Abstract:Background Lung retransplantation is offered to select patients with chronic allograft dysfunction. Given the increased risk of morbidity and mortality conferred by retransplantation, post‐transplant function should be considered in the decision of who and when to list. The aim of this study is to identify predictors of post‐operative disability in patients undergoing lung retransplantation. Methods Data were collected from the UNOS national dataset and included all patients who underwent lung retransplant from May 2005–March 2023. Pre‐ and post‐operative function was reported by the Karnofsky Performance Status (KPS) and patients were stratified based on their needs. Cumulative link mixed effects models identified associations between pre‐transplant variables and post‐transplant function. Results A total of 1275 lung retransplant patients were included. After adjusting for between‐group differences, pre‐operative functional status was predictive of post‐transplant function; patients requiring Total Assistance ( n = 740) were 74% more likely than No/Some Assistance patients (n = 535) to require more assistance in follow‐up (OR 1.74, 95% CI 1.13–2.68, p = .012). Estimated one year survival of Total Assistance patients is lower than No/Some Assistance Recipients (72% vs. 82%, CI 69%–75%; 79%–86%) but similar to overall re‐transplant survival (76%, CI 74%–79%). Conclusion Both survival and regain of function in patients requiring Total Assistance prior to retransplant may be higher than previously reported. Pre‐operative functional status is predictive of post‐operative function and should weigh in the selection, timing and post‐operative care of patients considered for lung retransplantation.

Body mass index percentage and survival in pediatric patients listed for lung transplantation: A modern-era multi-institutional analysis

Abstract:Background: Prior studies suggest that being underweight by body mass index percentiles (BMI%) or thinness grade did not affect post-transplant survival in pediatric lung transplant (LTx) recipients regardless of cystic fibrosis (CF) or non-CF diagnosis. Graft and overall survival from the time of listing was instead evaluated based on listing BMI%, the current standard of practice for BMI definitions in pediatrics, to ascertain the impact of a "severely low" subcategory. Methods: The UNOS registry was queried for children listed for LTx (aged 2 to <18 years) from January 1986 to March 2020. BMI% at listing and transplant were calculated per CDC guidelines according to age in years, sex, and reported BMI%. Patients were divided by listing BMI%: severely low (<3rd), low (3-<5th), normal (5-<85th), overweight (85-<95th), and obese (≥95th). Kaplan-Meier curves were generated to assess differences in overall survival since listing based on BMI% classes. Cox proportional-hazards models were developed to assess risk factors for overall and graft survival, including listing BMI%, transplant listing era (≥2005), and listing age, by reporting hazard ratios (HR). Results: Listing BMI% was calculable for 1,876 patients. The proportion of patients with CF differed significantly between BMI% groups (p < 0.001). Patients listed with a non-CF diagnosis comprised 34% of those in the severely low category, and 88% of those listed with an obese BMI%. Compared to patients with a normal listing BMI%, the cohort with severely low BMI% had worse overall survival regardless of LTx (p = 0.009) and graft survival (p = 0.034). Compared to patients with a low BMI%, those with a severely low BMI% had significantly poorer graft survival as well (p = 0.040). Mean graft survival was not significantly different between groups that remained at listing BMI% vs those that improved in category despite an overall small sample size. Independent predictors of poorer survival from the time of listing include severely low vs low-normal BMI% (HR = 1.20) and listing age (HR = 1.02). Conclusion: The proportion of children listed at severely low BMI% has steadily decreased with time, yet pediatric LTx candidates listed with a severely low BMI% had poorer graft and overall survival compared to those of normal BMI%. Severely low listing BMI% was an independent prognostic factor for higher mortality risk from the time of placement on the waitlist. BMI% may be a modifiable target for improving survival regardless of transplantation.

