Abstract:Abstract Objectives: To develop a microbial test method to ascertain the passage of airborne bacteria through the medical device packaging System after sterilization, and to apply this test method to flexible packages under mechanical pressure changes. Methods: Petri dishes filled with nutrient agar were integrated into the packaging unit prior to sterilization. We examined paper packaging consisting of 1 (single-paper packaging [P]), 2 (double-paper packaging [PP] and textile and paper double packaging [TP]), and 3 (double packaging with transport packaging [TPP]) layers. After sterilization, the test packages were pressed five times per minute for 1 or 3 hours by a mechanical device weighing 1 kg. This exposure took place in rooms with an average airborne bacterial count of 35 (room 1) or 440 (room 2) CFU/m 3 . The packaging was opened following culture at 37° C for 48 hours to determine the number of colonies formed. Results: The proportion of contaminated packages rose with the duration of mechanical stress and increased airborne bacteria concentration. Thus, mechanical pressure change for 3 hours resulted in the contamination of 60% (P), 15% (PP), 0% (TP), and 0% (TPP) of the packages in room 1, whereas 100% (P), 65% (PP), 73% (TP), and 0% (TPP) of the packages in room 2 were contaminated. Conclusions: This test method allows sterile packaging Systems to be tested for contamination under practical conditions without extensive laboratory preparation. Contamination as a resuit of laboratory errors can be ruled out almost certainly.

Abstract:The article discusses issues related to the creation and use of packaging materials for different types of products, including those specially designed for placing medical devices in them, before sterilization. The changes taking place related to the improvement of the barrier properties of packages to extend the preservation period of sterility of medical devices (both new types and complex in design) sterilized by different methods are noted. It is shown how over the course of several years new materials have been developed, technologies/methods of processing and reprocessing of raw materials and materials have changed, which make it possible to obtain packaging materials of different chemical nature and properties. Changes in existing packages from traditional materials (metal) to of single-use sterilization packages of a new generation with improved manufacturing technologies that provide mechanical, barrier and antimicrobial properties of packages differing in types, sizes, material properties are noted. The priority in the use of packaging materials for medical devices remains for materials that meet the requirements of GOST ISO 11607 "Packaging for medical devices subject to terminal sterilization", since the standard imposes more stringent requirements for the execution of packaging materials.

Abstract:Radiation and ethylene oxide terminal sterilization are the two most frequently used processes in the medical device industry to render product within the final sterile barrier package free from viable microorganisms. They are efficacious, safe, and efficient approaches to the manufacture of sterile product. Terminal sterilization is routinely applied to a wide variety of commodity healthcare products (drapes, gowns, etc.) and implantable medical devices (bare metal stents, heart valves, vessel closure devices, etc.) along with products used during implantation procedures (catheters, guidewires, etc.). Terminal sterilization is also routinely used for processing combination products where devices, drugs, and/or biologics are combined on a single product. High patient safety, robust standards, routine process controls, and low-cost manufacturing are appealing aspects of terminal sterilization. As the field of combination products continues to expand and evolve, opportunity exists to expand the application of terminal sterilization to new combination products. Material compatibility challenges must be overcome to realize these opportunities. This article introduces the reader to terminal sterilization concepts, technologies, and the related standards that span different industries (pharmaceutical, medical device, biopharmaceuticals, etc.) and provides guidance on the application of these technologies. Guidance and examples of the application of terminal sterilization are discussed using experiences with drug eluting stents and bioresorbable vascular restoration devices. The examples provide insight into selecting the sterilization method, developing the process around it, and finally qualifying/validating the product in preparation for regulatory approval and commercialization. Future activities, including new sterilization technologies, are briefly discussed.

Abstract:The packaging concept has been adopted from plants’ seed as it is guarded in a closed shell till it germinates in a suitable season. Therefore, a type of natural packaging results in protection, preservation, and production. In the modern era, packaging aims to secure the product's safety until usage. Delivering a product to the customers requires a suitable container and closure with non-interaction with the product by any external influence. The primary container protection is strengthened by the secondary ones, resulting in safe storage and enhanced stability. The display of essential information on the label with elegance ensures the integrity of the product. The label displays the name, strength and quality, identification number, storage instructions, usage directions, the product's expiry date, and the manufacturer's name and address. This information provides compliance aid to the patients. According to the regulatory body’s guidelines, all forms of counterfeiting must be guarded against. Packaging processes and related equipment need validation, and quality control of the equivalent is to be followed. The article discusses the packaging components' role and various types (materials, measurement, etc.), technology, design, regulatory guidelines and tests applied, types of hazards, labelling procedures with guidelines, and quality assurance and control. This review paper highlights structural features of various parts of containers, strip packaging, blister packaging, bubble packaging, and various types of injection vials that were not addressed in a previous publication on the packaging system.Graphical abstract

