Abstract:Objective To study the value of the clinical application of stereography and measurement of liver volume in cirrhotic patients with portal hypertension. Methods By use of the personal computer and the software of three-dimensional reconstruction and measurement system of liver faultage photograph, the liver volume in vivo was successfully measured and the three-dimensional image of the liver rebuilt in 46 posthepatitic cirrhotics who were selected for orthotopic liver transplantation and in 30 noncirrhotic controls, and comparison with the reference volume of recipient liver was obtained by means of water deplacement after transplantation. Results The liver volume of cirrhotic patients with portal hypertension measured by software and water deplacement was (983.33±206.11) cm 3, and (904.93±209.56) cm 3 respectively. Comparison by means of linear regression analysis between volume measurement on three-dimensional reconstruction software and reference volume showed a nearly ideal correlation coefficient( r =0.969, P 0.01), the average error was 8.66 % .The average of liver volume in controls was (1 287.00±96.18) cm 3, and was positively correlated to the height,weight and body surface area ( r =0.845,0.833,0.932 respectively, P 0.01),and was different from that of cirrhotics. Liver volume of cirrhoitic patients with portal hypertension was related to their Child-Pugh classification. The liver volume of patients in Child C group was significantly smaller than that of patients of Child B group, and was significantly correlated with Alb ( r =0.496, P 0.01) and TBIL( r =-0.493, P 0.01),PT( r =-0.517, P 0.01), but was not significantly correlated with ALT( r =0.206, P 0.05),portal pressure( r =-0.093, P =0.539) and portosystemic shunt index ( r =0.044, P =0.769). Conclusion The volume measurement of the liver by the three-dimensional software is relatively accurate. Liver volume of cirrhotic patients with portal hypertension was significantly related to their liver function,and can reflect the liver reserve function.

Abstract:Liver transplantation makes it possible to cure portal hypertension thoroughly.A new pattern of combined application of multiple treatments based on liver transplantation has emerged.Conventional surgical treatments such as splenectomy,devascularization and transjugular intrahepatic portosystemic shunting have important effects on the prognosis of liver transplant recipients.Portal vein stenosis,thrombosis as well as small-for-size liver graft lead to post-transplant portal hypertension and liver dysfunction.The author in this article provides an overview of the effects of various conventional treatments on the prognosis of recipients and the treatment of portal hypertension after liver transplantation,aiming to seek for optimal strategy.

Abstract:Objective:To explore the measurement of portal vein pressure (PVP) based on computational fluid dynamics (CFD) and its clinical value in the preoperative planning of resection of hepatic carcinoma.Methods:From January 2020 to April 2021, CT data of 16 patients with hepatocellular carcinoma were imported into 3d-doctor 4.0 software to reconstruct the liver 3D model(including portal vein) at Yuebei people’s Hospital Affiliated to Medical College of Shantou University: (1) PVP was simulated by ANSYS 19.0 software, and compared with the actual PVP measured during the operation or the intervention. Pearson correlation was used to analyze the correlation between the actual PVP and the simulated PVP; (2) The future liver remnant(FLR) was calculated according to the preoperative surgical planning; (3) The FLR combined with the simulated PVP was used to determine the selection of treatment for hepatocellular carcinoma. Continuous variables were compared using Student’s t-test, Pearson test was used for correlation analysis. Results:There was no significant difference between the simulated PVP and the actual PVP [(22.2±9.2) cmH 2O vs. (20.9±8.4) cmH 2O, t=0.419, P>0.05]. The simulated PVP was positively correlated with the actual PVP ( r=0.965, P<0.01). Based on the simulated PVP combined with the FLR, 14 patients underwent hepatectomy, 2 patients underwent the terminal portal vein embolization combined with TACE. Conclusion:The PVP level based on CFD simulation reflects the actual PVP level, and combined with the FLR can guide the choice of hepatocellular carcinoma treatment.
Flow Resistance Analysis of Clinically Significant Portal Hypertension in Patients with Liver Cirrhosis.

