Abstract:In this contribution we investigate the problem of wind disturbance and aerodynamic parameter estimation of a Gun-Launched Micro Air Vehicle (GLMAV) which is a new Micro Air Vehicle (MAV) concept, intended for outdoor flights, and using two-bladed coaxial contra-rotating rotors. A comprehensive GLMAV modeling is presented for hover and nearhover flight conditions and identified from experimental results. Hence we use a recent extension of the Extended Kalman Filter (EKF) with Sliding Window (EKFSW) to mainly improve the wind-disturbance estimation from very noisy accelerometer signals.

Abstract:Current spinal testing protocols generally adopt pure moments combined with axial compression. However, daily activities involve multi-axis loads, and multi-axis loading has been shown to impact intervertebral disc (IVD) cell viability. Therefore, integrating in-vivo load data with spine simulators is critical to understand how loading affects the IVD, but doing so is challenging due to load coupling and variable load rates. This study addresses these challenges through the Load Informed Kinematic Evaluation (LIKE) protocol, which was evaluated using the root mean squared error (RMSE) between desired and actual loads in each axis. Stage 1 involves obtaining the kinematics from six-axis load control tests replicating 20 Orthoload activities at a reduced test speed. Stage 2 applies these kinematics in five axes, with axial compression applied in load control, at the reduced speed and at the physiological test rate. Stage 3 enables long-term tests through six-axis kinematic control combined with diurnal height correction to account for the natural height fluctuations of the IVD. Stage 1 yielded RMSEs within twice the load cell noise floor. Low RMSEs were maintained during stage 2 at reduced speed (Tx:0.80 ± 0.30 N; Ty:0.77 ± 0.29 N; Tz:1.79 ± 0.50 N; Rx:0.02 ± 0.01Nm; Ry:0.02 ± 0.01Nm; and Rz:0.02 ± 0.01Nm) and at the physiological test rate (Tx:3.45 ± 1.81 N; Ty:3.82 ± 1.99 N; Tz:11.32 ± 8.69 N; Rx:0.13 ± 0.07Nm; Ry:0.16 ± 0.11Nm; and Rz:0.07 ± 0.04Nm). To address unwanted oscillations observed in longer tests (>2h), Stage 3 was introduced to enable the stable and consistent replication of activities at a physiological test rate. Despite higher RMSEs the axial error was 85.5 ± 24.27 N (equivalent to ∼ 0.16 MPa), with shear RMSEs similar to other testing systems conducting pure moment tests at slower rates. The LIKE protocol enables the replication of physiological loads, providing opportunities for enhanced investigations of IVD mechanobiology, and the pre-clinical evaluation of IVD devices and therapies.

Abstract:Individuals with transfemoral amputation experience relative motion between their residual limb and prosthetic socket, which can cause inefficient dynamic load transmission and secondary comorbidities that limit mobility. Accurately measuring the relative position and orientation of the residual limb relative to the prosthetic socket during dynamic activities can provide great insight into the complex mechanics of the socket/limb interface. Five participants with transfemoral amputation were recruited for this study. All participants had a well-fitting, ischial containment socket and were also fit with a compression/release stabilization socket. Participants underwent an 8-wk, randomized crossover trial to compare differences between socket types. Dynamic stereo x-ray was used to quantify three-dimensional residual bone kinematics relative to the prosthetic socket during treadmill walking at self-selected speed. Comfort, satisfaction, and utility were also assessed. There were no significant differences in relative femur kinematics between socket types in the three rotational degrees of freedom, as well as anterior-posterior and medial-lateral translation ( p > 0.05). The ischial containment socket demonstrated significantly less proximal-distal translation (pistoning) of the femur compared to the compression/release stabilization socket during the gait cycle ( p < 0.05), suggesting that the compression/release stabilization socket provided less control of the residual femur during distal translation. No significant differences in comfort and utility were found between socket types ( p > 0.05). The quantitative, dynamic analytical tools used in the study were sensitive to distinguish differences in three-dimensional residual femur motion between two socket types, which can serve as a platform for future comparative effectiveness studies of socket technology.

