Abstract:Patient-specific clinical target volume (CTV) delineation is the most difficult step in defining the planning target volume from the gross tumor volume. Delineating patient specific CTVs is a labor-intensive process, prone to human error, with poor inter-observer consistency. Automated volumetric expansions have been used to reduce these concerns, but these ignore patient specific anatomic and clinical variables, as well as the clinical experience of the radiation oncologist. We developed an automated CTV delineation system for lung cancer, which was trained to learn the physician’s CTV contour pattern using a deep 3D neural network. Our network took both patients’ CT images and their corresponding internal target volume (ITV) as input, and output CTV delineation. ITVs were contoured by physicians after studying the internal motion of the target from the 4D CT scans. In our study, the network was trained from twenty-five NSCLC patients in right upper lung lobe (RUL). Then, the trained network was tested on four additional datasets from RUL NSCLC patients. Each CTV delineation output by the trained network was compared with the corresponding CTV contoured by physicians. One challenge to train a 3D network for CTV delineation is the high imbalance between the CTV volume and the background volume. Instead of using hyperparameters to balance as existing methods did, we directly optimize a loss function robust to high imbalance. This improves the stability of the model, especially when the balance dynamically changes in images. Three out of four CTV contours that were automatically delineated by our trained model agree very well with the physician’s CTV contour. Only one has visible discrepancies near the apex of the upper lobe, but still looks reasonable. As shown in Table 1, the performance of the trained neural network was also quantitatively evaluated using precision and recall between the predicted CTVs and the ground truth CTVs. The average precision is 0.902, and recall is 0.871. The recall and precision are the fraction of true volume among the total ground-truth and the fraction of true volume among the predicted volume, respectively. The automated CTV delineation was utilized for RUL NSCLC patients using the deep learning method solely based on CT images with manually contoured ITV. On the limited testing samples, the model contoured CTVs reasonably well. Abstract:Background and purpose: Clinical targeted volume (CTV) delineation accounting for the patient-specific microscopic tumor spread can be a difficult step in defining the treatment volume. We developed an intelligent and automated CTV delineation system for locally advanced non-small cell lung carcinoma (NSCLC) to cover the microscopic tumor spread while avoiding organs-at-risk (OAR). Materials and methods: A 3D UNet with a customized loss function was used, which takes both the patients' respiration-correlated ("4D") CT scan and the physician contoured internal gross target volume (iGTV) as inputs, and outputs the CTV delineation. Among the 84 identified patients, 60 were randomly selected to train the network, and the remaining as testing. The model performance was evaluated and compared with cropped expansions using the shape similarities to the physicians' contours (the ground-truth) and the avoidance of critical OARs. Results: On the testing datasets, all model-predicted CTV contours followed closely to the ground truth, and were acceptable by physicians. The average dice score was 0.86. Our model-generated contours demonstrated better agreement with the ground-truth than the cropped 5 mm/8 mm expansion method (median of median surface distance of 1.0 mm vs 1.9 mm/2.0 mm), with a small overlap volume with OARs (0.4 cm(3) for the esophagus and 1.2 cm(3) for the heart). Conclusions: The CTVs generated by our CTV delineation system agree with the physician's contours. This approach demonstrates the capability of intelligent volumetric expansions with the potential to be used in clinical practice.

