Abstract:Abstract Objectives: To develop a microbial test method to ascertain the passage of airborne bacteria through the medical device packaging System after sterilization, and to apply this test method to flexible packages under mechanical pressure changes. Methods: Petri dishes filled with nutrient agar were integrated into the packaging unit prior to sterilization. We examined paper packaging consisting of 1 (single-paper packaging [P]), 2 (double-paper packaging [PP] and textile and paper double packaging [TP]), and 3 (double packaging with transport packaging [TPP]) layers. After sterilization, the test packages were pressed five times per minute for 1 or 3 hours by a mechanical device weighing 1 kg. This exposure took place in rooms with an average airborne bacterial count of 35 (room 1) or 440 (room 2) CFU/m 3 . The packaging was opened following culture at 37° C for 48 hours to determine the number of colonies formed. Results: The proportion of contaminated packages rose with the duration of mechanical stress and increased airborne bacteria concentration. Thus, mechanical pressure change for 3 hours resulted in the contamination of 60% (P), 15% (PP), 0% (TP), and 0% (TPP) of the packages in room 1, whereas 100% (P), 65% (PP), 73% (TP), and 0% (TPP) of the packages in room 2 were contaminated. Conclusions: This test method allows sterile packaging Systems to be tested for contamination under practical conditions without extensive laboratory preparation. Contamination as a resuit of laboratory errors can be ruled out almost certainly.

Abstract:Antimicrobial food packaging is an innovative technique used to reduce or inhibit the growth of pathogenic and spoilage microorganisms in foods. It involves the incorporation of natural antimicrobials, such as essential oils, enzymes, and bacteriocins, into the packaging matrix. The incorporation of antimicrobials into the packaging matrix allows for relatively high concentrations of the gas/vapor form of the antimicrobials on the food surface to be released and maintained for a longer period of time, enhancing the shelf-life and quality of various types of foods and preventing the possibility of recontamination after packaging. Film packaging, direct coating, and gaseous release are three common application methods of antimicrobial food packaging. There are some advantages and disadvantages associated with each method. Antimicrobial food packaging is able to inactivate both foodborne pathogens and spoilage microorganisms in the different types of food, hence improving the food safety and shelf-life.

Abstract:Radiation and ethylene oxide terminal sterilization are the two most frequently used processes in the medical device industry to render product within the final sterile barrier package free from viable microorganisms. They are efficacious, safe, and efficient approaches to the manufacture of sterile product. Terminal sterilization is routinely applied to a wide variety of commodity healthcare products (drapes, gowns, etc.) and implantable medical devices (bare metal stents, heart valves, vessel closure devices, etc.) along with products used during implantation procedures (catheters, guidewires, etc.). Terminal sterilization is also routinely used for processing combination products where devices, drugs, and/or biologics are combined on a single product. High patient safety, robust standards, routine process controls, and low-cost manufacturing are appealing aspects of terminal sterilization. As the field of combination products continues to expand and evolve, opportunity exists to expand the application of terminal sterilization to new combination products. Material compatibility challenges must be overcome to realize these opportunities. This article introduces the reader to terminal sterilization concepts, technologies, and the related standards that span different industries (pharmaceutical, medical device, biopharmaceuticals, etc.) and provides guidance on the application of these technologies. Guidance and examples of the application of terminal sterilization are discussed using experiences with drug eluting stents and bioresorbable vascular restoration devices. The examples provide insight into selecting the sterilization method, developing the process around it, and finally qualifying/validating the product in preparation for regulatory approval and commercialization. Future activities, including new sterilization technologies, are briefly discussed.

Abstract:Bio-effects of non-thermal plasma include lethality of microorganisms and stimulation of mammalian cells,which brings up a series of techniques in disinfection and clinical medicine.In this paper,the applications of non-thermal plasma in medical device sterilization,water and air cleaning,as well as food and package treatment are reviewed.Commercialized plasma sterilizer for medical devices can be classified into vacuum and atmospheric pressure,the volume of the former is much larger while the latter is more simple and easy to operate.For disinfection of water,air and food,it is not yet commercialized due to its energy consumption,efficiency or chemical residues.In the last decade,the developed plasma sources and techniques for medicine are successfully applied to treatment of decayed tooth,skin,wound,and cancer,with some of them approved for clinical practice.In the future,the research focus will be identifying the interactions between plasmas and cells,and developing more effective and practical equipment for different applications.