Abstract:Abstract: Introduction: The lack of knowledge of mechanical loads in orthotic joints can lead to oversized or breaking components. Previous studies suffer from small sample size and technical limitations. The goal of this study was to develop and validate a method that allows the direct measurement of moments in sagittal, frontal and transverse planes in knee and ankle joints of existing custom made orthoses. Methods: We developed a modular measurement system based on standard joint components, which were instrumented with strain gauges. To ensure sufficient signals and reduce cross talk an iterative approach based on FEM simulation was utilized. The system also contains inertial sensors for mobile gait analysis. Results: Instrumented joints show good results regarding linearity, hysteresis and cross talk. First pilot trials with post-polio and ICP patients demonstrated that joint loads depend on several factors and not solely on body weight. If combined with conventional gait analysis, measurement results can characterize the individual muscle situation of the patient. Conclusion: A novel method for obtaining data on loads in orthotic components was developed and validated. It provides the basis to develop safety testing standards and clinical guidelines, as well as allowing individual optimization of orthotic devices.

Abstract:Vertical loading rate could be associated with residuum and whole body injuries affecting individuals fitted with transtibial prostheses. The objective of this study was to outline one out of five automated methods of extraction of vertical loading rate that stacked up the best against manual detection, which is considered the gold standard during pseudo-prosthetic gait. The load applied on the long axis of the leg of three males was recorded using a transducer fitted between a prosthetic foot and physiotherapy boot while walking on a treadmill for circa 30 min. The automated method of extraction of vertical loading rate, combining the lowest absolute average and range of 95% CI difference compared to the manual method, was deemed the most accurate and precise. The average slope of the loading rate detected manually over 150 strides was 5.56 ± 1.33 kN/s, while the other slopes ranged from 4.43 ± 0.98 kN/s to 6.52 ± 1.64 kN/s depending on the automated detection method. An original method proposed here, relying on progressive loading gradient-based automated extraction, produced the closest results (6%) to manual selection. This work contributes to continuous efforts made by providers of prosthetic and rehabilitation care to generate evidence informing reflective clinical decision-making.

Abstract:Background: Residual limb discomfort and injury is a common experience for people living with lower limb amputation. Frequently, inadequate load distribution between the prosthetic device and the residual limb is the root cause of this issue. To advance our understanding of prosthetic interface fit, tools are needed to evaluate the mechanical interaction at the prosthetic interface, allowing interface designs to be evaluated and optimised. Objective: Present a methodology report designed to facilitate comprehension of the mechanical interaction between the prosthetic interface and the residual limb. As a pilot study, this methodology is used to compare a hands-on and hands-off interface for a single transtibial prosthesis user using secondary Magnetic Resonance Imaging (MRI) data. Methodology: MRI data of the residual limb while wearing a prosthetic interface is segmented into a hard tissue and a skin surface model. These models are exported as stereolithography (STL) files. Two methods are used to analyse the interface designs. Firstly, CloudCompare software is used to compute the nearest vertex on the skin surface for every vertex on the compiled internal bony surface for both interface types. Secondly, CloudCompare software is used to compare registered skin surfaces of the residual limb while wearing the hands-on and hands-off interfaces. Findings: The maximum and minimum nearest distances between the internal bony surface and skin surface were similar between interface types. However, the distribution of nearest distances was different. When comparing the skin surface while wearing both interfaces, where the fit is more compressive can be visualized. For the dataset used in this study, the classic features of a hands-on Patella Tendon Bearing interface and hands-off pressure cast interface could be identified. Conclusion: The methodology presented in this report may give researchers a further tool to better understand how interface designs affect the soft tissues of the residual limb.