Abstract:This study aimed to develop an automated delineation method of nasopharynx gross tumor volume (GTVnx) for nasopharyngeal carcinoma (NPC) in computed tomography (CT) image for radiotherapy applications. Inspired by ResNet and SENet's strong ability to extract image features, we proposed a modified version of the 3D U-Net model with Res-blocks and SE-block for delineation of GTVnx. Besides, an automatic pre-processing method was proposed to crop the 3D region of interest (ROI) of GTVnx. Radiotherapy simulation CT images and corresponding manually delineated target of 205 NPC patients diagnosed with stage T1-T4 were used as datasets for training. Automated delineation models were generated based on CT combining contrast-enhanced CT (CE-CT) and CT alone, respectively. We compared the automatic delineation results against the manual delineated contours by radiation oncologists with 5-fold cross-validation to evaluate the performance of the proposed model. We also compared with the framework using 3D CNN and 2D DDNN, respectively. Besides, the model generated by one medical group was assessed against the other two separate medical groups. Precision (PR), Sensitivity (SE), Dice Similarity Coefficient (DSC), Average Symmetric Surface Distance (ASSD), and 95% Hausdorff Distance (HD95) are calculated for quantitative evaluation. Experimental results show that the proposed method outperforms other automatic methods on the CT images. Automated delineation models based on CT combining CE-CT is superior to that based on CT alone. The presented method could be useful and robust for the 3D delineation of GTVnx for NPC in CT images during the planning of radiotherapy.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Purpose</h3><p>Accurately delineating clinical target volumes (CTV) is essential for completing radiotherapy plans but is time‐consuming, labor‐intensive and prone to inter‐observer variation. Automating CTV delineation has the benefits of both speeding up contouring process and improving the quality of contours. Recently, auto‐segmentation approaches based on deep learning have achieved some improvements. However, unlike organ segmentation, the CTV contains potential tumor spread tissues or subclinical disease tissues, resulting in poorly defined margin interface and irregular shape. It is not reasonable to directly apply the deep learning segmentation algorithms to CTV tasks without considering the unique characteristics of shape and margin. In this work, we propose a novel automatic CTV delineation algorithm based on deep learning addressing the unique shape and margin challenges.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>Our deep learning method, called RA‐CTVNet, segments the CTV from cervical cancer CT images. RA‐CTVNet denotes our automatic CTV delineation algorithm based on deep learning with Area‐aware reweight strategy and Recursive refinement strategy. (1) In order to process the whole‐volume CT images and delineate all CTVs in one shot, our method is built upon the popular 3D Unet architecture. We further extend it with robust residual learning and squeeze‐and‐excitation blocks for better feature representation.(2) We propose area‐aware reweight strategy which assigns different weights for different slices. The core is adjusting model's attention to each slice. (3) In terms of the trade‐off between providing performance improvements and meeting the limitations of GPU memory, we exploit a new recursive refinement strategy to address margin challenge.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>This retrospective study included 462 patients diagnosed with cervical cancer who received radiotherapy from June 2017 to May 2019. Extensive experiments were conducted to evaluate performance of RA‐CTVNet. First, compared to different network architectures, RA‐CTVNet achieved improvements in Dice similarity coefficient (DSC). Second, we conducted ablation study. The results showed that compared to the backbone, area‐aware reweight strategy increased DSC by 3.3% on average and recursive refinement strategy further increased DSC by 1.6% on average. Then, we compared our method with three human experts. Our RA‐CTVNet performed better than two experts while comparably to the third expert. Finally, multicenter evaluation was conducted to verify the accuracy and generalizability.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>Our findings show that deep learning is able to offer an efficient frame‐work for automatic CTV delineation. The tailored RA‐CTVNet can improve the quality of CTV contours, which has great potential for reducing the burden of experts and increasing the accuracy of delineation. In the future, if with more training data, further improvements are possible, bringing this approach closer to real clinical practice.</p></section>

Deep learning based clinical target volumes contouring for prostate cancer: Easy and efficient application

Abstract:Background: Radiotherapy has been crucial in prostate cancer treatment. However, manual segmentation is labor intensive and highly variable among radiation oncologists. In this study, a deep learning based automated contouring model is constructed for clinical target volumes (CTVs) of intact and postoperative prostate cancer. Methods: Computed tomography (CT) data sets of 197 prostate cancer patients were collected. Two auto-delineation models were built for radical radiotherapy and postoperative radiotherapy of prostate cancer respectively, and each model included CTVn for pelvic lymph nodes and CTVp for prostate tumors or prostate tumor beds. Results: In the radical radiotherapy model, the volumetric dice (VD) coefficient of CTVn calculated by AI, was higher than that of the one delineated by the junior physicians (0.85 vs. 0.82, p = 0.018); In the postoperative radiotherapy model, the quantitative parameter of CTVn and CTVp, counted by AI, was better than that of the junior physicians. The median delineation time for AI was 0.23 min in the postoperative model and 0.26 min in the radical model, which were significantly shorter than those of the physicians (50.40 and 45.43 min, respectively, p < 0.001). The correction time of the senior physician for AI was much shorter compared with that for the junior physicians in both models (p < 0.001). Conclusion: Using deep learning and attention mechanism, a highly consistent and time-saving contouring model was built for CTVs of pelvic lymph nodes and prostate tumors or prostate tumor beds for prostate cancer, which also might be a good approach to train junior radiation oncologists.
Clinical evaluation on automatic segmentation results of convolutional neural networks in rectal cancer radiotherapy

