Abstract:In order to form correct drug treatment decision,we should judge comprehensively the diagnosis,illness,patient and pharmacy.Firstly,the perfect diagnosis should include not only all the contents of the diagnosis,but also all the diseases that the patient suffered from.Secondly,full assessment of illness condition should be made,including the risk factors,disease characteristics,disease progression and prognosis.Thirdly,individual assessing patient is also important,including the biological characteristics of the patients,as well as society and cultural background.Fourthly,assessment of treatment protocol should be done,including the overall arrangement of treatments measures,drug therapeutic schedules,implementation and response of the treatment plan.

Abstract:Abstract Background and Aim Both intestinal symptoms and comorbidities exist in irritable bowel syndrome (IBS) patients and influence their quality of life (QOL). More research is needed to determine how these variables impact the QOL of IBS patients. This study aimed to determine which specific factors had a higher influence on QOL and to further compare the effects of intestinal symptoms and comorbidities on QOL. Methods IBS patients were recruited from six tertiary hospitals in different regions of China. QOL, gastrointestinal symptoms, and comorbidities were assessed by different scales. Correlation analysis, multiple linear regression, and mediation model were used for statistics. Results Four hundred fifty‐three IBS patients (39.7% women, mean age 45 years) were included and no significant differences in QOL were found across demographic characteristics. Abnormal defecation (r = −0.398), fatigue (r = −0.266), and weakness (r = −0.286) were found to show higher correlation with QOL. More than 40% of IBS patients were found to suffer from varying degrees of anxiety or depression, and anxiety (r = −0.564) and depression (r = −0.411) were significantly negatively correlated with QOL (P < 0.001). Psychological factors showed the strongest impact (β′ = −0.451) and play a strong mediating role in the impact of physiological symptoms on QOL. Anxiety was found to be the strongest factor (β′ = −0.421). Conclusion Compared with other symptoms, psychological symptoms, particularly anxiety, are more common and have a more negative influence on QOL. The QOL of IBS patients is also significantly impacted by abnormal defecation, abdominal distension, and systemic extraintestinal somatic symptoms. In the treatment of IBS patients with unhealthy mental status, psychotherapy might be prioritized.

Relationships Between Self-Reflectiveness and Clinical Symptoms in Individuals During Pre-Morbid and Early Clinical Stages of Psychosis

Abstract:Background Self-reflectiveness,one dimension of cognitive insight,plays a protective role in an individual's mental state.Both high and low levels of self-reflectiveness have been reported in patients with schizophrenia and individuals at clinical high risk for the illness.Aims This study aimed to explore the relationship patterns between self-reflectiveness and clinical symptoms in individuals during the pre-morbid and early clinical stages of psychosis.Methods A total of 181 subjects,including individuals with attenuated positive symptoms(APS,n=122)and patients with first-episode psychosis(FEP,n=59),completed the Beck Cognitive Insight Scale and were evaluated using the Schedule of Assessment of Insight and Positive and Negative Syndrome Scale.All subjects were classified into three groups according to their level of self-reflectiveness:low level(LSR,n=59),medium level(MSR,n=67)and high level(HSR,n=55).Both linear and non-linear relationships between self-reflectiveness and clinical symptoms were explored.Results More individuals with APS were classified into the MSR group,while more patients with FEP were classified into the LSR group.The LSR group demonstrated less awareness of illness than the MSR and HSR groups,more stereotyped thinking and poorer impulse control but less anxiety than the MSR group,and lower levels of blunted affect and guilt feelings than the HSR group.The MSR group demonstrated lower stereotyped thinking than the HSR group.Compared to the LSR group,the MSR group had increased self-reflectiveness,improved awareness of illness,decreased stereotyped thinking,and better impulse control,but increased feelings of guilt.The HSR group showed increased stereotyped thinking when compared to the MSR group,but the other variables did not change significantly between these two groups.Overall,self-reflectiveness demonstrated an approximately inverse S-shaped relationship with the awareness of illness,a U-shaped relationship with stereotyped thinking and poor impulse control,and an almost linear relationship with anxiety and guilt feelings.Conclusions Self-reflectiveness demonstrates complex relationships with clinical symptoms and fails to exert significant positive effects when reaching a certain high level.