Abstract:The effects of inertial loads on the interface stresses between trans-tibial residual limb and prosthetic socket were investigated. The motion of the limb and prosthesis was monitored using a Vicon motion analysis system and the ground reaction force was measured by a force platform. Equivalent loads at the knee joint during walking were calculated in two cases with and without consideration of the material inertia. A 3D nonlinear finite element (FE) model based on the actual geometry of residual limb, internal bones and socket liner was developed to study the mechanical interaction between socket and residual limb during walking. To simulate the friction/slip boundary conditions between the skin and liner, automated surface-to-surface contact was used. The prediction results indicated that interface pressure and shear stress had the similar double-peaked waveform shape in stance phase. The average difference in interface stresses between the two cases with and without consideration of inertial forces was 8.4% in stance phase and 20.1% in swing phase. The maximum difference during stance phase is up to 19%. This suggests that it is preferable to consider the material inertia effect in a fully dynamic FE model.

Abstract:Background: Anterior cervical discectomy and fusion (ACDF) sacrifices segmental mobility, which can lead to the acceleration of adjacent segment degeneration. The challenge has promoted cervical artificial disc replacement (CADR) as a substitute for ACDF. However, CADR has revealed a series of new issues that are not found in ACDF, such as hypermobility, subsidence, and wear phenomenon. This study designed a cervical subtotal discectomy prosthesis (CSDP) consisting of a cervical disc prosthesis structure (CDP structure), cervical vertebra fixation structure (CVF structure), link structure, and locking screw, aiming to facilitate motion control and reduce subsidence. The aim of this study was to assess the biomechanics of the CSDP using finite element (FE) analysis, friction-wear test, and non-human primates implantation study. Study Design: For the FE analysis, based on an intact FE C 2 -C 7 spinal model, a CSDP was implanted at C 5 -C 6 to establish the CSDP FE model and compare it with the Prestige LP prosthesis (Medtronic Sofamor Danek, Minneapolis, MN, United States). The range of motion (ROM), bone-implant interface stress, and facet joint force were calculated under flexion extension, lateral bending, and axial rotation. In addition, CSDP was elevated 1 mm to mimic an improper implantation technique to analyze the biomechanics of CSDP errors in the FE model. Moreover, the friction-wear test was conducted in vitro to research CSDP durability and observe surface wear morphology and total wear volume. Finally, the CSDP was implanted into non-human primates, and its properties were evaluated and verified by radiology. Results: In the FE analysis, the ROM of the CSDP FE model was close to that of the intact FE model in the operative and adjacent segments. In the operative segment, the CSDP error FE model increased ROM in flexion extension, lateral bending, and axial rotation. The maximum stress in the CSDP FE model was similar to that of the intact FE model and was located in the peripheral cortical bone region. The facet joint force changes were minimal in extension, lateral bending, and axial rotation loads in CSDP. In the friction-wear test, after the 150-W movement simulation, both the CVF-link-junction and the CDP-link-junction had slight wear. In the CSDP non-human primate implantation study, no subsidence, dislocation, or loosening was observed. Conclusion: In the FE analysis, the biomechanical parameters of the CSDP FE model were relatively close to those of the intact FE model when compared with the Prestige LP FE model. In terms of CSDP error FE models, we demonstrated that the implantation position influences CSDP performance, such as ROM, bone-implant interface stress, and facet joint force. In addition, we performed a friction-wear test on the CSDP to prove its durability. Finally, CSDP studies with non-human primates have shown that the CSDP is effective.

A novel posture control device to induce high-rate complex loads for spine biomechanical studies