Abstract:Purpose: Image segmentation can be time-consuming and lacks consistency between different oncologists, which is essential in conformal radiotherapy techniques. We aimed to evaluate automatic delineation results generated by convolutional neural networks (CNNs) from geometry and dosimetry perspectives and explore the reliability of these segmentation tools in rectal cancer. Methods: Forty-seven rectal cancer cases treated from February 2018 to April 2019 were randomly collected retrospectively in our cancer center. The oncologists delineated regions of interest (ROIs) on planning CT images as the ground truth, including clinical target volume (CTV), bladder, small intestine, and femoral heads. The corresponding automatic segmentation results were generated by DeepLabv3+ and ResUNet, and we also used Atlas-Based Autosegmentation (ABAS) software for comparison. The geometry evaluation was carried out using the volumetric Dice similarity coefficient (DSC) and surface DSC, and critical dose parameters were assessed based on replanning optimized by clinically approved or automatically generated CTVs and organs at risk (OARs), i.e. , the Plan ref and Plan test . Pearson test was used to explore the correlation between geometric metrics and dose parameters. Results: In geometric evaluation, DeepLabv3+ performed better in DCS metrics for the CTV (volumetric DSC, mean = 0.96, P< 0.01; surface DSC, mean = 0.78, P< 0.01) and small intestine (volumetric DSC, mean = 0.91, P< 0.01; surface DSC, mean = 0.62, P< 0.01), ResUNet had advantages in volumetric DSC of the bladder (mean = 0.97, P< 0.05). For critical dose parameters analysis between Plan ref and Plan test , there was a significant difference for target volumes (P 0.05). For the correlation test, a negative correlation was found between DSC metrics (volumetric, surface DSC) and dosimetric parameters (δD95, δD95, HI, CI) for target volumes (P 0.05). Conclusions: CNNs show remarkable repeatability and time-saving in automatic segmentation, and their accuracy also has a certain potential in clinical practice. Meanwhile, clinical aspects, such as dose distribution, may need to be considered when comparing the performance of auto-segmentation methods.

Abstract:In post-operative radiotherapy for prostate cancer, the cancerous prostate gland has been surgically removed, so the clinical target volume (CTV) to be irradiated encompasses the microscopic spread of tumor cells, which cannot be visualized in typical clinical images such as computed tomography or magnetic resonance imaging. In current clinical practice, physicians segment CTVs manually based on their relationship with nearby organs and other clinical information, per clinical guidelines. Automating post-operative prostate CTV segmentation with traditional image segmentation methods has been a major challenge. Here, we propose a deep learning model to overcome this problem by segmenting nearby organs first, then using their relationship with the CTV to assist CTV segmentation. The model proposed is trained using labels clinically approved and used for patient treatment, which are subject to relatively large inter-physician variations due to the absence of a visual ground truth. The model achieves an average Dice similarity coefficient (DSC) of 0.87 on a holdout dataset of 50 patients, much better than established methods, such as atlas-based methods (DSC<0.7). The uncertainties associated with automatically segmented CTV contours are also estimated to help physicians inspect and revise the contours, especially in areas with large inter-physician variations. We also use a 4-point grading system to show that the clinical quality of the automatically segmented CTV contours is equal to that of approved clinical contours manually drawn by physicians.

Abstract:Purpose: The aim of this study is to develop a practicable automatic clinical target volume (CTV) delineation method for radiotherapy of breast cancer after modified radical mastectomy. Methods: Unlike breast conserving surgery, the radiotherapy CTV for modified radical mastectomy involves several regions, including CTV in the chest wall ( CTV cw ), supra- and infra-clavicular region ( CTV sc ), and internal mammary lymphatic region ( CTV im ). For accurate and efficient segmentation of the CTVs in radiotherapy of breast cancer after modified radical mastectomy, a multi-scale convolutional neural network with an orientation attention mechanism is proposed to capture the corresponding features in different perception fields. A channel-specific local Dice loss, alongside several data augmentation methods, is also designed specifically to stabilize the model training and improve the generalization performance of the model. The segmentation performance is quantitatively evaluated by statistical metrics and qualitatively evaluated by clinicians in terms of consistency and time efficiency. Results: The proposed method is trained and evaluated on the self-collected dataset, which contains 110 computed tomography scans from patients with breast cancer who underwent modified mastectomy. The experimental results show that the proposed segmentation method achieved superior performance in terms of Dice similarity coefficient (DSC), Hausdorff distance (HD) and Average symmetric surface distance (ASSD) compared with baseline approaches. Conclusion: Both quantitative and qualitative evaluation results demonstrated that the specifically designed method is practical and effective in automatic contouring of CTVs for radiotherapy of breast cancer after modified radical mastectomy. Clinicians can significantly save time on manual delineation while obtaining contouring results with high consistency by employing this method.

