Abstract:Purpose: The objective of this study was to compare and assess the accuracy of computed tomography (CT)-based target delineation with that of magnetic resonance imaging (MRI)-based on high-dose-rate brachytherapy (HDR-BT) for patients with cervical cancer. Material and methods: Data of 20 patients with locally advanced cervical cancer were collected and evaluated. Dimensions, conformity, and dose parameters of high-risk clinical target volume (CTVHR) as well as D-0.(1cc), D-1(cc), and D-2cc of organs at risk (OARs) based on MRI were compared with those based on CT. Results: Average age of 20 patients included was 57.8 years. Width, thickness, and volumes of CT-based CTVHR (CTVHR-CT) were significantly overestimated compared with those of MRI-based CTVHR (CTVHR-MR). Mean values of dice similarity coefficient (DSC), Hausdorff distance (HD), and centroid distance (AV) of CTVHR were 0.82 cm, 0.96 cm, and 0.35 cm, respectively. Dose values of CTVHR-CT were significantly lower compared with those of CTVHR-MR. Concerning OARs, geometrical and dosimetric values on CT were comparable to those on MRI. Conclusions: The delineated ranges of CTVHR were significantly over-estimated on CT compared with MRI. D-98 and D-90 of CTVHR-CT were lower than CTVHR-MR. DSC and AV of CTVHR and CTVIR were similar to each other; however, there was a difference in terms of HD. CT images regarding pre-BT MR images for delineating were not enough and MRI fusion is still required.

Abstract:Background and purpose: Clinical targeted volume (CTV) delineation accounting for the patient-specific microscopic tumor spread can be a difficult step in defining the treatment volume. We developed an intelligent and automated CTV delineation system for locally advanced non-small cell lung carcinoma (NSCLC) to cover the microscopic tumor spread while avoiding organs-at-risk (OAR). Materials and methods: A 3D UNet with a customized loss function was used, which takes both the patients' respiration-correlated ("4D") CT scan and the physician contoured internal gross target volume (iGTV) as inputs, and outputs the CTV delineation. Among the 84 identified patients, 60 were randomly selected to train the network, and the remaining as testing. The model performance was evaluated and compared with cropped expansions using the shape similarities to the physicians' contours (the ground-truth) and the avoidance of critical OARs. Results: On the testing datasets, all model-predicted CTV contours followed closely to the ground truth, and were acceptable by physicians. The average dice score was 0.86. Our model-generated contours demonstrated better agreement with the ground-truth than the cropped 5 mm/8 mm expansion method (median of median surface distance of 1.0 mm vs 1.9 mm/2.0 mm), with a small overlap volume with OARs (0.4 cm(3) for the esophagus and 1.2 cm(3) for the heart). Conclusions: The CTVs generated by our CTV delineation system agree with the physician's contours. This approach demonstrates the capability of intelligent volumetric expansions with the potential to be used in clinical practice.
Automated Clinical Target Volume Delineation for Non-Small Cell Lung Cancer Patients Using Deep 3D Networks

Abstract:Patient-specific clinical target volume (CTV) delineation is the most difficult step in defining the planning target volume from the gross tumor volume. Delineating patient specific CTVs is a labor-intensive process, prone to human error, with poor inter-observer consistency. Automated volumetric expansions have been used to reduce these concerns, but these ignore patient specific anatomic and clinical variables, as well as the clinical experience of the radiation oncologist. We developed an automated CTV delineation system for lung cancer, which was trained to learn the physician’s CTV contour pattern using a deep 3D neural network. Our network took both patients’ CT images and their corresponding internal target volume (ITV) as input, and output CTV delineation. ITVs were contoured by physicians after studying the internal motion of the target from the 4D CT scans. In our study, the network was trained from twenty-five NSCLC patients in right upper lung lobe (RUL). Then, the trained network was tested on four additional datasets from RUL NSCLC patients. Each CTV delineation output by the trained network was compared with the corresponding CTV contoured by physicians. One challenge to train a 3D network for CTV delineation is the high imbalance between the CTV volume and the background volume. Instead of using hyperparameters to balance as existing methods did, we directly optimize a loss function robust to high imbalance. This improves the stability of the model, especially when the balance dynamically changes in images. Three out of four CTV contours that were automatically delineated by our trained model agree very well with the physician’s CTV contour. Only one has visible discrepancies near the apex of the upper lobe, but still looks reasonable. As shown in Table 1, the performance of the trained neural network was also quantitatively evaluated using precision and recall between the predicted CTVs and the ground truth CTVs. The average precision is 0.902, and recall is 0.871. The recall and precision are the fraction of true volume among the total ground-truth and the fraction of true volume among the predicted volume, respectively. The automated CTV delineation was utilized for RUL NSCLC patients using the deep learning method solely based on CT images with manually contoured ITV. On the limited testing samples, the model contoured CTVs reasonably well. Quantitative and Qualitative Evaluation of a Deep Learning Auto Contouring Model for Prostate Cancer Patients with Hydrogel Spacer