Abstract:The concept of using paper in medical packaging has been accepted widely. as it provides the essential permeability to allow ingress of the sterilising media whilst preventing the post-penetration of bacteria. thereby maintaining sterility of the medical device itself. In Europe this widespread application of paper has demanded the introduction of proper standards for sterilization papers and converted items, such as bags and pouches. In this article these standards are discussed, i.e. British Standards, German DIN 58953, French Pharmacopoeia and Eucomed (European Confederation of Medical Suppliers Association). Comparisons of these standards, including results of our testing, also are presented t o underline the essential differences between specifications for the same grade of paper. In particular, special note is made in the case of air permeability of papers used for gas sterilization. The most reasonable solution is put forward by Eucomed, in which permeability performance is more fully addressed. The principles of various methods for the determination of microbial barrier performances of sterilization papers also are described and some comments in this respect are given.

Abstract:Objectives: The goal of this research was to evaluate how material curl, package structure and handling of pouches containing medical devices affect rates of contact between non-sterile surfaces and sterile devices during aseptic transfer. Methods: One hundred and thirty-six individuals with practical experience in aseptic technique were recruited. Participants were asked to present the contents of four different pouch designs (a standard, one designed to curl in, another to curl out and one that incorporated a tab) using two transfer techniques. During the first block of trials "standard technique" was used; participants presented using their typical methods to the sterile field. Trials in the second block employed "modified technique"; participants were instructed to grab the package at the top center and present package contents using a single, fluid motion. The outside of the pouch and the backs of the participants' hands were coated using a simulated contaminant before each trial. The simulant was undetectable in the visible spectrum, but fluoresced under a black light. The dependent variable was recorded in a binary fashion and analyzed using a generalized linear mixed model. Results: Participants were between 20-57 and the averaged year 5.1 years of experience in aseptic technique. The data analysis was based on generalized linear mixed effects (GLMM) model, which accommodates the repeated measurements within the same participant. The effect of the pouch design was significant (P‹0.001), but the effect of aseptic technique did not suggest significance (P = 0.088). Specifically, pouches designed with the material curled outward resulted in significantly fewer contacts with non-sterile surfaces than the other styles, including the inward, tab, and standard styles; this was true regardless of the used aseptic technique, standard (P = 0.0171, P = 0.0466, P = 0.0061, respectively) or modified (P‹0.0001 for all comparisons)). Conclusion: Results presented here contribute to a growing body of knowledge that investigates packaging as a potential route of contamination for sterile devices during aseptic presentation. Specifically, we provide insights regarding how both package design and opening technique can be informed in ways that build safety into the healthcare system.

Abstract:Objective To monitor disinfection package prior to sterilization process,so as to guarantee the effectiveness of disinfection and sterilization.Methods According to requirement in Disinfection technique standard Open-package examinations of 729 disinfection packages were performed in central supply room,including cleaning quality,package description strap,in-package description card,external condition and internal condition.Results 35.39% of packages had problems in above aspects.After monitoring and feedback,the qualification rate of package before disinfection rose from 40.31% in January to 85.37% in December.Conclusion Behavior change requires involvement of quality control department.PDCA recycle management software can be continuously used for planning,implementation,feedback and evaluation.

