Abstract:Background and purpose: Clinical targeted volume (CTV) delineation accounting for the patient-specific microscopic tumor spread can be a difficult step in defining the treatment volume. We developed an intelligent and automated CTV delineation system for locally advanced non-small cell lung carcinoma (NSCLC) to cover the microscopic tumor spread while avoiding organs-at-risk (OAR). Materials and methods: A 3D UNet with a customized loss function was used, which takes both the patients' respiration-correlated ("4D") CT scan and the physician contoured internal gross target volume (iGTV) as inputs, and outputs the CTV delineation. Among the 84 identified patients, 60 were randomly selected to train the network, and the remaining as testing. The model performance was evaluated and compared with cropped expansions using the shape similarities to the physicians' contours (the ground-truth) and the avoidance of critical OARs. Results: On the testing datasets, all model-predicted CTV contours followed closely to the ground truth, and were acceptable by physicians. The average dice score was 0.86. Our model-generated contours demonstrated better agreement with the ground-truth than the cropped 5 mm/8 mm expansion method (median of median surface distance of 1.0 mm vs 1.9 mm/2.0 mm), with a small overlap volume with OARs (0.4 cm(3) for the esophagus and 1.2 cm(3) for the heart). Conclusions: The CTVs generated by our CTV delineation system agree with the physician's contours. This approach demonstrates the capability of intelligent volumetric expansions with the potential to be used in clinical practice.

Abstract:Purpose: In clinical work, accurately measuring the volume and the size of breast cancer is significant to develop a treatment plan. However, it is time-consuming, and inter- and intra-observer variations among radiologists exist. The purpose of this study was to assess the performance of a Res-UNet convolutional neural network based on automatic segmentation for size and volumetric measurement of mass enhancement breast cancer on magnetic resonance imaging (MRI). Materials and methods: A total of 1,000 female breast cancer patients who underwent preoperative 1.5-T dynamic contrast-enhanced MRI prior to treatment were selected from January 2015 to October 2021 and randomly divided into a training cohort (n = 800) and a testing cohort (n = 200). Compared with the masks named ground truth delineated manually by radiologists, the model performance on segmentation was evaluated with dice similarity coefficient (DSC) and intraclass correlation coefficient (ICC). The performance of tumor (T) stage classification was evaluated with accuracy, sensitivity, and specificity. Results: In the test cohort, the DSC of automatic segmentation reached 0.89. Excellent concordance (ICC > 0.95) of the maximal and minimal diameter and good concordance (ICC > 0.80) of volumetric measurement were shown between the model and the radiologists. The trained model took approximately 10-15 s to provide automatic segmentation and classified the T stage with an overall accuracy of 0.93, sensitivity of 0.94, 0.94, and 0.75, and specificity of 0.95, 0.92, and 0.99, respectively, in T1, T2, and T3. Conclusions: Our model demonstrated good performance and reliability for automatic segmentation for size and volumetric measurement of breast cancer, which can be time-saving and effective in clinical decision-making.
Deep learning-based segmentation for high-dose-rate brachytherapy in cervical cancer using 3D Prompt-ResUNet