Abstract:Modern environmental scenarios such as autonomous vehicles, aircrafts, and military vehicles position the human body in a nonstandard posture and induce multiplanar loads; however, current spine alignment methods and loading are based on sagittal and planar loads. The objective of this study is to develop a posture control device and demonstrate its ability to induce multiplanar loads to the human cadaver spinal columns. The inferior end of the device was designed to allow a full six degree-of-freedom control for positioning the specimen via a coupled x-y cross table, vertical lift platform, and triaxial rotation mechanism. The superior end of the device was designed such that the cranial fixation of the specimen could be attached to the piston of the electrohydraulic testing apparatus directly or via a rotary disc through a slider-crank mechanism. The former attachment induces complex forces and moments, while the latter induces controlled moments with minimal forces. The usability of the posture control device was demonstrated by conducting experiments with a thoracolumbar spinal column for combined forces and moments, and with a head-neck column for complex moments, and in both cases, the uniaxial travel of the piston was at a dynamic rate. The posture control device can be used to study the biomechanics of the spine under complex loads and with different postures and develop injury criteria for different field environments.
Development of a test rig for the diagnosis of arthritic knee joint defects in the case of lower extremity amputees in orthopaedic technology

Abstract:If the alignment of a leg prosthesis is inadequate there will be the danger of arthritic defects occurring in the preserved knee joint. Acoustic-kinetic joint analysis in orthopaedic technology allows monitoring of the knee joint of an amputee after the alignment of a leg prosthesis and to recognize early joint defects resulting in adaptation of the alignment of the prothesis. A test rig was developed which includes acoustic emission analysis of the knee, video-based gait analysis, ground reaction force measurement and the measurement of the holding forces of the hands. During the examination a subject must do 3 one-legged knee bends during 10 sec with the preserved leg. To support the subject two grab handles were fixed with force sensors to a frame. A series of 6 measurements were recorded with breaks of 2 min. The recorded forces of the leg show a double hump pattern during the knee bend. The rising edge of the flexion phase was not uniform. The forces show no conclusive uniform pattern. The fitness of the probands acts as an indicator for the pattern of the force signals. The signals of the measurements show the necessity of the support for both hands to execute a one-legged knee bend. The understanding of the 3 measured forces is important for the arthritic diagnosis of the knee joint.

Abstract:Purpose Few finite element studies have investigated changes in cervical biomechanics with various prosthesis design parameters using hybrid surgery (HS), and none have investigated those combined different HS strategies. The aim of our study was to investigate the effect of ball-and-socket prosthesis geometry on the biomechanical performance of the cervical spine combined with two HS constructs. Methods Two HS strategies were conducted: (1) ACDF at C4–C5 and anterior cervical disc replacement (ACDR) at C5–C6 (ACDF/ACDR), and (2) ACDR/ACDF. Three different prostheses were used for each HS strategy: prosthesis with the core located at the center of the inferior endplate with a radius of 5 mm (BS-5) or 6 mm (BS-6), or with a 5 mm radius core located 1 mm posterior to the center of the inferior endplate (PBS-5). Flexion and extension motions were simulated under displacement control. Results The flexion motions in supra- and infra-adjacent levels increased in all cases. The corresponding extension motions increased with all prostheses in ACDR/ACDF group. The stiffness in flexion and extension increased with all HS models, except for the extension stiffness with ACDF/ACDR. The facet stresses between the index and infra-adjacent level in ACDR/ACDF were significantly greater than those in the intact model . The stresses on the BS-5 UHMWPE core were greater than the yield stress. Conclusion The core radii and position did not significantly affect the moments, ROM, and facet stress in extension. However, the moments and ROM in flexion were easily affected by the position. The results implied that the large core radii and posterior core position in ACDR designs may reduce the risk of subsidence and wear in the long term as they showed relative low stress . The ACDF/ACDR surgery at C4–C6 level may be an optimal treatment for avoiding accelerating the degeneration of adjacent segments.