Abstract:Cirrhosis-induced clinically significant portal hypertension (CSPH) is a fatal disease. Early detection of CSPH is vitally important to reduce the patients’ mortality rate. In this study, combined with three-dimensional image construction technology and computational fluid dynamics (CFD), an image-based flow resistance analysis was proposed. The flow resistance analysis was performed for nine cirrhosis patients with CSPH and ten participants without liver diseases, respectively. The results showed that the flow resistance coefficient of the portal vein system in CSPH patients was significantly lower than that in the control group (0.97 ± 0.11 Pa/(mL/s) for CSPH patients; 1.80 ± 0.40 Pa/(mL/s) for the control group; P=0.028). In contrast, although main portal vein dilation was found in CSPH patients, the cross-sectional area enlargement was not statistically significant (186.01 ± 57.48 mm2 for CSPH patients; 166.26 ± 33.74 mm2 for the control group; P=0.39). The research outcomes indicated that the flow resistance analysis was more sensitive than the commonly used vessel size measurement in the detection of CSPH. In summary, we suggest using flow resistance analysis as a supplementary noninvasive method to detect cirrhosis patients with CSPH.

Abstract:Hepatic venous pressure gradient (HVPG) measurement is the standard technique of assessing portal pressure. Portal hypertension is mainly caused by increased intrahepatic resistance to portal blood flow related to structural changes, including fibrosis and vascular distortion. Here, we used dynamic contrast-enhancement (DCE) MRI to quantify segmented liver contours and vascular morphology in evaluating portal pressure. Our study showed the MRI measurements correlated well with HVPG measurement. A non-invasive prediction model combining liver vascular morphology and liver surface nodularity (LSN) can potentially be used to assess clinically significant portal hypertension (CSPH).
Alpha-1 Adrenergic Receptors in Liver Plasma Membranes of Cirrhotic Patients with Portal Hypertension:A Quantitative Study

Abstract:OBJECTIVE:To investigate the changes of alpha-1 adrenergic receptors in cirrhotic liver and the relationship between the changes and the pathogenesis of portal hypertension.METHODS:The concentration and affinity of alpha-1 adrenergic receptors in the liver plasma membranes of posthepatitic cirrhotic patients with portal hypertension were quantitatively measured using radioligand binding analysis.RESULTS:Compared with 8 controls without hepatic pathological changes, the maximal binding capacity (Bmax) of 9 posthepatitic cirrhotic patients decreased (129.1 +/- 12.0 vs 142.1 +/- 14.1 fmol/mg protein, P > 0.05), dissociation constant (Kd) values increased (0.3945 +/- 0.0974 vs 0.2382 +/- 0.0548 nmol/L, P < 0.01), and the receptor maximal content (RMC) decreased (417.4 +/- 76.8 vs 739.9 +/- 167.6 fmol/g liver, P < 0.01).CONCLUSIONS:The decreased concentration and affinity of alpha-1 adrenergic receptors may play an important role in the metabolic disturbances of catecholamines often seen in some cirrhotic patients, and have implications in the pathogenesis of portal hypertension.
A Non‐invasive Method for Evaluating Cirrhotic Portal Hypertension by Administration of 99mtc‐mibi Per Rectum

Abstract:A study was performed to evaluate radio-isotopic imaging using technetium-99m hexakis 2-methoxyisobutyl isonitrile administered per rectum to assess portal collateral circulation.The heart-liver ratios (H/L; mean +/- standard deviation) in 15 controls, 13 cases of histologically confirmed viral hepatitis and 57 cirrhosis patients were 0.27 +/- 0.11, 0.43 +/- 0.14 and 1.00 +/- 0.28, respectively (P < 0.001). Among the cirrhosis patients those with the Child-Pugh classification A, B and C had H/L of 0.56 +/- 0.14, 1.00 +/- 0.20 and 1.19 +/- 0.26, respectively (P < 0.001). A high value of H/L was associated with a high risk of hepatic encephalopathy (1.25 +/- 0.17, P < 0.01) and oesophageal varices (1.02 +/- 0.20, P < 0.01). There were associations between H/L and serum bilirubin (P < 0.01), albumin (P < 0.05) and prothrombin time (P < 0.05). The results also showed a good correlation between H/L and portal vein pressure measured during operation in 13 patients (P < 0.001, r = 0.87). The regression equation: gamma = 6.77 + 32.5 H/L, allowed portal vein pressure to be estimated.The prognostic value of the test was supported by the fact that good correlations were observed between the H/L ratio and widely accepted prognostic classification (Child-Pugh). It is suggested that this new method could be a reliable non-invasive way to give an indication of the degree of portasystemic shunting to evaluate the prognosis and to follow up the effects of medications for reducing portal hypertension in patients with cirrhotic portal hypertension.
Serum Levels of Epinephrine and Norepinephrine in Cirrhotic Patients with Portal Hypertension.