Abstract:Objective. To develop and evaluate a 3D Prompt-ResUNet module that utilized the prompt-based model combined with 3D nnUNet for rapid and consistent autosegmentation of high-risk clinical target volume and organ at risk in high-dose-rate brachytherapy for cervical cancer patients. Approach. We used 73 computed tomography (CT) and 62 magnetic resonance imaging (MRI) scans from 135 (103 for training, 16 for validation, and 16 for testing) cervical cancer patients across two hospitals for HRCTV and OAR segmentation. A novel comparison of the deep learning neural networks 3D Prompt-ResUNet, nnUNet, and SAM-Med3D was applied for the segmentation. Evaluation was conducted in two parts: geometric and clinical assessments. Quantitative metrics included the Dice similarity coefficient (DSC), 95th percentile Hausdorff distance (HD95%), Jaccard index (JI), and Matthews correlation coefficient (MCC). Clinical evaluation involved interobserver comparison, 4-grade expert scoring, and a double-blinded Turing test. Main results. The Prompt-ResUNet model performed most similarly to experienced radiation oncologists, outperforming less experienced ones. During testing, the DSC, HD95% (mm), JI, and MCC value (mean ± SD) for HRCTV were 0.92±0.03, 2.91 ± 0.69, 0.85± 0.04, and 0.92 ± 0.02, respectively. For the bladder, these values were 0.93 ± 0.05, 3.07 ± 1.05, 0.87 ± 0.08, and 0.93 ± 0.05, respectively. For the rectum, they were 0.87 ± 0.03, 3.54 ± 1.46, 0.78 ± 0.05, and 0.87 ± 0.03, respectively. For the sigmoid, they were 0.76 ± 0.11, 7.54 ± 5.54, 0.63 ± 0.14, and 0.78 ± 0.09, respectively. The Prompt-ResUNet achieved a clinical viability score of at least 2 in all evaluation cases (100%) for both HRCTV and bladder and exceeded the 30% positive rate benchmark for all evaluated structures in the Turing test. Significance. The Prompt-ResUNet architecture demonstrated high consistency with ground truth (GT) in autosegmentation of HRCTV and OARs, reducing interobserver variability and shortening treatment times.

Abstract:Purpose To create and evaluate a three‐dimensional (3D) Prompt‐nnUnet module that utilizes the prompts‐based model combined with 3D nnUnet for producing the rapid and consistent autosegmentation of high‐risk clinical target volume (HR CTV) and organ at risk (OAR) in high‐dose‐rate brachytherapy (HDR BT) for patients with postoperative endometrial carcinoma (EC). Methods and materials On two experimental batches, a total of 321 computed tomography (CT) scans were obtained for HR CTV segmentation from 321 patients with EC, and 125 CT scans for OARs segmentation from 125 patients. The numbers of training/validation/test were 257/32/32 and 87/13/25 for HR CTV and OARs respectively. A novel comparison of the deep learning neural network 3D Prompt‐nnUnet and 3D nnUnet was applied for HR CTV and OARs segmentation. Three‐fold cross validation and several quantitative metrics were employed, including Dice similarity coefficient (DSC), Hausdorff distance (HD), 95th percentile of Hausdorff distance (HD95%), and intersection over union (IoU). Results The Prompt‐nnUnet included two forms of parameters Predict‐Prompt (PP) and Label‐Prompt (LP), with the LP performing most similarly to the experienced radiation oncologist and outperforming the less experienced ones. During the testing phase, the mean DSC values for the LP were 0.96 ± 0.02, 0.91 ± 0.02, and 0.83 ± 0.07 for HR CTV, rectum and urethra, respectively. The mean HD values (mm) were 2.73 ± 0.95, 8.18 ± 4.84, and 2.11 ± 0.50, respectively. The mean HD95% values (mm) were 1.66 ± 1.11, 3.07 ± 0.94, and 1.35 ± 0.55, respectively. Abstract:Inter-and intra-observer variation in delineating regions of interest (ROIs) occurs because of differences in expertise level and preferences of the radiation oncologists. We evaluated the accuracy of a segmentation model using the U-Net structure to delineate the prostate, bladder, and rectum in male pelvic CT images. The dataset used for training and testing the model consisted of raw CT scan images of 85 prostate cancer patients. We designed a 2D U-Net model to directly learn a mapping function that converts a 2D CT grayscale image to its corresponding 2D OAR segmented image. Our network contains blocks of convolution 2D layers with variable kernel sizes, channel number, and activation functions. On the left side of the U-Net model, we used three 3x3 convolutions, each followed by a rectified linear unit (ReLu) (activation function), and one max pooling operation. On the right side of the U-Net model, we used a 2x2 transposed convolution and two 3x3 convolution networks followed by a ReLu activation function. The automatic segmentation using the U-Net generated an average dice similarity coefficient (DC) and standard deviation (SD) of the following: DC +- SD (0.88 +- 0.12), (0.95 +- 0.04), and (0.92 +- 0.06) for the prostate, bladder, and rectum, respectively. Furthermore, the mean of average surface Hausdorff distance (ASHD) and SD were 1.2 +- 0.9 mm, 1.08 +- 0.8 mm, and 0.8 +- 0.6 mm for the prostate, bladder, and rectum, respectively. Our proposed method, which employs the U-Net structure, is highly accurate and reproducible for automated ROI segmentation. This provides a foundation to improve automatic delineation of the boundaries between the target and surrounding normal soft tissues on a standard radiation therapy planning CT scan.