Abstract:The implantation of uncemented prostheses requires the application of sufficient forces to achieve a press-fit of the implant in the bone. Excessive forces have to be omitted to limit bone damage. Force measurements along the force transmission path between mallet and implant are frequently used to investigate this trade-off. Placing a load cell at a position of interest (PoI), which might be the implant bone interface or the head taper junction, is technically challenging or even impossible so that nearby positions are chosen. Thus, a certain inertia and stiffness remain between the PoI and the sensor, and consequently the measured dynamic forces differ from those at the PoI. This experimental and numerical study aimed to investigate the amount of force reduction along the transmission path while joining femoral heads to stem tapers. Forces were measured in vitro at the tip of the mallet, directly above the polymer tip of the impactor and below the stem taper. Springs and masses were used to represent the responding tissue of a patient. A semi-empirical numerical model of the force transmission path was developed and validated in order to simulate a larger range of responding tissue properties than experimentally possible and to investigate the influence of different surgical instruments. A distinct attenuation was observed since the peak forces at the impactor reached 35% of the applied peak forces and 21% at the stem taper, respectively. The force curves were replicated with a median root mean square error of 3.8% of the corresponding mallet blow for the impactor and 3.6% for the stem. The force measurement position and the used surgical instruments have a strong influence on the measured forces. Consequently, the exact measurement conditions with regard to sensor positioning and used surgical instruments have to be specified and hence only studies with similar setups should be compared to avoid misestimation of the forces at the PoI. The proposed dynamic numerical model is a useful tool to calculate the impact of the chosen or changed mechanical parameters prior to executing experiments and also to extrapolate the effect of changing the applied forces to the resulting forces at the PoI.

Evaluation of the reduction, tightening and gripping performance of an innovative set screw technology for instrumented posterior lumbar fusion: A biomechanical study

Abstract:Background: Instrumented posterior lumbar fusion with top-loading pedicle screw systems (PSS) requires fully tightened set screws to achieve a secure fixation and symmetric load condition. This assumes a complete reduction of the rod by 90°, which is not always attainable in situ, especially under constraint. The objective of this work is to compare the mechanical performance of different innovative set screw technologies, which should improve the tightening process. Hypothesis: The hypotheses of the study are that modifications to the screw and screwdriver unit can (1) improve the quality of set screw tightening and (2) increase the axial gripping capacity of the construct. Materials and methods: The four set screw technologies under investigation include a standard set screw with a flat surface (F-S; control group), a set screw with a convex surface (C-S) and a shaft tip method screwdriver used in combination with both flat (F-STM) and convex set screws (C-STM). The quality of set screw tightening is categorized as follows: failed=the rod is not completely reduced; reduced=the rod is successfully reduced but the set screw is not correctly fixed; good=remaining cases. An axial gripping capacity test is performed by a universal testing machine (Instron®) with a force capacity of 5kN. Results: Regarding the quality of set screw tightening, comparisons between F-S vs. F-STM, F-S vs. C-STM and between C-S vs. C-STM show statistically significant differences (p<0.001). The axial gripping capacity test shows mean gripping forces of 1223N (STD 331) in the F-STM group and of 1724N (STD 168) in the C-STM group with statistically significant differences between both groups (p=0.003). Discussion: Several biomechanical and clinical case studies have identified possible effects of misaligned rod-screw interfaces such as screw pull-out during rod reduction, screw loosening, screw or rod breakage, misalignment, adjacent segment degeneration and worsening of the clinical outcome. C-STM-technology thus supports controlled fixation in the sense of applying appropriate forces for correction or fixation during PSS assembly as well as friction-reduced final alignment and tightening with the aim to reduce implant loosening, hardware failure and reoperations, while respecting anatomical and biomechanical balance.
Identification of a lumped-parameter model of the intervertebral joint from experimental data