Abstract:A sensitive and precise assay of guanase was based on the conversion of 14C-guanine to uric acid in the presence of excess xanthine oxidase. The enzyme was entirely soluble in rat tissues and no inhibitors of it were detected. The most active tissues were red blood cells, lung and lactating mammary gland, with more than twice the activity of liver. The contained blood could account for the low activity in adult skeletal muscle and some other tissues. All fetal tissues examined were without activity. Activity in mammary gland rose fourfold during lactation and dropped precipitously during involution, with a secondary rise associated in time with the loss of cells from the gland. Reticulocytes present normally and after hemorrhagic anemia appeared to account for substantially all of the high guanase activity in red blood cells. The guanase level could be used to predict the degree of reticulocytosis in rats within confidence limits of +/- 0.2%. The virtual absence of guanase in human red cells was confirmed, even in bloods containing elevated reticulocyte numbers.
Pilot study of dose fractionation in carcinoma of the base of the tongue: uninterrupted vs. split-course irradiation.

Abstract:A prospective study of 4 fractionation regimes for advanced carcinoma of the base of the tongue is presented. The treatment categories were: uninterrupted irradiation with daily fractions of 170 r up to 6,ooo r; uninterrupted irradiation with 3 fractions per week of 285 r up to 6,ooo r; a split-course technique with daily fractions of 300 r up to 3,000 r, followed by a rest period of approximately 3 weeks, then a repetition of the initial irradiation; and a similar split-course technique with 3 fractions per week of 500 r each.Normal tissue reactions, completion of therapy, tumor response, and 2 year disease free survival are discussed.The results of this study favor the split-course technique with daily fractions of 300 r; however, the limited number of cases involved does not permit a valid conclusion.The need exists for a larger scale, properly planned study to compare the split-course technique with the usual conventional uninterrupted therapy for various cancer sites.

Abstract:Background Pediatric lung transplant (LT) outcomes correlate with prolonged length of stay (pLOS), yet a paucity of research exists predicting pLOS in this population. We aim to identify factors associated with pLOS following pediatric LT, and to create a risk score identifying individual patients at increased risk of pLOS. Methods Using the OPTN database, we analyzed 733 pediatric patients who received an LT between January 2000‐July 2022. We used LASSO regularization to identify factors predicting pLOS. A risk score was calculated using odds ratios for each variable in the model. Results LASSO was run on 51 factors and identified 13 to be included in the model. The variables with the highest negative impact on LOS were recipient ethnicity (Asian, Native American, Pacific Islanders, or Mixed Race) (OR = 3.187), recipient requiring life support (OR = 2.354), and recipient age 2–10 years old (OR = 2.203). In contrast, low cold ischemia time (OR = 0.562), time on waitlist 21–60 days (OR = 0.284), and diagnosis of primary pulmonary hypertension (OR = 0.307) had protective effects on LOS. Using the risk score, we stratified patients into three equal groups: low (mean LOS = 20.60 days), medium (mean LOS = 22.53 days), and high risk (mean LOS = 36.72 days) of pLOS. The c‐statistic for the model was 0.7931 and 0.7757 for the risk score. Conclusions Using this model and risk score, physicians and healthcare systems could identify patients at risk of pLOS and could intervene on preventable variables before transplantation.

Abstract:Histochemical staining for sulphhydryl (-SH) groups and disulphide (S-S) bonds was undertaken in 10 cases of cutaneous amyloidosis using the new fluorescent thiol reagent. A weak S-S fluorescence (but not -SH) was detected in dermal amyloid, suggesting that cross-linking of -SH groups may occur in amyloidogenesis. Since in normal skin S-S bonds were stained only in the horny layer and negligibly in the dermis, our findings seem to support the view that amyloid is derived at least partially from epidermal cells.