Abstract:To quantitatively and qualitatively evaluate a deep learning auto contouring model for prostate radiotherapy patients with pretreatment insertion of a hydrogel spacer (about water equivalent with no contrast) between prostate and rectum. The model employs convolutional neural networks (CNN) to learn features from input images that can be used to generate semantic segmentation. The study used 163 patients from three specialized GU radiation oncologists (referred to as A/B/C). The first 135 patients (A/B/C = 82/39/14) were used for training (125) and validation (10). The validation patients were randomly selected. The validated model was tested on 28 patients (A/B/C = 18/6/4) accrued during model development. There was no change of practice during the whole period. A simulation CT and MR were taken on the same day for each patient. In manual contouring, with MR fused to CT, spacer was contoured on T2 MR, prostate on CT with MR guidance, and other structures on CT only. The model was trained to auto contour prostate, proximal seminal vesicles (SV), bladder, rectum, penile bulb, femurs and spacer on CT without MR. Quantitatively, auto contours were evaluated against manual contours using the following metrics: sensitivity (% of voxels correctly drawn), false positive rate (FPR, % of voxels overdrawn), dice similarity coefficient (DSC), 95-percentile of Hausdorff distance (HD) and mean distances (dmean) between the two contours over all slices. The structures with high DSC were qualitatively evaluated by the original attending using a 1 (acceptable with minor editing), 2 (editable with efficiency gain over manual contouring) and 3 (rejected for no efficiency gain or gross error) scoring system. A gross error on rectum occurred for two patients (A/B = 1/1). These two points were excluded from quantitative analysis but counted as rejected in qualitative evaluation. On average, DSC was high for femurs (>0.95) and bladder (0.91), moderate for prostate (0.85) and rectum (0.81), but low for bulb (0.67), proximal SV (0.62) and spacer (0.52). For right femur/left femur/bladder/prostate/rectum, sensitivity = 0.93/0.92/0.88/0.86/0.81, FPR = 1.8%/1.5%/4.5%/15%/17%, 95% 95%-HD = 2.8/2.6/12.1/7.4/9.5 mm, and dmean = 0.9/1.0/2.6/2.5/2.4 mm. Qualitatively, femurs scored 1 in all cases. The average scores for bladder/prostate/rectum = 1.28/1.44/1.50, 1.83/2.17/1.67, 1.25/1.50/1.25 for physicians A, B, C, respectively, and 1.39/1.61/1.50 overall. Prostate and rectum both scored well below 2, despite their lower quantitative performance, as some errors caused by the inaccurate prediction of spacer without MR were deemed easily correctable by the physicians. The model produced clinically satisfactory results, both quantitatively and qualitatively, for femurs, bladder, prostate and rectum. The results for proximal SV and bulb were less ideal. The model drew the spacer in the correct location, but could not draw it accurately due to lack of contrast on CT.