Abstract:Abstract Enabling high-level electrical performance and smooth optical signal transmission is a key requirement for microelectronics packaging materials. This becomes even more relevant for biomedical applications, in which protection requirements must be met alongside standard performance and signal transmission benchmarks. The major challenge: medical packaging must to provide absolute reliability. Microelectronic components face potentially damaging working environment hazards such as high humidity, extreme temperatures, corrosive chemicals, and bodily fluid – especially in the application cases of implantable, ingestible, and wearable devices. Hermeticity is an important factor in the performance of medical packaging. Non-hermetic packaging, sometimes known as “quasi-hermetic” uses organic materials, such as silicone or plastic over-moldings, that are not designed to withstand extreme conditions and break down over time. Material break down in these types of packages can lead to limited protection by allowing permeation of water and other gases through the polymer structure. Non-hermetic packages typically reach critical permeation levels after a very short period of time. For microelectronic components, even the smallest, hardly detectable traces of hydrogen and water vapor inside a package can compromise the performance and reliability of the encapsulated chips and circuits and potentially lead to interconnect failure, one of the most common reliability failure modes in microelectronic applications. The only way to overcome the challenge of achieving absolute reliability is by using hermetic packaging technologies with inorganic materials, such as glass, ceramics and metal. Unique manufacturing processes of these hermetic components create vacuum-tight seals that prevent moisture and harmful gases from penetrating into the package. Hermetically sealed packages deliver uncompromised reliability, offering long-term protection of sensitive electronics in medical devices – even after thousands of surgical procedures and steam sterilization cycles. This technical presentation will introduce the different reliability levels of packaging materials and provide an in-depth comparison of the respective advantages, disadvantages, and typical application areas of various hermetic packaging technologies. The latest insights into materials and technologies available for high-reliability packaging of medical electronics will be presented.

Abstract:The concept of using paper in medical packaging has been accepted widely. as it provides the essential permeability to allow ingress of the sterilising media whilst preventing the post-penetration of bacteria. thereby maintaining sterility of the medical device itself. In Europe this widespread application of paper has demanded the introduction of proper standards for sterilization papers and converted items, such as bags and pouches. In this article these standards are discussed, i.e. British Standards, German DIN 58953, French Pharmacopoeia and Eucomed (European Confederation of Medical Suppliers Association). Comparisons of these standards, including results of our testing, also are presented t o underline the essential differences between specifications for the same grade of paper. In particular, special note is made in the case of air permeability of papers used for gas sterilization. The most reasonable solution is put forward by Eucomed, in which permeability performance is more fully addressed. The principles of various methods for the determination of microbial barrier performances of sterilization papers also are described and some comments in this respect are given.

Throughout delivery and storage, the packaging may interact either directly or indirectly with the drug product or medical device, potentially leading to chemical reactions between the two. The role of packaging is paramount in ensuring success, safeguarding the product, and facilitating its sale. Similar to other items found in supermarkets, prescription pharmaceuticals necessitate packaging that addresses various needs, including security, promptness, safety, product identity, quality assurance, patient well-being, and product excellence. Packaging represents both a scientific and artistic endeavour, involving the consideration of numerous factors, beginning with the fundamental design and technology utilized to package the product securely, while also ensuring its protection, presentation, and compliance with manufacturing standards during transportation, storage, and consumption. To uphold the physiochemical, biological, and chemical stability of drugs, packaging professionals design containers capable of withstanding the pressures encountered during supply and shipping processes. Enhancements in the field of prescription drug development have long emphasized the importance of packaging expertise. This serialization process is crucial for bolstering drug security within the supply chain while maintaining drug quality, thereby minimizing the risk of counterfeit drugs infiltrating the distribution network.