Abstract:Through predictive simulations, multibody models can aid the treatment of spinal pathologies by identifying optimal surgical procedures. Critical to achieving accurate predictions is the definition of the intervertebral joint. The joint pose is often defined by virtual palpation. Intervertebral joint stiffnesses are either derived from literature, or specimen-specific stiffnesses are calculated with optimisation methods. This study tested the feasibility of an optimisation method for determining the specimen-specific stiffnesses and investigated the influence of the assigned joint pose on the subject-specific estimated stiffness. Furthermore, the influence of the joint pose and the stiffness on the accuracy of the predicted motion was investigated. A computed tomography based model of a lumbar spine segment was created. Joints were defined from virtually palpated landmarks sampled with a Latin Hypercube technique from a possible Cartesian space. An optimisation method was used to determine specimen-specific stiffnesses for 500 models. A two-factor analysis was performed by running forward dynamic simulations for ten different stiffnesses for each successfully optimised model. The optimisations calculated a large range of stiffnesses, indicating the optimised specimen-specific stiffnesses were highly sensitive to the assigned joint pose and related uncertainties. A limited number of combinations of optimised joint stiffnesses and joint poses could accurately predict the kinematics. The two-factor analysis indicated that, for the ranges explored, the joint pose definition was more important than the stiffness. To obtain kinematic prediction errors below 1 mm and 1° and suitable specimen-specific stiffnesses the precision of virtually palpated landmarks for joint definition should be better than 2.9 mm.

Abstract:Introduction: The cervical spine, pivotal for mobility and overall body function, can be affected by cervical spondylosis, a major contributor to neural disorders. Prevalent in both general and military populations, especially among pilots, cervical spondylosis induces pain and limits spinal capabilities. Anterior Cervical Discectomy and Fusion (ACDF) surgery, proposed by Cloward in the 1950s, is a promising solution for restoring natural cervical curvature. The study objective was to investigate the impacts of ACDF implant design on postsurgical cervical biomechanics and neurorehabilitation outcomes by utilizing a biofield head-neck finite element (FE) platform that can facilitate scenario-specific perturbations of neck muscle activations. This study addresses the critical need to enhance computational models, specifically FE modeling, for ACDF implant design. Materials and methods: We utilized a validated head-neck FE model to investigate spine-implant biomechanical interactions. An S-shaped dynamic cage incorporating titanium (Ti) and polyetheretherketone (PEEK) materials was modeled at the C4/C5 level. The loading conditions were carefully designed to mimic helmet-to-helmet impact in American football, providing a realistic and challenging scenario. The analysis included intervertebral joint motion, disk pressure, and implant von Mises stress. Results: The PEEK implant demonstrated an increased motion in flexion and lateral bending at the contiguous spinal (C4/C5) level. In flexion, the Ti implant showed a modest 5% difference under 0% activation conditions, while PEEK exhibited a more substantial 14% difference. In bending, PEEK showed a 24% difference under 0% activation conditions, contrasting with Ti's 17%. The inclusion of the head resulted in an average increase of 18% in neck angle and 14% in C4/C5 angle. Disk pressure was influenced by implant material, muscle activation level, and the presence of the head. Polyetheretherketone exhibited lower stress values at all intervertebral disc levels, with a significant effect at the C6/C7 levels. Muscle activation level significantly influenced disk stress at all levels, with higher activation yielding higher stress. Titanium implant consistently showed higher disk stress values than PEEK, with an orders-of-magnitude difference in von Mises stress. Excluding the head significantly affected disk and implant stress, emphasizing its importance in accurate implant performance simulation. Conclusions: This study emphasized the use of a biofidelic head-neck model to assess ACDF implant designs. Our results indicated that including neck muscles and head structures improves biomechanical outcome measures. Furthermore, unlike Ti implants, our findings showed that PEEK implants maintain neck motion at the affected level and reduce disk stresses. Practitioners can use this information to enhance postsurgery outcomes and reduce the likelihood of secondary surgeries. Therefore, this study makes an important contribution to computational biomechanics and implant design domains by advancing computational modeling and theoretical knowledge on ACDF-spine interaction dynamics.
Optimization Reduces Knee-Joint Forces During Walking and Squatting: Validating the Inverse Dynamics Approach for Full Body Movements on Instrumented Knee Prostheses

