Ultrasound guided infraclavicular brachial plexus block

Abstract:Objective To evaluate clinic effectiveness for infraclavicular brachial plexus block guided by sonography. Methods Infraclavicular brachial plexus block was performed via coracoid approach in 33 patients. The three cords of brachial plexus locating periphery axillary artery, and the needle at all times during advancement were observed by ultrasound, then local anaesthetic was carried out around each of three cords. The onset time and anaesthetic effectiveness was recorded. Results Infraclavicular brachial plexus were successfully identified in the transverse view as oval hypoechoic structures with small internal punctuate echoes. After injection,the nerves were infiltration by local anesthetic. In 33 patients, the successful blockage were 33(100) patients, including 31 patients complete block and 2 patients required to add fentanyl via vein. Mean operation time,onset time and complete block time was ( 3.81 ± 0.28 )min, ( 4.39 ± 0.11 )min, and ( 369.38 ± 20.64 ) min,respectively. Twenty minutes after injection, the complete blockade rate of all 7 nerves reached 100. Conclusions The use of ultrasound improves the success rate and decreases the complications of brachial plexus block.
