postoperative ascites,whereas 5/13 patients with D0 or D1 lymph node dissection had ascites.Postoperative survival time ranged from 2 to 39 months,the median survival time was 15 months,and 4 patients were still alive.Conclusions The key to successful surgical treatment of a patient with gastric carcinoma associated with liver cirrhosis and portal hypertension is the access of strictly ruled indication and a reasonable surgical procedure.Minor procedure is advocated in gastric surgery.Preoperative hepatic function,preoperative ascites and tumor stage are factors affecting postoperative survival.
Clinical analysis of laparoscopic splenectomy for portal hypertension

Abstract:Objective To investigate the clinical outcome of laparoscopic splenectomy (LS) in the treatment of portal hypertension.Methods The clinical data of 85 patients with portal hypertension were retrospectively analyzed from March 2015 to June 2016, including 40 cases who received open pericardial devascularization in traditional operation group and 45 cases who underwent laparoscopic splenectomy in LS group.Statistical analysis were performed by using SPSS 11.5 software.Perioperative measurement data including liver and kidney function and quality of life were expressed as mean±standard deviation (±s), and were examined by using t test.Postoperative complications were analyzed by Chi-square test.A P value <0.05 was considered as statistically significant difference.Results The operation time of the LS group was shorter than that of the traditional operation group (t=21.393, P<0.001), and the intraoperative blood loss was less than that of the traditional operation group (t=49.471, P<0.001).There was no significant difference of preoperative liver and kidney function between two groups.After operation, the levels of ALT, AST and TBIL in the LS group were lower than those in the traditional operation group respectively (t=13.323, 5.307, 6.313, P<0.001).There was no significant difference of quality of life between the two groups before operation.The quality of life in the LS group was significantly better than that in the traditional operation group 6 months after operation.Complications such as bleeding, ascites and hepatorenal syndrome in LS group were significantly lower than those in traditional operation group (X2=5.977, P=0.014).Conclusion Laparoscopic splenectomy for portal hypertension has good curative effect, could significantly improve the liver function of patients, and could reduce the incidence of complications, with the clinical application value.

Abstract:AIM:To evaluate the feasibility,safety,and effectiveness of consecutive laparoscopic cholecystectomy(LC)plus splenectomy(LS)in liver cirrhosis patients.METHODS:From 2003 to 2013,17(group 1)patients with liver cirrhosis complicated by hypersplenism and symptomatic gallstones were treated with combined LC and LS,while 58(group 2)patients with liver cirrhosis and hypersplenism received LS alone.An additional 14(group 3)patients who received traditional open procedures during the same period were included as controls.Data were retrospectively collected and reviewed in regard to demographic characteristics and preoperative,intraoperative and postoperative features.Differences between the three groups were assessed by statistical analysis.RESULTS:The three groups showed no significant differences in the demographic characteristics or preoperative status.However,the patients treated with LC and LS required significantly longer operative time,shorter postoperative stay as well as shorter time of return to the first oral intake,and suffered less intraoperative blood loss as well as fewer postoperative surgical infections than the patients treated with traditional open procedures(group 1 vs group 3,P<0.05 for all).The patients treated with LC and LS showed no significant differences in the intraoperative and postoperative variables from those treated with LS alone(group 1 vs group 2).All patients showed significant improvements in the haematological responses(preoperative period vs postoperative period,P<0.05 for all).None of the patients treated with LC and LS presented with any gallstone-associated symptoms following discharge,while the patients treated with the traditional open procedures expressed complaints of discomfort related to their surgical incisions.CONCLUSION:Consecutive LC and LS is an appropriate treatment option for liver cirrhosis patients with gallstones and hypersplenism,especially for those with Child-Pugh A and B.

Abstract:Objective To analyze the clinical experience of splenectomy in situ in the patients with liver cirrhosis and portal hypertension.Methods The clinical data of 348 patients with portal hypertension received in situ splenectomy in our department from Jan 2006 to May 2012 were analyzed retrospectively.The procedures of the operation were described in details.Results During the operation,the hilus of spleen was separated at first,then the vessels and ligaments around the spleen disconnected.The key point of the operation was to set up the space behind the pancreas.This technique is beneficial in avoiding complications such as massive hemorrhage,gastric trauma and pancreatic leakage,etc.Conclusions Comparing with the conventional procedures,in situ splenectomy shows both safety and convenience in surgical treatment of patients with giant splenomegaly and liver cirrhosis and portal hypertension.