Evaluation of Deep Learning‐based Auto‐segmentation Algorithms for Delineating Clinical Target Volume and Organs at Risk Involving Data for 125 Cervical Cancer Patients

Abstract:Objective To evaluate the accuracy of a deep learning-based auto-segmentation mode to that of manual contouring by one medical resident, where both entities tried to mimic the delineation "habits" of the same clinical senior physician. Methods This study included 125 cervical cancer patients whose clinical target volumes (CTVs) and organs at risk (OARs) were delineated by the same senior physician. Of these 125 cases, 100 were used for model training and the remaining 25 for model testing. In addition, the medical resident instructed by the senior physician for approximately 8 months delineated the CTVs and OARs for the testing cases. The dice similarity coefficient (DSC) and the Hausdorff Distance (HD) were used to evaluate the delineation accuracy for CTV, bladder, rectum, small intestine, femoral-head-left, and femoral-head-right. Results The DSC values of the auto-segmentation model and manual contouring by the resident were, respectively, 0.86 and 0.83 for the CTV (P < 0.05), 0.91 and 0.91 for the bladder (P > 0.05), 0.88 and 0.84 for the femoral-head-right (P < 0.05), 0.88 and 0.84 for the femoral-head-left (P < 0.05), 0.86 and 0.81 for the small intestine (P < 0.05), and 0.81 and 0.84 for the rectum (P > 0.05). The HD (mm) values were, respectively, 14.84 and 18.37 for the CTV (P < 0.05), 7.82 and 7.63 for the bladder (P > 0.05), 6.18 and 6.75 for the femoral-head-right (P > 0.05), 6.17 and 6.31 for the femoral-head-left (P > 0.05), 22.21 and 26.70 for the small intestine (P > 0.05), and 7.04 and 6.13 for the rectum (P > 0.05). The auto-segmentation model took approximately 2 min to delineate the CTV and OARs while the resident took approximately 90 min to complete the same task. Conclusion The auto-segmentation model was as accurate as the medical resident but with much better efficiency in this study. Furthermore, the auto-segmentation approach offers additional perceivable advantages of being consistent and ever improving when compared with manual approaches.
Automatic delineation of cervical cancer target volumes in small samples based on multi-decoder and semi-supervised learning and clinical application

Abstract:Radiotherapy has been demonstrated to be one of the most significant treatments for cervical cancer, during which accurate and efficient delineation of target volumes is critical. To alleviate the data demand of deep learning and promote the establishment and promotion of auto-segmentation models in small and medium-sized oncology departments and single centres, we proposed an auto-segmentation algorithm to determine the cervical cancer target volume in small samples based on multi-decoder and semi-supervised learning (MDSSL), and we evaluated the accuracy via an independent test cohort. In this study, we retrospectively collected computed tomography (CT) datasets from 71 pelvic cervical cancer patients, and a 3:4 ratio was used for the training and testing sets. The clinical target volumes (CTVs) of the primary tumour area (CTV1) and pelvic lymph drainage area (CTV2) were delineated. For definitive radiotherapy (dRT), the primary gross target volume (GTVp) was simultaneously delineated. According to the data characteristics for small samples, the MDSSL network structure based on 3D U-Net was established to train the model by combining clinical anatomical information, which was compared with other segmentation methods, including supervised learning (SL) and transfer learning (TL). The dice similarity coefficient (DSC), 95% Hausdorff distance (HD95) and average surface distance (ASD) were used to evaluate the segmentation performance. The ability of the segmentation algorithm to improve the efficiency of online adaptive radiation therapy (ART) was assessed via geometric indicators and a subjective evaluation of radiation oncologists (ROs) in prospective clinical applications. Compared with the SL model and TL model, the proposed MDSSL model displayed the best DSC, HD95 and ASD overall, especially for the GTVp of dRT. We calculated the above geometric indicators in the range of the ground truth (head-foot direction). In the test set, the DSC, HD95 and ASD of the MDSSL model were 0.80/5.85 mm/0.95 mm for CTV1 of post-operative radiotherapy (pRT), 0.84/ 4.88 mm/0.73 mm for CTV2 of pRT, 0.84/6.58 mm/0.89 mm for GTVp of dRT, 0.85/5.36 mm/1.35 mm for CTV1 of dRT, and 0.84/4.09 mm/0.73 mm for CTV2 of dRT, respectively. In a prospective clinical study of online ART, the target volume modification time (MTime) was 3–5 min for dRT and 2–4 min for pRT, and the main duration of CTV1 modification was approximately 2 min. The introduction of the MDSSL method successfully improved the accuracy of auto-segmentation for the cervical cancer target volume in small samples, showed good consistency with RO delineation and satisfied clinical requirements. In this prospective online ART study, the application of the segmentation model was demonstrated to be useful for reducing the target volume delineation time and improving the efficiency of the online ART workflow, which can contribute to the development and promotion of cervical cancer online ART.