Abstract:<h3>Purpose/Objective(s)</h3> To evaluate the feasibility and safety of creating radiation treatment plans based on a convolutional neural network auto-contouring (AC) model for patients receiving radiation for localized prostate cancer with a radio-opaque rectal spacer hydrogel. <h3>Materials/Methods</h3> The deep learning model produces ACs for target volumes (prostate, proximal seminal vesicle), OARs (rectum, bladder, penile bulb, femoral heads), and rectal hydrogel spacers. ACs were compared to manual contours (MC) by MDs for clinical use. Individual volumes and overall performance (composite score) were previously evaluated and reported, qualitatively using a 1 (minor discrepancy), 2 (moderate discrepancy), 3 (significant discrepancy), and 4 (rejected, gross error) scale and quantitatively using Dice Similarity Coefficient (DSC) and mean distance to agreement. VMAT treatment plans were created on unedited AC volumes from five composite 1 (C1) cases and five composite 2 (C2) cases and dose volume metrics were computed for the final MCs. Dosimetry comparison was performed to assess the safety of delivering a treatment plan created from unedited ACs. Cases were planned to 50.4 Gy in 28 fractions to the SVs, with a SIB of 70 Gy the prostate. PTV expansions were CTV+5 mm (CTV+4 mm posteriorly). <h3>Results</h3> Of 62 initial cases evaluated, 11 were scored as a C1, 28 were scored as a C2, and 23 received a C3. 10 cases were randomly selected for treatment planning. For prostate (CTV P) in selected C1 and C2 cases, mean score was 1 and 1.8 and mean DSC was 0.919 and 0.876, respectively. Clinical goals for planning and dosimetric data from treatment plans are shown in Table 1. The mean V70 Gy for prostate CTV/PTV was 99.83%/90.05% for C1 cases and 99.46%/89.15% for C2 cases. All clinical goals for OARs were met in both cohorts. <h3>Conclusion</h3> 63% of all cases evaluated received a C1 or C2 score. Our results indicate the treatment plans generated from unedited ACs on these cases respected all clinical goals for OARs. Although C1 plans produced better PTV coverage than C2, almost all plans had sub-optimal PTV coverage, indicating the need for MDs to edit target volumes before treatment planning.

Abstract:This study aimed to develop an automated delineation method of nasopharynx gross tumor volume (GTVnx) for nasopharyngeal carcinoma (NPC) in computed tomography (CT) image for radiotherapy applications. Inspired by ResNet and SENet's strong ability to extract image features, we proposed a modified version of the 3D U-Net model with Res-blocks and SE-block for delineation of GTVnx. Besides, an automatic pre-processing method was proposed to crop the 3D region of interest (ROI) of GTVnx. Radiotherapy simulation CT images and corresponding manually delineated target of 205 NPC patients diagnosed with stage T1-T4 were used as datasets for training. Automated delineation models were generated based on CT combining contrast-enhanced CT (CE-CT) and CT alone, respectively. We compared the automatic delineation results against the manual delineated contours by radiation oncologists with 5-fold cross-validation to evaluate the performance of the proposed model. We also compared with the framework using 3D CNN and 2D DDNN, respectively. Besides, the model generated by one medical group was assessed against the other two separate medical groups. Precision (PR), Sensitivity (SE), Dice Similarity Coefficient (DSC), Average Symmetric Surface Distance (ASSD), and 95% Hausdorff Distance (HD95) are calculated for quantitative evaluation. Experimental results show that the proposed method outperforms other automatic methods on the CT images. Automated delineation models based on CT combining CE-CT is superior to that based on CT alone. The presented method could be useful and robust for the 3D delineation of GTVnx for NPC in CT images during the planning of radiotherapy.

Abstract:Purpose: Image segmentation can be time-consuming and lacks consistency between different oncologists, which is essential in conformal radiotherapy techniques. We aimed to evaluate automatic delineation results generated by convolutional neural networks (CNNs) from geometry and dosimetry perspectives and explore the reliability of these segmentation tools in rectal cancer. Methods: Forty-seven rectal cancer cases treated from February 2018 to April 2019 were randomly collected retrospectively in our cancer center. The oncologists delineated regions of interest (ROIs) on planning CT images as the ground truth, including clinical target volume (CTV), bladder, small intestine, and femoral heads. The corresponding automatic segmentation results were generated by DeepLabv3+ and ResUNet, and we also used Atlas-Based Autosegmentation (ABAS) software for comparison. The geometry evaluation was carried out using the volumetric Dice similarity coefficient (DSC) and surface DSC, and critical dose parameters were assessed based on replanning optimized by clinically approved or automatically generated CTVs and organs at risk (OARs), i.e. , the Plan ref and Plan test . Pearson test was used to explore the correlation between geometric metrics and dose parameters. Results: In geometric evaluation, DeepLabv3+ performed better in DCS metrics for the CTV (volumetric DSC, mean = 0.96, P< 0.01; surface DSC, mean = 0.78, P< 0.01) and small intestine (volumetric DSC, mean = 0.91, P< 0.01; surface DSC, mean = 0.62, P< 0.01), ResUNet had advantages in volumetric DSC of the bladder (mean = 0.97, P< 0.05). For critical dose parameters analysis between Plan ref and Plan test , there was a significant difference for target volumes (P 0.05). For the correlation test, a negative correlation was found between DSC metrics (volumetric, surface DSC) and dosimetric parameters (δD95, δD95, HI, CI) for target volumes (P 0.05). Conclusions: CNNs show remarkable repeatability and time-saving in automatic segmentation, and their accuracy also has a certain potential in clinical practice. Meanwhile, clinical aspects, such as dose distribution, may need to be considered when comparing the performance of auto-segmentation methods.

