Abstract:Objective To investigate the correlation between adult dental anxiety and oral health status, and access the related factors contribute to dental anxiety. Methods A total of 1 203 dental patients completed a questionnaire that contained Corah” s dental anxiety scales ( DAS ) and dental fear survey ( DFS ) . The general characteristics were de-scribed, such as socio-demographic, dental attendances and oral health status. Results The prevalence of high dental anxiety ( DAS≥13) was 23. 4%. The regression analysis indicated that gender, age, education, dental attendances and oral health status were correlated with dental anxiety. The level of dental anxiety of patients with regular scaling was sig-nificant decreased. Patients with ‘always bleeding on brushing” and ‘always dentine hypersensitivity” had significantly higher level of dental anxiety. ‘Drilling with the handpiece” and ‘injecting the anesthetic” were the most important fac-tors to contribute to dental anxiety. Conclusion There was high prevalence of dental anxiety in Chinese adult patients. The level of dental anxiety may influence by oral health status and awareness.
Dental Fear and Its Possible Relationship with Periodontal Status in Chinese Adults: A Preliminary Study

Abstract:Background: The aim of the present study was to describe the characteristics of dental fear of Chinese adult patients with periodontal disease and provide information for clinical assessment.Methods: A total of 1203 dental patients completed questionnaires that included Corach's Dental Anxiety Scales (DAS), Dental Fear Survey (DFS) and the short-form Dental Anxiety Inventory (S-DAI). Among all the patients, 366 cases were self-reported periodontal disease. The general characteristics were described, such as socio-demographics, dental attendances and oral health behaviors. The statistical analysis was performed by t-test, Mann-Whitney U test and linear regression respectively to evaluate correlations between dental fear and general characteristics according to the three scales.Results: The prevalence of dental fear was 74% among 1203 patients, 23.4% of total with high dental fear, while 27.3% in the patients with periodontal disease. The average score of DAS and DFS for patients with periodontal disease was significantly higher than those without periodontal disease. The regression analysis indicated that gender, age, periodontal status, dental attendances and oral health behaviors were correlated with dental fear. Among 366 patients with periodontal disease, gender, dental attendances and oral health behaviors had correlation with dental fear. The analysis of DFS scale exhibited that 'drilling with handpiece' and 'injecting the anesthetic' were the most important factors to contribute to dental fear.Conclusions: There was high prevalence of dental fear in Chinese adult patients, particularly in patients with periodontal disease, and high level of dental fear may lead to poor periodontal status.

Abstract:[Objective]To determine the prevalence of dental anxiety(DA) in the elderly in old-age care institutions.[Methods]The prevalence of DA in 140 elders was investigated by using the modified Dental Anxiety Scale(MDAS).[Results]The mean MDAS score was 6.8071±3.2848 in the elderly in old-age care institutions.The prevalence rate of DA was 7.86%.Among the elders,there was significant difference for the comparison of MDAS scores in degree of education,different pain tolerance and experiences of dental care.The order of factors about MDAS scores were(from high to low):pain tolerance,age,mental state,degree of education,character,experiences of dental care,oral hygiene,family financial situations, relatives experiences of dental care,gender and health situation.The pain tolerance had highly significant influence among these factors. [Conclusions]The prevalence rate of DA was low in the elderly in old-age care institutions.The prevalence of DA was significantly influenced by difference in pain tolerance and age.

Abstract:To explore the current status and influencing factors of dental anxiety among the elders in communities of Hangzhou. A total of 600 elders were selected by multi-stage stratified cluster random sampling and examined by Coarh's dental anxiety scale (DAS). The status and influencing factors of dental anxiety among them were analyzed by single and multi-factor analysis methods. A total of 596 questionnaires were answered 99.3%(596/600). And there were 573 valid questionnaires with a validity rate of 96.1%(573/596). The respondents' DAS score was 7.51±3.18. Their prevalence rate of DA was 34.9%. Among them, there was significant difference for the comparison of prevalence rate of DA in gender, age, different pain tolerance and experiences of dental care (χ2=9.45, 18.24, 33.34, 38.66, 12.62, P<0.05). The results of logistic regression model showed that there was a higher risk of DA occurrence among females, age, poor pain tolerance and elders with painful dental experiences. Gender, age, pain tolerance and experiences of dental care are influencing factors of oral health behaviors among the elders in communities of Hangzhou. Key words: Elder; Dental anxiety