Abstract:Because of the redundancy of our motor system, movements can be performed in many ways. While multiple motor control strategies can all lead to the desired behavior, they result in different joint and muscle forces. This creates opportunities to explore this redundancy, for example, for pain avoidance or reducing the risk of further injury. To assess the effect of different motor control optimization strategies, a direct measurement of muscle and joint forces is desirable, but problematic for medical and ethical reasons. Computational modeling might provide a solution by calculating approximations of these forces. In this study, we used a full-body computational musculoskeletal model to (a) predict forces measured in knee prostheses during walking and squatting and (b) study the effect of different motor control strategies (i.e., minimizing joint force vs. muscle activation) on the joint load and prediction error. We found that musculoskeletal models can accurately predict knee joint forces with a root mean squared error of <0.5 body weight (BW) in the superior direction and about 0.1 BW in the medial and anterior directions. Generally, minimization of joint forces produced the best predictions. Furthermore, minimizing muscle activation resulted in maximum knee forces of about 4 BW for walking and 2.5 BW for squatting. Minimizing joint forces resulted in maximum knee forces of 2.25 BW and 2.12 BW, that is, a reduction of 44% and 15%, respectively. Thus, changing the muscular coordination strategy can strongly affect knee joint forces. Patients with a knee prosthesis may adapt their neuromuscular activation to reduce joint forces during locomotion.

Multi-Material 3-D Viscoelastic Model of a Transtibial Residuum from In-vivo Indentation and MRI Data

Abstract:Although the socket is critical in a prosthetic system for a person with limb amputation, the methods of its design are largely artisanal. A roadblock for a repeatable and quantitative socket design process is the lack of predictive and patient specific biomechanical models of the residuum. This study presents the evaluation of such a model using a combined experimental-numerical approach. The model geometry and tissue boundaries are derived from MRI. The soft tissue non-linear elastic and viscoelastic mechanical behavior was evaluated using inverse finite element analysis (FEA) of in-vivo indentation experiments. A custom designed robotic in-vivo indentation system was used to provide a rich experimental data set of force versus time at 18 sites across a limb. During FEA, the tissues were represented by two layers, namely the skin-adipose layer and an underlying muscle-soft tissue complex. The non-linear elastic behavior was modeled using 2nd order Ogden hyperelastic formulations, and viscoelasticity was modeled using the quasi-linear theory of viscoelasticity. To determine the material parameters for each tissue, an inverse FEA based optimization routine was used that minimizes the combined mean of the squared force differences between the numerical and experimental force-time curves for indentations at 4 distinct anatomical regions on the residuum. The optimization provided the following material parameters for the skin-adipose layer: c=5.22 kPa, m=4.79, gamma=3.57 MPa, tau=0.32 s and for the muscle-soft tissue complex: c=5.20 kPa, m=4.78, gamma=3.47 MPa, tau=0.34 s. These parameters were evaluated to predict the force-time curves for the remaining 14 anatomical locations. The mean percentage error (mean absolute error/ maximum experimental force) for these predictions was 7 +/- 3%. The mean percentage error at the 4 sites used for the optimization was 4%.

Abstract:The AMTI VIVOTM six degree of freedom joint simulator allows reproducible preclinical testing of joint endoprostheses under specific kinematic and loading conditions. When testing total knee endoprosthesis, the articulating femoral and tibial components are each mounted on an actuator with two and four degrees of freedom, respectively. To approximate realistic physiological conditions with respect to soft tissues, the joint simulator features an integrated virtual ligament model that calculates the restoring forces of the ligament apparatus to be applied by the actuators. During joint motion, the locations of the ligament insertion points are calculated depending on both actuators' coordinates. In the present study, we demonstrate that unintended elastic deformations of the actuators due to the specifically high contact forces in the artificial knee joint have a considerable impact on the calculated ligament forces. This study aims to investigate the effect of this structural compliance on experimental results. While the built-in algorithm for calculating the ligament forces cannot be altered by the user, a reduction of the ligament force deviations due to the elastic deformations could be achieved by preloading the articulating implant components in the reference configuration. As a proof of concept, a knee flexion motion with varying ligament conditions was simulated on the VIVO simulator and compared to data derived from a musculoskeletal multibody model of a total knee endoprosthesis.