Abstract:OBJECTIVETo investigate the role of catecholamines in the pathogenesis of portal hypertension.METHODSSerum epinephrine (E) and norepinephrine (NE) concentrations in the peripheral vein, portal vein and superior vena cava (SVC) were measured by high pressure liquid chromatography in 38 portal hypertensive patients, 28 idiopathic hypertensive patients and 34 controls, respectively.RESULTSPeripheral venous E concentrations in portal hypertensive patients and controls were 57.5 +/- 37.4 ng/L and 23.5 +/- 11.2 ng/L, respectively (P < 0.01), and peripheral venous NE concentrations were 451.1 +/- 381.2 ng/L and 183.0 +/- 83.3 ng/L, respectively (P < 0.01). Compared with controls, peripheral venous E (54.9 +/- 39.9 ng/L vs 23.5 +/- 11.2 ng/L, P < 0.01) and NE (524.3 +/- 219.9 ng/L vs 183.0 +/- 83.3 ng/L, P < 0.01) in idiopathic hypertensive patients were also significantly increased. E and NE in SVC, portal vein and peripheral artery in portal hypertensive patients were also increased, but only the elevation of E in SVC was statistically significant (207.2 +/- 55.4 ng/L vs 83.7 +/- 46.7 ng/L, P < 0.05).CONCLUSIONOur results reveal significant metabolic disorders of E and NE in portal hypertensive patients, which may play an important role in the pathogenesis of portal hypertension.
The Clinical Significance of the Change of Blood-Lipid in Patients with Hepatitis B-related Cirrhosis.

Abstract:Objective To investigate the relation between the clinical significance and the levels of serum total cholesterol(TC),triglyceride(TG),high-density lipoprotein(HDL-C),low density lipoprotein(LDL-C) and very low density lipoprotein(VLDL-C).Method A retrospective analysis of 120 cases of clinical data of patients with liver cirrhosis was performed,and the clinical data of 30 normal healthy individuals in our outpatient clinic were involved as a control group.Result Compared with normal healthy persons,lipid levels in patients with hepatitis B cirrhosis were significantly lower(P0.05).Levels of TC,TG,HDL-C and LDL-C were positive correlated with levels of albumin and cholinesterase(P 0.01).Levels of TC,TG and LDL-C were negative correlated with the levels of prothrombin time and MELD(Model for end-stage liver disease) classification(P0.01).Conclusion The levels of blood lipid in patients with hepatitis B-related cirrhosis are significantly lower compared with normal persons.And levels of some blood lipids are significantly correlated with liver damage in patients with hepatitis B liver cirrhosis.
Influence of Nitric Oxide and Endothelin on Levels of Liver Function and Portal Hyperdynamics in Patients with Liver Cirrhosis