Abstract:Objective To understand the current status of dental anxiety(DA)and it's influencing factors among the elderly in communities of Hangzhou. Methods A total of 600seniors were randomly selected by multi-stage stratified cluster sampling method and surveyed using the Coarh's Dental Anxiety Scale(DAS). One-way ANOVA and multiple linear regression analysis were used to analyze the data. Results 573out of 600questionnaires qualified for the study were retrieved.The average DAS score was 7.51±3.18.The prevalence of high DA was 6.1%.One-way ANOVA showed that the differences of DA scores were significant among seniors with gender,age,monthly income,pain tolerance,experience of dental care and oral hygiene(P<0.05).Meanwhile,multiple linear regression analysis revealed that pain tolerance,gender,age and monthly income were closely related to the DAS score.Usually,seniors of female with younger age,low income and poor pain tolerance were more likely to have higher scores of DA. Conclusions The prevalence of high DA is low among the elderly in communities of Hangzhou city.Pain tolerance,gender,age and monthly income are influencing factors for DA.
Sea-Level Rise and Adaptation Responses for Coastal Construction: A Spatial-Temporal Decision Making Tool

Abstract:Most infrastructure, settlements and facilities are located near the coast and are highly vulnerable to sea-level rise (SLR), coastal erosion and storms. Continued population growth in low-lying coastal areas will increase vulnerability to these hazards. The impacts of SLR are highly variable across regions and difficult to predict over time and space. Principally, due to constraints on the adaptive capacities of coastal areas, SLR increases the challenge of achieving sustainable development in these areas in developing countries, and they are likely to be hit hardest. Generally speaking, vulnerability and adaptation to climate change are urgent issues, especially among developing countries. The dilemmas confronting decision makers are: how and when to adapt to SLR. The complexity can easily overwhelm the ability of decision makers to thoroughly investigate the outcomes of adaptation alternatives. Therefore developing and implementing effective adaptation options is crucial for future coastal development. However, in many cases, it is difficult to determine whether taking a specific action to prepare for SLR is justified, due to uncertainty in the timing and magnitude of impacts. By considering the uncertain nature of projected changes in climate and addressing these dilemmas, this paper intends to provide a dynamic model for a comprehensive vulnerability assessment of coastal areas to assist decision makers to identify and evaluate effective adaptation alternatives for reducing climate change impacts. To achieve this outcome, two modeling techniques are combined: (1) System Dynamics, and (2) Geographical Information Systems. A combination of these approaches would provide the potential to address temporal and spatial problems concurrently.

Abstract:A survey of the literature purporting to deal with the effects of water deprivation shows that most of it has really been concerned with the results of a deficiency of salt. In 1935, however, Kerpel-Fronius distinguished between the changes in the body due solely to a deficiency of salt, and those due simply to a shortage of water, but this important distinction has been little appreciated either in this country or in America, although Nadal, Pedersen & Maddock [1941] set out to contrast the features of these two syndromes in man. Unfortunately, in one of Nadal et al. two subjects the period of water deprivation followed directly upon one of salt deficiency, while in the other deprivation of water was accompanied by fasting. Hence, although results of great interest were obtained, the effects of water deprivation were complicated in one person by salt deficiency, and in the other by fasting. Admittedly, in clinical medicine, deprivation of water is usually accompanied by one or other of these complications, but warfare by sea or land may subject men with plenty of food to a shortage of water. The present paper describes the results of metabolism studies on two subjects, who abstained from water for 3 and 4 days respectively, during which they took a 'dry' diet adequate in protein, salt and calories. Although a balance experiment on a human being cannot be carried to the point where the biochemical cause of death becomes clearly stigmatized, it is possible to distinguish physico-chemical happenings in the early days of water deprivation which would, if they went on long enough, provide a reasonable modus moriendi. It is this chain of events which it is now proposed to describe.