Abstract:The article discusses issues related to the creation and use of packaging materials for different types of products, including those specially designed for placing medical devices in them, before sterilization. The changes taking place related to the improvement of the barrier properties of packages to extend the preservation period of sterility of medical devices (both new types and complex in design) sterilized by different methods are noted. It is shown how over the course of several years new materials have been developed, technologies/methods of processing and reprocessing of raw materials and materials have changed, which make it possible to obtain packaging materials of different chemical nature and properties. Changes in existing packages from traditional materials (metal) to of single-use sterilization packages of a new generation with improved manufacturing technologies that provide mechanical, barrier and antimicrobial properties of packages differing in types, sizes, material properties are noted. The priority in the use of packaging materials for medical devices remains for materials that meet the requirements of GOST ISO 11607 "Packaging for medical devices subject to terminal sterilization", since the standard imposes more stringent requirements for the execution of packaging materials.

Abstract:Capsule-based dry powder inhalers (cDPIs) are widely utilized in the delivery of pharmaceutical powders to the lungs. In these systems, the fundamental nature of the interactions between the drug/formulation powder, the capsules, the inhaler device, and the patient must be fully elucidated in order to develop robust manufacturing procedures and provide reproducible lung deposition of the drug payload. Though many commercially available DPIs utilize a capsule-based dose metering system, an in-depth analysis of the critical factors associated with the use of the capsule component has not yet been performed. This review is intended to provide information on critical factors to be considered for the application of a quality by design (QbD) approach for cDPI development. The quality target product profile (QTPP) defines the critical quality attributes (CQAs) which need to be understood to define the critical material attributes (CMA) and critical process parameters (CPP) for cDPI development as well as manufacturing and control.

Abstract:Abstract Enabling high-level electrical performance and smooth optical signal transmission is a key requirement for microelectronics packaging materials. This becomes even more relevant for biomedical applications, in which protection requirements must be met alongside standard performance and signal transmission benchmarks. The major challenge: medical packaging must to provide absolute reliability. Microelectronic components face potentially damaging working environment hazards such as high humidity, extreme temperatures, corrosive chemicals, and bodily fluid – especially in the application cases of implantable, ingestible, and wearable devices. Hermeticity is an important factor in the performance of medical packaging. Non-hermetic packaging, sometimes known as “quasi-hermetic” uses organic materials, such as silicone or plastic over-moldings, that are not designed to withstand extreme conditions and break down over time. Material break down in these types of packages can lead to limited protection by allowing permeation of water and other gases through the polymer structure. Non-hermetic packages typically reach critical permeation levels after a very short period of time. For microelectronic components, even the smallest, hardly detectable traces of hydrogen and water vapor inside a package can compromise the performance and reliability of the encapsulated chips and circuits and potentially lead to interconnect failure, one of the most common reliability failure modes in microelectronic applications. The only way to overcome the challenge of achieving absolute reliability is by using hermetic packaging technologies with inorganic materials, such as glass, ceramics and metal. Unique manufacturing processes of these hermetic components create vacuum-tight seals that prevent moisture and harmful gases from penetrating into the package. Hermetically sealed packages deliver uncompromised reliability, offering long-term protection of sensitive electronics in medical devices – even after thousands of surgical procedures and steam sterilization cycles. This technical presentation will introduce the different reliability levels of packaging materials and provide an in-depth comparison of the respective advantages, disadvantages, and typical application areas of various hermetic packaging technologies. The latest insights into materials and technologies available for high-reliability packaging of medical electronics will be presented.

Abstract:Objective:To improve hospital infection control and management of central sterile supply.Methods:The hospital sterilization and supply center establishes the standard workflow from equipment recovery,equipment cleaning,cleaning cleanliness inspection,device packaging,instrument sterilization to dissemination to strengthen quality monitoring,focusing on staff training,communication and coordination and sector departments.Results:Hospital infection control management level and effectiveness were improved significantly to achieve the integration of resources through saving the capital and operating costs of the relevant sections of the cleaning and disinfection of facilities to avoid re-contamination of surgical instruments on the staff and surroundings.Conclusion:The measures taken are feasible and productive.