Abstract:Pediatric HT recipients with decreased pre‐ and post‐transplant functional status are at higher risk for graft failure and mortality. These patients may benefit from early intervention aimed at improving functional status. Background Pretransplant functional, motor, cognitive, and academic deficits are common in pediatric patients requiring heart transplantation (HT); some persist post‐HT. We assessed the association between these quality of life (QoL) deficits and post‐HT outcomes. Methods Using SRTR data 2008–2023, we evaluated the functional, motor, cognitive, and academic status of pediatric HT recipients from listing to 15 years post‐HT. We compared all‐cause graft survival among patients with vs. without pre‐HT deficits using Cox regressions. Among patients with a functioning graft at 1 year, we assessed the association between deficits at that time and subsequent graft failure. Results In 6153 HT recipients, 85.3% had pre‐HT functional status ≤ 80%, 53.1% of patients could not tolerate a full academic load; this decreased to 15.2% and 15.0% postoperatively, and was sustained. Definite or probable cognitive deficits were seen in 17.4% of patients and motor delays in 18.6%; both remained stable post‐HT at 18.0% and 16.4%, respectively. Graft survival was worse in those with either pre‐ or post‐HT deficits in functional status. Abstract:Background/Aims:Liver transplantation is the most effective treatment for the sickest patients with acute-on-chronic liver failure (ACLF). However, the influence of donor age on liver transplantation, especially in ACLF patients, is still unclear. Methods:In this study, we used the data of the Scientific Registry of Transplant Recipients. We included patients with ACLF who received liver transplantation from January 1, 2007, to December 31, 2017, and the total number was 13,857. We allocated the ACLF recipients by age into group I (donor age ≤17 years, n=647); group II (donor age 18-59 years, n=11,423); and group III (donor age ≥60 years, n=1,787). Overall survival (OS), graft survival, and mortality were compared among the three age groups and the four ACLF grades. Cox regression was also analyzed. Results:The 1-, 3-, and 5-year OS rates were 89.6%, 85.5%, and 82.0% in group I; 89.4%, 83.4%, and 78.2% in group II; and 86.8%, 78.4%, and 71.4% in group III, respectively (p<0.001). When we analyzed the different effects of donor age on OS with different ACLF grades, in groups II and III, we observed statistical differences. Finally, the cubic spline curve told us that the relative death rate changed linearly with increasing donor age. Conclusions:Donor age is related to OS and graft survival of ACLF patients after transplantation, and poorer results were associated with elderly donors. In addition, different donor ages have different effects on recipients with different ACLF grades.
Age-related survival disparity associated with lung transplantation in cystic fibrosis: An analysis of the registry of the International Society for Heart and Lung Transplantation

Abstract:Background: A survival advantage has been demonstrated for lung transplantation (LTx) in the cystic fibrosis (CF) population, but children may be at higher risk of post-transplant mortality than adults. Methods: The registry of the International Society for Heart and Lung Transplantation (ISHLT) was queried for lung transplants performed during the period 1998 to 2014 in patients with CF. Period differences were assessed by dividing the sample into LTxs performed between 2006 and 2014 and those performed between 1998 and 2005. Age ≥18 years distinguished adult from pediatric recipients. Supplemental analyses of United States of America (USA) LTx recipients with CF utilized 1998 to 2014 data from the United Network for Organ Sharing (UNOS) registry. Results: A total of 7,245 patients from the ISHLT registry were selected for analysis. Kaplan-Meier curves confirmed a survival disadvantage of children as compared to adults with CF in 2006 to 2014 (p<0.001), and in the earlier era of 1998 to 2005 (p = 0.002). Univariate Cox proportional hazards regression revealed that age <18 years at transplantation [hazard ratio (HR) = 1.367; 95% confidence interval (CI) 1.225 to 1.526; p < 0.001] and LTxs performed in 1998 to 2005 (HR = 1.131; 95% CI 1.042 to 1.227; p = 0.003) were associated with greater mortality hazard. Multivariable Cox regression adjusting for potential confounders and interacting LTx era with recipient age group confirmed the persistence of age-related survival disparities after LTx in CF in the contemporary era. Analysis of UNOS data demonstrated survival disparities between children and adult CF patients receiving LTx in majority-adult programs. Conclusions: An age-related survival disparity after LTx has persisted in the international CF population. Data from the USA suggest this disparity is not fully explained by differences in center volume between adult and pediatric programs in the contemporary era.
Outcomes of adolescent recipients after lung transplantation: An analysis of the International Society for Heart and Lung Transplantation Registry

