Surgical Outcome of Coronary Artery Bypass Grafting in 54 Octogenarians

Abstract:Objective To investigate the surgical outcomes of coronary artery bypass grafting in octogenarians.Methods From June,2001 to Feb,2007,fifty four patients(43 mens,11 womens)80 years of age or older[80-99 yrs,mean(83.00±5.99)yrs] underwent isolated coronary artery bypass grafting.Nine patients underwent conventional(on-pump)CABG and 45 patients underwent off-pump CABG(OPCAB).The number of bypass grafts was 1 to 5(mean 2.98±0.46).Intermammary artery graft was used in 42 patients.Results Four deaths occurred peri-operatively.The mortality was 7.41%.Major postoperative complications included low cardiac output in 5 cases,respiratory dysfunction in 6 cases(3 patients underwent trachyotomy),and acute kidney injury in 3 cases(2 patients underwent CVVH).The atrail fibrillation occurred in 38 patients.Intraorta balloon pump(IABP)were used intra-and post-operatively in 3 patients.The average ICU stay was(3.90±1.96)days.Mean postoperative in-hostpital stay was(12.11±4.41)days.Forty-six patients were followed up from 1-58 months.Nine patients died during the follow-up period.93.5% patients were symptomatic freedom after the operation.Conclusion CABG can be done with acceptable postoperative morbidity and mortality in octogenarians.Intensive perioperative treatment should be stressed in octogenarians who underwent CABG.