Abstract:There are a large number of atypical symptoms and undifferentiated diseases in general practice, in which sociological factors are deeply involved. So to incorporate social factors into clinical thinking and to interpret disease from the perspective of the patient′s social life will enrich the multidisciplinary medical model and holistic care in general practice. This article shares author′s clinical experience through introducing typical cases, in which the patient′s social life and behavior characteristics played certain roles in disease development. The article also explores a new way from the sociological perspectives to interpret the complex symptoms that are difficult to be explained clinically by biomedicine or psychology.

Abstract:Clinical reasoning is a crucial skill and defining characteristic of the medical profession, which relates to intricate cognitive and decision-making processes that are needed to solve real-world clinical problems. However, much of our current competency-based medical education systems have focused on imparting swathes of content knowledge and skills to our medical trainees, without an adequate emphasis on strengthening the cognitive schema and psychological processes that govern actual decision-making in clinical environments. Nonetheless, flawed clinical reasoning has serious repercussions on patient care, as it is associated with diagnostic errors, inappropriate investigations, and incongruent or suboptimal management plans that can result in significant morbidity and even mortality. In this article, we discuss the psychological constructs of clinical reasoning in the form of cognitive 'thought processing' models and real-world contextual or emotional influences on clinical decision-making. In addition, we propose practical strategies, including pedagogical development of a personal cognitive schema, mitigating strategies to combat cognitive bias and flawed reasoning, and emotional regulation and self-care techniques, which can be adopted in medical training to optimize physicians' clinical reasoning in real-world practice that effectively translates learnt knowledge and skill sets into good decisions and outcomes.

Abstract:Poor cognitive insight, including low self-reflectiveness and high self-certainty, contributes to poor clinical insight, which includes awareness of illness, relabelling of specific symptoms, and treatment compliance. However, inconsistent results regarding cognitive insight among individuals at clinical high risk of psychosis (CHR) have been reported. This study investigated the difference in cognitive insight among groups with different severity of positive symptoms and analysed the effect of cognitive insight on clinical insight in each group. All participants, including CHR individuals with 3 or 4 points (L-Pitem, n = 85) and 5 points (H-Pitem, n = 37) on any positive-symptom item of the Scale of Prodromal Syndromes, and patients with first-episode psychosis (FEP, n = 59), were measured cognitive and clinical insight using the Beck Cognitive Insight Scale and the Schedule of Assessment of Insight, respectively. The self-reflectiveness of cognitive insight was highest in the L-Pitem group and lowest in the FEP group. Self-reflectiveness was positively associated with awareness of illness in the L-Pitem and FEP groups; both self-reflectiveness and self-certainty was positively associated with treatment compliance in the L-Pitem group. Improving self-reflectiveness of cognitive insight may conduce to good clinical insight. Self-certainty may have different implication to individuals with mild prodromal symptoms.

