Sedation with midazolam and fentanyl in patients undergoing brachial plexux block

Abstract:Objective To study the availability of co-administration of midazolam and fentanyl for sedation and analgesia during brachial plexux block.Methods One hundred patients scheduled for unilateral upper limbs surgery were randomized into control and drug-treated group with 50 cases each.Brachial plexux nerve was located with a stimulator.The control group did not receive any drug before puncture.In drug-treated group,fentanyl(1μg/kg) was intravenously administered as a bolus and midazolam was administered intermittently(1mg/3 min) until the sedation of patient reached to Ramsay III.Blood pressure,heart rate,percutaneous oxygen saturation(SpO_2),degree of sedation and effect of block were measured.The amnesia and satisfaction of patients were also investigated.(Results)The VAS pain scores of brachial plexux block in control group were significantly higher than those in drug-treated group(3.6±0.5 vs.0.4±0.2,P<0.05).The increase in heart rate and blood pressure had been found in control group during 0-5min after puncture.While a long(sedation) of Ramsay 3-5 lasting(58.2±8.6)min was produced by fentanyl and midazolam(0.03-0.06mg/kg).The heart rate and blood pressure were not changed significantly and minor supplement of sedative and(analgesic) drugs was used during operation in drug-treated group.Moreover,when compared with(control) group,the drug-treated patients had better satisfaction and amnesia on pricking or surgery.(Conclusion)Intravenous midazolam and fentanyl could produce sedation and analgesia in patients(undergoing) peripheral nerve block and elevate the efficacy of never block.