Abstract:BACKGROUND: In recent years, high-precision image-guided intensity-modulated radiation therapy combined with three-dimensional (3D) high-dose-rate (HDR) brachytherapy (BT) has become a recommended technique for radical radiotherapy for cervical cancer. This study first employed contrast-limited adaptive histogram equalization (CLAHE) for preprocessing of input data to achieve image enhancement. In this way, rapid and accurate automatic delineation of the clinical target volume (CTV) and organs at risk (OARs) in 3D BT for cervical cancer was achieved.METHODS: Two hundred cervical cancer patients who underwent radical radiotherapy from January 2016 to December 2018 were selected. After collecting the computed tomography (CT) image data of a patient, we constructed the radiotherapy CTV and OAR image libraries. A RefineNet-based deep learning protocol was used to segment the CTV and OARs for 3D BT for cervical cancer. In this study, a total of 1,000 rounds of training were carried out, and the model with the best performance was selected for subsequent iterative tuning. Finally, the clinical test was carried out, in which the CT images of 10 cases were tested one by one. The manual delineation results and the model output results for the CTV and OARs were compared to measure the performance of the model.RESULTS: Compared with the manually delineated CTV, the RefineNet model-based segmented CTV had a higher Dice similarity coefficient (DSC), Hausdorff distance (HD), and overlap index (OI), which were 0.861, 6.005, and 0.839, respectively. For OARs, the RefineNet-based model obtained the best results for bladder segmentation (DSC: 85.96%), respectively. The mean duration of RefineNet-based automatic contour processing of the CTV was 70 s, and the mean durations of RefineNet-based automatic delineation of the bladder, rectum, sigmoid colon, and small intestine were 67, 67.4, 63.8, and 60.8 s, respectively. The total time saved by RefineNet was approximately 60%.CONCLUSIONS: The RefineNet-based automatic delineation model for 3D BT for cervical cancer is a stable and highly consistent automatic delineation algorithmic model that has the potential to improve the consistency of target region delineation, simplify the radiotherapy procedure, and achieve rapid and accurate automatic delineation of CTVs and OARs.

Abstract:Purpose To explore the accuracy and feasibility of a proposed deep learning (DL) algorithm for clinical target volume (CTV) delineation in cervical cancer radiotherapy and evaluate whether it can perform well in external cervical cancer and endometrial cancer cases for generalization validation. Methods A total of 332 patients were enrolled in this study. A state‐of‐the‐art network called ResCANet, which added the cascade multi‐scale convolution in the skip connections to eliminate semantic differences between different feature layers based on ResNet‐UNet. The atrous spatial pyramid pooling in the deepest feature layer combined the semantic information of different receptive fields without losing information. A total of 236 cervical cancer cases were randomly grouped into 5‐fold cross‐training (n = 189) and validation (n = 47) cohorts. External validations were performed in a separate cohort of 54 cervical cancer and 42 endometrial cancer cases. The performances of the proposed network were evaluated by dice similarity coefficient (DSC), sensitivity (SEN), positive predictive value (PPV), 95% Hausdorff distance (95HD), and oncologist clinical score when comparing them with manual delineation in validation cohorts. Results In internal validation cohorts, the mean DSC, SEN, PPV, 95HD for ResCANet achieved 74.8%, 81.5%, 73.5%, and 10.5 mm. In external independent validation cohorts, ResCANet achieved 73.4%, 72.9%, 75.3%, 12.5 mm for cervical cancer cases and 77.1%, 81.1%, 75.5%, 10.3 mm for endometrial cancer cases, respectively. The clinical assessment score showed that minor and no revisions (delineation time was shortened to within 30 min) accounted for about 85% of all cases in DL‐aided automatic delineation. Conclusions We demonstrated the problem of model generalizability for DL‐based automatic delineation. The proposed network can improve the performance of automatic delineation for cervical cancer and shorten manual delineation time at no expense to quality. The network showed excellent clinical viability, which can also be even generalized for endometrial cancer with excellent performance.