Abstract:Objectives: The goal of this research was to evaluate how material curl, package structure and handling of pouches containing medical devices affect rates of contact between non-sterile surfaces and sterile devices during aseptic transfer. Methods: One hundred and thirty-six individuals with practical experience in aseptic technique were recruited. Participants were asked to present the contents of four different pouch designs (a standard, one designed to curl in, another to curl out and one that incorporated a tab) using two transfer techniques. During the first block of trials "standard technique" was used; participants presented using their typical methods to the sterile field. Trials in the second block employed "modified technique"; participants were instructed to grab the package at the top center and present package contents using a single, fluid motion. The outside of the pouch and the backs of the participants' hands were coated using a simulated contaminant before each trial. The simulant was undetectable in the visible spectrum, but fluoresced under a black light. The dependent variable was recorded in a binary fashion and analyzed using a generalized linear mixed model. Results: Participants were between 20-57 and the averaged year 5.1 years of experience in aseptic technique. The data analysis was based on generalized linear mixed effects (GLMM) model, which accommodates the repeated measurements within the same participant. The effect of the pouch design was significant (P‹0.001), but the effect of aseptic technique did not suggest significance (P = 0.088). Specifically, pouches designed with the material curled outward resulted in significantly fewer contacts with non-sterile surfaces than the other styles, including the inward, tab, and standard styles; this was true regardless of the used aseptic technique, standard (P = 0.0171, P = 0.0466, P = 0.0061, respectively) or modified (P‹0.0001 for all comparisons)). Conclusion: Results presented here contribute to a growing body of knowledge that investigates packaging as a potential route of contamination for sterile devices during aseptic presentation. Specifically, we provide insights regarding how both package design and opening technique can be informed in ways that build safety into the healthcare system.

Abstract:Determining what biological testing is required for the development and approval of a new medical device can be complex. Biological evaluation of medical devices is governed by standards such as ISO 10993, the Japanese Ministry of Health, Labor and Welfare Notifications and Ordinances, and China’s GB/T 16886 national standards. This chapter highlights the processes and steps involved in the evaluation of new medical devices and materials. It discusses how chemical characterization and toxicology data are used to evaluate risk and establish biocompatibility. Abstract:The issue of medication noncompliance has resulted in major risks to public safety and financial loss. The new omnipresent medicine enabled by the Internet of things offers fascinating new possibilities. Additionally, an in-home healthcare station (IHHS), it is necessary to meet the rapidly increasing need for routine nursing and on-site diagnosis and prognosis. This article proposes a universal and preventive strategy to drug management based on intelligent and interactive packaging (I2Pack) and IMedBox. The controlled delamination material (CDM) seals and regulates wireless technologies in novel medicine packaging. As such, wearable biomedical sensors may capture a variety of crucial parameters via wireless communication. On-site treatment and prediction of these critical factors are made possible by high-performance architecture. The user interface is also highlighted to make surgery easier for the elderly, disabled, and patients. Land testing incorporates and validates an approach for prototyping I2Pack and iMedBox. Additionally, sustainability, increased product safety, and quality standards are crucial throughout the life sciences. To achieve these standards, intelligent packaging is also used in the food and pharmaceutical industries. These technologies will continuously monitor the quality of a product and communicate with the user. Data carriers, indications, and sensors are the three most important groups. They are not widely used at the moment, although their potential is well understood. Intelligent packaging should be used in these sectors and the functionality of the systems and the values presented in this analysis.

A review of Spaulding's classification system for effective cleaning, disinfection and sterilization of reusable medical devices: Viewed through a modern-day lens that will inform and enable future sustainability

