Posterolateral minimally invasion versus standard posterolateral approach in total hip replacement

Abstract:Objective To summarize the surgical experience and approach the short-term results of single-incision minimally invasive posterolateral approach in total hip arthroplasty(MIS-THA).Methods One hundred consecutive patients(100 hips) were divided into the MIS group and THA group that were performed through a standard posterolateral approach at the same period.Operative time and blood loss,hemoglobin concentration,the hematocrit count and the number of blood transfusions,postoperative pain,implant position and functional outcome were recorded and compared between the two groups.Functional outcome was assessed with the modified Harris hip score.Results The mean length of the incision and postoperative hospitalization was 8.5 cm,10.5 d versus 15.2 cm,11.8 d,respectively.The mean blood loss was significantly less in the MIS group,as was the level of postoperative pain(P<0.05).Other operative variables as well as implant position were comparable.There were two skin abrasion and one ecchymoma in MIS group,one deep venous thrombosis and one dislocation in standard group.Significantly fewer patients from the MIS group required walking aids at 6 weeks and 3 months(P<0.05).The modified Harris hip score was higher only at 6 weeks.Functional outcome and pain became comparable thereafter.Conclusions THA through posterolateral mini-incision approach is a reliable minimally invasive approach.Its advantages are less postoperative pain,shorter postoperative hospitalization and faster initial recuperation.