Abstract:Objective. To develop and evaluate a 3D Prompt-ResUNet module that utilized the prompt-based model combined with 3D nnUNet for rapid and consistent autosegmentation of high-risk clinical target volume and organ at risk in high-dose-rate brachytherapy for cervical cancer patients. Approach. We used 73 computed tomography (CT) and 62 magnetic resonance imaging (MRI) scans from 135 (103 for training, 16 for validation, and 16 for testing) cervical cancer patients across two hospitals for HRCTV and OAR segmentation. A novel comparison of the deep learning neural networks 3D Prompt-ResUNet, nnUNet, and SAM-Med3D was applied for the segmentation. Evaluation was conducted in two parts: geometric and clinical assessments. Quantitative metrics included the Dice similarity coefficient (DSC), 95th percentile Hausdorff distance (HD95%), Jaccard index (JI), and Matthews correlation coefficient (MCC). Clinical evaluation involved interobserver comparison, 4-grade expert scoring, and a double-blinded Turing test. Main results. The Prompt-ResUNet model performed most similarly to experienced radiation oncologists, outperforming less experienced ones. During testing, the DSC, HD95% (mm), JI, and MCC value (mean ± SD) for HRCTV were 0.92±0.03, 2.91 ± 0.69, 0.85± 0.04, and 0.92 ± 0.02, respectively. For the bladder, these values were 0.93 ± 0.05, 3.07 ± 1.05, 0.87 ± 0.08, and 0.93 ± 0.05, respectively. For the rectum, they were 0.87 ± 0.03, 3.54 ± 1.46, 0.78 ± 0.05, and 0.87 ± 0.03, respectively. For the sigmoid, they were 0.76 ± 0.11, 7.54 ± 5.54, 0.63 ± 0.14, and 0.78 ± 0.09, respectively. The Prompt-ResUNet achieved a clinical viability score of at least 2 in all evaluation cases (100%) for both HRCTV and bladder and exceeded the 30% positive rate benchmark for all evaluated structures in the Turing test. Significance. The Prompt-ResUNet architecture demonstrated high consistency with ground truth (GT) in autosegmentation of HRCTV and OARs, reducing interobserver variability and shortening treatment times.

Abstract:Objective Clinical target volume (CTV) autosegmentation for cervical cancer is desirable for radiation therapy. Data heterogeneity and interobserver variability (IOV) limit the clinical adaptability of such methods. The adaptive method is proposed to improve the adaptability of CNN-based autosegmentation of CTV contours in cervical cancer. Methods This study included 400 cervical cancer treatment planning cases with CTV delineated by radiation oncologists from three hospitals. The datasets were divided into five subdatasets (80 cases each). The cases in datasets 1, 2, and 3 were delineated by physicians A, B, and C, respectively. The cases in datasets 4 and 5 were delineated by multiple physicians. Dataset 1 was divided into training (50 cases), validation (10 cases), and testing (20 cases) cohorts, and they were used to construct the pretrained model. Datasets 2-5 were regarded as host datasets to evaluate the accuracy of the pretrained model. In the adaptive process, the pretrained model was fine-tuned to measure improvements by gradually adding more training cases selected from the host datasets. The accuracy of the autosegmentation model on each host dataset was evaluated using the corresponding test cases. The Dice similarity coefficient (DSC) and 95% Hausdorff distance (HD_95) were used to evaluate the accuracy. Results Before and after adaptive improvements, the average DSC values on the host datasets were 0.818 versus 0.882, 0.763 versus 0.810, 0.727 versus 0.772, and 0.679 versus 0.789, which are improvements of 7.82%, 6.16%, 6.19%, and 16.05%, respectively. The average HD_95 values were 11.143 mm versus 6.853 mm, 22.402 mm versus 14.076 mm, 28.145 mm versus 16.437 mm, and 33.034 mm versus 16.441 mm, which are improvements of 37.94%, 37.17%, 41.60%, and 50.23%, respectively. Conclusion The proposed method improved the adaptability of the CNN-based autosegmentation model when applied to host datasets.