Abstract:Objective:To investigate the influence of nitric oxide(NO) and endothelin(ET)levels on liver function and portal hyperdynamics in patients with liver cirrhosis.Methods:The plasma levels of NO and ET of 51 cirrhotic patients[who were divided into group A(n=19),B(n=21) and C(n=11) according to the liver function] and 31 normal controls were measured by Griess and radioimmunoassay,respectively.Color Doppler ultrasonography was used to measure the portal hypernamics(including portal venous and splenic venous blood flow) and automatic biochemistry analyzer was used to measure the liver function.Results:The NO and ET level in the plasma of the cirrhotic patients(including Child-Pugh A,B and C)was significantly higher than that of the controls(P(0.01)).The NO and ET level in the plasma of the cirrhotic patients increased with the degeneration of the liver function and the difference was obvious between different grades(P0.01).The ratio of NO/ET in the plasma of the cirrhotic patients(including Child-Pugh A,B and C)was significantly higher than that of the controls(P0.05).The difference of NO/ET levels was significant between group A and group C(P0.01),but the difference was not obvious between group B and group C(P0.05).The plasma level of portal venous and splenic venous blood flow(PVBF and SVBF) was significantly higher in the cirrhotic patients than that in the controls(P0.01).The increase was more obvious in group C.The difference was significant between group C and group A(P0.01)but there was no difference between group C and group A.The plasma of NO and ET had a positive correlation with portal venous and splenic venous blood flow(PVBF and SVBF) in the cirrhotic patients,and it had a positive correlation with PVBF and SVBF in the controls.The plasma levels of NO and ET were significantly higher in ascites patients than those in the controls(P0.01)and the no ascites patients (P0.05).Conclusions:The serum levels of NO and ET are of great value in the evaluation of the liver function and the plasma levels of NO and ET may play an important role in the pathogenesis and promotion of portal hyperdynamics in patients with cirrhosis.
99mTc-Phy portal perfusion index imaging helps predict the severity of hepatitis B virus cirrhosis: a preliminary study.

Abstract:ObjectiveThe aim was to perform exploratory research on the application of technetium phytate (Tc-99m-Phy) portal perfusion index (PPI) imaging in predicting the complications of hepatitis B cirrhosis and their severity.Patients and methodsA total of 65 hepatitis B cirrhosis patients were stratified, respectively, into three groups from classes A to C according to Child-Pugh scores and five groups from stages 1 to 5 according to the five-stage prognostic system. PPIs were compared and analyzed, respectively, among the three and five groups. The correlations between PPIs and major biochemical indices of liver function were also analyzed. One-way analysis of variance was used to compare the PPIs among the various groups and a nonparametric Spearman test was used to analyze the correlations between PPIs and various biochemical indices.ResultsPPIs of the five groups decreased gradually from stage 1 to stage 5 (73.038.49, 52.96 +/- 16.22, 46.24 +/- 15.25, 29.99 +/- 17.36, and 11.50 +/- 6.37, respectively); with the exception of the difference between stages 2 and 3 (P=0.252), the differences between the remaining groups were statistically significant (P<0.05). The PPI showed positive correlations with serum total protein, serum albumin, and albumin/globulin results (r=0.292, 0.559, 0.520, respectively; P<0.05), and negative correlations with serum globulin (r=-0.366, P<0.05).ConclusionTechnetium phytate PPI could be a promising noninvasive and effective method for predicting the complications of hepatitis B cirrhosis and their severity; a lower PPI value indicates a higher severity of complications for hepatitis B cirrhosis patients. PPI can provide very meaningful reference data for clinical practice.
A hemodynamic study of the relationship between the left and right liver volumes and the blood flow distribution in portal vein branches

Abstract:Background Cirrhosis patients often exhibit clinical symptoms such as right liver atrophy, portal hypertension, spleen enlargement and increased blood supply, which exhibit considerable variation between the left and right liver sections. These differences are hypothesized to stem from disparities in blood flow within the left and right portal vein (PV) branches. However, rigorous quantitative evidence remains scarce. Purpose We mainly aim at quantitatively revealing the relationship between the blood flow rates of two PV branches and liver volumes, and providing quantitative evidence and theoretical support for the diagnosis and treatment of cirrhosis from the perspective of hemodynamics. Methods Five cirrhotic patients and two healthy volunteers from Beijing Friendship Hospital are investigated. Their PV blood flow models are established based on computed tomography (CT) images and finite volume simulations. The volume of the left and right liver lobes are measured in 3‐matic. The distributions of blood source in the PV branches are tracked by streamline analysis. The blood flow rates are quantitatively counted by integrating the blood source velocities. Linear analysis is performed to build the relationship between liver volumes and PV blood flow distributions. Results Streamline analysis reveals significant differences in blood distribution between the left and right PV branches. The majority of blood from the superior mesenteric vein (SMV) flowed into the right portal vein (RPV), while most blood from the splenic vein (SV) entered the left portal vein (LPV). The main PV pressure drop linearly increases with the SV blood velocity for all PV structures of patients and healthy volunteers. The flow rate ratio QRPV/QLPV demonstrates an increase in tandem with the volume ratio VR/VL, exhibiting a linear correlation with the Pearson correlation coefficient being 0.93. Conclusion The differences in the blood distributions are consistent with the clinicians' knowledge and validate our simulations. We demonstrated a linear increase in PV pressure with elevated SV blood velocity. Additionally, the volumes of the left and right hepatic lobes exhibited a positive correlation with blood flow rates in the corresponding PV branches.