Abstract:Objectives This qualitative study aimed to develop an understanding of patients' barriers and facilitators of conversations about dentine hypersensitivity (DH) with their dentist. Methods The Theoretical Domains Framework shaped the topic guide for 26 participants who were troubled (High H) or not particularly troubled (Low L) by DH. Inductive thematic analysis of anonymised, transcribed, online focus group conversations was undertaken to identify reasons for non-discussion of DH during dental consultations. Results Participants reported that dentists did not routinely discuss DH, nor did participants feel they could bring it up. There was a perception among participants that DH was not a 'proper' condition and DH pain was not worthy of dentists' attention. Participants reported dealing with DH through lifestyle changes, and had little faith that dentists could offer more than 'just toothpaste' and voiced a need for a 'miracle cure'. Dental anxiety was a reported barrier to conversations. A positive attitude to DH by a dental team that normalised the condition and acknowledged its impacts on patients was voiced as a potential facilitator to DH discussions. Conclusions DH conversations do not routinely take place in dental practice because patients believe DH is not serious, DH pain is not legitimate and there is no credible solution dentists can offer. Patients and dental teams need to be supported through behavioural science tools to change these perceptions for DH conversations to take place more routinely. Clinical Significance As DH cannot be diagnosed by visual inspection of teeth, patients cannot be managed unless they raise the issue of their DH with their dentist. The paper helps clinicians manage patients who are reticent to discuss DH with them and suggests how dentists may intervene to help.

Abstract:Anxiety is an adaptive emotional response to potentially threatening or dangerous situations; moderated by the sympathetic nervous system.1,2 Dental anxiety is common and presents before, during or after dental treatment.3 The physiological response includes an increase in heart rate, blood pressure, respiratory rate, and cardiac output.1,2 Consequently, extensive distress leads to avoidance of dental treatment and multiple failed appointments, impacting both oral and general health.4 Dental anxiety can generate a variety of negative consequences for both the dentist and the patient.4 Evidence‐based strategies are essential for mitigating and relieving anxiety in the dental clinic.2,3 Psychotherapeutic behavioural strategies can modify the patient's experience through a minimally invasive approach with nil or negligible side effects, depending on patient characteristics, anxiety level and clinical situations.2,3 These therapies involve muscle relaxation, guided imagery, physiological monitoring, utilising biofeedback, hypnosis, acupuncture, distraction and desensitisation. Pharmacological intervention utilises either relative analgesia (nitrous oxide), conscious intravenous sedation or oral sedation, which can have undesirable side effects, risks and contraindications. These modalities increase the cost and availability of dental treatment.5 © 2022 Australian Dental Association.

The psychosocial background of dental care avoidance: A qualitative study

Abstract:Introduction Dental care avoidance affects individuals' oral health status. There is limited literature examining the context of dental avoidance behavior. Aim The aim of the study is to examine attitudes toward dental care services among adults. Methods Overall, 16 patients participated in semi‐structured interviews. The interviews were analyzed using inductive thematic analysis. Through independent reviews of the themes that comprise the patients' experience, a consensus was established among the researchers. Results Ten major themes were identified that encapsulate patients' attitudes toward dental care attendance: Effect of environment and equipment, Fear and anesthesia, Procrastination, Dissatisfaction with the dental care system, Expectations of dentists, Quality of dental care, Edentulousness and social status, Financial involvement, Aesthetics, and Dentists' behavior. Compliance with oral health maintenance can be enhanced by specific training for professionals and dentistry students and by fine tuning the dental care system. Conclusions This is the first comprehensive study in Hungary investigating attitudes among dentist patients toward dental care. The results suggest that there are many factors that may affect dental care attendance. Thus, these findings make a potentially fundamental contribution to the improvement of dental care and support practitioners in enriching their understanding of dental avoidance behavior.

Abstract:After analyzing the characteristics of the user in the human-computer interaction process, a user model and an interaction model based on multi-agent intelligence have been set up respectively, and a kind of interaction protocol for eye gazing interaction has been proposed. To verify the models and protocol, some experiments have been done mainly with the eye gazing human-computer interface (HCI), and the results show that the problems which used to exist in the similar interaction processes have been overcome, moreover, the prompt messages from the computer, when it is working, have enforced the user's learning ability by multimedia, such as vision and voice.
Novel non-pharmacological strategies for managing dentophobia in adult patients - literature review