Abstract:Purpose To propose a novel deep-learning-based auto-segmentation model for CTV delineation in cervical cancer and to evaluate whether it can perform comparably well to manual delineation by a three-stage multicenter evaluation framework. Methods An adversarial deep-learning-based auto-segmentation model was trained and configured for cervical cancer CTV contouring using CT data from 237 patients. Then CT scans of additional 20 consecutive patients with locally advanced cervical cancer were collected to perform a three-stage multicenter randomized controlled evaluation involving nine oncologists from six medical centers. This evaluation system is a combination of objective performance metrics, radiation oncologist assessment, and finally the head-to-head Turing imitation test. Accuracy and effectiveness were evaluated step by step. The intra-observer consistency of each oncologist was also tested. Results In stage-1 evaluation, the mean DSC and the 95HD value of the proposed model were 0.88 and 3.46 mm, respectively. In stage-2, the oncologist grading evaluation showed the majority of AI contours were comparable to the GT contours. The average CTV scores for AI and GT were 2.68 vs. 2.71 in week 0 (P = .206), and 2.62 vs. 2.63 in week 2 (P = .552), with no significant statistical differences. In stage-3, the Turing imitation test showed that the percentage of AI contours, which were judged to be better than GT contours by >= 5 oncologists, was 60.0% in week 0 and 42.5% in week 2. Most oncologists demonstrated good consistency between the 2 weeks (P > 0.05). Conclusions The tested AI model was demonstrated to be accurate and comparable to the manual CTV segmentation in cervical cancer patients when assessed by our three-stage evaluation framework.

Abstract:Background: The current clinical workflow for esophageal gross tumor volume (GTV) contouring relies on manual delineation of high labor-costs and interuser variability. Purpose: To validate the clinical applicability of a deep learning (DL) multi-modality esophageal GTV contouring model, developed at 1 institution whereas tested at multiple ones. Methods and Materials: We collected 606 esophageal cancer patients from four institutions. 252 institution-1 patients had a treatment planning-CT (pCT) and a pair of diagnostic FDG-PETCT; 354 patients from other 3 institutions had only pCT. A two-streamed DL model for GTV segmentation was developed using pCT and PETCT scans of a 148 patient institution-1 subset. This built model had the flexibility of segmenting GTVs via only pCT or pCT+PETCT combined. For independent evaluation, the rest 104 institution-1 patients behaved as unseen internal testing, and 354 institutions 2-4 patients were used for external testing. We evaluated manual revision degrees by human experts to assess the contour-editing effort. The performance of the deep model was compared against 4 radiation oncologists in a multiuser study with 20 random external patients. Contouring accuracy and time were recorded for the pre-and post-DL assisted delineation process. Results: Our model achieved high segmentation accuracy in internal testing (mean Dice score: 0.81 using pCT and 0.83 using pCT+PET) and generalized well to external evaluation (mean DSC: 0.80). Expert assessment showed that the predicted contours of 88% patients need only minor or no revision. In multi-user evaluation, with the assistance of a deep model, inter-observer variation and required contouring time were reduced by 37.6% and 48.0%, respectively. Conclusions: Deep learning predicted GTV contours were in close agreement with the ground truth and could be adopted clinically with mostly minor or no changes.

Abstract:Purpose To explore the accuracy and feasibility of a proposed deep learning (DL) algorithm for clinical target volume (CTV) delineation in cervical cancer radiotherapy and evaluate whether it can perform well in external cervical cancer and endometrial cancer cases for generalization validation. Methods A total of 332 patients were enrolled in this study. A state‐of‐the‐art network called ResCANet, which added the cascade multi‐scale convolution in the skip connections to eliminate semantic differences between different feature layers based on ResNet‐UNet. The atrous spatial pyramid pooling in the deepest feature layer combined the semantic information of different receptive fields without losing information. A total of 236 cervical cancer cases were randomly grouped into 5‐fold cross‐training (n = 189) and validation (n = 47) cohorts. External validations were performed in a separate cohort of 54 cervical cancer and 42 endometrial cancer cases. The performances of the proposed network were evaluated by dice similarity coefficient (DSC), sensitivity (SEN), positive predictive value (PPV), 95% Hausdorff distance (95HD), and oncologist clinical score when comparing them with manual delineation in validation cohorts. Results In internal validation cohorts, the mean DSC, SEN, PPV, 95HD for ResCANet achieved 74.8%, 81.5%, 73.5%, and 10.5 mm. In external independent validation cohorts, ResCANet achieved 73.4%, 72.9%, 75.3%, 12.5 mm for cervical cancer cases and 77.1%, 81.1%, 75.5%, 10.3 mm for endometrial cancer cases, respectively. The clinical assessment score showed that minor and no revisions (delineation time was shortened to within 30 min) accounted for about 85% of all cases in DL‐aided automatic delineation. Conclusions We demonstrated the problem of model generalizability for DL‐based automatic delineation. The proposed network can improve the performance of automatic delineation for cervical cancer and shorten manual delineation time at no expense to quality. The network showed excellent clinical viability, which can also be even generalized for endometrial cancer with excellent performance.