Abstract:Background: Recipient adolescent age for non-lung solid-organ transplantation is associated with higher rates of rejection, graft loss and mortality. Although there have been no studies specifically examining adolescent outcomes after lung transplantation (LTx), limited data from the International Society of Heart and Lung Transplantation (ISHLT) Registry suggest that a similar association may exist. Recently, adolescence has been defined as 10 to 24 years of age, taking into account the biologic and sociologic transitions that occur during this age interval. Methods: The ISHLT Registry was used to examine the survival outcomes of LTx recipients 10 to 24 years of age between 2005 and 2013. Given the developmental changes that occur in adolescence, survival outcomes for the tertiles of adolescence (10 to 14, 15 to 19 and 20 to 24 years old) were also examined. Results: Adolescents made up 9% (n = 2,319) of the 24,730 LTxs undertaken during the study period. Kaplan-Meier survival estimates at 3 years showed lower adolescent survival (65%) when compared with younger children (73%, p = 0.006) and adults 25 to 34 (75%, p < 0.00001) and 35 to 49 (71%, p < 0.00001) years of age, without a significant survival difference compared with those 50 to 65 years old. Critically, 15- to 19-year-old recipients had the poorest outcomes, with reduced 1-year survival (82%) compared with those 10 to 14 years old (88%, p = 0.02), and reduced 3-year survival (59%) compared with those 10 to 14 (73%, p < 0.00001) and 20 to 24 (66%, p < 0.0001) years old. Conclusions: Adolescent LTx recipients have poorer overall survival when compared with younger children and adults, with those 15 to 19 years old having the highest risk of death. This survival disparity among age groups likely reflects the difficult period of adolescence and its biologic and social transitions, which may influence both immunologic function and adherence.

Improvements in Patient-Reported Functioning after Lung Transplant is Associated with Improved Quality of Life and Survival

Abstract:Lung transplantation aims to improve health–related quality of life (HRQL) and survival. While lung function improvements are associated with these outcomes, the association between physical functioning and these outcomes is less clear. We investigated the association between changes in patient-reported physical functioning and HRQL, chronic lung allograft dysfunction (CLAD), and survival after lung transplantation. This single–center prospective cohort study analyzed 220 lung transplant recipients who completed the 15–item Lung Transplant Valued Life Activities (LT–VLA) before and repeatedly after transplant. HRQL was assessed using generic, respiratory disease–specific, and utility measures. Associations between 0.3–point changes (the minimally important difference) in LT–VLA as a time–varying predictor on HRQL, CLAD, and mortality were tested using linear regression and Cox proportional hazard models. Models were adjusted for demographics, disease diagnosis, and post–operative lung function as a time–varying covariate. Participants were 45% female and 75% White, with a mean age of 56 (±12) years. Each 0.3–point improvement in LT–VLA was associated with substantially improved HRQL across all measures (adjusted p–values <0.01). Each 0.3–point improvement in LT–VLA was associated with a 13% reduced hazard of CLAD (adjusted HR: 0.87, 95% CI: 0.76–0.99, p=0.03) and a 19% reduced hazard of mortality (adjusted HR: 0.81, 95% CI: 0.67–0.95, p=0.01). Improvements in patient–reported physical functioning after lung transplantation are associated with improved HRQL and reduced risk of CLAD and death, independent of allograft function. The simplicity of the LT–VLA suggests it could be a valuable monitoring or outcome measure in both clinical and research settings.