Abstract:Dentophobia concerns a substantial portion of the adult population, often leading to avoidance of dental care and subsequent deterioration in oral health. This comprehensive review explores the multifaceted nature of odontophobia and examines various non-pharmacological strategies aimed at its management in adult patients. Factors contributing to dentophobia, including past traumatic experiences, feelings of vulnerability, and mistrust in dental professionals, are discussed, highlighting the complex interplay of psychological, physiological, and environmental influences. Novel approaches such as Virtual Reality Exposure Therapy offer promising avenues for systematically desensitising patients to their fears and enhancing treatment acceptance. Aromatherapy utilising essential oils like chamomile, orange, and lavender, as well as dog-assisted therapy, have shown potential in creating calming environments and reducing patient anxiety during dental procedures. Muscle relaxation therapy, biofeedback techniques, and process simulations provide additional tools for addressing the physiological and cognitive aspects of odontophobia. Cognitive-behavioural therapy interventions, including brief sessions focused on cognitive restructuring and exposure therapy, demonstrate efficacy in reducing fear of dentists and improving treatment outcomes. By incorporating these diverse non-pharmacological strategies into dental practice, clinicians can enhance patient experiences, increase treatment acceptance and adherence, and ultimately improve oral health outcomes. While these interventions show promising results, further research is needed to refine their implementation, optimize their effectiveness, and ensure accessibility to patients with dentophobia. By addressing the multifaceted nature of dental anxiety and adopting a patient-centred approach, clinicians can provide holistic care, fostering better oral health and overall well-being in their patients.
Analysis of Clinical Related Factors on Dental Anxiety in Adult Dental Outpatients

Abstract:Objective To explore the clinical related factors of dental anxiety in adult outpatients. Methods A total of 376 adult patients who were treated in Odontopathy Prevention Institute and the dental clinic of Zhongshan Hospital Affiliated to Fudan University from June to October 2017 were enrolled. The dental anxiety scale (DAS) was used to assess the occurrence of dental anxiety. SPSS 18.0 software was used for statistical analysis. Results There were 50 adult outpatients with anxiety or severe anxiety and 326 cases without anxiety. There was no statistically significant difference in degree of education between the two groups (4/26, 6//27, 10/74, 30/199, χ2=0.83, P>0.05). However, significant differences were found in the age (27/98, 12/96, 11/44), gender (42/181, 8/145), dental treatment history (7/11, 43/315) and childhood dental psychological fear (12/128, 38/198) between two groups(χ2=18.37, 14.57, 10.74, 4.32, all P<0.05). Multivariate Logistic regression was used to analyze the association of factors affecting dental anxiety. The results showed that it was significantly correlated with age (OR=50.66, 3.37), gender (OR=65.25), history of dental treatment (OR=14.22) and childhood dental psychological fear (OR=3.64) (all P<0.05). Conclusions Adult dental anxiety is common in clinical practices. There is a close relationship between the occurrence of anxiety and the patient′s gender, age, history of dental treatment and childhood dental psychological fear. Abstract:AIM In young, post-thrombotic patients, venous distensibility is decreased not only in the affected lower limb, but also in the contralateral limb and in the jugular vein when compared to age-matched control subjects. In the present study, we investigated venous wall mechanical properties in young, asymptomatic thrombophilic patients. METHODS Eleven young (24+/-0.4 years) control subjects and 9 age-matched patients (21.1+/-1.8 years) with proven thrombophilic molecular defects, but without any signs or history of previous deep vein thrombosis, were compared. Anterolateral and mediolateral diameters of the common femoral, axillary and internal jugular veins were measured by ultrasonography in situ. Pressure alterations were induced by altering body positions and by pressure-controlled Valsalva tests. Distensibility was calculated from diameter and pressure changes. RESULTS In thrombophilic patients, resting diameter of both the common femoral and of internal jugular veins at low transmural pressure was larger than those for the control subjects. Distensibility, however, was significantly less when high pressures were applied. Alterations in diameter of the axillary vein were minimal. CONCLUSION Our measurements suggest that there are generalized changes in venous mechanical properties in thrombophilic patients even before the appearance of thrombotic processes. These biomechanical alterations of the venous wall and/or surrounding connective tissue are similar to those found in connection with aging and in post-thrombotic patients. The pathological mechanisms behind these processes are unknown.