More precise measurement of irregular splenic volume in cirrhotic patients with portal hypertension

Abstract:Background and aims Splenomegaly often occurs in cirrhotic patients with portal hypertension (PHT), and therefore the efficacy and accuracy of conventional methods for measuring splenic volume are a matter of question in these patients. Here, we developed a novel approach to assess true splenic volume more precisely. Methods High-quality thin-slice computed tomography data of 112 cirrhotic patients with PHT were obtained and reviewed. Both the conventional measurement and a novel formula obtained from three-dimensional reconstruction software were used to estimate splenic volume, and the accuracy was compared and verified. Results In PHT patients, the splenic volume calculated using the conventional method was significantly less than that calculated using the three-dimensional software. We found that the splenic volume was significantly positively correlated with splenic indices of length (L), thickness (T), and width (W), and also the diameter of the splenic vein. Using these indices, we propose two novel formulas using the software to estimate the splenic volume more accurately: SV = 69.686 × L + 53.077 × W + 103.525 × T + 314.510 × diameter of splenic vein − 2266.209 (p<0.01, R 2 =0.805). And a more practical simplified formula: SV′ = 0.504 × L × W × Τ + 319.762 × diameter of splenic vein − 81.66 (p<0.01, R 2 =0.784). Conclusions Although the conventional formula has been widely used for years, it is not suitable for an enlarged spleen. We developed two novel formulas for estimating splenic volume from clinical data that were more appropriate for cirrhotic patients with PHT.
Contrast‐Enhanced Sonography for Quantitative Assessment of Portal Hypertension in Patients with Liver Cirrhosis

Abstract:ObjectivesThe clinical utility of contrast‐enhanced sonography in portal hypertension remains unclear. We explored the feasibility of using contrast‐enhanced sonography for noninvasive assessment of portal venous pressure.MethodsTwenty healthy individuals (control group; 9 men; mean age, 46.4 years) and 18 patients with portal hypertension (15 men; mean age, 46.2 years) were enrolled in this study. The portal hypertension group included patients who underwent splenectomy and pericardial blood vessel disarticulation at our hospital from October 2010 to March 2011. One week before surgery, patients with portal hypertension underwent preoperative liver contrast‐enhanced sonography. Two‐dimensional, Doppler, and contrast‐enhanced sonographic parameters were compared between the groups. Portal venous pressure was measured intraoperatively by portal vein puncture in the portal hypertension group, and its relationship with the other parameters was analyzed.ResultsThe 2‐dimensional, Doppler, and contrast‐enhanced sonographic parameters differed between the groups (P < .01). Portal venous pressure was inversely correlated with the area under the portal vein/hepatic artery time‐intensity curve ratio (Qp/Qa), portal vein/hepatic artery strength ratio (Ip/Ia), and portal vein/hepatic artery wash‐in perfusion slope ratio (βp/βa), with correlation coefficients of −0.701, −0.625, and −0.494, respectively.ConclusionsMeasurement of the liver contrast‐enhanced sonographic parameters Qp/Qa, Ip/Ia, and βp/βa could be used as a new quantitative method for noninvasively assessing portal venous pressure.
The relationship of portal hemodynamics to nitric oxide and endothelin levels in patients with liver cirrhosis

Abstract:To study the relationships of portal hemodynamics to vasoactive substances - nitric oxide (NO) and endothelin (ET) in cirrhosis patients, 48 patients with liver cirrhosis (LC) and 32 normal controls (NC) were studied, including 18 patients with compensated LC and 30 patients with decompensated LC. Portal venous and splenic venous blood flow (PVBF and SVBF) were measured by color duplex doppler ultrasonography, and the blood coucentrations of ET and NO were detected. After analyzing the correlation between PVBF and SVBF to NO(ET), the results suggest that protal hy- perdynamic changes, which exist in cirrhosis patients from beginning to the end, were possibly due to the high NO level and the low sensitiving to ET, and it is useful for the clinical treatment.