Challenging the Sterrad 100NX sterilizer with different carrier materials and wrappings under experimental "clean" and "dirty" conditions

Abstract:Background: Sterrad sterilizers have been developed for the sterilization of thermolabile materials. The aim of the present study was to challenge the efficacy of this low-temperature hydrogen peroxide-based sterilization system with different carrier materials and wrappings under experimental "clean" and "dirty" conditions. Methods: We tested the sporocidal effect of the Sterrad 100NX sterilizer (Advanced Sterilization Products, Irvine, CA) on the carrier materials titanium, polyethylene, and polyurethane with single versus 3 wrappings of inoculated carriers. To simulate insufficient cleaning or crystalline residues, carriers were charged with spore inocula containing organic and inorganic burdens. Results: Our qualitative results show that irrespective of the number of wrappings in the "clean" condition, sterilization by the Sterrad 100NX was equally effective on all 3 carrier materials, reaching a log-10 reduction rate of ≥ 6 under standard half-cycle conditions. Any additional organic or inorganic challenge significantly impaired the sterilization outcome. Conclusion: Results of our current study emphasize the utmost importance of thorough and reliable cleaning of medical devices before being exposed to a subsequent hydrogen peroxide sterilization process. Any institution using this sterilization technology should have a well-established and validated cleaning process and enforce a rigorous quality assurance program for all steps of the presterilization processing of medical devices.

Abstract:Due to the unique thermal absorption and release capabilities, phase-change materials (PCMs) are used in various industrial fields, such as photo-thermal storage and building-energy saving. In recent years, more and more research has been dedicated to applying PCMs to the medical field with substantial progress, opening new avenues for disease treatment and healthcare. The safety and reliability of PCMs may be taken into serious consider for continuous application in the medical field, while the relevant review on the related development is currently lacking. In this work, the methods for enhancing the applicability and reliability of PCMs in the medical field are systematically summarized, including microencapsulation, electrospinning technology, and porous framework encapsulation, which effectively address the issues of liquid leakage. Subsequently, the emerging advances of PCMs in medical healthcare, including medical dressings treatment, drug delivery, cold chain transport, and bio-bone cement are summarized. By exploring and analyzing the encapsulation methods, principles as well as the emerging advances of PCMs in medical field, the challenges and perspectives promoting the applications are identified, presenting the guidelines for further development. This work systematically summarizes the encapsulation methods for enhancing the applicability and reliability of phase change materials (PCMs) from the perspective of safety and reliability, as well as the emerging advances of PCMs in medical healthcare, clarifying the challenges and prospects for promoting the applications in the medical field, presenting the guideline for the further development. image
A review of Spaulding's classification system for effective cleaning, disinfection and sterilization of reusable medical devices: Viewed through a modern-day lens that will inform and enable future sustainability

Abstract:Despite advances in medicine and innovations in many underpinning fields including disease prevention and control, the Spaulding classification system, originally proposed in 1957, remains widely used for defining the disinfection and sterilization of contaminated re-usable medical devices and surgical instruments. Screening PubMed and Scopus databases using a PRISMA guiding framework generated 272 relevant publications that were used in this review. Findings revealed that there is a need to evolve how medical devices are designed, and processed by cleaning, disinfection (and/or sterilization) to mitigate patient risks, including acquiring an infection. This Spaulding Classification remains in use as it is logical, easily applied and understood by users (microbiologists, epidemiologists, manufacturers, industry) and by regulators. However, substantial changes have occurred over the past 65 years that challenge interpretation and application of this system that includes inter alia emergence of new pathogens (viruses, mycobacteria, protozoa, fungi), a greater understanding of innate and adaptive microbial tolerance to disinfection, toxicity risks, increased number of vulnerable patients and associated patient procedures, and greater complexity in design and use of medical devices. Common cited examples include endoscopes that enable non- or minimal invasive procedures but are highly sophisticated with various types of materials (polymers, electronic components etc), long narrow channels, right angle and heat-sensitive components and various accessories (e.g., values) that can be contaminated with high levels of microbial bioburden and patient tissues after use. Contaminated flexible duodenoscopes have been a source of several significant infection outbreaks, where at least 9 reported cases were caused by multidrug resistant organisms [MDROs] with no obvious breach in processing detected. Despite this, there is evidence of the lack of attention to cleaning and maintenance of these devices and associated equipment. Over the last few decades there is increasing genomic evidence of innate and adaptive resistance to chemical disinfectant methods along with adaptive tolerance to environmental stresses. To reduce these risks, it has been proposed to elevate classification of higher-risk flexible endoscopes (such as duodenoscopes) from semi-critical [contact with mucous membrane and intact skin] to critical use [contact with sterile tissue and blood] that entails a transition to using low-temperature sterilization modalities instead of routinely using high-level disinfection; thus, increasing the margin of safety for endoscope processing. This timely review addresses important issues surrounding use of the Spaulding classification system to meet modern-day needs. It specifically addresses the need for automated, robust cleaning and drying methods combined with using real-time monitoring of device processing. There is a need to understand entire end-to-end processing of devices instead of adopting silo approaches that in the future will be informed by artificial intelligence and deep-learning/machine learning. For example, combinational solutions that address the formation of complex biofilms that harbour pathogenic and opportunistic microorganisms on the surfaces of processed devices. Emerging trends are addressed including future sustainability for the medical devices sector that can be enabled via a new Quintuple Helix Hub approach that combines academia, industry, healthcare, regulators, and society to unlock real world solutions.

