Comparison in Three Different Kinds of Retroperitoneoscopic Nephroureterectomy with Bladder Cuff(Report of 110 Cases)

Abstract:Objective:To select the optimal operation method for different patients,we compared three different kinds of retroperitoneoscopic nephroureterectomy.Methods:Three kinds of retroperitoneoscopic nephroureterectomy were performed in 110 cases.32 cases were underwent retroperitoneoscopic nephroureterectomy+cut of the ureteral orifice with bladder cuff by hypogastric incision(operation method A).Retroperitoneoscopic nephroureterectomy+transurethral resection of the ureteral orifice with bladder cuff(operation method B) was performed in 19 cases.Transurethral resection of the ureteral orifice with bladder cuff + retroperitoneoscopic nephroureterectomy(operation method C) was performed in 59 cases.Results:The operation was successfully completed in all the 110 cases.Operative time,blood loss and hospital stay time of three operation methods were not significantly.3 patients died in perioperative period.58 patients were followed till now,mean follow-up time was 38.3 months.During follow up period,4 patients died of cancer metastasis,1 patient died of pneumothorax and 1 patient died of cerebrovascular disease.The difference of early complication rate,contralateral affection and bladder tumor recurrence rate among three operation methods were not significant.But the rate of death and metastasis in operation method C were less.Conclusions:Carcinoma of renal pelvis or ureter complicate with bladder cancer should select transurethral resection of the ureteral orifice with bladder cuff + retroperitoneoscopic nephroureterectomy. Only carcinoma of renal pelvis or ureter may choose underwent retroperitoneoscopic nephroureterectomy+ cut of the ureteral orifice with bladder cuff by hypogastric incision.