Abstract:Psychosomatic symptoms are common and result in high health care costs. An etiological understanding is of importance for psychotherapy. In the present study, a psychosomatic model (the “axis of psychosomatic totality”) developed from clinical experience and previous literature was empirically examined. Twenty patients with psychosomatic symptoms were included in the study. To obtain the greatest possible stratification, all diagnosis groups between the ages of 18 and 65 years eligible for treatment in a psychosomatic clinic were included, for example, primarily mood disorders and neurotic, stress-related and somatoform disorders. A semistructured interview was conducted, audio-documented, and transcribed. A codebook was created based on the first codings. Data from the interviews were analyzed using grounded theory methods. The codes showed good interrater reliability. Statements about bodily symptoms could be assigned to the symbolic pole, asymbolic poles I and II, the imaginary zone, and the logic pole of the axis of psychosomatic totality. Most symptoms could be assigned to the imaginary zone, which reflects the unconscious imaginary body image (n = 157). Symbolic symptoms arising against the background of unconscious conflicts were also frequently mentioned (n = 99), while symptoms on the asymbolic poles I and II were reported less frequently (n = 37, n = 18). In particular, the transitions from the imaginary zone to the adjacent poles were fluid. The dimensions of the axis of psychosomatic totality were directly reflected in the bodily symptoms reported in the interviews. Abstract:Als "klinisches Denken" bezeichnet man alle Denkprozesse von Ärztinnen und Ärzten zum Stellen einer Diagnose und zur Festlegung eines Therapie- und Betreuungsplans. Artificial Intelligence (AI) wird das menschliche klinisch-diagnostische Denken erweitern, verbessern und beschleunigen, es aber vermutlich nicht ersetzen. Ihre Anwendung in der Medizin hat das Potenzial, medizinische Diagnosefehler drastisch zu reduzieren und den Ärzten mehr Zeit für die Betreuung ihrer Patienten zu verschaffen. "Clinical reasoning" refers to all the thought processes that physicians use to make a diagnosis and determine a treatment and care plan. Artificial intelligence (AI) will enhance, improve, and accelerate human clinical diagnostic thinking, but it is unlikely to replace it. Its application in medicine has the potential to drastically reduce medical diagnostic errors and give doctors more time to care for their patients. Here, we provide an overview of some of the key elements of clinical diagnostic reasoning and the potential impacts of AI on clinical reasoning. Das klinisch-diagnostische Denken ist eine der Kernkompetenzen aller klinisch tätigen Ärztinnen und Ärzte. Ein besseres Verständnis des klinisch-diagnostischen Denkens hilft, Diagnosefehler zu vermeiden und AI-Systeme adäquat anzuwenden. Die AI wird das menschliche klinisch-diagnostische Denken erweitern, verbessern und beschleunigen, aber sehr wahrscheinlich nicht ersetzen. Die AI hat das Potenzial, medizinische diagnostische Fehler drastisch zu reduzieren und mehr Zeit für Ärztinnen und Ärzte verfügbar zu machen, die diese für die Betreuung ihrer Patienten brauchen. Clinical intuition in the nursing process and decision-making-A mixed-studies review

Abstract:Aims and objectives: To review what is characteristic of registered nurses' intuition in clinical settings, in relationships and in the nursing process. Background: Intuition is a controversial concept and nurses believe that there are difficulties in how they should explain their nursing actions or decisions based on intuition. Much of the evidence from the body of research indicates that nurses value their intuition in a variety of clinical settings. More information on how nurses integrate intuition as a core element in daily clinical work would contribute to an improved understanding on how they go about this. Intuition deserves a place in evidence-based activities, where intuition is an important component associated with the nursing process. Design: An integrative review strengthened with a mixed-studies review. Methods: Literature searches were conducted in the databases CINAHL, PubMed and PsycINFO, and literature published 1985-2016 were included. The findings in the studies were analysed with content analysis, and the synthesis process entailed a reasoning between the authors. Results: After a quality assessment, 16 studies were included. The analysis and synthesis resulted in three categories. The characteristics of intuition in the nurse's daily clinical activities include application, assertiveness and experiences; in the relationships with patients' intuition include unique connections, mental and bodily responses, and personal qualities; and in the nursing process include support and guidance, component and clues in decision-making, and validating decisions. Conclusion: Intuition is more than simply a "gut feeling," and it is a process based on knowledge and care experience and has a place beside research-based evidence. Nurses integrate both analysis and synthesis of intuition alongside objective data when making decisions. They should rely on their intuition and use this knowledge in clinical practice as a support in decision-making, which increases the quality and safety of patient care. Relevance to clinical practice: We find that intuition plays a key role in more or less all of the steps in the nursing process as a base for decision-making that supports safe patient care, and is a validated component of nursing clinical care expertise.