Dual convolution-transformer UNet (DCT-UNet) for organs at risk and clinical target volume segmentation in MRI for cervical cancer brachytherapy

Abstract:Objective. MRI is the standard imaging modality for high-dose-rate brachytherapy of cervical cancer. Precise contouring of organs at risk (OARs) and high-risk clinical target volume (HR-CTV) from MRI is a crucial step for radiotherapy planning and treatment. However, conventional manual contouring has limitations in terms of accuracy as well as procedural time. To overcome these, we propose a deep learning approach to automatically segment OARs (bladder, rectum, and sigmoid colon) and HR-CTV from female pelvic MRI. Approach. In the proposed pipeline, a coarse multi-organ segmentation model first segments all structures, from which a region of interest is computed for each structure. Then, each organ is segmented using an organ-specific fine segmentation model separately trained for each organ. To account for variable sizes of HR-CTV, a size-adaptive multi-model approach was employed. For coarse and fine segmentations, we designed a dual convolution-transformer UNet (DCT-UNet) which uses dual-path encoder consisting of convolution and transformer blocks. To evaluate our model, OAR segmentations were compared to the clinical contours drawn by the attending radiation oncologist. For HR-CTV, four sets of contours (clinical + three additional sets) were obtained to produce a consensus ground truth as well as for inter/intra-observer variability analysis. Main results. DCT-UNet achieved dice similarity coefficient (mean±SD) of 0.932±0.032 (bladder), 0.786±0.090 (rectum), 0.663±0.180 (sigmoid colon), and 0.741±0.076 (HR-CTV), outperforming other state-of-the-art models. Notably, the size-adaptive multi-model significantly improved HR-CTV segmentation compared to a single-model. Furthermore, significant inter/intra-observer variability was observed, and our model showed comparable performance to all observers. Computation time for the entire pipeline per subject was 12.59±0.79 seconds, which is significantly shorter than the typical manual contouring time of >15 minutes. Significance. These experimental results demonstrate that our model has great utility in cervical cancer brachytherapy by enabling fast and accurate automatic segmentation, and has potential in improving consistency in contouring.

Comprehensive evaluation of a deep learning model for automatic organs at risk segmentation on heterogeneous computed tomography images for abdominal radiotherapy

Abstract:Purpose To develop a deep learning model (AbsegNet) that produces accurate contours of 16 organs at risk (OARs) for abdominal malignancies, as an essential part of a fully automated radiation treatment planning. Material and methods Three datasets with 544 computed tomography (CT) scans were retrospectively collected. Dataset 1 was split into 300 training cases and 128 test cases (cohort 1) for AbsegNet. Dataset 2, including cohort 2 (n = 24) and cohort 3 (n = 20), were used to validate AbsegNet externally. Dataset 3, including cohort 4 (n = 40) and cohort 5 (n = 32) were used to clinically assess the accuracy of AbsegNet-generated contours. Each cohort was from a different center. The Dice similarity coefficient (DSC) and 95th-percentile Hausdorff distance (HD95) were calculated to evaluate the delineation quality for each OAR. Clinical accuracy evaluation was classified into three levels: no revision, minor revisions (0% < volumetric revision degrees (VRD) ≤ 10%), moderate revisions (10% ≤ VRD < 20%), and major revisions (VRD ≥ 20%). Results For all OARs, AbsegNet achieved a mean DSC of 86.73%, 85.65%, and 88.04% in cohort 1, cohort 2, and cohort 3, respectively, and a mean HD95 of 8.92mm, 10.18mm, and 12.40mm, respectively. The performance of AbsegNet outperformed SwinUNETR, DeepLabV3+, Attention-UNet, UNet and 3D-UNet. When experts evaluated contours from cohorts 4 and 5, four OARs (liver, kidney_L, kidney_R, and spleen) of all patients were scored as having no revision, and over 87.5% of patients with contours of the stomach, esophagus, adrenals, or rectum were considered as having no or minor revisions. Only 15.0% of patients with contours of the colon and small bowel required major revisions. Conclusions We propose a novel deep learning model to delineate OARs on diverse datasets. Most contours produced by AbsegNet are accurate and robust, therefore clinically applicable and helpful to facilitate radiotherapy workflow.