Abstract:Despite advances in medicine and innovations in many underpinning fields including disease prevention and control, the Spaulding classification system, originally proposed in 1957, remains widely used for defining the disinfection and sterilization of contaminated re-usable medical devices and surgical instruments. Screening PubMed and Scopus databases using a PRISMA guiding framework generated 272 relevant publications that were used in this review. Findings revealed that there is a need to evolve how medical devices are designed, and processed by cleaning, disinfection (and/or sterilization) to mitigate patient risks, including acquiring an infection. This Spaulding Classification remains in use as it is logical, easily applied and understood by users (microbiologists, epidemiologists, manufacturers, industry) and by regulators. However, substantial changes have occurred over the past 65 years that challenge interpretation and application of this system that includes inter alia emergence of new pathogens (viruses, mycobacteria, protozoa, fungi), a greater understanding of innate and adaptive microbial tolerance to disinfection, toxicity risks, increased number of vulnerable patients and associated patient procedures, and greater complexity in design and use of medical devices. Common cited examples include endoscopes that enable non- or minimal invasive procedures but are highly sophisticated with various types of materials (polymers, electronic components etc), long narrow channels, right angle and heat-sensitive components and various accessories (e.g., values) that can be contaminated with high levels of microbial bioburden and patient tissues after use. Contaminated flexible duodenoscopes have been a source of several significant infection outbreaks, where at least 9 reported cases were caused by multidrug resistant organisms [MDROs] with no obvious breach in processing detected. Despite this, there is evidence of the lack of attention to cleaning and maintenance of these devices and associated equipment. Over the last few decades there is increasing genomic evidence of innate and adaptive resistance to chemical disinfectant methods along with adaptive tolerance to environmental stresses. To reduce these risks, it has been proposed to elevate classification of higher-risk flexible endoscopes (such as duodenoscopes) from semi-critical [contact with mucous membrane and intact skin] to critical use [contact with sterile tissue and blood] that entails a transition to using low-temperature sterilization modalities instead of routinely using high-level disinfection; thus, increasing the margin of safety for endoscope processing. This timely review addresses important issues surrounding use of the Spaulding classification system to meet modern-day needs. It specifically addresses the need for automated, robust cleaning and drying methods combined with using real-time monitoring of device processing. There is a need to understand entire end-to-end processing of devices instead of adopting silo approaches that in the future will be informed by artificial intelligence and deep-learning/machine learning. For example, combinational solutions that address the formation of complex biofilms that harbour pathogenic and opportunistic microorganisms on the surfaces of processed devices. Emerging trends are addressed including future sustainability for the medical devices sector that can be enabled via a new Quintuple Helix Hub approach that combines academia, industry, healthcare, regulators, and society to unlock real world solutions.

Cost-effective Analysis of Sterile Product Packaging

Abstract:Objective We compared the cost component of different kinds of packaging materials in order to provide a suitable way to reduce the cost of medical treatment.Methods Cost component of 3 different kinds of packaging materials (traditional cotton, import pulp mold plastic and domestic pulp mold plastic) were analyzed.Results Compared with cotton,the cost of plastic material reduced by 35.2% to 56.8%,and domestic pulp mold plastic material reduced by 56.8%.Conclusions All of the materials didn't reduce the sterilization effect,and pulpmold plastic could save more money.

Abstract:The design features of medical electronic devices are considered. It is shown that the development of medical electronics is not an easy task and the designer must be prepared to face problems and regulations that are more strict than for household appliances, due to the fact that the failure of these devices to work properly can lead to tragic consequences, up to death. A number of difficulties that arise in the process of designing medical equipment is shown, which is presented in the form of a hierarchy of decisions. The main standards for designing devices of such type are given, among which there are IEC-60601-1-1 and IEC-60601-1-2 that deserve special attention, since compliance with their requirements is crucial for certification and release to the market of manufactured equipment. The use of a risk management matrix is proposed, which is used to provide quantitative indicators for each possible risk associated with a medical device in any mode of operation and possible malfunction.

Abstract:Biomaterials used as implants or as medical devices come into human contact and must first be sterilized and maintained sterile with certainty until usage to ensure success. Sterility of materials is usually assured by less than one in one million devices having a microbial presence. Sterilization of biomaterials is most commonly achieved using radiation, chemical or thermal methods. The use of ionizing radiation and ethylene oxide offer advantages for scale-up, and are typically preferred as methods for industrial level sterilization. Autoclave sterilization and chemical surface treatments are most commonly used for disinfection of glass and metallic devices. While patient safety should be the primary consideration in the choice of appropriate methods to ensure sterility, attention should also be paid to the impact of the sterilization method used on the biomaterial and any potential changes in functional performance.

Since formal classifications for medical device sterilization and disinfection were established in the 1970's, microbial adaptation under adverse environmental conditions has evolved rapidly. MDR microbial biofilms that adhere to medical devices and recurrently infect patients pose a significant threat in hospitals. Therefore, it is essential to mitigate the risk associated with MDR outbreaks by establishing novel recommendations for medical device sterilization, in a world of MDR. MDR pathogens typically thrive on devices with flexible accessories, which are easily contaminated with biofilms due to previous patient use and faulty sterilization or reprocessing procedures. To prevent danger to immunocompromised individuals, there is a need to regulate the classification of reprocessed medical device sterilization. This article aims to assess the risks of improper sterilization of medical devices in the era of MDR when sterilization procedures for critical medical devices are not followed to standard. Further, we discuss key regulatory recommendations for consistent sterilization of critical medical devices in contrast to the risks of disinfection reusable medical devices.