Abstract:Purpose: In clinical work, accurately measuring the volume and the size of breast cancer is significant to develop a treatment plan. However, it is time-consuming, and inter- and intra-observer variations among radiologists exist. The purpose of this study was to assess the performance of a Res-UNet convolutional neural network based on automatic segmentation for size and volumetric measurement of mass enhancement breast cancer on magnetic resonance imaging (MRI). Materials and methods: A total of 1,000 female breast cancer patients who underwent preoperative 1.5-T dynamic contrast-enhanced MRI prior to treatment were selected from January 2015 to October 2021 and randomly divided into a training cohort (n = 800) and a testing cohort (n = 200). Compared with the masks named ground truth delineated manually by radiologists, the model performance on segmentation was evaluated with dice similarity coefficient (DSC) and intraclass correlation coefficient (ICC). The performance of tumor (T) stage classification was evaluated with accuracy, sensitivity, and specificity. Results: In the test cohort, the DSC of automatic segmentation reached 0.89. Excellent concordance (ICC > 0.95) of the maximal and minimal diameter and good concordance (ICC > 0.80) of volumetric measurement were shown between the model and the radiologists. The trained model took approximately 10-15 s to provide automatic segmentation and classified the T stage with an overall accuracy of 0.93, sensitivity of 0.94, 0.94, and 0.75, and specificity of 0.95, 0.92, and 0.99, respectively, in T1, T2, and T3. Conclusions: Our model demonstrated good performance and reliability for automatic segmentation for size and volumetric measurement of breast cancer, which can be time-saving and effective in clinical decision-making.

Abstract:Purpose We developed a deep learning model to achieve automatic multitarget delineation on planning CT (pCT) and synthetic CT (sCT) images generated from cone-beam CT (CBCT) images. The geometric and dosimetric impact of the model was evaluated for breast cancer adaptive radiation therapy. Methods We retrospectively analyzed 1,127 patients treated with radiotherapy after breast-conserving surgery from two medical institutions. The CBCT images for patient setup acquired utilizing breath-hold guided by optical surface monitoring system were used to generate sCT with a generative adversarial network. Organs at risk (OARs), clinical target volume (CTV), and tumor bed (TB) were delineated automatically with a 3D U-Net model on pCT and sCT images. The geometric accuracy of the model was evaluated with metrics, including Dice similarity coefficient (DSC) and 95% Hausdorff distance (HD95). Dosimetric evaluation was performed by quick dose recalculation on sCT images relying on gamma analysis and dose-volume histogram (DVH) parameters. The relationship between ΔD95, ΔV95 and DSC-CTV was assessed to quantify the clinical impact of the geometric changes of CTV. Results The ranges of DSC and HD95 were 0.73–0.97 and 2.22–9.36 mm for pCT, 0.63–0.95 and 2.30–19.57 mm for sCT from institution A, 0.70–0.97 and 2.10–11.43 mm for pCT from institution B, respectively. The quality of sCT was excellent with an average mean absolute error (MAE) of 71.58 ± 8.78 HU. The mean gamma pass rate (3%/3 mm criterion) was 91.46 ± 4.63%. DSC-CTV down to 0.65 accounted for a variation of more than 6% of V95 and 3 Gy of D95. DSC-CTV up to 0.80 accounted for a variation of less than 4% of V95 and 2 Gy of D95. The mean ΔD90/ΔD95 of CTV and TB were less than 2Gy/4Gy, 4Gy/5Gy for all the patients. The cardiac dose difference in left breast cancer cases was larger than that in right breast cancer cases. Conclusions The accurate multitarget delineation is achievable on pCT and sCT via deep learning. The results show that dose distribution needs to be considered to evaluate the clinical impact of geometric variations during breast cancer radiotherapy.