Abstract:Purpose: Image segmentation can be time-consuming and lacks consistency between different oncologists, which is essential in conformal radiotherapy techniques. We aimed to evaluate automatic delineation results generated by convolutional neural networks (CNNs) from geometry and dosimetry perspectives and explore the reliability of these segmentation tools in rectal cancer. Methods: Forty-seven rectal cancer cases treated from February 2018 to April 2019 were randomly collected retrospectively in our cancer center. The oncologists delineated regions of interest (ROIs) on planning CT images as the ground truth, including clinical target volume (CTV), bladder, small intestine, and femoral heads. The corresponding automatic segmentation results were generated by DeepLabv3+ and ResUNet, and we also used Atlas-Based Autosegmentation (ABAS) software for comparison. The geometry evaluation was carried out using the volumetric Dice similarity coefficient (DSC) and surface DSC, and critical dose parameters were assessed based on replanning optimized by clinically approved or automatically generated CTVs and organs at risk (OARs), i.e. , the Plan ref and Plan test . Pearson test was used to explore the correlation between geometric metrics and dose parameters. Results: In geometric evaluation, DeepLabv3+ performed better in DCS metrics for the CTV (volumetric DSC, mean = 0.96, P< 0.01; surface DSC, mean = 0.78, P< 0.01) and small intestine (volumetric DSC, mean = 0.91, P< 0.01; surface DSC, mean = 0.62, P< 0.01), ResUNet had advantages in volumetric DSC of the bladder (mean = 0.97, P< 0.05). For critical dose parameters analysis between Plan ref and Plan test , there was a significant difference for target volumes (P 0.05). For the correlation test, a negative correlation was found between DSC metrics (volumetric, surface DSC) and dosimetric parameters (δD95, δD95, HI, CI) for target volumes (P 0.05). Conclusions: CNNs show remarkable repeatability and time-saving in automatic segmentation, and their accuracy also has a certain potential in clinical practice. Meanwhile, clinical aspects, such as dose distribution, may need to be considered when comparing the performance of auto-segmentation methods.

Abstract:The segmentation of organs at risk is an important part of radiotherapy. The current method of manual segmentation depends on the knowledge and experience of physicians, which is very time-consuming and difficult to ensure the accuracy, consistency and repeatability. Therefore, a deep convolutional neural network (DCNN) is proposed for the automatic and accurate segmentation of head and neck organs at risk. The data of 496 patients with nasopharyngeal carcinoma were reviewed. Among them, 376 cases were randomly selected for training set, 60 cases for validation set and 60 cases for test set. Using the three-dimensional (3D) U-NET DCNN, combined with two loss functions of Dice Loss and Generalized Dice Loss, the automatic segmentation neural network model for the head and neck organs at risk was trained. The evaluation parameters are Dice similarity coefficient and Jaccard distance. The average Dice Similarity coefficient of the 19 organs at risk was 0.91, and the Jaccard distance was 0.15. The results demonstrate that 3D U-NET DCNN combined with Dice Loss function can be better applied to automatic segmentation of head and neck organs at risk.
A Deep Learning-Based Auto-Segmentation System for Organs-at-risk on Whole-Body Computed Tomography Images for Radiation Therapy.

