Effect of Different Positive End-expiratory Pressure on Cardiac Preload and Predictability of Different Cardiac Preload Indexes on Patients Treated with Mechanical Ventilation

Abstract:Objective To investigate the effect on the hemodynamics and heart function when different posi- tive end-expiratory pressure (PEEP) levels were used on patients treated with mechanical ventilation(MV) and campare the predictability of central venous pressure(CVP), stroke volume variation (SVV) and pulse pressure variation (PPV) to the changes of cardiac preload by the different PEEP in critically ill patients with MV.Methods Twelve critical patients were enrolled in this study with MV in intensive care unit(ICU) from July 2006 to July 2007. The changes in heart rate (HR),mean arterial boold pressure (MAP),systemic vascular resistance index (SVRI), cardiac index (CI),CVP,SVV,PPV,peak airway pressure (Ppeak),mean of airway pressure (Pmean) were deter- mined with different randomly selected PEEE levels of 0,5,10,15mmHg under volume contral ventilation with VT=10mL/kg,and also to maintain normocapnia (PCO2 35~45mmHg) throughout the study.Results There were no significant changes in SVV,PPV and other values except Pmean between PEEP0 and PEEP5 during mechanical ventilation with VT levels of 10mL/kg,but when applying higher PEEP led to a significant increase on HR,SVRI, CVP,SVV,PPV,Ppeak and Pmean,while CI decreased significantly and MAP had no changes.Linear regression analysis between changes of CI and SVV induced by high PEEP showed a significant correlation(r=-0.887 at PEEP10,r=-0.721 at PEEP15).And the changes of CI and PPV induced by high PEEP also showed the significant correlation(r=-0.734 at PEEP10,r=-0.935 at PEEP15),all P-value 0.01.Conclusion Cardiac preload can be de- creased significantly by the high PEEP and SVV and PPV can be the useful marker of the changes in cardiac output induced by high PEEP.