Abstract:Objective. MRI is the standard imaging modality for high-dose-rate brachytherapy of cervical cancer. Precise contouring of organs at risk (OARs) and high-risk clinical target volume (HR-CTV) from MRI is a crucial step for radiotherapy planning and treatment. However, conventional manual contouring has limitations in terms of accuracy as well as procedural time. To overcome these, we propose a deep learning approach to automatically segment OARs (bladder, rectum, and sigmoid colon) and HR-CTV from female pelvic MRI. Approach. In the proposed pipeline, a coarse multi-organ segmentation model first segments all structures, from which a region of interest is computed for each structure. Then, each organ is segmented using an organ-specific fine segmentation model separately trained for each organ. To account for variable sizes of HR-CTV, a size-adaptive multi-model approach was employed. For coarse and fine segmentations, we designed a dual convolution-transformer UNet (DCT-UNet) which uses dual-path encoder consisting of convolution and transformer blocks. To evaluate our model, OAR segmentations were compared to the clinical contours drawn by the attending radiation oncologist. For HR-CTV, four sets of contours (clinical + three additional sets) were obtained to produce a consensus ground truth as well as for inter/intra-observer variability analysis. Main results. DCT-UNet achieved dice similarity coefficient (mean±SD) of 0.932±0.032 (bladder), 0.786±0.090 (rectum), 0.663±0.180 (sigmoid colon), and 0.741±0.076 (HR-CTV), outperforming other state-of-the-art models. Notably, the size-adaptive multi-model significantly improved HR-CTV segmentation compared to a single-model. Furthermore, significant inter/intra-observer variability was observed, and our model showed comparable performance to all observers. Computation time for the entire pipeline per subject was 12.59±0.79 seconds, which is significantly shorter than the typical manual contouring time of >15 minutes. Significance. These experimental results demonstrate that our model has great utility in cervical cancer brachytherapy by enabling fast and accurate automatic segmentation, and has potential in improving consistency in contouring.

Abstract:Purpose Delineation of the clinical target volume (CTV) and organs‐at‐risk (OARs) is important in cervical cancer radiotherapy. But it is generally labor‐intensive, time‐consuming, and subjective. This paper proposes a parallel‐path attention fusion network (PPAF‐net) to overcome these disadvantages in the delineation task. Methods The PPAF‐net utilizes both the texture and structure information of CTV and OARs by employing a U‐Net network to capture the high‐level texture information, and an up‐sampling and down‐sampling (USDS) network to capture the low‐level structure information to accentuate the boundaries of CTV and OARs. Multi‐level features extracted from both networks are then fused together through an attention module to generate the delineation result. Results The dataset contains 276 computed tomography (CT) scans of patients with cervical cancer of staging IB‐IIA. The images are provided by the West China Hospital of Sichuan University. Simulation results demonstrate that PPAF‐net performs favorably on the delineation of the CTV and OARs (e.g., rectum, bladder and etc.) and achieves the state‐of‐the‐art delineation accuracy, respectively, for the CTV and OARs. In terms of the Dice Similarity Coefficient (DSC) and the Hausdorff Distance (HD), 88.61% and 2.25 cm for the CTV, 92.27% and 0.73 cm for the rectum, 96.74% and 0.68 cm for the bladder, 96.38% and 0.65 cm for the left kidney, 96.79% and 0.63 cm for the right kidney, 93.42% and 0.52 cm for the left femoral head, 93.69% and 0.51 cm for the right femoral head, 87.53% and 1.07 cm for the small intestine, and 91.50% and 0.84 cm for the spinal cord. Conclusions The proposed automatic delineation network PPAF‐net performs well on CTV and OARs segmentation tasks, which has great potential for reducing the burden of radiation oncologists and increasing the accuracy of delineation. In future, radiation oncologists from the West China Hospital of Sichuan University will further evaluate the results of network delineation, making this method helpful in clinical practice.