Abstract:Background and purpose: Delineating organs at risk (OARs) on computed tomography (CT) images is an essential step in radiation therapy; however, it is notoriously time-consuming and prone to interobserver variation. Herein, we report a deep learning-based automatic segmentation (AS) algorithm (WBNet) that can accurately and efficiently delineate all major OARs in the entire body directly on CT scans. Materials and methods: We collected 755 CT scans of the head and neck, thorax, abdomen, and pelvis and manually delineated 50 OARs on the CT images. The CT images with contours were split into training and test sets consisting of 505 and 250 cases, respectively, to develop and validate WBNet. The volumetric Dice similarity coefficient (DSC) and 95th-percentile Hausdorff distance (95% HD) were calculated to evaluate delineation quality for each OAR. We compared the performance of WBNet with three AS algorithms: one commercial multi-atlas-based automatic segmentation (ABAS) software, and two deep learning-based AS algorithms, namely, AnatomyNet and nnU-Net. We have also evaluated the time saving and dose accuracy of WBNet. Results: WBNet achieved average DSCs of 0.84 and 0.81 on in-house and public datasets, respectively, which outperformed ABAS, AnatomyNet, and nnU-Net. WBNet could reduce the delineation time significantly and perform well in treatment planning, with clinically acceptable dose differences compared with those in manual delineation. RefineNet-based automatic delineation of the clinical target volume and organs at risk for three-dimensional brachytherapy for cervical cancer

Abstract:BACKGROUND: In recent years, high-precision image-guided intensity-modulated radiation therapy combined with three-dimensional (3D) high-dose-rate (HDR) brachytherapy (BT) has become a recommended technique for radical radiotherapy for cervical cancer. This study first employed contrast-limited adaptive histogram equalization (CLAHE) for preprocessing of input data to achieve image enhancement. In this way, rapid and accurate automatic delineation of the clinical target volume (CTV) and organs at risk (OARs) in 3D BT for cervical cancer was achieved.METHODS: Two hundred cervical cancer patients who underwent radical radiotherapy from January 2016 to December 2018 were selected. After collecting the computed tomography (CT) image data of a patient, we constructed the radiotherapy CTV and OAR image libraries. A RefineNet-based deep learning protocol was used to segment the CTV and OARs for 3D BT for cervical cancer. In this study, a total of 1,000 rounds of training were carried out, and the model with the best performance was selected for subsequent iterative tuning. Finally, the clinical test was carried out, in which the CT images of 10 cases were tested one by one. The manual delineation results and the model output results for the CTV and OARs were compared to measure the performance of the model.RESULTS: Compared with the manually delineated CTV, the RefineNet model-based segmented CTV had a higher Dice similarity coefficient (DSC), Hausdorff distance (HD), and overlap index (OI), which were 0.861, 6.005, and 0.839, respectively. For OARs, the RefineNet-based model obtained the best results for bladder segmentation (DSC: 85.96%), respectively. The mean duration of RefineNet-based automatic contour processing of the CTV was 70 s, and the mean durations of RefineNet-based automatic delineation of the bladder, rectum, sigmoid colon, and small intestine were 67, 67.4, 63.8, and 60.8 s, respectively. The total time saved by RefineNet was approximately 60%.CONCLUSIONS: The RefineNet-based automatic delineation model for 3D BT for cervical cancer is a stable and highly consistent automatic delineation algorithmic model that has the potential to improve the consistency of target region delineation, simplify the radiotherapy procedure, and achieve rapid and accurate automatic delineation of CTVs and OARs.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Purpose</h3><p>Segmentation of the whole breast and fibroglandular tissue (FGT) is important for quantitatively analyzing the breast cancer risk in the dynamic contrast-enhanced magnetic resonance (DCE-MR) images. The purpose of this study is to improve the accuracy and efficiency of the segmentation of the whole breast and FGT in 3-D fat-suppressed DCE-MR images with a versatile deep learning (DL) framework.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>We randomly collected 100 breast DCE-MR scans from Shanghai Cancer Hospital of Fudan University. The MR scans in the dataset were different in both the spatial resolution and the MR scanners employed. Furthermore, four breast density categories were assessed by radiologists based on Breast Imaging Reporting and Data System (BI-RADS) of American College of Radiology. The dataset was separated into the training and the testing sets, while keeping a balanced distribution of scans with different imaging parameters and density categories. The nnU-Net has been recently proposed to automatically adapt preprocessing strategies and network architectures for a given medical image dataset, thus showing a great potential in the systematic adaptation of DL methods to different datasets. In this study, we applied the nnU-Net to segment the whole breast and FGT in 3-D fat-suppressed DCE-MR images. Five-fold cross validation was employed to train and validate the segmentation method.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>The segmentation performance was evaluated with the volume and surface agreement metrics between the DL-based automatic and the manually delineated masks, as quantified with the following measures: the average Dice volume overlap (0.968 <em>±</em> 0.017 and 0.877 <em>±</em> 0.081), the average surface distances (0.201 <em>±</em> 0.080 mm and 0.310 <em>±</em> 0.043 mm), and the Pearson correlation coefficient of masks (0.995 and 0.972) between the automatic and the manually delineated masks, as calculated for the whole breast and the FGT segmentation, respectively. The correlation coefficient between the breast densities obtained with the DL-based segmentation and the manual delineation was 0.981. There was a positive bias of 0.8% (DL-based relative to manual) in breast density measurement with the Bland-Altman plot. The execution time of the DL-based segmentation was approximately 20 s for the whole breast segmentation and 15 s for the FGT segmentation.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>Our DL-based segmentation framework using nnU-Net could robustly achieve high accuracy and efficiency across variable MR imaging settings without extra pre- or post-processing procedures. It would be useful for developing DCE-MR-based CAD systems to quantify breast cancer risk and to be integrated into the clinical workflow.</p>