Abstract:Purpose To develop and externally validate an automatic Artificial Intelligence (AI) tool for delineating gross tumor volume (GTV) in esophageal squamous cell carcinoma (ESCC) patients, which can assist in the neo-adjuvant or radical radiation therapy treatment planning. Methods and Materials In this multi-institutional study, contrast-enhanced CT images from 580 eligible ESCC patients were retrospectively collected. The GTV contours delineated by two experts via consensus were used as ground truth. A three-dimensional deep learning model was developed for GTV contouring in the training cohort and internally and externally validated in three validation cohorts. The AI tool was compared against twelve board-certified experts in 25 patients randomly selected from the external validation cohort to evaluate its assistance in improving contouring performance and reducing variation. Contouring performance was measured using dice similarity coefficient (DSC) and average surface distance (ASD). Additionally, our previously established radiomics model for predicting pathological complete response (pCR) was utilized to compare AI-generated and ground truth contours, in order to assess the potential of the AI contouring tool in radiomics analysis. Results The AI tool demonstrated good GTV contouring performance in multi-center validation cohorts, with median DSC values of 0.865, 0.876, and 0.866, and median ASD values of 0.939 mm, 0.789 mm, and 0.875 mm, respectively. Furthermore, the AI tool significantly improved contouring performance for half of twelve board-certified experts (DSC values, 0.794-0.835 vs 0.856-0.881, P = 0.003-0.048), reduced the intra- and inter-observer variations by 37.4% and 55.2%, respectively, and saved contouring time by 77.6%. In the radiomics analysis, 88.7% of radiomic features from ground truth and AI-generated contours demonstrated stable reproducibility, and similar pCR prediction performance for these contours ( P = 0.430) were observed. Conclusions Our AI contouring tool can improve GTV contouring performance and facilitate radiomics analysis in ESCC patients, which indicated its potential on GTV contouring during radiation therapy treatment planning and radiomics studies.

Abstract:OBJECTIVES: Because radiotherapy is indispensible for treating cervical cancer, it is critical to accurately and efficiently delineate the radiation targets. We evaluated a deep learning (DL)-based auto-segmentation algorithm for automatic contouring of clinical target volumes (CTVs) in cervical cancers.METHODS: Computed tomography (CT) datasets from 535 cervical cancers treated with definitive or postoperative radiotherapy were collected. A DL tool based on VB-Net was developed to delineate CTVs of the pelvic lymph drainage area (dCTV1) and parametrial area (dCTV2) in the definitive radiotherapy group. The training/validation/test number is 157/20/23. CTV of the pelvic lymph drainage area (pCTV1) was delineated in the postoperative radiotherapy group. The training/validation/test number is 272/30/33. Dice similarity coefficient (DSC), mean surface distance (MSD), and Hausdorff distance (HD) were used to evaluate the contouring accuracy. Contouring times were recorded for efficiency comparison.RESULTS: The mean DSC, MSD, and HD values for our DL-based tool were 0.88/1.32 mm/21.60 mm for dCTV1, 0.70/2.42 mm/22.44 mm for dCTV2, and 0.86/1.15 mm/20.78 mm for pCTV1. Only minor modifications were needed for 63.5% of auto-segmentations to meet the clinical requirements. The contouring accuracy of the DL-based tool was comparable to that of senior radiation oncologists and was superior to that of junior/intermediate radiation oncologists. Additionally, DL assistance improved the performance of junior radiation oncologists for dCTV2 and pCTV1 contouring (mean DSC increases: 0.20 for dCTV2, 0.03 for pCTV1; mean contouring time decrease: 9.8 min for dCTV2, 28.9 min for pCTV1).CONCLUSIONS: DL-based auto-segmentation improves CTV contouring accuracy, reduces contouring time, and improves clinical efficiency for treating cervical cancer.

Abstract:The delineation of the clinical target volumes (CTVs) for radiation therapy is time-consuming, requires intensive training and shows high inter-observer variability. Supervised deep-learning methods depend heavily on consistent training data; thus, State-of-the-Art research focuses on making CTV labels more homogeneous and strictly bounding them to current standards. International consensus expert guidelines standardize CTV delineation by conditioning the extension of the clinical target volume on the surrounding anatomical structures. Training strategies that directly follow the construction rules given in the expert guidelines or the possibility of quantifying the conformance of manually drawn contours to the guidelines are still missing. Seventy-one anatomical structures that are relevant to CTV delineation in head- and neck-cancer patients, according to the expert guidelines, were segmented on 104 computed tomography scans, to assess the possibility of automating their segmentation by State-of-the-Art deep learning methods. All 71 anatomical structures were subdivided into three subsets of non-overlapping structures, and a 3D nnU-Net model with five-fold cross-validation was trained for each subset, to automatically segment the structures on planning computed tomography scans. We report the DICE, Hausdorff distance and surface DICE for 71 + 5 anatomical structures, for most of which no previous segmentation accuracies have been reported. For those structures for which prediction values have been reported, our segmentation accuracy matched or exceeded the reported values. The predictions from our models were always better than those predicted by the TotalSegmentator. The sDICE with 2 mm margin was larger than 80% for almost all the structures. Individual structures with decreased segmentation accuracy are analyzed and discussed with respect to their impact on the CTV delineation following the expert guidelines. No deviation is expected to affect the rule-based automation of the CTV delineation.