Abstract:Objective:In this study, the liver, spleen, and hepatic portal vein in the portal venous phase images of abdominal enhanced computed tomography (CT) are artificially segmented and annotated, and the radiomics features are extracted from them. A model for predicting portal pressure in patients with hepatitis B virus (HBV) related cirrhosis is constructed by combining radiomics features with clinical indicators.Methods:A total of 171 patients who had abdominal enhancement CT examination and trans-jugular hepatic venous pressure gradient (HVPG) measurement at the same time were enrolled from January 2016 to May 2020 in the Zhongshan Hospital Affiliated to Fudan University. The liver, spleen, and hepatic portal vein in the portal venous phase images of the CT were manually labeled by using ITK-SNAP 3.8 software. The radiomics features of these three sites were extracted using Python programming, and an HVPG prediction model was established.Results:A total of 171 patients was included in the study. The average age was (51.1±10.3)years, of which 134(78.4%) were males, and the average HVPG was 16.87±5.695. A total of 2 553 radiomics features were extracted from three sites of the portal venous phase images of abdominal enhanced CT in each patient. The 2 553 features extracted were screened using LASSO, and by combing with clinical features and radiomics features, the predictive model of HVPG was obtained: m_HVPG=31.622+ 0.028 8T×total bile acids-6.31(portal venous wavelet-LHH_glcm_ClusterShade)=0.253(portal venous wavelet-LHL_glszm_LargeAreaLowGrayLevelEmphasis)-20.9(spleen wavelet-LLH_glcm_Correlation)-0.000 127(liver original_shape_SurfaceArea)+ 2.79(liver wavelet-LLH_glcm_ClusterShade). The coefficient of determination R2 was 0.345. Conclusions:The study suggests that radiomics features of the liver, spleen, and portal venous combined with clinical features may be used as a non-invasive method to assess the portal pressure in patients with HBV-related cirrhosis.
A Study on the Sensitivity of Different Methods in the Diagnosis of Portal Hypertension Associated with Liver Cirrhosis

Abstract:Objective To investigate the sensitivity of different methods in the diagnosis of portal hypertension associated with liver cirrhosis. Methods 99 patients with portal hypertension associated liver cirrhosis, and 49 healthy subjects as control were enrolled in this study. Their portal and splenic vein widths, and the conditions of varicose vein in esophagus and gastric fundus were determined, and portal pressures were measured by radionuclide method. The positive percentage of different methods for the diagnosis of portal hypertension were analysed. Results Most of the patients with portal hypertension had the extension of portal and splenic vein in the fifferent areas and in various degrees. There were no significant differences in the degrees of portal and splenic vein extension among the patients with mild, moderate or severe esophageal varicose vein. The width of portal and splenic vein was not related with portal pressure. Radionuclide method was the most sensitive in diagnosing portal hypertension. Conclusion Portal hypertension can be diagnosed according to the conditions of vessels and spleen. The sensitivity of different methods in diagnosing portal hypertension were different, and that of radionuclide method was the highest. The width of portal and splenic vein had no close relation with portal pressure and the degree of esophageal varicose vein.
[Diagnostic Value of Heart-Liver Nuclein Ratio in Cirrhotic Portal Hypertension].