Abstract:Objective: To evaluate the automatic segmentation approach for organ at risk (OARs) and compare the parameters of dose volume histogram (DVH) in radiotherapy. Methodology: Thirty-three patients were selected to contour OARs using automatic segmentation approach which based on U-Net, applying them to a number of the nasopharyngeal carcinoma (NPC), breast, and rectal cancer respectively. The automatic contours were transferred to the Pinnacle System to evaluate contour accuracy and compare the DVH parameters. Results: The time for manual contour was 56.5 ± 9, 23.12 ± 4.23 and 45.23 ± 2.39min for the OARs of NPC, breast and rectal cancer, and for automatic contour was 1.5 ± 0.23, 1.45 ± 0.78 and 1.8 ± 0.56 min. Automatic contours of Eye with the best Dice-similarity coefficients (DSC) of 0.907 ± 0.02 while with the poorest DSC of 0.459 ± 0.112 of Spinal Cord for NPC; And Lung with the best DSC of 0.944 ± 0.03 while with the poorest DSC of 0.709 ± 0.1 of Spinal Cord for breast; And Bladder with the best DSC of 0.91 ± 0.04 while with the poorest DSC of 0.43 ± 0.1 of Femoral heads for rectal cancer. The contours of Spinal Cord in H & N had poor results due to the division of the medulla oblongata. The contours of Femoral head, which different from what we expect, also due to manual contour result in poor DSC. Conclusion: The automatic contour approach based deep learning method with sufficient accuracy for research purposes. However, the value of DSC does not fully reflect the accuracy of dose distribution, but can cause dose changes due to the changes in the OARs volume and DSC from the data. Considering the significantly time-saving and good performance in partial OARs, the automatic contouring also plays a supervisory role.

Abstract:On-line segmentation of the uterus can aid effective image-based guidance for precise delivery of dose to the target tissue (the uterocervix) during cervix cancer radiotherapy. 3D ultrasound (US) can be used to image the uterus, however, finding the position of uterine boundary in US images is a challenging task due to large daily positional and shape changes in the uterus, large variation in bladder filling, and the limitations of 3D US images such as low resolution in the elevational direction and imaging aberrations. Previous studies on uterus segmentation mainly focused on developing semi-automatic algorithms where require manual initialization to be done by an expert clinician. Due to limited studies on the automatic 3D uterus segmentation, the aim of the current study was to overcome the need for manual initialization in the semi-automatic algorithms using the recent deep learning-based algorithms. Therefore, we developed 2D UNet-based networks that are trained based on two scenarios. In the first scenario, we trained 3 different networks on each plane (i.e., sagittal, coronal, axial) individually. In the second scenario, our proposed network was trained using all the planes of each 3D volume. Our proposed schematic can overcome the initial manual selection of previous semi-automatic algorithm.