Abstract:Accurate delineation of clinical target volume (CTV) is one of the most crucial aspects of treatment planning in radiation therapy. However, the quality of this process depends on the expertise level of the individual observer. This study aimed to investigate whether deep-learning based automatic delineation could provide greater accuracy, inter-observer consistency and efficiency of CTV contour compared with manual delineation for pN2 NSCLC receiving post-operation radiation therapy (PORT). A very deep dilated residual network was used to achieve the effective automatic delineation of CTV. The dilated module with different atrous rates is able to extract multi-scale features from CT, leading to a greater robustness of the model. Eleven junior radiation oncologists (work experience ≤ 5 years) delineated CTV on 19 patients using two contour methods: (1) a manual contour from scratch (MC) and (2) a user adjustment of the deep-learning based auto-delineation contour (UADC). Three senior radiation oncologists (work experience > 10 years) also contoured the CTV on these patients and the majority voting was used to generate the ground truth (GT). The accuracy of junior’s delineation was evaluated in terms of mean distance to agreement (MDA) and Dice similarity coefficient (DSC) with the GT as reference. MDA is a distance parameter while DSC is a volume overlap index. Therefore, smaller MDA and higher DSC indicate a greater contour accuracy. The coefficient of variation (COV) was rendered to measure the inter-observer variability. COV is defined as the standard deviation (SD) divided by the mean CTV volume of all observers and a larger COV implies a greater variability. For each of the CTV contouring tasks, the time consumption was recorded. A total of 418 unique CTV sets were generated. The UADC presented a significantly smaller MDA (mm) per individual CTV set compared with MC (mean ± SD: 2.79 ± 0.91 vs. 3.07 ± 0.98, P < 0.001). Similarly, the DSC of UADC was also significantly greater than that of MC (mean ± SD: 0.75 ± 0.06 vs. 0.72 ± 0.07, P < 0.001). For inter-observer variability, UADC introduced a remarkably decreased COV in comparison with MC (mean ± SD: 0.129 ± 0.040 vs. 0.183 ± 0.043, P < 0.001). The median contouring time for MC and UADC was 14.99 min (P25, P75: 9.80, 20.87) and 10.42 min (P25, P75: 7.20, 17.03) respectively, resulting in a 31% reduction (absolute: 4.57 min) of time consumption (P < 0.001). Compared with the manual contour, user adjustment of deep-learning based auto-delineation of PORT-CTV for pN2 NSCLC is a promising strategy to offer superior accuracy and consistency within a shortened time span, leading to higher quality and efficiency of CTV contouring.
Automatic delineation of the clinical target volume and organs at risk by deep learning for rectal cancer postoperative radiotherapy

Abstract:Background and purpose: Manual delineation of clinical target volumes (CTVs) and organs at risk (OARs) is time-consuming, and automatic contouring tools lack clinical validation. We aimed to construct and validate the use of convolutional neural networks (CNNs) to set better contouring standards for rectal cancer radiotherapy. Materials and methods: We retrospectively collected and evaluated computed tomography (CT) scans of 199 rectal cancer patients treated at our hospital from February 2018 to April 2019. Two CNNs-DeepLabv3+ for extracting high-level semantic information and ResUNet for extracting low-level visual features-were used for the CTV and small intestine contouring, and bladder and femoral head contouring, respectively. Contouring quality was compared using the paired t test. Five-point objective grading was performed independently by two experienced radiation oncologists and verified by a third. The CNN manual correction time was recorded. Results: CTVs calculated using DeepLabv3+ (CTVDeepLabv3+) had significant quantitative parameter advantages over CTVResUNet (volumetric Dice coefficient, 0.88 vs 0.87, P = 0.0005; surface Dice coefficient, 0.79 vs 0.78, P = 0.008). Among 315 graded cases, DeepLabv3+ obtained the highest scores with 284 cases, consistent with the objective criteria, whereas CTVResUNet had the minimum mean manual correction time (7.29 min). DeepLabv3+ performed better than ResUNet for small intestine contouring and ResUNet performed better for bladder and femoral head contouring. The manual correction time for OARs was <4 min for both models. Conclusion: CNNs at various feature resolution levels well delineate rectal cancer CTVs and OARs, displaying high quality and requiring shorter computation and manual correction time.
