Complete revascularization of blood circulation in patients with acute myocardial infarction and multivessel disease:safety and effectiveness

Abstract:Objective To assess the safety and long-term clinical outcomes of patients with acute myocardial infarction (AMI) and multivessel disease (MVD) who received primary percutaneous coronary intervention (PCI) followed by complete revascularization using drug-eluting stents during index hospitalization.Methods A total of 153 patients with AMI and MVD underwent primary PCI, with 92 receiving complete revascularization at index hospitalization (group A) and 61 receiving PCI for infarct-related artery only (group B). Clinical and procedural characteristics, in-hospital and 2-year major adverse cardiac events (MACE), including death, reoccurrence of non-fatal myocardial infarction, and target vessel revascularization, were compared between the 2 groups. Results Baseline clinical characteristics, including the age, sex, sites of infarction, number of diseased vessels, distribution of IRA and successful rate of primary PCI, were comparable between the two groups (all P>0.05). In group A, the second PCI was performed (9.2±2.2) d after the first procedure. The number of stents deployed (3.0±0.9 vs. 1.1±0.2, P<0.01) and the length of hospitalization ([17.4±5.1] d vs. [11.5±4.9] d, P<0.01) were more in group A than in group B. In-hospital death and MACE rate were similar between the two groups (group A: 4.3% vs. group B: 4.9%, P>0.05). At 2-year follow-up, the rate of re-hospitalization was significantly lower in group A than in group B (28.3% vs. 67.7%, P<0.01), mainly due to reduced need for revascularization of non-IRA. The incidences of death (6.5% vs. 8.2%), reinfarction (3.3% vs. 3.3%), TVR (3.3% vs. 6.6%), MACE (10.9% vs. 14.8%), definite stent thrombosis (1.1% vs. 1.6%) and overall 2-year MACE-free survival (89.1% vs. 85.2%) were all similar in the two groups (all P>0.05).Conclusion Complete revascularization for patients with AMI and MVD at index hospitalization is safe and can significantly reduce the need for re-hospitalization and long-term MACE incidence.