Contemporary Pediatric Heart Transplant Waitlist Mortality

Abstract:Background: In 2016, the United Network for Organ Sharing revised its pediatric heart transplant (HT) allocation policy. Objectives: This study sought to determine whether the 2016 revisions are associated with reduced waitlist mortality and capture patient-specific risks. Methods: Children listed for HT from 1999 to 2023 were identified using Organ Procurement and Transplantation Network data and grouped into 3 eras (era 1: 1999-2006; era 2: 2006-2016; era 3: 2016-2023) based on when the United Network for Organ Sharing implemented allocation changes. Fine-Gray competing risks modeling was used to identify factors associated with death or delisting for deterioration. Fixed-effects analysis was used to determine whether allocation changes were associated with mortality. Results: Waitlist mortality declined 8 percentage points (PP) across eras (21%, 17%, and 13%, respectively; P < 0.01). At listing, era 3 children were less sick than era 1 children, with 6 PP less ECMO use (P < 0.01), 11 PP less ventilator use (P < 0.01), and 1 PP less dialysis use (P < 0.01). Ventricular assist device (VAD) use was 13 PP higher, and VAD mortality decreased 9 PP (P < 0.01). Non-White mortality declined 10 PP (P < 0.01). ABO-incompatible listings increased 27 PP, and blood group O infant mortality decreased 13 PP (P < 0.01). In multivariable analyses, the 2016 revisions were not associated with lower waitlist mortality, whereas VAD use (in era 3), ABO-incompatible transplant, improved patient selection, and narrowing racial disparities were. Match-run analyses demonstrated poor correlation between individual waitlist mortality risk and the match-run order. Conclusions: The 2016 allocation revisions were not independently associated with the decline in pediatric HT waitlist mortality. The 3-tier classification system fails to adequately capture patient-specific risks. A more flexible allocation system that accurately reflects patient-specific risks and considers transplant benefit is urgently needed.

Abstract:13 Background: The use of protein biomarkers for serum-based detection of invasive breast cancer has been problematic due in large part to intrinsic variability in the population as a whole. The Provista Biomarker Assay (PBA) has been shown previously to be highly sensitive across a wide spectrum of clinical signals. The detection of autoantibodies (AAb) holds great promise due to the inherent specificity these biomarkers provide. However, as a clinical-quality diagnostic, it is necessary to utilize a panel comprised of multiple autoantibodies, as individual sensitivity is general low. Our hypothesis was that combining serum protein and autoantibodies together in a panel would result in an increased ability to correctly identify breast cancer. METHODS To maximize clinical performance (sensitivity and specificity), we utilized the Meso Scale Discovery ELISA-based system to measure AAbs from patient sera. We analyzed 134 prospectively collected patient serum samples for a variety of autoantibody targets. These data, together with serum protein biomarker concentrations, produced an analytically robust test that is also highly specific. RESULTS Consistent with our hypothesis, the inclusion of autoantibodies with serum proteins in the Provista Biomarker Assay improved test specificity above that observed in serum protein biomarkers alone. Individual protein targets had limited ability to differentiate benign conditions from invasive breast cancer. However, factoring in autoantibody values resulted in improved overall accuracy to differentiate serum from benign and invasive breast cancer patients. CONCLUSIONS In the novel approach of combining these two different classes of analytes, we exploit the specificity of AAbs while maintaining high sensitivity through the inclusion of more generalized cancer-associated protein biomarkers. The combination of these two approaches clearly offers unique performance benefits in the early detection of invasive breast cancer that is greater than either component alone.
Validation of a novel donor lung scoring system based on the updated lung Composite Allocation Score

Abstract:Lung transplantation (LTx) continues to have lower rates of long-term graft survival compared with other organs. Additionally, lung utilization rates from brain-dead donors remain substantially lower compared with other solid organs, despite a growing need for LTx and the significant risk of waitlist mortality. This study aims to examine the effects of using a combination of the recently described novel lung donor (LUNDON) acceptability score and the newly adopted recipient lung Composite Allocation Score (CAS) to guide transplantation. We performed a review of nearly 18 000 adult primary lung transplants from 2015-2022 across the US with retroactive calculations of the CAS value. The medium-CAS group (29.6-34.5) had superior 1-year posttransplant survival. Importantly, the combination of high-CAS (> 34.5) recipients with low LUNDON score (≤ 40) donors had the worst survival at 1 year compared with any other combination. Additionally, we constructed a model that predicts 1-year and 3-year survival using the LUNDON acceptability score and CAS values. These results suggest that caution should be exercised when using marginally acceptable donor lungs in high-priority recipients. The use of the LUNDON score with CAS value can potentially guide clinical decision-making for optimal donor-recipient matches for LTx.
The effect of low aluminum water and desferrioxamine on the outcome of dialysis encephalopathy.