Abstract:Given the complexity of the sterilization process, and the risk involved in absence of strict adherence to the protocol described by the medical device manufacturers, terminally sterilized devices are emerging and being promoted in the field of medical practices. The characteristics associated with conventional reprocessing are demanding logistics, costs of delay, operations and adverse events, and unacceptable liability. Demanding logistics were a result of decoupled staff between the operating room and sterilize processing department, understaffed and high-volume processing with an additional burden due to inventory management and inefficient training. Other costs arose from upkeep, delay in operating room, and surgical-site infections. Liability arose from the repeatedly use of an unquantifiable process thus adding uncertainties, limited shelf life of the reprocessed implants, contingency of flash sterilization and introduction of newer technology with higher demand on cleaning performances. In contrast, terminally sterilized single-use devices do not carry any of the aforementioned-characteristics, deeming it to be the simplest solution to the current conundrum. This review serves to provide an evaluation of logistics, costs, and potential adverse effects, both directly and indirectly, associated with current practices in the sterile processing department, and also describes as to how the use of terminally sterilized devices can help circumvent those.

Abstract:Biomaterials used as implants or as medical devices come into human contact and must first be sterilized and maintained sterile with certainty until usage to ensure success. Sterility of materials is usually assured by less than one in one million devices having a microbial presence. Sterilization of biomaterials is most commonly achieved using radiation, chemical or thermal methods. The use of ionizing radiation and ethylene oxide offer advantages for scale-up, and are typically preferred as methods for industrial level sterilization. Autoclave sterilization and chemical surface treatments are most commonly used for disinfection of glass and metallic devices. While patient safety should be the primary consideration in the choice of appropriate methods to ensure sterility, attention should also be paid to the impact of the sterilization method used on the biomaterial and any potential changes in functional performance.

Abstract:The issue of medication noncompliance has resulted in major risks to public safety and financial loss. The new omnipresent medicine enabled by the Internet of things offers fascinating new possibilities. The controlled delamination material (CDM) seals and regulates wireless technologies in novel medicine packaging. As such, wearable biomedical sensors may capture a variety of crucial parameters via wireless communication. On-site treatment and prediction of these critical factors are made possible by high-performance architecture. The user interface is also highlighted to make surgery easier for the elderly, disabled, and patients. Land testing incorporates and validates an approach for prototyping I2Pack and iMedBox. Additionally, sustainability, increased product safety, and quality standards are crucial throughout the life sciences. To achieve these standards, intelligent packaging is also used in the food and pharmaceutical industries. These technologies will continuously monitor the quality of a product and communicate with the user. Data carriers, indications, and sensors are the three most important groups. They are not widely used at the moment, although their potential is well understood. Intelligent packaging should be used in these sectors and the functionality of the systems and the values presented in this analysis.