Automatic delineation of the clinical target volume and organs at risk by deep learning for rectal cancer postoperative radiotherapy

Abstract:Background and purpose: Manual delineation of clinical target volumes (CTVs) and organs at risk (OARs) is time-consuming, and automatic contouring tools lack clinical validation. We aimed to construct and validate the use of convolutional neural networks (CNNs) to set better contouring standards for rectal cancer radiotherapy. Materials and methods: We retrospectively collected and evaluated computed tomography (CT) scans of 199 rectal cancer patients treated at our hospital from February 2018 to April 2019. Two CNNs-DeepLabv3+ for extracting high-level semantic information and ResUNet for extracting low-level visual features-were used for the CTV and small intestine contouring, and bladder and femoral head contouring, respectively. Contouring quality was compared using the paired t test. Five-point objective grading was performed independently by two experienced radiation oncologists and verified by a third. The CNN manual correction time was recorded. Results: CTVs calculated using DeepLabv3+ (CTVDeepLabv3+) had significant quantitative parameter advantages over CTVResUNet (volumetric Dice coefficient, 0.88 vs 0.87, P = 0.0005; surface Dice coefficient, 0.79 vs 0.78, P = 0.008). Among 315 graded cases, DeepLabv3+ obtained the highest scores with 284 cases, consistent with the objective criteria, whereas CTVResUNet had the minimum mean manual correction time (7.29 min). DeepLabv3+ performed better than ResUNet for small intestine contouring and ResUNet performed better for bladder and femoral head contouring. The manual correction time for OARs was <4 min for both models. Conclusion: CNNs at various feature resolution levels well delineate rectal cancer CTVs and OARs, displaying high quality and requiring shorter computation and manual correction time.

Abstract:Objective Adaptive planning to accommodate anatomic changes during treatment often requires repeated segmentation. In this study, prior patient‐specific data was integrateda into a registration‐guided multi‐channel multi‐path (Rg‐MCMP) segmentation framework to improve the accuracy of repeated clinical target volume (CTV) segmentation. Methods This study was based on CT image datasets for a total of 90 cervical cancer patients who received two courses of radiotherapy. A total of 15 patients were selected randomly as the test set. In the Rg‐MCMP segmentation framework, the first‐course CT images (CT1) were registered to second‐course CT images (CT2) to yield aligned CT images (aCT1), and the CTV in the first course (CTV1) was propagated to yield aligned CTV contours (aCTV1). Then, aCT1, aCTV1, and CT2 were combined as the inputs for 3D U‐Net consisting of a channel‐based multi‐path feature extraction network. The performance of the Rg‐MCMP segmentation framework was evaluated and compared with the single‐channel single‐path model (SCSP), the standalone registration methods, and the registration‐guided multi‐channel single‐path (Rg‐MCSP) model. The Dice similarity coefficient (DSC), 95% Hausdorff distance (HD95), and average surface distance (ASD) were used as the metrics. Results The average DSC of CTV for the deformable image DIR‐MCMP model was found to be 0.892, greater than that of the standalone DIR (0.856), SCSP (0.837), and DIR‐MCSP (0.877), which were improvements of 4.2%, 6.6%, and 1.7%, respectively. Similarly, the rigid body DIR‐MCMP model yielded an average DSC of 0.875, which exceeded standalone RB (0.787), SCSP (0.837), and registration‐guided multi‐channel single‐path (0.848), which were improvements of 11.2%, 4.5%, and 3.2%, respectively. Abstract:Purpose: More and more automatic segmentation tools are being introduced in routine clinical practice. However, physicians need to spend a considerable amount of time in examining the generated contours slice by slice. This greatly reduces the benefit of the tool's automaticity. In order to overcome this shortcoming, we developed an automatic quality assurance (QA) method for automatic segmentation using convolutional neural networks (CNNs). Materials and Methods: The study cohort comprised 680 patients with early-stage breast cancer who received whole breast radiation. The overall architecture of the automatic QA method for deep learning-based segmentation included the following two main parts: a segmentation CNN model and a QA network that was established based on ResNet-101. The inputs were from computed tomography, segmentation probability maps, and uncertainty maps. Two kinds of Dice similarity coefficient (DSC) outputs were tested. One predicted the DSC quality level of each slice ([0.95, 1] for "good," [0.8, 0.95] for "medium," and [0, 0.8] for "bad" quality), and the other predicted the DSC value of each slice directly. The performances of the method to predict the quality levels were evaluated with quantitative metrics: balanced accuracy, F score, and the area under the receiving operator characteristic curve (AUC). The mean absolute error (MAE) was used to evaluate the DSC value outputs. Results: The proposed methods involved two types of output, both of which achieved promising accuracy in terms of predicting the quality level. For the good, medium, and bad quality level prediction, the balanced accuracy was 0.97, 0.94, and 0.89, respectively; the F score was 0.98, 0.91, and 0.81, respectively; and the AUC was 0.96, 0.93, and 0.88, respectively. For the DSC value prediction, the MAE was 0.06 ± 0.19. The prediction time was approximately 2 s per patient. Conclusions: Our method could predict the segmentation quality automatically. It can provide useful information for physicians regarding further verification and revision of automatic contours. The integration of our method into current automatic segmentation pipelines can improve the efficiency of radiotherapy contouring.
Comprehensive evaluation of a deep learning model for automatic organs at risk segmentation on heterogeneous computed tomography images for abdominal radiotherapy