Abstract:Five patients with dialysis encephalopathy were transferred to deionized water; all died after a variable period, 4 of progressive dementia. Six other patients with encephalopathy were started either on deionized or reverse osmosis water as well as desferrioxamine on dialysis. Serial EEG's were performed in this group. In 2 patients the EEG's showed rapid and sustained improvement after the institution of desferrioxamine, whereas there had been no EEG change after 15 and 32 months of reverse osmosis water. In 2 patients there was similar clinical and EEG improvement, related to water change and the institution of desferrioxamine. Two patients died of progressive dementia in spite of both water change and desferrioxamine. Thus, while not a panacea, desferrioxamine has a definitive therapeutic effect on the clinical and EEG manifestations of dialysis encephalopathy, when used in conjunction with deionized or reverse osmosis water.

Abstract:OBJECTIVES: Health-related quality of life has been well studied across transplantation fields, but factors associated with lung transplant preoperative and postoperative quality of life remain unknown. Here, we determine factors associated with health-related quality of life in lung transplant candidates and recipients to identify patients at risk of lower health-related quality of life.MATERIALS AND METHODS: From January 2021 to May 2022, health-related quality of life was measured in candidates and recipients using the RAND 36-Item Short Form Health Survey questionnaire. We reviewed demographic parameters and clinical information and scored frailty according to the modified 5-item frailty index. We performed Fisher exact test and the Pearson chi-square test and used linear regression models to determine covariate associations on physical component summary, mental component summary, and self-reported health scores (α = 0.05).RESULTS: Eleven candidates and 17 recipients comp-leted the survey. Compared with candidates, transplant recipients reported significantly higher scores in 4 of the 8 health domains and in the physical component summary (P < .01), mental component summary (P = .05), and self-reported health score (P < .01). In candidates, higher body mass index and higher modified 5-item frailty index scores were negatively associated with the physical component summary and mental component summary, respec-tively (P < .05). In recipients, higher body mass index and higher lung allocation scores were associated with lower values for the physical component summary (-2.29; P < .05) and self-reported health score (-0.33; P < .05), respectively.CONCLUSIONS: Body mass index, the modified 5-item frailty index, and the lung allocation score were significantly associated with health-related quality of life in lung transplant recipients. Future interventions should target these modifiable associations to maximize candidate and recipient health-related quality of life.

Abstract:Objectives The Starzl Network for Excellence in Pediatric Transplantation identified optimizing immunosuppression (IS) as a priority practice improvement area for patients, families, and providers. We aimed to evaluate associations between clinical characteristics, early IS, and outcomes. Methods We analyzed pediatric liver transplant (LT) data from 2013 to 2018 in the United Network for Organ Sharing (UNOS) and the Society of Pediatric Liver Transplantation (SPLIT) registries. Results We included 2542 LT recipients in UNOS and 1590 in SPLIT. IS choice varied between centers with steroid induction and mycophenolate mofetil (MMF) use each ranging from 0% to 100% across centers. Clinical characteristics associated with early IS choice were inconsistent between the two data sets. T‐cell depleting antibody use was associated with improved 1‐year graft (hazard ratio [HR] 0.50, 95% confidence interval [CI] 0.34−0.76) and patient (HR 0.40, 95% CI 0.20−0.79) survival in UNOS but decreased 1‐year patient survival (HR 4.12, 95% CI 1.31−12.93) and increased acute rejection (HR 1.58, 95% CI 1.07−2.34) in SPLIT. Non‐T‐cell depleting antibody use was not associated with differential risk of survival nor rejection. MMF use was associated with improved 1‐year graft survival (HR 0.73, 95% CI 0.54−0.99) in UNOS only. Conclusions Variation exists in center choice of early IS regimen. UNOS and SPLIT data provide conflicting associations between IS and outcomes in multivariable analysis. These results highlight the need for future multicenter collaborative work to identify evidence‐based IS best practices.