Abstract:Objective To study the objective existence of Yin(not-yet-manifested)symptoms of Traditional Chinese Medicine in order to enlarge the category of syndrome differentiation of TCM.Methods The 615 healthy subjects who had the physical check were involved in the observation.According to the theories of Zangxiang(viscera manifestation)and Qi,Blood and Body Fluid of TCM,the abnormal findings of those who had no symptoms were divided into Zangfu(viscera)Yin(not-yet-manifested)symptoms(including the heart Yin symptoms,liver and gall bladder Yin symptoms,spleen and stomach Yin symptoms,lung Yin symptoms,and kidney Yin symptoms)and Qi,Blood and Body Fluid Yin symptoms.The data were processed with statistics.Results Totally 197 items were found abnormal and the 119 cases were found to be Yin symptom patients.Of them,there were 57 heart Yin symptom patients,51 liver and gall bladder Yin symptom patients,6 spleen and stomach Yin symptom patients,4 lung Yin symptom patients,17 kidney Yin symptom patients,and 31 Qi,Blood and Body Fluid Yin symptom patients.Conclusion Yin symptom of TCM was an objective existence and occupied a relatively high percentage in those who had no symptom and sign in physical check.The theory of Yin symptom will make the category of TCM differentiation widened and the level of diagnosis enhanced.
Study on symptom dimensions and clinical characteristics in patients with obsessive-compulsive disorder

Abstract:BACKGROUND AND OBJECTIVE: The aim of this study was to explore the symptom dimensions and clinical characteristics of obsessive-compulsive disorder in the context of Chinese culture.METHODS: In this cross-sectional study, the severity of obsessive-compulsive symptoms, the distribution of symptoms, and symptom scores of 263 patients with obsessive-compulsive disorder were assessed using the Yale-Brown Obsessive-Compulsive Scale and Yale-Brown Obsessive-Compulsive Inventory Symptoms Checklist. System cluster analysis and Pearson analysis were performed to explore the relationships between the main clinical characteristics and symptom dimensions.RESULTS: Cluster analysis identified four symptom dimensions of obsessive-compulsive disorder: (1) symmetry precision; (2) contamination cleaning; (3) aggression examination; and (4) taboo thinking. The symmetry precision dimension showed an association with years of education. The compulsive score, total Yale-Brown Obsessive Compulsive Scale score, contamination cleaning dimension, and aggression examination dimension had significant relationships. Age, age at onset, obsessive score, and compulsive score had a significant correlation with the taboo-thinking dimension.CONCLUSION: The symptom dimensions of obsessive-compulsive disorder in China are similar to those in other regions. Each of the four symptom dimensions had distinct clinical characteristics.

Abstract:Context: Artificial intelligence refers to a set of systems that are capable of performing functions similar to human intelligent functions. Today, artificial intelligence has been successfully incorporated into clinical decision support systems (CDSS). Evidence Acquisition: The current study aimed to briefly present a narrative mini-review on clinical reasoning and artificial intelligence. Data were gathered from Google Scholar, ScienceDirect, and PubMed databases using the "clinical decision support system, artificial intelligence, and clinical reasoning" keywords. Results: Clinical decision support systems are divided into two categories: knowledge-based and data-driven. The first category is called the rule-based expert system, and the second category is also named the machine-learning system. The usefulness of the mentioned systems and artificial intelligence in interpreting algorithmic and statistical information, where the human element can easily make a mistake, is that they are much more efficient and work with fewer errors. However, when it comes to dealing with a patient and his complaints and symptoms, because of the requirement for clinical judgment, the human element works much better in obtaining a mental image of the patient’s condition. Artificial intelligence is specifically used in scenarios such as the diagnosis of electrolyte disorders, interpreting ECG findings, and recognizing the causes of myocardial hypertrophy. Nonetheless, artificial intelligence has challenges, such as a lack of responsibility for medical decisions and treatment errors. Conclusions: Referring to the above-mentioned benefits and challenges of artificial intelligence, artificial and human intelligence cannot be superior to each other, and both have an irreplaceable role in clinical decision-making. The new view is that the goal of CDSS is to help the physician make better decisions by processing vast pieces of information as a whole entity rather than individually.