Abstract:Background: Parental dental fear and anxiety (DFA) is an important factor, which has an impact on adolescence receiving dental treatment and maintenance of their oral health. It is necessary to recognize and know how parental DFA affects the dental treatment of children and adolescents. Aim: This narrative review was planned with the objective of evaluating parental DFA influence on adolescent dental treatment. Materials and Methods: A broad search of literature published between 2005 and 2021 from electronic databases through Scopus, PubMed, Web of Science, Google Scholar was performed. We included the studies in which parental dental anxiety was a major criterion affecting different dental health conditions. Articles referring to adolescents facing different oral health problems were also included. This narrative review included 12 articles of which 8 cross-sectional studies, 3 longitudinal studies, and 1 descriptive study, all of which met the inclusion criteria and the specified age group of adolescents ranging between 10 and 19 years. Results: After screening 83 abstracts, 12 articles were selected, which included all the inclusion criteria. In this study, we found that parental DFA showed a positive association with their adolescent's DFA, which hinders the dental treatment received. Conclusion: Parental DFA influences the adolescent behavior and can impact the seeking of dental treatment. Hence, it is important to address parental DFA prior to the intervention and treatment. An appropriate address will facilitate in reducing or eliminating DFA in adolescents.

Abstract:Background: Dental anxiety is marked by intense and irrational fear of dental procedures affecting millions of individuals worldwide. The purpose of this study was to investigate the relationship between dental anxiety, its influencing factors, and the preferred management techniques among adults seeking dental care in Riyadh. Methods: A cross-sectional questionnaire study was carried out among 1734 patients who visited dental clinics in Riyadh. A self-administered questionnaire was developed and validated, comprising 28 closed-ended questions; it demonstrated good reliability and internal consistency (Cohen's kappa coefficient = 0.90, Cronbach's alpha coefficient = 0.86), it contained pre-validated Modified Dental Anxiety Scale to quantitatively assess the level of dental anxiety. Data analysis involved descriptive analysis, Chi-square test, Pearson correlation coefficient and multiple linear regression. Results: Majority (59.2%) of participants reported moderate anxiety, while 10.9% experienced high anxiety which was significantly linked to factors such as fear of pain (37.8%) and anaesthetic needles (25.8%). Topical anaesthetic gel (64.5%), pre-treatment explanations (78.9%), and conducive clinic environment (79.4%) were perceived as effective anxiety alleviators. A negative correlation existed between dental anxiety and preferred management techniques. Dental anxiety had significant association between gender (β = 0.910) and age (β = 0.263). Conclusion: This study revealed that majority of participants had moderate dental anxiety, linked significantly to influencing factors like frequency and purpose of dental visits and past dental experiences. The study also found the preferred anxiety management methods among participants which included topical anaesthetic gel, pre-treatment explanations, and a comfortable clinic environment.

Abstract:BackgroundDentistry in Australia combines business and health care service, that is, the majority of patients pay money for tangible dental procedures such as fluoride applications, dental radiographs, dental fillings, crowns, and dentures among others. There is evidence that patients question dentists’ behaviours and attitudes during a dental visit when those highly technical procedures are performed. However, little is known about how patients’ experience dental care as a whole. This paper illustrates the findings from a qualitative study recently undertaken in general dental practice in Australia. It focuses on patients’ experiences of dental care, particularly on the relationship between patients and dentists during the provision of preventive care and advice in general dental practices.MethodsSeventeen patients were interviewed. Data analysis consisted of transcript coding, detailed memo writing, and data interpretation.ResultsPatients described their experiences when visiting dental practices with and without a structured preventive approach in place, together with the historical, biological, financial, psychosocial and habitual dimensions of their experience. Potential barriers that could hinder preventive activities as well as facilitators for prevention were also described. The offer of preventive dental care and advice was an amazing revelation for this group of patients as they realized that dentists could practice dentistry without having to “drill and fill” their teeth. All patients, regardless of the practice they came from or their level of clinical risk of developing dental caries, valued having a caring dentist who respected them and listened to their concerns without “blaming” them for their oral health status. These patients complied with and supported the preventive care options because they were being “treated as a person not as a patient” by their dentists. Patients valued dentists who made them aware of existing preventive options, educated them about how to maintain a healthy mouth and teeth, and supported and reassured them frequently during visits.ConclusionsPatients valued having a supportive and caring dentist and a dedicated dental team. The experience of having a dedicated, supportive and caring dentist helped patients to take control of their own oral health. These dentists and dental teams produced profound changes in not just the oral health care routines of patients, but in the way patients thought about their own oral health and the role of dental professionals.