Abstract:A new method for evaluating portal systemic collaterals with administering 99mTc-MIBI per rectum was described in 80 cases. The heart to liver uptake ratio (H/L) in 15 healthy subjects was 0.267 +/- 0.1, slightly increased (0.455 +/- 0.13) in 8 patients with viral hepatitis and significantly increased in 68 cirrhotics. There was a good correlation between H/L ratio and the Child-Pugh classification with the ratio in Child A group of 0.57 +/- 0.12, in Child B group of 1.0 +/- 0.19, and in Child C group of 1.19 +/- 0.25 respectively (P < 0.01). For the diagnosis of portal hypertension, tentatively taking the ratio of > or = 0.62 as positive the sensitivity was 96%, specificity 89%, and accuracy 94%. The authors believe that 99mTc-MIBI imaging is a noninvasive method very useful in evaluating porta-systemic shunts.
Clinical value of contrast-enhanced ultrasonography and serum markers in assessing portal hypertension in patients with decompensated liver cirrhosis

Abstract:Objective To analyze the possibility of assessing portal hypertension in patients with decompensated liver cirrhosis via contrast-enhanced ultrasonography and common serum markers.Methods Fifty-four patients with decompensated liver cirrhosis were divided into two groups according to hepatic venous pressure gradient (HVPG):HVPG＜12 mmHg (1 mmHg=0.133 kPa) and HVPG ≥12 mmHg.The non-invasive index of routine blood test,liver function,coagulation function,Child-pugh score (CPS) and the results of contrast-enhanced ultrasound such as hepatic artery arrival time (HAAT),hepatic vein arrival time (HVAT)、portal vein arrival time (PVAT),hepatic artery to henatic vein arrive transmit time (HA-HVTT) and portal vein to hepatic vein arrive transmit time (PV-HVTT) were assessed by univariate analysis and multivariate Logistic regression analysis,and then were used to generate a diagnostic model.The receiver operating characteristic curve was also used for analysis.Results The non-invasive model is Y =-0.217 × PV-HVTT + 1.526 × CPS-7.097.When the area under ROC curve (AUROC) was ≥0.857 and the best cutoff value was ≥0.631,and the sensitivity and specificity in judging HVPG≥ 12 mmHg were 87.5 ％ and 78.6％,respectively.Conclusions The model composed of PV-HVTT and CPS could be used to assess portal hypertension.

Abstract:BACKGROUND: Hepatic venous pressure gradient (HVPG) is the gold standard for diagnosis of portal hypertension (PH). However, its use can be limited because it is an invasive procedure. Therefore, it is necessary to explore a non-invasive method to assess PH.AIM: To investigate the correlation of computed tomography (CT) perfusion of the liver with HVPG and Child-Pugh score in hepatitis B virus (HBV)-related PH.METHODS: Twenty-eight patients (4 female, 24 male) with gastroesophageal variceal bleeding induced by HBV-related PH were recruited in our study. All patients received CT perfusion of the liver before transjugular intrahepatic portosystemic stent-shunt (TIPS) therapy. Quantitative parameters of CT perfusion of the liver, including liver blood flow (LBF), liver blood volume (LBV), hepatic artery fraction, splenic blood flow and splenic blood volume were measured. HVPG was recorded during TIPS therapy. Correlation of liver perfusion with Child-Pugh score and HVPG were analyzed, and the receiver operating characteristic curve was analyzed. Based on HVPG (> 12 mmHg vs ≤ 12 mmHg), patients were divided into moderate and severe groups, and all parameters were compared.RESULTS: Based on HVPG, 18 patients were classified into the moderate group and 10 patients were classified into the severe group. The Child-Pugh score, HVPG, LBF and LBV were significantly higher in the moderate group compared to the severe group (all P < 0.05). LBF and LBV were negatively associated with HVPG (r = -0.473, P < 0.05 and r = -0.503, P < 0.01, respectively), whereas splenic blood flow was positively associated with hepatic artery fraction (r = 0.434, P < 0.05). LBV was negatively correlated with Child-Pugh score. Child-Pugh score was not related to HVPG. Using a cutoff value of 17.85 mL/min/100 g for LBV, the sensitivity and specificity of HVPG ≥ 12 mmHg for diagnosis were 80% and 89%, respectively.CONCLUSION: LBV and LBF were negatively correlated with HVPG and Child-Pugh scores. CT perfusion imaging is a potential non-invasive quantitative predictor for PH in HBV-related liver cirrhosis.