Abstract:Objective To investigate the operative skills and clinical safety of the treatment for recur-rent upper digestive tract hemorrhage with portal hypertension by laparoscopic surgical technique. Methods The clinical data of 56 patients who suffered from recurrent upper digestive tract hemorrhage with portal hyper-tension by laparoscopic surgical technique from May. 2010 to Apr. 2015 were retrospectively analyzed. Results In 56 cases, Total laparoscopic splenectomy plus portoazygous devascularization wered in 51 cases, 13 cases of primary splenic pedicle from breaking method is adopted, the secondary spleen was 38 cases. The operative time was (2. 8±0. 5)hours. The intraoperative bleeding was (156. 8±33. 5)ml. The time taken for the bowel to return function was (3. 1±1. 3)days. The hospitalization time was(6. 1±1. 3)days. Postoperative pleural effu-sion was in 2 cases, mild ascites in 1 case;2 cases of mild pancreatic leakage were cured by abdominal drain-age. There was no perioperative mortality in the whole group. Due to bleeding, 2 cases changed to hand-assis-ted laparoscopic surgery, three cases of complete conversion to open surgery. The conversion rate of non com-plete laparoscopic surgery was 8. 9%. The total patients were followed up for 1 year, rebleeding wered in 3 cases, the rebleeding rate was 5. 4%,the 1,3,5 years survival rate were 96. 4% (54/56), 87. 5%(28/32),75. 0% (15/20). Conclusions Laparoscopic splenectomy and portoazygous devascularization for portal hypertension with recurrent upper digestive tract bleeding, with less trauma, less bleeding, faster recov-ery, the advantages of the low incidence of complications was advantageous. As long as strict indications, ac-curate preoperative assessment, intraoperative fine operation and make transit emergency treatment, after care-ful management, the procedure is safe and feasible. Its long-term effect is positive.

Abstract:Background: In the recent years, laparoscopic splenectomy and esophagogastric devascularization (LSD) for liver cirrhosis and portal hypertension rapidly gained the interest of hepatobiliary surgeons due to its minimal invasion. This study aimed to gather and analyze available data from the observational studies that have compared LSD and open splenectomy and esophagogastric devascularization (OSD) for liver cirrhosis and portal hypertension. Materials and Methods: All the studies comparing LSD and OSD for liver cirrhosis and portal hypertension were searched on the available databases, including the Cochrane Central Register of Controlled Trials, Medline, Science Citation Index, EMBASE, China National Knowledge Infrastructure, Wanfang Database, and China Biomedical Database. Data were analyzed using Review Manager software version 5.0. Results: After the literature search, a total of 17 studies were included in the meta-analysis, which involved 1093 patients: 552 in the laparoscopic group and 541 in the open group. The laparoscopic group was shown to have a lower overall postoperative complication rate (0.43; 95% confidence interval [CI; 0.29-0.64]) than the open group (P<.0001), which was not associated with heterogeneity between the studies. The laparoscopic group was shown to have a lower intraoperative blood loss (-320.62; 95% CI [-552.35 to -88.9]), shorter time of oral intake (-29.08 hours; 95% CI [-35.28 to -22.88]), and shorter hospital stay (95% CI [-6.19 to -2.19]) than those of the open group (P<.00001). The operative time of the laparoscopic group was 42.16 minutes longer (95% CI [32.20-52.11]) compared with the open group (P<.00001). There was no significant difference of hospitalization costs between the studies. Conclusion: This meta-analysis demonstrated that laparoscopic left lateral resection is a safe and feasible option associated with a reduced overall complication rate. The current evidence suggested that it could be performed routinely in liver centers.

Abstract:BACKGROUND: Laparoscopic splenectomy is considered the gold standard for resecting normal-to-moderately bigger spleens in benign conditions, and in addition could be tried for patients with malignant splenic disorders. However, the safety of laparoscopic splenectomy in patients with hypersplenism is not well-known. This study aimed to investigate the efficacy and safety of laparoscopic splenectomy for patients with hypersplenism secondary to liver cirrhosis by comparing with the open splenectomy.DATA SOURCES: Several databases were searched to identify comparative studies fulfilling the predefined selection criteria from January 2000 to June 2015. The subsequent key words were utilized for browsing "laparoscopy" or "laparoscopic", "open", "splenectomy", and "liver cirrhosis". Studies evaluating laparoscopic and open splenectomy for patients with liver cirrhosis were incorporated. Two evaluators personally strained the title and abstract of each publication. Citations with contemplated compliance within our eligibility criteria underwent compressed review. Meta-analysis was carried out according to the recommendations of the Cochrane Collaboration software (review manager 5.1).RESULTS: Seven studies containing 509 patients were included. Compared with the open splenectomy group, patients in the laparoscopic splenectomy group had significantly less intraoperative blood loss (MD=210.30; 95% CI: 11.28-409.32; P=0.04), longer operative time (MD=-31.58; 95% CI:-53.34-9.82; P=0.004), shorter duration of postoperative hospital stay (MD=3.41; 95% CI: 2.39-4.43; P<0.01), lower incidence of postoperative complications (RR=1.34; 95% CI: 0.88-2.01; P=0.17), and decreased liver damage [ALT (MD=8.52; 95% CI: 0.19-16.85; P=0.05) and total bilirubin (MD=5.12; 95% CI: 0.37-9.87;P=0.03)].CONCLUSION: Hypersplenism secondary to cirrhosis and portal hypertension should not be a contraindication for laparoscopic splenectomy.
