Abstract:With the expanded enrolment of students,how to improve the quality of graduate education has become a prominent issue at present.Clinical ability is a core in producing graduate students of clinical medicine program.This study analyzes the main problems from the practice of training clinical ability for graduate students of otorhinolaryngology,which mainly include the uneven clinical foundation,less clinical practice chance,the contradiction of the skill training between single and comprehensive professional skills.Furthermore,the authors have come up with the corresponding solutions.

Abstract:In order to form correct drug treatment decision,we should judge comprehensively the diagnosis,illness,patient and pharmacy.Firstly,the perfect diagnosis should include not only all the contents of the diagnosis,but also all the diseases that the patient suffered from.Secondly,full assessment of illness condition should be made,including the risk factors,disease characteristics,disease progression and prognosis.Thirdly,individual assessing patient is also important,including the biological characteristics of the patients,as well as society and cultural background.Fourthly,assessment of treatment protocol should be done,including the overall arrangement of treatments measures,drug therapeutic schedules,implementation and response of the treatment plan.

Abstract:Clinical reasoning is a crucial skill and defining characteristic of the medical profession, which relates to intricate cognitive and decision-making processes that are needed to solve real-world clinical problems. However, much of our current competency-based medical education systems have focused on imparting swathes of content knowledge and skills to our medical trainees, without an adequate emphasis on strengthening the cognitive schema and psychological processes that govern actual decision-making in clinical environments. Nonetheless, flawed clinical reasoning has serious repercussions on patient care, as it is associated with diagnostic errors, inappropriate investigations, and incongruent or suboptimal management plans that can result in significant morbidity and even mortality. In this article, we discuss the psychological constructs of clinical reasoning in the form of cognitive 'thought processing' models and real-world contextual or emotional influences on clinical decision-making. In addition, we propose practical strategies, including pedagogical development of a personal cognitive schema, mitigating strategies to combat cognitive bias and flawed reasoning, and emotional regulation and self-care techniques, which can be adopted in medical training to optimize physicians' clinical reasoning in real-world practice that effectively translates learnt knowledge and skill sets into good decisions and outcomes.

Abstract:Objective To understand the medical students in clinical training in cognitive thinking ability, and to explore the clinical and basic thinking skills training prerequisites. Methods A cluster sampling method was used to investigate the cognition of 189 clinical undergraduates on clinical thinking ability training in longyan city from 2011 to 2013 year. The contents of the investiga-tion included cognition on clinical thinking ability training and basic conditions and prerequisites of clinical thinking ability. The results were statistically analyzed using SPSS 19.0 software. Enumeration data were analyzed by chi-square test and measurement data by t test of normal distribution and rank sum test of non-normal distribution. Results Of undergraduate medical students clinical thinking ability training on cognitive and not enough, only 16.93%(32/189) of the students think that really need, after practice that improve very needs students to 55.03%(104/189), without a student that requires absolutely no training, practice and compare difference to have statistical significance (P=0.000); training in basic condition before and after the regular teaching, clinical teaching practice, the prevention of medical disputes, differences of the clinical thinking ability were significant (P<0.05);the necessary condition of doctor patient communication skills, specialist consultation examination, to discuss difficult cases had very significant difference before and after training(P=0.000). Conclusion The clinical thinking ability of medical students is of great significance to build a harmonious relation-ship between doctors and patients, practice can improve the clinical thinking ability of the students in a certain extent, but there are still some problems in the current training hospital clinical thinking ability, need to further improve the training system model, in order to improve the clinical thinking ability of medical students.

Abstract:Chinese Medicine, being one of the most creative disciplines, with the superiority of its disciplinary theory and thinking pattern closely related to the studies of Sinology.Chinese Medicine has taken on its great aspect with good favorable prospects of timing, geographical and unity and coordination of human relationship, hence, the development of CM needs the recognition of its original advantage, making clear of its sources of original thought.The XIANG thought (iconic thinking) is the basic connotation and basic features of traditional Chinese thinking. The imported western culture depresses the XIANG (iconic) thinking of original thought, and, as a result, conceptual thinking of scientism and western medicine became the mainstream. Actually, conceptual thinking shouldn't be rejected, whereas the original thinking and original superiority characterized by our own nation should also be manifested. The knowledge, both oriental and occidental, are to be accepted simultaneously, while Chinese medicine and western medicine should be all mastered thoroughly and integrated as a whole so as to further proceed together.

Abstract:"Xiang thinking" is the main thinking mode in traditional Chinese medicine(TCM) with both philosophical and scientific connotations, and has an important influence on the emergence and development of TCM. This study systematically expounded the philosophical connotation and characteristics of "Xiang thinking", and its application in the construction of TCM theory, clinical syndrome differentiation and treatment, the formation of medicinal properties, and interpretation of efficacy of Chinese medicine. "Xiang thinking" in TCM develops and changes continuously with practical application, and its historical evolution can be summarized into three stages, i.e., "Ying Xiang(corresponding to Xiang)" "Fa Xiang(following Xiang)", and "Bian Xiang(differentiating Xiang)". The understanding of Xiang is based on the means and methods of human observation of things and the backgrounds of the philosophy, science, and development. The result of comparison depends on the depth and breadth of Xiang. In the real world, Xiang is showing new construction characteristics with multiple dimensions and levels. Therefore, this study proposed the thinking of "Bian Xiang", which is expected to provide a thinking approach that can realize the transformation from "Xiang thinking" to scientific research for the exploration and innovative research on life origin in the field of contemporary and TCM.

Abstract:The presented research examines Chinese students’ conceptualization of critical thinking through a qualitative study interviewing 46 Chinese college students. This study finds that Chinese students tend to conceptualize critical thinking as a multi-faceted concept. Cognitive thinking skills, intellectual autonomy, and the omnipresence of positive and negative aspects are commonly mentioned in the students’ definitions of critical thinking (among other concepts). The study finds that there are unique qualities in Chinese students’ conceptualizations of critical thinking that need to be understood in Chinese sociocultural contexts. This project proposes ways to understand and study Chinese students not previously considered in critical thinking literature.

Quantitative Imaging in Medicine and Surgery

Abstract:Following tremendous economic progress, society in China is also undergoing fundamental changes, as is the healthcare system. Currently the training of Chinese young doctors and their future work placement are all undergoing re-structuring. We compiled some thoughts and opinions on the topic of 'should clinicians in China engage in research?', and publish them as a special report in this issue of Quantitative Imaging in Medicine and Surgery (QIMS). The contributors included some editorial members of this journal, and a few personal friends. Besides a few minor linguistic corrections, opinions from the contributors have not been edited, as we want authors' to write their own independent views. However, it is possible there is a selection bias of the contributors of this paper; more likely those who are interested in the medical research are selected and therefore the views of the contributors may not be generalizable. To compare the structure and funding of China with other countries, authors from UK, The Netherlands, France, and USA are also invited.