Multiscale Local Enhancement Deep Convolutional Networks for the Automated 3D Segmentation of Gross Tumor Volumes in Nasopharyngeal Carcinoma: A Multi-Institutional Dataset Study

Abstract:Purpose Accurate segmentation of gross target volume (GTV) from computed tomography (CT) images is a prerequisite in radiotherapy for nasopharyngeal carcinoma (NPC). However, this task is very challenging due to the low contrast at the boundary of the tumor and the great variety of sizes and morphologies of tumors between different stages. Meanwhile, the data source also seriously affect the results of segmentation. In this paper, we propose a novel three-dimensional (3D) automatic segmentation algorithm that adopts cascaded multiscale local enhancement of convolutional neural networks (CNNs) and conduct experiments on multi-institutional datasets to address the above problems. Materials and Methods In this study, we retrospectively collected CT images of 257 NPC patients to test the performance of the proposed automatic segmentation model, and conducted experiments on two additional multi-institutional datasets. Our novel segmentation framework consists of three parts. First, the segmentation framework is based on a 3D Res-UNet backbone model that has excellent segmentation performance. Then, we adopt a multiscale dilated convolution block to enhance the receptive field and focus on the target area and boundary for segmentation improvement. Finally, a central localization cascade model for local enhancement is designed to concentrate on the GTV region for fine segmentation to improve the robustness. The Dice similarity coefficient (DSC), positive predictive value (PPV), sensitivity (SEN), average symmetric surface distance (ASSD) and 95% Hausdorff distance (HD95) are utilized as qualitative evaluation criteria to estimate the performance of our automated segmentation algorithm. Results The experimental results show that compared with other state-of-the-art methods, our modified version 3D Res-UNet backbone has excellent performance and achieves the best results in terms of the quantitative metrics DSC, PPR, ASSD and HD95, which reached 74.49 ± 7.81%, 79.97 ± 13.90%, 1.49 ± 0.65 mm and 5.06 ± 3.30 mm, respectively. It should be noted that the receptive field enhancement mechanism and cascade architecture can have a great impact on the stable output of automatic segmentation results with high accuracy, which is critical for an algorithm. The final DSC, SEN, ASSD and HD95 values can be increased to 76.23 ± 6.45%, 79.14 ± 12.48%, 1.39 ± 5.44mm, 4.72 ± 3.04mm. In addition, the outcomes of multi-institution experiments demonstrate that our model is robust and generalizable and can achieve good performance through transfer learning. Conclusions The proposed algorithm could accurately segment NPC in CT images from multi-institutional datasets and thereby may improve and facilitate clinical applications.

Abstract:Automatic breast ultrasound (ABUS) imaging provides complementary information when other imaging modalities (i.e., mammography) are not conclusive in the task of tumor detection. It enables sectional plane visualization, simplified temporal comparison and coronal depiction, and features higher reproducibility than conventional ultrasound imaging. Although the 3D ABUS acquisition significantly reduces the acquisition time and cost, the manual segmentation of tumor on 3D ABUS could be time-consuming and labor-intensive due to its high slice number. This work aims to develop a deep-learning-based method to automatically segment the breast tumor on 3D ABUS. We integrated mask scoring-based self-correlation strategy into the R-CNN-based method to force the final segmented tumor contour to be more reasonable. We tested the performance of the proposed method using 3D ABUS of 40 patients who are confirmed with breast tumor through four-fold cross validation test. The comparison between our results and the ground truth contours was quantified by metrics including the Dice similarity coefficient (DSC), Jaccard index (JAC), 95% Hausdorff distance (HD95). The mean DSC, JAC, and HD95 were 0.85 ± 0.10, 0.75 ± 0.14, and 1.65 ± 1.20 (mm), respectively.