Abstract:Purpose To develop a deep learning model (AbsegNet) that produces accurate contours of 16 organs at risk (OARs) for abdominal malignancies, as an essential part of a fully automated radiation treatment planning. Material and methods Three datasets with 544 computed tomography (CT) scans were retrospectively collected. Dataset 1 was split into 300 training cases and 128 test cases (cohort 1) for AbsegNet. Dataset 2, including cohort 2 (n = 24) and cohort 3 (n = 20), were used to validate AbsegNet externally. Dataset 3, including cohort 4 (n = 40) and cohort 5 (n = 32) were used to clinically assess the accuracy of AbsegNet-generated contours. Each cohort was from a different center. The Dice similarity coefficient (DSC) and 95th-percentile Hausdorff distance (HD95) were calculated to evaluate the delineation quality for each OAR. Clinical accuracy evaluation was classified into three levels: no revision, minor revisions (0% < volumetric revision degrees (VRD) ≤ 10%), moderate revisions (10% ≤ VRD < 20%), and major revisions (VRD ≥ 20%). Results For all OARs, AbsegNet achieved a mean DSC of 86.73%, 85.65%, and 88.04% in cohort 1, cohort 2, and cohort 3, respectively, and a mean HD95 of 8.92mm, 10.18mm, and 12.40mm, respectively. The performance of AbsegNet outperformed SwinUNETR, DeepLabV3+, Attention-UNet, UNet and 3D-UNet. When experts evaluated contours from cohorts 4 and 5, four OARs (liver, kidney_L, kidney_R, and spleen) of all patients were scored as having no revision, and over 87.5% of patients with contours of the stomach, esophagus, adrenals, or rectum were considered as having no or minor revisions. Only 15.0% of patients with contours of the colon and small bowel required major revisions. Conclusions We propose a novel deep learning model to delineate OARs on diverse datasets. Most contours produced by AbsegNet are accurate and robust, therefore clinically applicable and helpful to facilitate radiotherapy workflow.

Abstract:Background and purpose: The geometrical accuracy of auto-segmentation using convolutional neural networks (CNNs) has been demonstrated. This study aimed to investigate the dose-volume impact of differences between automatic and manual OARs for locally advanced (LA) and peripherally located early-stage (ES) non-small cell lung cancer (NSCLC). Material and methods: A single CNN was created for automatic delineation of the heart, lungs, main left and right bronchus, esophagus, spinal cord and trachea using 55/10/40 patients for training/validation/testing. Dice score coefficient (DSC) and 95th percentile Hausdorff distance (HD95) were used for geometrical analysis. A new treatment plan based on the auto-segmented OARs was created for each test patient using 3D for ES-NSCLC (SBRT, 3-8 fractions) and IMRT for LA-NSCLC (24-35 fractions). The correlation between geometrical metrics and dose-volume differences was investigated. Results: The average (±1 SD) DSC and HD95 were 0.82 ± 0.07 and 16.2 ± 22.4 mm, while the average dose-volume differences were 0.5 ± 1.5 Gy (ES) and 1.5 ± 2.8 Gy (LA). The geometrical metrics did not correlate with the observed dose-volume differences (average Pearson for DSC: -0.27 ± 0.18 (ES) and -0.09 ± 0.12 (LA); HD95: 0.1 ± 0.3 mm (ES) and 0.2 ± 0.2 mm (LA)). Conclusions: After post-processing, manual adjustments of automatic contours are only needed for clinically relevant OARs situated close to the tumor or within an entry or exit beam e.g., the heart and the esophagus for LA-NSCLC and the bronchi for ES-NSCLC. The lungs do not need to be checked further in detail.