Abstract:Radiotherapy has been demonstrated to be one of the most significant treatments for cervical cancer, during which accurate and efficient delineation of target volumes is critical. To alleviate the data demand of deep learning and promote the establishment and promotion of auto-segmentation models in small and medium-sized oncology departments and single centres, we proposed an auto-segmentation algorithm to determine the cervical cancer target volume in small samples based on multi-decoder and semi-supervised learning (MDSSL), and we evaluated the accuracy via an independent test cohort. In this study, we retrospectively collected computed tomography (CT) datasets from 71 pelvic cervical cancer patients, and a 3:4 ratio was used for the training and testing sets. The clinical target volumes (CTVs) of the primary tumour area (CTV1) and pelvic lymph drainage area (CTV2) were delineated. For definitive radiotherapy (dRT), the primary gross target volume (GTVp) was simultaneously delineated. According to the data characteristics for small samples, the MDSSL network structure based on 3D U-Net was established to train the model by combining clinical anatomical information, which was compared with other segmentation methods, including supervised learning (SL) and transfer learning (TL). The dice similarity coefficient (DSC), 95% Hausdorff distance (HD95) and average surface distance (ASD) were used to evaluate the segmentation performance. The ability of the segmentation algorithm to improve the efficiency of online adaptive radiation therapy (ART) was assessed via geometric indicators and a subjective evaluation of radiation oncologists (ROs) in prospective clinical applications. Compared with the SL model and TL model, the proposed MDSSL model displayed the best DSC, HD95 and ASD overall, especially for the GTVp of dRT. We calculated the above geometric indicators in the range of the ground truth (head-foot direction). In the test set, the DSC, HD95 and ASD of the MDSSL model were 0.80/5.85 mm/0.95 mm for CTV1 of post-operative radiotherapy (pRT), 0.84/ 4.88 mm/0.73 mm for CTV2 of pRT, 0.84/6.58 mm/0.89 mm for GTVp of dRT, 0.85/5.36 mm/1.35 mm for CTV1 of dRT, and 0.84/4.09 mm/0.73 mm for CTV2 of dRT, respectively. In a prospective clinical study of online ART, the target volume modification time (MTime) was 3–5 min for dRT and 2–4 min for pRT, and the main duration of CTV1 modification was approximately 2 min. The introduction of the MDSSL method successfully improved the accuracy of auto-segmentation for the cervical cancer target volume in small samples, showed good consistency with RO delineation and satisfied clinical requirements. In this prospective online ART study, the application of the segmentation model was demonstrated to be useful for reducing the target volume delineation time and improving the efficiency of the online ART workflow, which can contribute to the development and promotion of cervical cancer online ART.

Abstract:Accurate delineation of the clinical target volume (CTV) and organs at risk (OARs) in pelvic plays a crucial role in the follow-up radiotherapy of the patient after hysterectomy. However, manual delineation is a time-consuming task which is susceptible to inter-observer variation. Automatic organ delineation is reliant on the global context contained in the extracted feature map to segment the original CT image. Currently, numerous deep learning networks, like U-Net++, are widely used in this task. In this paper, we use a standard U-Net++ as a fundamental structure followed by using dilated convolutions to replace its standard convolutions. We observed that dilated convolutions enlarged the size of the receptive field significantly, fusing more global contextual information and increasing the accuracy of segmenting organs. We evaluate the performance of standard U-Net++ and improved U-Net++(with double-deck dilated convolution) on the multiple organs segmentation in pelvic respectively. Our data set consist of CT images of 70 patients after hysterectomy. We use the dice similarity coefficient to quantify the segmentation accuracy. The result of our experiment demonstrates that the improved U-Net++ outperforms the standard U-Net++ on the segmentation of bladder, CTV and rectum. The dice scores show in follow: 93.2%±4.2% vs 91.2%±3.7% for bladder, 89%±3.6% vs 85%±2.6% for CTV and 87.6%±3.6% vs 84.7%±2.1% for rectum. It shows that the segmentation result of the improved U-Net ++ network is closer to the results manually drawn by the doctor than the standard U-Net++.
